Abstract
Objective To summarize the experience of transumbilic laparoscopic appendectomy (TULA) and laparoscopic appendectomy (LA) in the treatment of acute appendicitis in children.Methods Forty-three children underwent transumbilic laparoscopic appendectomy (TULA) from September 2009 to April 2011,and 83 underwent laparoscopic appendectomy (LA) from January 2006 to December 2008 in our hospital.The intraoperative blood loss,operation time,time of resuming intestinal mobility,postoperative hospital stay,wound infection,abdominal drainage rate,time for removal of abdominal drainage tube,abdominal abscess rate,intestinal obstruction rate,and reoperation rates were compared between the TULA and LA group.Results The operating time in TULA group (95.7± 25.3)min was longer than the LA group (80.9 ± 23.8)min,with significant difference (P<0.01).There were no significant difference between the TULA and LA group in the intraoperative blood loss ( 12.9 ± 9.4 ml vs.12.9 ± 9.2 ml),time of resuming intestinal mobility (43.3 ± 10.3 h vs.46.1 ±11.1 h),postoperative hospital stay (3.8 ± 1.4 d vs.3.7 ± 1.5),wound infection rate (2.4% vs.2.4%),abdominal drainage rate (9.3% vs.12.0%),time for removal of abdominal drainage tube (25.9±5.4 h vs.24.7±7.1 h),abdominal abscess rate (4.6% vs.4.8%),intestinal obstruction rate (4.6% vs.6.0%),and reoperation rates (2.3% vs.2.4%),However,the cosmetic results of TULA group were better than that in LA group.Conclusions The TULA is feasible and safe for children with appendicitis and has better cosmetic outcomes.
