Abstract

Diabetic foot disease is one of the most feared of all diabetes complications. Diabetic foot ulceration (DFU) can lead to significant disability, increase the risk of developing an infection and put the individual at risk of amputation. Charcot neuroarthropathy (CN) which is typified by bone and joint disorganization may lead to major foot deformities and subsequent risk of ulceration. In this chapter, we discuss the contemporary management of the diabetic foot which includes care provision through a multidisciplinary diabetic foot service alongside current concepts in DFU and CN care. Prompt recognition, early referral to a specialist service, and high-quality standard care including offloading are key management principles. Many advanced wound care technologies are now available with good quality evidence to support their use although most currently lack robust cost-effectiveness data. The role of diabetic foot surgery to achieve healing and maintain remission is also receiving widespread acceptance and is discussed briefly.
