Abstract

A 50-year-old woman with a medical history of polycystic ovarian syndrome presented for acute onset of a pruritic rash on bilateral upper and lower extremities. On examination, erythematous edematous plaques consistent with urticaria were present on the bilateral upper and lower extremities. A punch biopsy of a thigh lesion showed urticaria with no evidence of vasculitis. She was prescribed cetirizine for acute urticaria. At her 1-month follow-up visit, she reported continued urticaria nightly along with erythema and swelling of the hands and fingers, dizziness, and nausea.
