Abstract
전신마취 시 기관 내 삽관법으로는 경구 기관내 삽관법, 경비 기관내 삽관법, 굴곡성 후두경법, 역행성 기관내 삽관 법 등이 있는데 이중 경비 기관내 삽관은 수술의 편의를 위 하여두경부 수술환자에서많이 사용하고있다. 그러나 경비 기관 내 삽관시에는 비출혈, 비점막 및 비인 두점막의 손상, 비용(nasal polyp) 또는 비강내 이물의 이동 에 의한 출혈 및 기도 폐쇄, 비갑개의 점막 열상에 의한 경 비기관 튜브 폐쇄 등의 초기에 발생할 수 있는 합병증과 삽 관시 튜브에 의한 기도 손상 및 식도 손상에 의한 기흉, 장 시간 삽관 튜브의 거치에 의한 비괴사, 부비동염, 중이염 및 균혈증 등의 후기에 발생하는 합병증을 볼 수 있는데 일 부의 시술자들은 삽관튜브가 비강을 통과할 때 점막 손상 에 의한 비출혈 및 비중격과 하비갑개 사이에서 발생하는 저항 등의합병증을 경험하기도한다. 따라서, 본 연구에서는 비강의 해부학적인 구조의 특성 으로 인한 삽관시 발생할 수 있는 점막 손상으로 인한 비출 혈과 비중격과 하비갑개 사이의 저항을 줄이기 위하여 computed tomography (CT)를 이용한 비강의 평가 방법 및 평가 후 해부학적 구조에 따른 전처치에 관하여 연구를 시 행하였다.
