Abstract
Objective: To improve clinical medication safety and avoid medication errors in Shanghai Public Health Clinical Center.Methods: Various errors occurring during August 2008 to August 2010 in the pharmacy intravenous admixture services(PIVAS) of the center were analyzed in accordance with drug package inserts,Pharmacopeia of the People's Republic of China and Clinical Medication Information,meanwhile the continuous improvement measures were formulated.Results: The rate of external errors in the PIVAS was 0.02%,and the rate of internal errors was 0.18%.The errors occurred mainly in the following steps: medical orders(by doctors),prescription check,arrangement of drugs,admixture of drugs,the check of finished products,as well as drug returning,etc.Conclusion: By taking the following measures,such as carrying out errors reporting system,enhancing prescription-checking ability of pharmacists,strengthening pharmacists management,intensifying drug management system,etc,the incidence of errors in PIVAS can be reduced and the rational clinical medication can be promoted.
