Abstract
IntroductionLaparoscopic treatment of parastomal hernia is controversial because of its high rate of recurrence. There is still no consensus on what the ideal technique should be. We present a new technical option and discuss surgical principles to be observed for this type of lateral hernia. Case reportWe describe a new technique that satisfies the criteria for reconstruction of the entire lateral abdominal wall using a new type of mesh, a low-density polypropylene-coated titanium. The technique performs a complete overlap of the entire posterior half of the hemi-abdominal wall, consisting of a combined fixation with clamps and cyanoacrylate (to the costal margin and the iliac crest) and a tunneling loop segment by means of a mesh fenestration without a center hole. ConclusionLaparoscopic repair of parastomal hernia seems safe and effective. The technique needs to be standardized and centralized. We present a new option that embodies the principles of the two known repairs, such as «keyhole» and Sugarbaker, adding the concept of comprehensive treatment of the affected side of the hemi-abdominal wall.
