Abstract
症例は12歳の男児で，5歳時より腹痛を繰り返し，急性膵炎とそれによる仮性膵嚢胞と診断され加療を受けていたが原因は判然としなかった．12歳時に腹痛が再燃し，CTで胃重複症と胆石症を指摘され，手術を施行した．まず胆嚢を摘出した．胃幽門部大彎の重複胃が，横行結腸間膜に沿って舌状に張り出した分葉膵と癒着していた．両者を接合部で切離すると互いに交通していたが，胃と重複胃の交通はみられなかった．分葉膵断端からの造影により主膵管が描出された．重複胃の粘膜および分葉膵を切離した．消化管重複症が胃に生じることは比較的まれであるが，重複胃と膵が直接交通した症例は，PubMedで検索したかぎり本症例を含めてこの50年で15例（本邦では4例）と極めてまれであり，診断と治療に難渋した報告が大半を占める．本症例では重複胃内容のドレナージが不良になることで分葉膵が膵炎を繰り返したと考えられた．
