Abstract
This paper reviews the ignition failure of a solid rocket engine in a flight test. A fault tree is established to investigate and analyze the causes. After a series of ground tests to reproduce the faults, it is considered that the humidity of the igniter is the main reason affecting the normal ignition of the engine, and follow-up improvement measures are formulated.
