Abstract
In consultation with direct care staff, the authors developed and supervised the implementation of a program to treat the intensive fear response for a nonverbal severely retarded woman. At the time of referral, the subject was tearing clothing, stopping up toilets with sanitary pads, and would only tolerate the wearing of sanitary pads for five seconds. The treatment consisted of a five phase in vivo desensitization program in which the subject's tolerance for wearing pads was gradually increased. Tolerance was developed by reinforcing appropriate responses at each step in the hierarchical program until the subject was able to successfully wear a sanitary pad without exhibiting avoidance behaviors.
