Abstract
This paper discusses the implementation of public policies at the local level in areas of high concentration of poverty by the study of mediation and structural dynamics established by the implementing agents in the population and government. We analyze how these agents perform their practical implementation and by the construction of relational mechanisms and their network structure and its consequences in people’s access to public services. The results are intended to contribute to the understanding of the explanatory role of location in the results of public policies in the Brazilian federal arrangement. The empirical results were generated in qualitative studies and analysis of social networks developed with Community Health Workers in different regions of Brazil between 2004 and 2005. We studied Agents from four Brazilian municipalities: Sobral, Sao Paulo, Taboao da Serra and Santo Andre. We did ethnographic research to understand their practices and also social network research, considering the networks from the agents and also from the implementation policy. A discussão mais ampla sobre as políticas públicas e a desigualdade social tem como um dos principais desafios explicitar os mecanismos inseridos no processo de formulação e implementação dessas políticas, que levam à variação dos seus resultados e consequências não antecipadas. Pelo fato de serem selecionados entre moradores da própria comunidade, os ACS acabam se tornando atores híbridos, burocratas comunitários que, ao implementar suas ações, têm ainda mais mudanças na construção de mecanismos e dinâmicas relacionais que modificam a forma como as políticas foram concebidas, garantindo a elas novas potencialidades, mas de forma heterogênea em cada local onde são implementadas. As considerações deste trabalho orientam-se, assim, pelo conhecimento da realidade das políticas públicas efetivamente implementadas, destacando em particular o impacto dos processos no resultado das políticas, suas consequências não intencionadas e seus efeitos, principalmente no acesso da população a recursos e serviços públicos. Realizamos uma análise dos efeitos produzidos por mudanças estruturais nos padrões de acesso às políticas públicas dos grupos que acumulam desvantagens sociais pelas dinâmicas de implementação das políticas, no estabelecimento de barreiras ou de ampliação aos recursos ofertados pelo poder público, discutindo principalmente o papel do agente implementador “da ponta”.
