Abstract

Vascular endothelium of the brain has a multitude of regulatory functions that impact vascular tone, inflammatory responses, oxidative stress, angiogenesis, and thrombosis and hemostasis. Brain capillary endothelium is the critical interface of the blood–brain barrier, with a wide range of specific functional and structural components. Endothelial dysfunction thus has many implications and can contribute to stroke and cerebrovascular disorders in a multitude of ways. Pharmacologic targeting of these endothelial functions represents an important future therapeutic direction for stroke. The approaches with the most promise include statins, antioxidants, and phosphodiesterase inhibitors.
