Abstract

A total of 53 patients with benign prostatic hyperplasia were treated with the Ho: YAG laser using a new technique termed Holmium Laser Resection of the Prostate (HoLRP). A high powered holmium laser was used and was set at 2.4 joules/pulse at 25 pulses/sec for an average power of 60 watts. The laser energy was applied directly to prostatic tissue via an end-firing 550 micron fiber. IPSS and QOL scores improved from one week after surgery, when compared to the preoperative scores, and lasted to 12 months postoperatively. Maximum flow rates also significantly improved at all measurement points compared to preoperative values. The retrograde urethrography revealed that a cavity was successfully formed and that the defect was identical to that of a conventional TURP. The operative factors of patients treated with HoLRP were compared with the patients treated with TURP. The average amount of resected tissue, the average volume of operative bleeding and the average duration of postoperative catheter drainage were greater in TURP than in HoLRP.Because of its effectiveness and safety, HoLRP is a useful surgical alternative for BPH.
