Abstract
The importance of quality and the intense competition in the industries encourage company to evaluate the risks in the production process so company can produce high quality and competitive product. This research aims to identify factors that cause defects in product, calculate the sigma level of the production process and arrange mitigation actions that can be done to minimize the risks in the food industry that has high level of defective products. This research used ix Sigma DMAIC (define, measure, analyse, improve, control) and FMEA-HOR (Failure Modes and Effect Analysis House of Risk) to analyse dominant risks and mitigation actions from expert’s perspective. The calculation results show that, sigma level in production process is 3.20. There are 5 risk agents in the production process that cause defect in product and 9 priority mitigation actions to minimize the risks. Example of priority actions such as invite online operators in the Daily Meeting System (DMS), conducting centre lining (marking position), and optimization of scheduling Preventive Maintenance (PM), make One Point Lesson (OPL), and standardization cleaning-inspection-lubrication (CIL).
