Abstract

Objective To explore the effect of lymphocytes active immunotherapy in the treatment of recurrent spontaneous abortion(RSA). Methods From January 2009 to June 2013, a total of 331 cases of RSA undergoing the lymphocytes active immunotherapy in Reproductive Medicine Center, Northern Jiangsu People's Hospital were chosen as study subjects.According to different donor of active immunogens, 331 cases were divided into group A (307 cases, using husband's lymphocyte) and group B(24 cases, using unrelated male's lymphocyte). According to recheck results of blocking antibodies(BA), those 78 cases who agreed to detect BA after 3 times lymphocytes active immunotherapies and follow-up successfully were divided into BA positive recheck group(40 cases) and BA negative recheck group(38 cases). Compare the successful rate of subsequent pregnancy between group A and group B, and between BA positive recheck group and BA negative recheck group.The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Reproductive Medicine Center, Northern Jiangsu People's Hospital.Informed consent was obtained from each participating patient.There was no statistically significant difference on average age between group A, B and those of BA positive recheck group and BA negative recheck group, respectivly (P>0.05). Results The rate of BA turn to positive after 3 times lymphocytes active immunotherapies was 51.4%(76/148). Among all 331 cases treated with lymphocytes active immunotherapies, there were 188 cases who were pregnant successfully again and delivered with live-birth newborns.Until the end of our study, there were 24 cases whose gestational ages were more than or equal 28 weeks, and the results of ultrasound image detections were normal, but had not deliveried yet; 23 cases were in pregnancy within 28 weeks and the results of ultrasound image detections were normal also, and 83 cases were not pregnant yet, 13 cases underwent spontaneous abortion once again. The successful rate of subsequent pregnancy for all patients in our study was 85.5%(212/248). After 3 times lymphocytes active immunotherapies , there were neither statistically significant difference on the successful rates of subsequent pregnancy between group A and B (84.4% vs 95.7%; χ2=2.112, P>0.05) nor between BA positive recheck group and BA negative recheck group(84.6% vs 88.2%; χ2=0.167, P>0.05). Conclusions The lymphocytes active immunotherapy maybe can improve immune function, elevates BA production and improve the pregnancy maintenance in BA lacking RSA patients.It is a safe and effective therapy.
