Abstract

Abstract. We describe a clinical case report of severe acute intoxication of ethanol and its surrogates with symptoms of exotoxic shock and multiple organ failure in a young patient, who is addicted to alcohol for a long time. We describe the etiology, pathogenesis, methods for diagnosing and treatment of acute poisoning with ethanol and its surrogates and complications of this disease. This research topic is relevant, as the acute alcohol poisoning is one of the main reasons of hospitalization of patients with toxic effect and leads to a large percent of lethal outcomes. Acute respiratory failure of the 3rd degree required an artificial lung ventilation through a tracheostomy cannula. The percutaneous puncture-dilation tracheostomy was performed to the patient in the resuscitation and intensive care ward. Performed actions improved the sanation measures of the tracheobronchial tree and prevented infectious complications of the lungs and upper respiratory tract. In the management of this patient, a multidisciplinary approach was used with the involvement of toxicologists, nephrologists, therapists, resuscitators, gastroenterologists. All of the above contributed to the successful treatment and recovery of the patient. Aim: to show on a clinical example the efficiency of a multidisplinary approach to the diagnosis and treatment of severe intoxication of ethanol and its surrogates developed complications such as exotoxic shock and multiple organ failure, and also the efficiency of long-term artificial lung ventilation through a tracheostomy cannula with low-traumatic percutaneous puncture-dilation method.
