Abstract
目的 探讨胎盘部位滋养细胞瘤(PSTT)的临床病理特点与鉴别诊断.方法对5例PSTT与8例绒癌进行临床病理学对比观察和免疫组化CK、HCG、HPL、PsAP 标记.结果 PSTT瘤细胞主要为中间型滋养细胞.有明显嗜血管浸润倾向,但血管结构仍保存.浸润分离平滑肌,但不全摧毁,间质中有广泛的纤维蛋白样物质沉积.CK和HPL呈弥漫性阳性,HCG、PsAP弱阳性.绒癌由细胞滋养细胞和合体滋养细胞混合组成,肿瘤细胞异型性明显,易见核分裂像.侵犯破坏血管和平滑肌,见大片出血和坏死.免疫组化CK及HCG弥漫性阳性而HPL部分弱阳性,PsAP阴性.结论 PSTT是一种极少见的滋养细胞肿瘤,形态学结合免疫组化HCG、HPL、PsAP检测有助于PSTT与其它滋养细胞疾病的鉴别。
