Photon-Counting Detector CT Virtual Monoenergetic Images for Coronary Artery Stenosis Quantification: Phantom and In Vivo Evaluation.

BACKGROUND. Calcium blooming causes stenosis overestimation on coronary CTA. OBJECTIVE. The purpose of this article was to evaluate the impact of virtual monoenergetic imaging (VMI) reconstruction level on coronary artery stenosis quantification using photon-counting detector (PCD) CT. METHODS. A phantom containing two custom-made vessels (representing 25% and 50% stenosis) underwent PCD CT acquisitions without and with simulated cardiac motion. A retrospective analysis was performed of 33 patients (seven women, 26 men; mean age, 71.3 ± 9.0 [SD] years; 64 coronary artery stenoses) who underwent coronary CTA by PCD CT followed by invasive coronary angiography (ICA). Scans were reconstructed at nine VMI energy levels (40-140 keV). Percentage diameter stenosis (PDS) was measured, and bias was determined from the ground-truth stenosis percentage in the phantom and ICA-derived quantitative coronary angiography measurements in patients. Extent of blooming artifact was measured in the phantom and in calcified and mixed plaques in patients. RESULTS. In the phantom, PDS decreased for 25% stenosis from 59.9% (40 keV) to 13.4% (140 keV) and for 50% stenosis from 81.6% (40 keV) to 42.3% (140 keV). PDS showed lowest bias for 25% stenosis at 90 keV (bias, 1.4%) and for 50% stenosis at 100 keV (bias, -0.4%). Blooming artifacts decreased for 25% stenosis from 61.5% (40 keV) to 35.4% (140 keV) and for 50% stenosis from 82.7% (40 keV) to 52.1% (140 keV). In patients, PDS for calcified plaque decreased from 70.8% (40 keV) to 57.3% (140 keV), for mixed plaque decreased from 69.8% (40 keV) to 56.3% (140 keV), and for noncalcified plaque was 46.6% at 40 keV and 54.6% at 140 keV. PDS showed lowest bias for calcified plaque at 100 keV (bias, 17.2%), for mixed plaque at 140 keV (bias, 5.0%), and for noncalcified plaque at 40 keV (bias, -0.5%). Blooming artifacts decreased for calcified plaque from 78.4% (40 keV) to 48.6% (140 keV) and for mixed plaque from 73.1% (40 keV) to 44.7% (140 keV). CONCLUSION. For calcified and mixed plaque, stenosis severity measurements and blooming artifacts decreased at increasing VMI reconstruction levels. CLINICAL IMPACT. PCD CT with VMI reconstruction helps overcome current limitations in stenosis quantification on coronary CTA.

Evaluation of ECG-Gated, High-Pitch Thoracoabdominal Angiographies With Dual-Source Photon-Counting Detector Computed Tomography.

The aim of this study was to evaluate the radiation dose, image quality, and the potential of virtual monoenergetic imaging (VMI) reconstructions of high-pitch computed tomography angiography (CTA) of the thoracoabdominal aorta on a dual-source photon-counting detector-CT (PCD-CT) in comparison with an energy-integrating detector-CT (EID-CT), with a special focus on low-contrast attenuation. Consecutive patients being referred for an electrocardiogram (ECG)-gated, high-pitch CTA of the thoracoabdominal aorta prior to transcatheter aortic valve replacement (TAVR), and examined on the PCD-CT, were included in this prospective single-center study. For comparison, a retrospective patient group with ECG-gated, high-pitch CTA examinations of the thoracoabdominal aorta on EID-CT with a comparable scan protocol was matched for gender, body mass index, height, and age. Virtual monoenergetic imaging reconstructions from 40 to 120 keV were performed. Enhancement and noise were measured in 7 vascular segments and the surrounding air as mean and standard deviation of CT values. The radiation dose was noted and signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated. Finally, a subgroup analysis was performed, comparing VMI reconstructions from 40 keV to 70 keV in patients with at least a 50% decrease in contrast attenuation between the ascending aorta and femoral arteries. Fifty patients (mean age 77.0±14.5 years; 31 women) were included. The radiation dose was significantly lower on the PCD-CT (4.2±1.4 vs. 7.2±2.2 mGy; p<0.001). With increasing keV, vascular noise, SNR, and CNR decreased. Intravascular attenuation was significantly higher on VMI at levels from 40 to 65, compared with levels of 120 keV (p<0.01 and p<0.005, respectively). On the PCD-CT, SNR was significantly higher in keV levels 40 and 70 (all p<0.001), and CNR was higher at keV levels 40 and 45 (each p<0.001), compared with scans on the EID-CT. At VMI ≤60 keV, image noise was also significantly higher than that in the control group. The subgroup analysis showed a drastically improved diagnostic performance of the low-keV images in patients with low-contrast attenuation. The ECG-gated CTA of the thoracoabdominal aorta in high-pitch mode on PCD-CT have significantly lower radiation dose and higher objective image quality than EID-CT. In addition, low-keV VMI can salvage suboptimal contrast studies, further reducing radiation dose by eliminating the need for repeat scans. ECG-gated CT-angiographies of the thoracoabdominal aorta can be acquired with a lower radtiation dose and a better image quality by using a dual-source photon-countinge detector CT. Furthermore, the inherent spectral data offers the possiblity to improve undiagnostic images and thus saves the patient from further radiation and contrast application.

Ultra-high resolution photon-counting CT with tin prefiltration for bone-metal interface visualization

PurposeTo investigate the metal artifact suppression potential of combining tin prefiltration and virtual monoenergetic imaging (VMI) for osseous microarchitecture depiction in ultra-high-resolution (UHR) photon-counting CT (PCCT) of the lower extremity. MethodDerived from tin-filtered UHR scans at 140 kVp, polychromatic datasets (T3D) and VMI reconstructions at 70, 110, 150, and 190 keV were compared in 117 patients with lower extremity metal implants (53 female; 62.1 ± 18.0 years). Three implant groups were investigated (total arthroplasty [n = 48], osteosynthetic material [n = 43], and external fixation [n = 26]). Image quality was assessed with regions of interest placed in the most pronounced artifacts and adjacent soft tissue, measuring the respective attenuation. Additionally, artifact extent, bone-metal interface interpretability and overall image quality were independently evaluated by three radiologists. ResultsArtifact reduction was superior with increasing keV level of VMI. While T3D was superior to VMI70keV (p ≥ 0.117), artifacts were more severe in T3D than in VMI ≥ 110 keV (all p ≤ 0.036). Image noise was highest for VMI70keV (all p < 0.001) and lowest for VMI110keV with comparable results for VMI110keV – VMI190keV. Subjective image quality regarding artifacts was superior for VMI ≥ 110 keV (all p ≤ 0.042) and comparable for VMI110keV – VMI190keV. Bone-metal interface interpretability was superior for VMI110keV (all p ≤ 0.001), while T3D, VMI150keV and VMI190keV were comparable. Overall image quality was deemed best for VMI110keV and VMI150keV. Interreader reliability was good in all cases (ICC ≥ 0.833). ConclusionsTin-filtered UHR-PCCT scans of the lower extremity combined with VMI reconstructions allow for efficient artifact reduction in the vicinity of bone-metal interfaces.

Reduction of contrast medium for transcatheter aortic valve replacement planning using a spectral detector CT: a prospective clinical trial

Abstract Introduction This study investigated the use of dual-energy spectral detector computed tomography (CT) and virtual monoenergetic imaging (VMI) reconstructions in pre-interventional transcatheter aortic valve replacement (TAVR) planning. We aimed to determine the minimum required contrast medium (CM) amount to maintain diagnostic CT imaging quality for TAVR planning. Methods In this prospective clinical trial, TAVR candidates received a standardized dual-layer spectral detector CT protocol. The CM amount (Iohexol 350 mg iodine/mL, standardized flow rate 3 mL/s) was reduced systematically after 15 patients by 10 mL, starting at 60 mL (institutional standard). We evaluated standard, and 40- and 60-keV VMI reconstructions. For image quality, we measured signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and diameters in multiple vessel sections (i.e., aortic annulus: diameter, perimeter, area; aorta/arteries: minimal diameter). Mixed regression models (MRM), including interaction terms and clinical characteristics, were used for comparison. Results Sixty consecutive patients (mean age, 79.4 ± 7.5 years; 28 females, 46.7%) were included. In pre-TAVR CT, the CM reduction to 40 mL is possible without affecting the image quality (MRM: SNR: –1.1, p = 0.726; CNR: 0.0, p = 0.999). VMI 40-keV reconstructions showed better results than standard reconstructions with significantly higher SNR (+ 6.04, p &lt; 0.001). Reduction to 30 mL CM resulted in a significant loss of quality (MRM: SNR: –12.9, p &lt; 0.001; CNR: –13.9, p &lt; 0.001), regardless of the reconstruction. Across the reconstructions, we observed no differences in the metric evaluation (p &gt; 0.914). Conclusion Among TAVR candidates undergoing pre-interventional CT at a dual-layer spectral detector system, applying 40 mL CM is sufficient to maintain diagnostic image quality. VMI 40-keV reconstructions improve the vessel attenuation and are recommended for evaluation. Clinical relevance statement Contrast medium reduction to 40 mL in pre-interventional transcatheter aortic valve replacement CT using dual-energy CT maintains image quality, while 40-keV virtual monoenergetic imaging reconstructions enhance vessel attenuation. These results offer valuable recommendations for interventional transcatheter aortic valve replacement evaluation and potentially improve nephroprotection in patients with compromised renal function. Improved Thermal Sensitivity Using Virtual Monochromatic Imaging Derived from Photon Counting Detector CT Data Sets: Ex Vivo Results of CT-Guided Cryoablation in Porcine Liver

PurposeTo investigate differences in thermal sensitivity of virtual monoenergetic imaging (VMI) series generated from photon-counting detector (PCD) CT data sets, regarding their use to improve discrimination of the ablation zone during percutaneous cryoablation.Materials and MethodsCT-guided cryoablation was performed using an ex vivo model of porcine liver on a PCD-CT system. The ablation zone was imaged continuously for 8 min by acquiring a CT scan every 5 s. Tissue temperature was measured using fiberoptic temperature probes placed parallel to the cryoprobe. CT-values and noise were measured at the tip of the temperature probes on each scan and on VMI series from 40 to 130 keV. Correlation of CT-values and temperature was assessed using linear regression analyses.ResultsFor the whole temperature range of [− 40, + 20] °C, we observed a linear correlation between CT-values and temperature in reference 70 keV images (R2 = 0.60, p < 0.001) with a thermal sensitivity of 1.4HU/°C. For the most dynamic range of [− 15, + 20] °C, the sensitivity increased to 2.4HU/°C (R2 = 0.50, p < 0.001). Using VMI reconstructions, the thermal sensitivity increased from 1.4 HU/°C at 70 keV to 1.5, 1.7 and 2.0HU/°C at 60, 50 and 40 keV, respectively (range [− 40, + 20] °C). For [− 15, + 20]°C, the thermal sensitivity increased from 2.4HU/°C at 70 keV to 2.5, 2.6 and 2.7HU/°C at 60, 50 and 40 keV, respectively. Both CT-values and noise also increased with decreasing VMI keV-levels.ConclusionDuring CT-guided cryoablation of porcine liver, low-keV VMI reconstructions derived from PCD-CT data sets exhibit improved thermal sensitivity being highest between + 20 and − 15 °C.Graphical

Photon Counting Detector CT-Based Virtual Noniodine Reconstruction Algorithm for In Vitro and In Vivo Coronary Artery Calcium Scoring: Impact of Virtual Monoenergetic and Quantum Iterative Reconstructions.

The aim of this study was to evaluate the impact of virtual monoenergetic imaging (VMI) and quantum iterative reconstruction (QIR) on the accuracy of coronary artery calcium scoring (CACS) using a virtual noniodine (VNI) reconstruction algorithm on a first-generation, clinical, photon counting detector computed tomography system. Coronary artery calcium scoring was evaluated in an anthropomorphic chest phantom simulating 3 different patient sizes by using 2 extension rings (small: 300 × 200 mm, medium: 350 × 250 mm, large: 400 × 300 mm) and in patients (n = 61; final analyses only in patients with coronary calcifications [n = 34; 65.4 ± 10.0 years; 73.5% male]), who underwent nonenhanced and contrast-enhanced, electrocardiogram-gated, cardiac computed tomography on a photon counting detector system. Phantom and patient data were reconstructed using a VNI reconstruction algorithm at different VMI (55-80 keV) and QIR (strength 1-4) levels (CACS VNI ). True noncontrast (TNC) scans at 70 keV and QIR "off" were used as reference for phantom and patient studies (CACS TNC ). In vitro and in vivo CACS VNI showed strong correlation ( r > 0.9, P < 0.001 for all) and excellent agreement (intraclass correlation coefficient > 0.9 for all) with CACS TNC at all investigated VMI and QIR levels. Phantom and patient CACS VNI significantly increased with decreasing keV levels (in vitro: from 475.2 ± 26.3 at 80 keV up to 652.5 ± 42.2 at 55 keV; in vivo: from 142.5 [7.4/737.7] at 80 keV up to 248.1 [31.2/1144] at 55 keV; P < 0.001 for all), resulting in an overestimation of CACS VNI at 55 keV compared with CACS TNC at 70 keV in some cases (in vitro: 625.8 ± 24.4; in vivo: 225.4 [35.1/959.7]). In vitro CACS increased with rising QIR at low keV. In vivo scores were significantly higher at QIR 1 compared with QIR 4 only at 60 and 80 keV (60 keV: 220.3 [29.6-1060] vs 219.5 [23.7/1048]; 80 keV: 152.0 [12.0/735.6] vs 142.5 [7.4/737.7]; P < 0.001). CACS VNI was closest to CACS TNC at 60 keV, QIR 2 (+0.1%) in the small; 55 keV, QIR 1 (±0%) in the medium; 55 keV, QIR 4 (-0.1%) in the large phantom; and at 60 keV, QIR 1 (-2.3%) in patients. Virtual monoenergetic imaging reconstructions have a significant impact on CACS VNI . The effects of different QIR levels are less consistent and seem to depend on several individual conditions, which should be further investigated.

Improving visualization of free fibula flap perforators and reducing radiation dose in dual-energy CT angiography.

The precise assessment of the perforators of the fibula free flap (FFF) is crucial for minimizing procedure-related complications when harvesting the FFF in patients with maxillofacial lesions. This study aims to investigate the utility of virtual noncontrast (VNC) images for radiation dose saving and to determine the optimal energy level of virtual monoenergetic imaging (VMI) reconstructions in dual-energy computed tomography (DECT) for visualization of the perforators of the fibula free flap (FFF). Data from 40 patients with maxillofacial lesions who received lower extremity DECT examinations in the noncontrast and arterial phase were collected in this retrospective, cross-sectional study. To compare VNC images from the arterial phase with true non-contrast images in a DECT protocol (M_0.5-TNC) and to compare VMI images with 0.5 linear images blending from the arterial phase (M_0.5-C), the attenuation, noise, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and subjective image quality were assessed in different arteries, muscles, and fat tissues. Two readers evaluated the image quality and visualization of the perforators. The dose-length product (DLP) and CT volume dose index (CTDIvol) were used to determine the radiation dose. Objective and subjective analyses showed no significant difference between the M_0.5-TNC and VNC images in the arteries and muscles (P>0.09 to P>0.99), and VNC imaging could reduce 50% of the radiation dose (P<0.001). Compared with those of the M_0.5-C images, the attenuation and CNR of VMI reconstructions at 40 kiloelectron volt (keV) and 60 keV were higher (P<0.001 to P=0.04). Noise was similar at 60 keV (all P>0.99) and increased at 40 keV (all P<0.001), and the SNR in arteries was increased at 60 keV (P<0.001 to P=0.02) in VMI reconstructions compared with those in the M_0.5-C images. The subjective scores in VMI reconstructions at 40 and 60 keV was higher than those in M_0.5-C images (all P<0.001). The image quality at 60 keV was superior to that at 40 keV (P<0.001), and there was no difference in the visualization of the perforators between 40 and 60 keV (P=0.31). VNC imaging is a reliable technique for replacing M_0.5-TNC and provides radiation dose saving. The image quality of the 40-keV and 60-keV VMI reconstructions was higher than that of the M_0.5-C images, and 60 keV provided the best assessment of perforators in the tibia.

Evaluation of run-off computed tomography angiography on a first-generation photon-counting detector CT scanner – Comparison with low-kVp energy-integrating CT

PurposeTo assess the overall imaging performance (radiation dose and image quality) of a photon-counting detector CT (PCD-CT) in comparison with a state-of-the-art energy-integrating detector CT (EID-CT) in run-off CTAs. MethodsConsecutive patients who underwent run-off CTA on a PCD-CT were included (PCD-CT cohort). A retrospective cohort of patients who had undergone run-off CTA on an EID-CT was matched for gender, body mass index, height, and age (EID-CT cohort). Virtual monoenergetic imaging (VMI) reconstructions for various keV settings (40–120 keV) were generated. CT values and noise were semiautomatically measured for 13 vascular segments of the abdomen, pelvis, and lower extremities. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated for each segment. Subjective image quality was evaluated by two radiologists along the dimensions ‘vessel attenuation’, ‘vessel sharpness’, and ‘overall image quality’ using 5-point Likert scales. ResultsForty patients (age 70.9 ± 9.8 years; 14 women) were included in the PCD-CT cohort and matched with a corresponding number of EID-CT patients. Overall, there was an inverse correlation of signal and noise but also of SNR and CNR with keV levels used for VMI reconstructions. SNR and CNR in the 40 – 60 keV range exceeded EID-CT levels significantly. Subjective image quality was substantially higher at lower keV levels and showed no significant difference to EID-CT. ConclusionLow keV VMI reconstructions of run-off CTA scans on a PCD-CT result in substantially higher SNR and CNR than 80 kVp and 100 kVp EID-CT acquisitions with equal subjective image quality.

Carotid and cerebrovascular dual-energy computed tomography angiography: Optimization of window settings for virtual monoenergetic imaging reconstruction

PurposeDedicated post-processing of dual-energy computed tomography angiography (DE-CTA) datasets has been shown to allow for increased vascular contrast. The goal of our study was to define optimal window settings for displaying virtual monoenergetic images (VMI) reconstructed from dual-energy carotid and cerebrovascular DE-CTA. MethodsFifty-seven patients who underwent clinically-indicated carotid and cerebrovascular third-generation dual-source DE-CTA were retrospectively evaluated. Standard linearly-blended (M_0.6), 70-keV traditional VMI (M70), and 40-keV noise-optimized VMI (M40+) reconstructions were analyzed. For M70 and M40+ datasets, the subjectively best window setting (width and level, B–W/L) was independently determined by two observers and subsequently related with aortic arch attenuation to calculate optimized values (O–W/L) using linear regression. Subjective evaluation of image quality (IQ) between W/L settings were assessed by two additional readers. Repeated measures analysis of variance were performed to compare W/L settings and IQ indices between M_0.6, M70, and M40 + . ResultsB–W/L and O–W/L for M70 were 580/210 and 560/200, and for M40+ were 1630/570 and 1560/550, respectively, higher than standard DE-CTA W/L settings (450/100). Highest subjective scores were observed for M40+ regarding overall IQ (all p < 0.001). ConclusionApplication of O–W/L settings is mandatory to optimize subjective IQ of VMI reconstructions of DE-CTA.

Can Dual-energy CT-based Virtual Monoenergetic Imaging Improve the Assessment of Hypodense Liver Metastases in Patients With Hepatic Steatosis?

To evaluate the impact of noise-optimized virtual monoenergetic imaging (VMI) on lesion demarcation and measuring accuracy of hypoattenuating liver metastases in patients with fatty liver disease compared to standard reconstructions. Twenty-eight patients (mean age 62.2 ± 7.7 years) with fatty liver disease and hypoattenuating liver metastases who underwent unenhanced and contrast-enhanced portal-venous dual-energy CT (DECT) were enrolled. Standard linearly blended and VMI series were reconstructed in 10-keV intervals. Lesion-to-parenchyma contrast-to-noise ratio (CNR) was calculated and the best VMI series was further investigated in a subjective evaluation of overall image quality and lesion demarcation. Size measurements were performed independently by measuring all hypodense lesions (n = 58) twice in a predefined sequence. Inter- and intra-rater agreement was assessed using intra-class correlation coefficient (ICC) statistics. The calculated CNR was greatest at 40-keV VMI (4.3 ± 2.6), significantly higher compared to standard reconstructions (2.9 ± 1.9; p < 0.001). Subjective ratings for overall image quality showed no significant difference between the 2 reconstruction techniques (both medians 4; p = 0.147), while lesion margin demarcation was found to be superior for 40-keV VMI (median 5; p ≤ 0.001). Inter- (ICC, 0.98 for 40-keV VMI; ICC, 0.93 for standard reconstruction) and intra-rater (ICC, 0.99 for 40-keV VMI; ICC, 0.94 for standard image series) analysis showed an excellent agreement for lesion measurements in both reconstruction techniques. Noise-optimized VMI reconstructions significantly improve contrast and lesion demarcation of hypoattenuating liver metastases in patients with the fatty liver disease compared to standard reconstruction.

Low-keV virtual monoenergetic imaging reconstructions of excretory phase spectral dual-energy CT in patients with urothelial carcinoma: A feasibility study

ObjectivesTo compare objective and subjective image quality between low keV virtual monoenergetic images (VMI) of the excretory phase and conventional venous phase images derived from spectral dual-energy CT (DECT) in the assessment of urothelial carcinoma. Methods26 consecutive patients with histologically confirmed urothelial carcinoma who received clinically indicated venous- and excretory phase abdominal CT scans were included retrospectively. Attenuation, image noise as well as signal- and contrast-to-noise-ratio (SNR, CNR) in venous and excretory phase CT and excretory phase VMI from 40 to 70 keV were obtained from ROI-based measurements in the following regions: urothelial carcinoma, liver, pancreas, renal cortex, subcutaneous fat, renal vein/artery, portal vein, urinary bladder wall, lymph nodes, prostate/uterus. Subjective vessel contrast and delineation of primary tumor manifestations and distant metastases were rated on 5-point Likert scales. ResultsIn comparison to venous phase CT, attenuation and SNR in excretory phase VMI40keV were higher (p < 0.001), except for liver parenchyma, where they were comparable (p = 0.07 and p = 0.17, respectively). Regarding image noise, no significant difference was found between venous phase CT and excretory phase VMI40keV (p-range: 0.08–1.00), except for liver, portal vein and renal artery, where it was lower in VMI40keV (p < 0.05). CNR of urothelial carcinoma to circumjacent bladder wall was significantly higher in excretory phase VMI40keV compared to venous phase CT. Subjective vessel contrast and delineation of primary tumor and distant metastases received equivalent or higher Likert scores in excretory phase VMI40keV than in venous phase CT. ConclusionThis feasibility study indicates that in the assessment of urothelial carcinoma, virtual monoenergetic excretory phase images at 40 keV acquired with spectral DECT could be feasible to maintain subjective and objective image quality as provided by conventional venous phase images. Still, equivalence with regards to metastatic lesion detection requires further investigation before employing this technique in a potential signal-scan, single-bolus approach.

Dual energy computed tomography virtual monoenergetic imaging: technique and clinical applications

Dual energy CT (DECT) has evolved into a commonly applied imaging technique in clinical routine due to its unique post-processing opportunities for improved evaluation of all body areas. Reconstruction of virtual monoenergetic imaging (VMI) series has shown beneficial effects for both non-contrast and contrast-enhanced DECT due to the flexibility to calculate low-keV VMI reconstructions to increase contrast and iodine attenuation, or to compute high-keV VMI reconstructions to reduce beam-hardening artefacts. The goal of this review article is to explain the technical background of VMI and noise-optimized VMI+ algorithms and to give an overview of useful clinical applications of the VMI technique in DECT of various body regions.

Virtual monoenergetic reconstruction of contrast-enhanced CT scans of the abdomen and pelvis at 40keV improves the detection of peritoneal metastatic deposits.

To evaluate the role of virtual monoenergeticimaging (VMI) in the detection of peritoneal metastatic disease in contrast-enhanced computed tomography (CT) of the abdomen and pelvis and to compare this technique to the conventional 120kV mixed dataset. Institutional review board approval was obtained with no informed consent required for this retrospective analysis. 43 consecutive patients with histopathologically confirmed peritoneal disease were scanned using a standard protocol on a 128-section dual-source, dual-energy CT system (100/140keV). Scans were retrospectively reconstructed at VMI energy levels from 40-110keV in 10keV increments and were analyzed both quantitatively and qualitatively. CNR values for peritoneal metastatic deposits were recorded using region of interest (ROI) analysis at each energy level for all VMI datasets. Subjective analysis was performed by two independent fellowship-trained readers with combined experience of greater than 15years. Qualitative parameters included diagnostic acceptability, subjective noise, and contrast resolution and confidence. The contrast-to-noise ratios (CNRs) for peritoneal metastatic deposits at the different VMI energy levels were compared using a one-way ANOVA with Tukey Post Test, and the optimal CNR was observed at 40keV (p < 0.0001). Qualitative parameters were compared using a Paired T Test. Subjective noise, diagnostic acceptability, and contrast resolution was significantly better on the conventional images, but readers reported increased confidence on VMI at 40keV (p < 0.001). VMI reconstruction of contrast-enhanced dual-energy CT scans of the abdomen and pelvis at 40keV maximizes the conspicuity of metastatic peritoneal deposits and improves radiologists' diagnostic confidence compared with conventional CT images. We recommend using virtual monoenergetic datasets at 40keV as a tool for improving the detection of these lesions in routine clinical practice.

Dual-Energy Computed Tomography Virtual Monoenergetic Imaging of Lung Cancer: Assessment of Optimal Energy Levels.

The aim of the study was to evaluate objective and subjective image qualities of virtual monoenergetic imaging (VMI) in dual-source dual-energy computed tomography (DECT) and optimal kiloelectron-volt (keV) levels for lung cancer. Fifty-nine lung cancer patients underwent chest DECT. Images were reconstructed as VMI series at energy levels of 40, 60, 80, and 100 keV and standard linear blending (M_0.3) for comparison. Objective and subjective image qualities were assessed. Lesion contrast peaked in 40-keV VMI reconstructions (2.5 ± 2.9) and 60 keV (1.9 ± 3.0), which was superior to M_0.3 (0.5 ± 2.7) for both comparisons (P < 0.001). Compared with M_0.3, subjective ratings were highest for 60-keV VMI series regarding general image quality (4.48 vs 4.52; P = 0.74) and increased for lesion demarcation (4.07 vs 4.84; P < 0.001), superior to all other VMI series (P < 0.001). Image sharpness was similar between both series. Image noise was rated superior in the 80-keV and M_0.3 series, followed by 60 keV. Virtual monoenergetic imaging reconstructions at 60-keV provided the best combination of subjective and objective image qualities in DECT of lung cancer.
