Abstract

An innovative, state-of-the-art, comprehensive nutrition assessment and case management system has been developed by nutritionists (R.D.s) and public health nurses with the San Bernardino County Public Health Department in California. The system consists of three major components: flowsheets for the infant, the child, the perinatal client, family planning, and the adolescent/adult/aging client; a manual that defines and describes nutrition assessment guidelines and interventions; and an appendix that includes resource/reference information. Distinct advantages and unique features of this system include the following: (a) decrease in charting time; (b) precise documentation; (c) a workable diagnostic tool for implementing nutrition interventions; (d) a standard for consistent, quality nutrition care; (e) computer adaptability; and (f) easy accessibility for quality assurance audits.
