Abstract

Necrosis of pulp with periapical changes is common detrimental sequelae of dental traumatic injuries to teeth if not treated early. Conventional endodontic treatment alone may be difficult or ineffective in such cases especially in teeth with morphological deviations (e.g. dilacerated root) wherein traditional treatment approach may be insufficient to eliminate the infection from the complex root canal system. In such cases, a combination of conventional and surgical endodontic therapy is deemed necessary to completely eliminate the periradicular pathosis and promote apical healing. Moreover, prompt placement of coronal restoration is essential in such cases to seal the canal and improve prognosis of endodontically treated teeth. The present case report describes successful endo-aesthetic management of a chronic large periapical lesion in left permanent mandibular central incisor with dilacerated root with combined root canal and surgical endodontic treatment followed by its rehabilitation with fiber post and porcelain fused to metal (PFM) crown.
