Abstract

An 8-month-old girl presented after a white opacity was noticed on the left eye. Evaluation showed an anterior pyramidal cataract. Right eye was normal. At 3 years of age, despite aggressive amblyopia therapy with part-time occlusion and atropinization of the right eye, examination demonstrated strong right eye fixation preference, so left eye cataract extraction was performed. During surgery, anterior left lens photography displayed an anterior pyramidal cataract, a pyramid-shaped plaque that protrudes into the anterior chamber, and cortical vacuoles and opacities (A).
