Abstract
肺定型カルチノイドのリンパ節転移は稀である．症例は72歳，男性．検診胸部X線で異常を指摘され近医を受診し，CTで左下葉に径3.1cmの腫瘤を認め，気管支鏡でカルチノイド疑いと診断され当科へ紹介となった．PETで腫瘤に異常集積を認めたが転移所見はなく，左下葉肺癌疑い，cT2aN0M0:Stage IBと診断した．手術を希望せず厳重経過観察となったが，軽度増大を認めたため手術となった．胸腔鏡下左下葉切除術および肺門縦隔リンパ節郭清を施行し，術後病理で肺定型カルチノイドと診断された．#12Lに1個リンパ節転移を認め，pT2aN1M0:Stage IIBの診断となった．術後経過は良好で，術後8日目にリハビリ目的にて転院となった．肺定型カルチノイドであってもリンパ節転移をきたす可能性があるため，術式選択は慎重に行うべきと考えられた．
