About 50% of (~13 million) reproductive aged women with Type I or II diabetes receive any preconception care. Even fewer receive contraceptive counseling, or other vital reproductive health services. This scoping review aimed to explore if reproductive aged women with diabetes receive adequate preventative reproductive health services compared to women without diabetes. A systematic search strategy of the published literature was built and applied in multiple databases (PubMed, Web of Science, Cochrane). Interventional and observational studies describing access to reproductive health services among reproductive-aged women with compared to those without diabetes were included. The Preferred Reporting Items for Scoping Reviews and Meta-Analyses (PRISMA) guidelines were followed. The search strategy identified 6785 unique records. A two reviewer method was used to screen abstracts, with 41 studies identified to be relevant. These studies were separated into the domains of contraception counseling (N=20), cervical cancer screening (N=12), counseling/screening for sexually transmitted infections (N=3), and preconception counseling (N=12). We did not include studies focusing on prenatal care, pregnancy, menstrual irregularities, sexual dysfunction, or breast cancer screening. While a vast amount of literature exists related to diabetes and pregnancy, this scoping review draws attention to the lack of research focused on the reproductive health of women with diabetes prior to pregnancy. The majority of available literature focuses on contraception and preconception management, consistently demonstrating that women with diabetes lack adequate preventative reproductive healthcare compared to their nondiabetic counterparts. Far less evidence exists for sexually transmitted infection risk assessment and screening. Physical Access Related User-Friendliness of Sexual and Reproductive Health Services for Women With Disabilities in Various Hospitals in a City in North India: An Integrated Qualitative and Quantitative Study.

Access to optimum health care services is vital for every woman. Women with disabilities (WWD), in particular, face multiple discrimination and social exclusion on this issue. The objective of the study was to evaluate the user-friendliness of sexual and reproductive health (SRH) services in terms of the physical access to health facilities in various hospitals in a city in north India. This cross-sectional evaluation study was conducted during 2013-2017 in five purposively selected major government hospitals in a city in north India. A disabled friendliness evaluation tool was used to analyze the barriers to physical access in terms of approach to SRHcare facilities. Data analysis was done through MicrosoftExcel (Microsoft, Redmond, WA, USA) and content analysis. Results: Overall, 270 respondents aged 15 years and above with at least 40% disability were interviewed for the study. Lack of access to SRH care in hospitals (infrastructure/equipment/communication/transport) was the main barrier (46.36%) reported by WWD. Most of the hospitals lacked any special provisions for people with disability (PWD). Proper ramps, stairs, toilets, etc. were not present in hospitals as denoted by their verbatim responses. All the hospitals scored low on access to sexual and reproductive health services for WWD.

ADMINISTRATION OF TETANUS TOXOID IMMUNIZATION TO PREGNANT MOTHERS DURING THE COVID-19 PANDEMIC

Background: The outbreak of the Covid-19 pandemic in Indonesia impacts women's reproductive health services, including pregnant women services that meet the standard, namely the administration of TT immunization. Officers at the Community Health Center (Puskesmas) are more focused on controlling the spread of COVID-19. The problem that arises due to pregnant women who are not administered Tetanus Toxoid during pregnancy is the occurrence of tetanus in the mother and baby. Objective: To determine the relationship between knowledge, attitudes, and support of MCH officers towards the administration of Tetanus Toxoid immunization during the COVID-19 pandemic at UPT PuskesmasPasundan, Garut Regency, for the period of June-July 2021.Method: Analytic observational with the cross-sectional approach. The study was conducted from June to July 2021. The population and research sample consisted of 50 people. A questionnaire was used as a research instrument.Analysis: Univariate to explain the research variables and bivariate analysis to explain the relationship between knowledge, attitudes of pregnant women, and the support of MCH officers in administering Tetanus Toxoid immunization during the covid-19 pandemic. Hypothesis testingused a statistical test, namely the Chi-Square test with SPSS 2013, and the significance level was 0.05. Results: Relationship between Knowledge and Administration of Tetanus Toxoid Immunization, p-value = 0.015 (p < 0.05). Relationship between Attitude and Administration of Tetanus Toxoid Immunization, p-value = 0.0001 (P <0.05). Relationship between support of MCH officers and Administration of Tetanus Toxoid immunization, p-value = 0.0001 (p < 0.05).Conclusion: There is a relationship between knowledge, attitudes, and support of MCH officerstowards the administration of Tetanus Toxoid immunization. It illustrates that the presence of COVID-19 slightly interferes with the administration of TT immunization to pregnant women.Suggestion: It is important to make various efforts and ways to increase the coverage of TT immunization for pregnant women during the covid-19 pandemic.

Stigma and Reproductive Health Service Access Among Women in Treatment for Substance Use Disorder.

We aimed to examine the role of perceived and direct stigma on access to reproductive health services among women in treatment for substance use disorders in Michigan. We draw on self-interview data from a cross-sectional study of female patients aged 18-50years who accessed substance use disorder treatment at 22 randomly selected facilities in Michigan from December 2015 to May 2017. We conducted logistic regressions to examine associations between perceived stigma and access to three types of reproductive health services (screening exams, birth control, and prenatal care), as well as direct stigma and access to birth control. The final sample included 260 women. A notable percentage of women reported inability to access reproductive health services (24% for screening exams, 14% for birth control, and 12% for prenatal care). Women with higher levels of perceived stigma because of substance use were significantly more likely to report inability to access screening exams (odds ratio [OR]: 2.14; confidence interval [CI]: 1.43-3.20) and birth control (OR: 2.17; CI: 1.36-3.77). Women reporting higher levels of direct stigma were also significantly more likely to report inability to access birth control (OR: 3.87; CI: 2.29-6.53), even after accounting for perceived stigma. Perceived and direct stigma because of substance use may be significant barriers to seeking and accessing reproductive health services for women. Health professionals should reduce stigma in health care in order to increase access to necessary services for this population.

Contraceptive use among cisgender women with bacterial sexually transmitted infections: A cross-sectional study.

Data on contraceptive use among women with bacterial sexually transmitted infections (STIs) are sparse, despite this population's high risk for unplanned pregnancy. This cross-sectional study included 1623 cisgender women recently diagnosed with a bacterial STI who completed a public health Partner Services interview between January 2017 and December 2019 in King County, WA, USA. Contraceptive methods were categorized as: (1) highly or moderately effective and (2) least effective or no method. Poisson regression models were used to assess associations between individual characteristics and contraceptive method. Almost two thirds of the women (62.6%) reported using highly or moderately effective contraception, with 30.3% of women using long-acting contraception (LARC). More than one in three women (37.4%) reported using least effective methods or no method. Black women were less likely to report using a highly or moderately effective method compared to White women (aRR 0.58, 95% CI 0.43-0.80) and women with private insurance were more likely to report using a highly or moderately effective method compared to those with public insurance (aRR 1.67, 95% CI 1.28-2.19). Given that many women with bacterial STIs are not desiring pregnancy, this study highlights the need for additional reproductive health services for women with recent STI diagnoses.

Acesso à contracepção durante a pandemia de Covid-19: barreiras e perspectivas

Abstract Background The SARS-CoV-2 pandemic, which started in December 2019, was declared by the World Health Organization (WHO) as an international public health emergency in January 2020, with an impact on access to sexual and reproductive health services for women. Objective To discuss contraception in the pandemic context, based on current public policies and the world and Brazilian scenario. Method This is an opinion article, which describes the barriers and possible solutions for access to qualified sexual and reproductive health care for women. Results Relevant topics were explored, such as: the change in the logistics of services, the reduction in the number of consultations, the difficulty in importing contraceptives, the lack of trained professionals, and the bureaucratization of access to contraceptives methods. The use of telemedicine and the strengthening of Primary Health Care are considered strategies to guarantee access and change the reality of women. Conclusion In this context, it is necessary to maintain the actions of sexual and reproductive planning services to prevent injuries from unplanned pregnancies and increase maternal morbidity and mortality.

The Meaning of Menstruation in Women with Mild Disabilities

Women with Disabilities have a greater social vulnerability both as Women and Persons with Disabilities. The stigma formed by the community greatly influences them in interpreting their menstruation. This study aims to explore the meaning of menstruation for women with mild disabilities. This research was conducted qualitatively using Alfred Schutz's Phenomenological Approach and supported by Symbolic Interactionism Theory and Radical Feminist Theory. The results of the study found the concept of menstruation, the concept of personal hygiene during menstruation, and gender relations in menstruation. The implications of this research are 1) Implementation of health policies related to reproductive health services for women with disabilities; 2) Menstruation socialization and counseling for Women with Disabilities and their families; 3) Capacity building of Social Welfare Institutions related to Gender and Reproductive Health; 4) Development of skills training for making cloth pads for women with disabilities and their families.

Reproductive Health Services for Women During COVID-19 Pandemic

Millions of women across the world risk losing access to essential reproductive health services during the prevalent COVID-19 pandemic. The global and national level healthcare contexts are rapidly changing, and strongly influencing the sexual and reproductive health (SRH) and rights of adolescent girls and women [1]. Sustainable SRH services are of paramount importance in this current pandemic scenario. The SRH rights include an individual’s overall well-being for the SRH and reproductive body and considered imperative for the personal, social and economic growth [2]. The Sustainable Development Goals (SDGs) 3 and 5 are concerned with SRH rights pertaining to gender equality, women empowerment, accessible family planning services and contraception use [3]. The deep-rooted, gender-based inequalities for females in accessing SRH care have come to the forefront during the COVID-19 pandemic. Disrupted SRH services, gender inequality, and gender-based violence threatens to become a pandemic within the existing pandemic according to the United Nations Population Fund (UNFPA) [1,4].

Providing reproductive health services for women who inject drugs: a pilot program

BackgroundNeedle syringe programs (NSPs), a proven harm reduction strategy for people who inject drugs, frequently offer limited healthcare services for their clients. Women who inject drugs face multiple barriers to accessing reproductive health care in traditional settings: personal histories of trauma, judgmental treatment from providers, and competing demands on their time. Our aim was to implement patient-centered reproductive healthcare services at a Seattle NSP.MethodsWe interviewed clients and staff of an NSP in Seattle and staff of other community-based organizations serving women who inject drugs, then used the Consolidated Framework for Implementation Research to code transcripts deductively. Based on our qualitative work, we implemented reproductive health care at the NSP program 1 day per week. We evaluated the implementation by surveying staff and clients and auditing charts over a 9-month period.ResultsClients and staff (N = 15 for clients, N = 13 for staff) noted a high unmet need for trauma-informed, accessible reproductive health care. We successfully implemented reproductive health care services including short- and long-acting contraception, sexually transmitted disease testing, and cervical cancer screening. Survey data was limited but demonstrated client satisfaction with services.ConclusionsIntegrating reproductive health care into an NSP’s clinical services is feasible and can be a source of low-barrier preventive care for women unable to seek gynecologic care elsewhere.

Developing integrated community-based HIV prevention, harm reduction, and sexual and reproductive health services for women who inject drugs

BackgroundDespite being a priority population for HIV prevention and harm reduction programs, the sexual and reproductive health (SRH) needs of women who inject drugs are being overlooked. Furthermore, models for providing integrated SRH, HIV, and harm reduction services for women who inject drugs are rare. This article reports the development of community-based outreach services that integrated family planning and other SRH interventions with HIV and harm reduction services for this population in coastal Kenya.MethodsUsing mixed-methods implementation research, a qualitative baseline needs assessment was conducted with women who inject drugs and harm reduction stakeholders using a combination of in-depth interviews and focus group discussions. The qualitative data from participants was subjected to thematic analysis using Nvivo. Based on the baseline needs assessment, integration of SRH into existing HIV and harm reduction services was implemented. After two years of implementation, an evaluation of the program was conducted using a combination of qualitative interviews and review of quantitative service delivery records and other program documents. The process, impacts, and challenges of integrating SRH into a community-based HIV prevention and harm reduction program were identified.ResultsThis article highlights: 1) low baseline utilization of family planning services among women who inject drugs, 2) improved utilization and high acceptability of outreach-based provision of SRH services including contraception among this population, 3) importance of training, capacity strengthening, technical support and financial resourcing of community-based organizations to integrate SRH into HIV prevention and harm reduction services, and 4) the value of beneficiary involvement, advocacy, and collaboration with other partners in the planning, designing and implementing of SRH interventions for women who inject drugs.ConclusionsWomen who inject drugs in this study had low utilization of family planning and other SRH services, which can be improved through the integration of contraceptive and other SRH interventions into existing outreach-based HIV prevention and harm reduction programs. This integration is acceptable to women who inject drugs, and is programmatically feasible. For successful integration, a rights-based beneficiary involvement, coupled with sustainable technical and financial capacity strengthening at the community level is essential.

Access to sexual and reproductive health services for women living with HIV in Latin America and the Caribbean: systematic review of the literature.

IntroductionSystematic reviews show that women living with HIV (WLHIV) have high unmet sexual and reproductive health (SRH) needs due to barriers to access sexual and reproductive health services (SRHS). In Latin America and the Caribbean (LAC), as of 2016, there were nearly one million WLHIV, but the existing evidence of their SRH needs comes from a few individual studies. This systematic review provides an overview of these women's needs to help define new and/or adapt existing public health strategies to the local context. This review synthesizes the evidence from the literature on the use of and access to SRHS related to family planning, antenatal care, abortion services and violence against WLHIV in LAC.MethodsUsing a systematic review of mixed studies, a search was performed in MEDLINE, EMBASE, LILACS, INASP, POPLINE, SCOPUS, for studies conducted in LAC, from 2004 to 2017, as well as contact with authors and hand search as needed. Two independent reviewers evaluated the quality of the studies using the Mixed Methods Appraisal Tool; inclusion was conducted according to the PRISMA flow diagram. An exploratory narrative synthesis followed by quantitative synthesis data was undertaken. Group analysis or meta‐analysis was not considered appropriate given the level of heterogeneity of the studies.ResultsA total of 18 studies in 13 LAC countries for a population of 5672 WLHIV were included. Data from individual studies reported unmet family planning needs; higher, but inconsistent use of condom as the sole contraceptive method OR=1.46 [1.26 to 1.69]; lesser use of other non‐permanent contraceptive methods OR=0.26 [0.22 to 0.31]; more unplanned pregnancies OR=1.30 [1.02 to 1.66]; more induced abortions OR=1.96 [1.60 to 2.39]; higher risk of immediate postpartum sterilization; and higher exposure to sexual and institutional violence by WLHIV when compared with women without HIV.ConclusionsThis review presents evidence from LAC about the SRH unmet needs of WLHIV that must be addressed by decreasing institutional and structural barriers, facilitating services and reducing stigma, and discrimination among healthcare providers to improve access to SRHS based on human rights, so women independently of their HIV status can make their own reproductive decisions, free of violence and coercion.

Joyce Banda and Ellen Johnson Sirleaf – confronting the neglect of maternal health and women’s rights in Malawi and Liberia

abstractSub-Saharan Africa accounts for the majority of maternal deaths in the world. The death of girls and women during childbirth as a result of pregnancy-related complications, or maternal deaths is termed unwanted or avoidable deaths due to the fact that we have the medical expertise to deal with them. A major factor that pushes up maternal deaths is the absence of or the poor state of sexual and reproductive health rights and services (SRHRS). Gender inequality, women’s lower social, political and economic status are recognised as contributing significantly to the neglect of universal access to and provision of reproductive health services for women. Africa as a continent has identified maternal deaths as a health policy priority. Reversing the trend has been dependent on a number of variables, not least country level attention to women’s health. In this article I place the spotlight on two examples of leaders who as women used the positional power of office as head of state to address the neglect of rural women’s health and access to services. Presidents Joyce Banda and Ellen Johnson Sirleaf introduced measures and committed government resources to maternal health services delivery to bring down the high number of maternal deaths of women in their respective countries, Malawi and Liberia. These measures offered women maternal care and a safe environment in which to give birth. The different initiatives taken by two presidential leaders to reduce maternal mortality are profiled against the growing pressure on Africa’s nations to take action to provide universal access to sexual and reproductive health services to women and girls.

Predictors of unplanned pregnancies among female students at South African Technical and Vocational Education and Training colleges: Findings from the 2014 Higher Education and Training HIV and AIDS survey.

Unplanned pregnancies among college/tertiary female students pose a serious public health concern in South Africa (SA) and are associated with adverse health and social outcomes that impact negatively on educational progress and future career prospects. To examine the potential predictors of unplanned pregnancy among female students at Technical and Vocational Education and Training (TVET) colleges in SA. This analysis used data drawn from the 2014 Higher Education and Training HIV and AIDS survey, which was a nationally representative survey of TVET college students in SA. Associations between unplanned pregnancy and the explanatory variables were assessed using bivariate analysis. Multivariate logistic regression analysis was used to identify the effect of several independent predictors of unplanned pregnancy. Of 1 002 female students who responded to the question on unplanned pregnancy, 74.6% reported having had an unplanned pregnancy. Predictors significantly associated with a reduced likelihood of unplanned pregnancy among female TVET students included living with a husband (odds ratio (OR) 0.28, 95% confidence interval (CI) 0.13 - 0.62; p=0.002), having two (OR 0.45, 95% CI 0.23 - 0.88; p=0.003) or three (OR 0.07, 95% CI 0.01 - 0.39; p=0.003) previous pregnancies, and not having had an abortion (OR 0.16, 95% CI 0.04 - 0.62; p=0.008). The high level of unplanned pregnancies is indicative of the state of women's reproductive health services at SA TVET colleges. The findings suggest that certain groups of female students are at increased risk of unplanned pregnancy and would benefit from targeted family planning interventions tailored to their needs.

To review the current evidence regarding the impact of the Zika virus epidemic on pregnancy and the implications for family planning and reproductive health services for women in affected countries. In the past 2 years, over the course of the Zika epidemic, much has been learned about the virus. Specifically in regard to pregnancy, a causative relationship has been established between the Zika virus and adverse fetal outcomes, and sexual transmission of the virus has been documented. Broad recommendations by government agencies and public health officials regarding avoidance of pregnancy for women at risk of acquiring the Zika virus have theoretical and documented implications for abortion and contraception demand and access. Additional research is needed to better understand the impact of Zika virus health recommendations and advisories on women's decision making around pregnancy, as well as their impact on demand, access and safety of family planning services.

Sexual and reproductive health services for women with disability: a qualitative study with service providers in the Philippines.

BackgroundThe Philippines has ratified the United Nations Convention on the Rights of Persons with Disabilities and recently passed domestic legislation protecting the sexual and reproductive rights of people with disability. However women in the Philippines continue to report barriers to sexual and reproductive health services, and there is limited empirical evidence available to inform policy makers’ efforts to respond. This study aims to contribute to the available evidence by examining service providers’ perceptions of disability and their experiences providing sexual and reproductive health services to women with disability.MethodsThe study was conducted as part of a larger three-year program of participatory action research that aims to improve the sexual and reproductive health of women with disabilities in the Philippines. Fourteen in-depth interviews and two focus group discussions were conducted with a total of thirty-two sexual and reproductive health service providers in Quezon City and Ligao. Qualitative data were analysed to identify key themes in participants’ discussion of service provision to women with disability.ResultsAnalysis of service providers’ accounts suggests a range of factors undermine provision of high quality sexual and reproductive health services to women with disability. Service providers often have limited awareness of the sexual and reproductive health needs of women with disability and inadequate understanding of their rights. Service providers have had very little training in relation to disability, and limited access to the resources that would enable them to provide a disability inclusive service. Some service providers hold prejudiced attitudes towards women with disability seeking sexual and reproductive health services, resulting in disability-based discrimination. Service providers are also often unaware of specific factors undermining the health of women with disability, such as violence and abuse.ConclusionRecent legislative change in the Philippines opens a window of opportunity to strengthen sexual and reproductive health service provision across the country. However the development of services that are disability-inclusive will require substantial efforts to address supply-side barriers such as prejudiced service provider attitudes and limited capacity. Disability inclusion must be prioritised for the national goal of responsible parenthood and reproductive health to be realised for all.

This study provides a recommendation for both central and local governments (especially in East Java) to accomodate reproductive health services for women with disabilities. Since The Convention on the rights of persons with disabilities through the Law no.19 in 2011, the government have not taken any significant action. Therefore, it is necessary to establish a more implementative model policy for the community. The purpose of this research acts as a reference to make an effective, efficient, and right-on-target reproductive health policy. The recommendation made in this study is expected to be a reference for various parties, including the government, in order to design a more concrete and proactive steps to overcome reproductive health issue for women with disabilities in Indonesia, and also to build strategies which unite communities with different backgrounds. In order to obtain empirical data and information, this study used descriptive research with qualitative approach. Several steps taken are (1) determining the location of the study purposively in 33 sub-districts of Malang regency: in clinic, health, education, and social department, boarding school, Bhakti Luhur, P2TP2A, and local NGOs. Malang regency is chosen because (a) it is the only city which starts concerning sexuality and reproductive health of women with disabilities issues; (b) The sex ratio in 2010 was around 98,78%, which means that the number of females is significantly larger than males. As seen from the population density of 33 sub-districts of Malang, the majority of women has high density above 2000/km². It implies that poverty is mostly suffered by women; (c) the number of reproductive health issues for women with disabilities is quite a lot, because it is regarded as a taboo. (2) Data Collection: (1) observation and (2) indepth interview, (3) informants, (4) data analysis. The result of this study indicates that there are still a lot of women with disabilities who do not understand about taking care of their reproductive organs, and thus impacting their health. The control over their reproductive organs is still low, which makes many of them suffered from sexual violences that lead to unwanted pregnancies. Some cases found during the study show that women with disabilities who become sexual violence victims did not get proper justice, because their statements are not legally accepted, and it is also reinforced by the communities’ stigma which calls person with mental disabilities as a psycho.

It implies that poverty is mostly suffered by women; (c) the number of reproductive health issues for women with disabilities is quite a lot, because it is regarded as a taboo. The result of this study indicates that there are still a lot of women with disabilities who do not understand about taking care of their reproductive organs, and thus impacting their health. The control over their reproductive organs is still low, which makes many of them suffered from sexual violences that lead to unwanted pregnancies. Some cases found during the study show that women with disabilities who become sexual violence victims did not get proper justice, because their statements are not legally accepted, and it is also reinforced by the communities’ stigma which calls person with mental disabilities as a psycho.
