Abstract
Cortical demyelination (CDM) is known to occur in chronic multiple sclerosis (MS). It is an important pathological substrate of progression in chronic disease. Not much is known about cortical demyelination in early MS. Recent neuroimaging and histopathological evidence show that CDM is common in early MS lesions. Conventional MRI is not sensitive to the presence of cortical lesions. Studies utilizing other imaging modalities reveal cortical involvement in early MS. Most histopathological studies in MS utilized tissue obtained at autopsy. These findings correlate with chronic disease. Histopathological studies utilizing brain biopsies in patients with MS provide real-time evidence of active cortical demyelination in early MS lesions. This paper reviews current evidence of CDM in early MS and present clinical correlates.
