Abstract

More than 40 years ago, the idea for what may be a landmark opportunity for pharmacy was introduced. In 1967, the American Academy of Pediatrics (AAP) first presented the concept of the medical home, with a focus on having a central location for a child's medical information. 1. In March 2007, the American Academy of Family Physicians, AAP, American College of Physicians, and American Osteopathic Association issued a joint statement on the principles of the PCMH. 2.
