Abstract

The development of safety management systems offshore has been driven in response to events that have had a catastrophic and/or environmental impact. A series of such events are described, starting with the Titanic (1912) and concluding with Deepwater Horizon/Macondo (2010). Each event is analyzed for its long-term impact on the management of offshore safety and in terms of the elements of SEMS (Safety and Environmental Management Systems). This chapter includes a detailed review of four of the key reports written on the Deepwater Horizon catastrophe, including an analysis of the recommendations made by these reports and how the industry has responded.
