Abstract

A 79-YEAR-OLD MAN (weight, 98.8 kg; height, 172.3 cm) was scheduled to undergo elective coronary artery bypass graft surgery for severe 3-vessel coronary artery disease. The patient's history was notable for chronic paroxysmal atrial flutter with variable atrioventricular heart block (Fig 1). After anesthetic induction, a pulmonary artery catheter was inserted through the right internal jugular vein. Central venous and mean pulmonary artery pressures were 25 and 40 mmHg, respectively. The following images were obtained during transesophageal echocardiography interrogation of the mitral and tricuspid valves before cardiopulmonary bypass (Figs 2 and 3; Video 1).
