Abstract

Postpartum weight retention (PPWR) is associated with adverse maternal outcomes. Gestational weight gain (GWG) and PPWR can be reduced with counseling, however it is estimated that one third of women receive no GWG counseling. We aim to see if implementation of GWG video module can decrease PPWR. This was an IRB-approved, prospective cohort study conducted from February to October 2019. Patients were recruited from a large academic practice during the first trimester of pregnancy. Group 1 were patients in the control cohort and received routine care, while group 2 were in the intervention cohort and watched the 5-minute GWG video module. Patients were included if they were ≥18 years old and had a singleton intrauterine pregnancy. Patients were excluded if they were non-English speaking, had a fetus with a known anomaly and/or demise, or if they had a documented eating disorder. Pre-pregnancy weight was recorded for all. Patients who attended their 6-week postpartum visit (PPV) had the difference between their postpartum and pre-pregnancy weights calculated. This was compared between both groups using the Mann-Whitney U test, with p<0.05 deemed statistically significant. There were 79 patients in group 1 and 76 patients in group 2 (Table 1). 68.35% (n=54) of group 1 attended an in-person postpartum visit, as compared to only 31.57% (n=24) of group 2. The average maternal weight at the PPV was 74.82 +/- 19.9kg and 85.94 +/- 17.76kg in groups 1 and 2, respectively (p=0.007). There was no difference between groups 1 and 2 when comparing the difference between pre-pregnancy and postpartum weight. The average weight difference was 6.877 +/- 6.867kg and 6.763 +/- 8.681kg in groups 1 and 2, respectively (p=0.657). A GWG video module did not improve PPWR. However, our video was only shown during the first trimester; it is possible that a video later in pregnancy or in the immediate postpartum period may show a different effect. Additionally, many patients in group 2 were postpartum during the COVID-19 pandemic, limiting access to in-office visits and possibly skewing results.View Large Image Figure ViewerDownload Hi-res image Download (PPT)
