Abstract

Approximately 70% of patients do not gain appropriate weight during pregnancy and almost one third of women receive no counseling about gestational weight gain (GWG) during prenatal care. The aim of this study is to assess if implementation of a video module improves patient adherence to recommended GWG guidelines. This was an IRB-approved, prospective cohort study conducted from February-October 2019. Patients were recruited from a large academic practice during their first trimester of pregnancy. Group 1 were patients in the control cohort and received routine care, while group 2 were in the intervention cohort. Patients were included if they were ≥18 years old and had a singleton intrauterine pregnancy. Patients were excluded if they were non-English speaking, had a fetus with a known anomaly and/or demise, or if they had a documented eating disorder. Pre-pregnancy weight was recorded for all. In addition to routine care, Group 2 watched the 5-minute GWG video module. On admission to the hospital for delivery, all patients’ GWG was calculated, and the overall GWG differences between the two groups were calculated. T-tests and Mann-Whitney U tests were used to compare groups, with a value of p<0.05 deemed statistically significant. During the study period, 155 patients were recruited, with 79 in group 1 and 76 in group 2. Demographic information was collected for both groups (Table 1). There was no significant difference in the appropriate amount of weight gained between the two groups; 24% (n=18) of patients in group 1 vs. 25% (n=17) of patients in group 2 (p=0.926). In women who did not gain the appropriate amount of weight, 60% (n=44) and 63% (n=43) of patients gained too much weight in groups 1 and 2, respectively, and 16% (n=12) and 12% (n=8) of patients did not gain enough weight in groups 1 and 2, respectively (p=0.745). In this study, a video module did not improve adherence to GWG guidelines. It is possible that a continuous intervention throughout the pregnancy, such as with app-based technology or a recurring video series, may better improve adherence.
