Abstract

Objective : determination of the mechanisms of the formation of changes in the periodontal tissues of children under conditions of affliction with diffuse nontoxic goiter by means of evaluation of the main etiological factors promoting the development of gingivitis. Methods . For this purpose 150 children at the age of 12 years and 145 children at the age of 15 years suffering from diffuse nontoxic goiter were clinically observed. The children were distributed into subgroups depending on the degree of severity of the thyroid pathology. The control group included somatically healthy children of the same age. To estimate periodontopathogenic situation in the oral cavity of the children from the groups of observation the following indices were detected: hygienic (OHI-S and Silness-Loe), local provoking factors (caries, teeth anomalies and deformations), condition of the local immunity by lysozyme activity, degree of dysbiosis by the urease activity; peculiarities of the diet and social-behavioral factors were analyzed by the findings of the conducted survey. Conclusion. The main periodontopathogenic factors are found in children afflicted with diffuse nontoxic goiter as well as in somatically healthy children, which become a favorable medium for the initiation of the inflammation in the gums. But much worse indicators of periodontal tissues against a background of diffuse nontoxic goiter in children suggest that the process of gingivitis is morphologically and functionally altered periodontal tissues. To our mind, the changes found are the result of metabolic disorders in the hard and soft tissues of the dento-alveolar apparatus in case of thyroid pathology.
