Abstract

An 18-month-old girl presented with a history of persistent cow's milk allergy following a severe anaphylactic reaction at two and a half months of age. She was found to be significantly sensitized to α-lactalbumin, β-lactoglobulin, casein and bovine serum albumin. Sublingual immunotherapy for 3 months followed by oral immunotherapy over a period of three years resulted in a reduction of specific IgE from greater than 100kU/L to 20kU/L, a significant increase in specific IgG4 and full tolerance to 90mL of milk as well as to various other dairy products. The authors emphasize the effectiveness of a short course of sublingual immunotherapy followed by a longer course of oral immunotherapy in such cases.
