Abstract
The editorial proposes a new classification of acute respiratory distress syndrome (ARDS). Acute lung injury (ALI) is regarded as the first stage of ARDS. Diagnostic tests based on the use of new technologies, such as determination of pulmonary extravascular water, pulmonary capillary permeability index, oxygenation index, and hemodynamic disorders are recommended. The early signs of acute lung injury should be detected to perform early appropriate therapy in order to prevent further complications and to reduce mortality rates.

В редакционной статье предлагается новая классификация острого респираторного дистресс синдрома (ОРДС). Ос трое повреждение легких (ОПЛ) рассматривается как первая стадия ОРДС. Острый респираторный дистресс синдром (ОРДС) является частым осложнением тяжелой соче танной травмы, сепсиса, неадекватной вентиляции лег ких, массивной инфузионно трансфузионной терапии, тромбоэмболии легочной артерии и пр. По данным литературы [3], ост рый респираторный дистресс синдром составляет лишь около 25% всех проявлений острого тяжелого респира торного синдрома (Severe Acute Respiratory Syndrome — SARS). Согласительной конференцией 1992 года было предложено выделить «острое повреждение легких» (ОПЛ), как состояние, предшествующее развитию ост рого респираторного дистресс синдрома, рекомендова ны диагностические критерии острого повреждения легких [4].
