Abstract
This report describes a patient with an ejection fraction of 0.16, initially attributed to toxic cardiomyopathy, in whom severe coronary artery disease with regions of metabolically active myocardium was later demonstrated. Coronary artery bypass graft surgery led to delayed improvement in ventricular function and exercise performance and the patient's return to work without cardiac transplantation
