Schizophrenia is a serious mental disorder with clinical manifestations such as: delusions (false beliefs), hallucinations, loss of contact wit reality (psychoisis), risk of violent behavior, abnormal thoughts that can inhibit daily activities that give rise to emotions, perceptions, thoughts, movement and behavior (Pertiwi et al., 2023). The purpose of this paper is to determine and provide psychiatric nursing care to Mrs. T with the main problem of the risk of violent behavior due to paranoid schizophrenia in the Dewaruci ward of dr. Amino Gondohutomo Mental Hospital, Cental Java Province in accordance with nursing standards. The method used is a descriptive method in the form of interviews, observations, documentation studies and literature studies. From the case review, it was found that the patients main complaint said “I was angry, shouted and even punched the door twice, ma’am”. There are 3 nursing problems, namely the risk of violent behavior, self-care deficit. Interventions are arranged base on TUM and TUK as well as on the client’s condition and can be implemented.

Schizophrenia is a chronic mental disorder known for unclear thoughts, strange behavior and unreal sensory experiences that affect a person's thinking, feeling and acting and can make it difficult to distinguish between what is real and what is not. Patients with schizophrenia who experience delusions often feel angry and may be violent towards people they believe have hurt them. Uncontrolled anger in patients with schizophrenia will cause the risk of violent behavior (Auliati & Lubis, 2023; Wulandari et al., 2024).

Mindfulness-Based Asmaul Husna and changes in general adaptive function response among schizophrenia: A Quasi-experimental study

Background. Poor functional capacity is one of the factors that increase the risk of recurrence of positive and negative symptoms in schizophrenia. Mindfulness is a treatment potentially help patients become fully accepting of their conditions and conscious of it, allowing them to regulate unpleasant emotions and increase awareness of positive emotions. Methods. This study aimed to investigate effect of mindfulness-based Asmaul Husna on the overall adaptive functioning of individuals with schizophrenia. This quasi-experiment times series study involved 36 participants selected with simple random sampling. The inclusion criteria of participants were adult Muslim with schizophrenia who had a PANSS-EC score <10, mild symptoms, risk of violent behavior. Participants with severe symptoms and complications from other diseases were excluded. Mindfulness-based Asmaul Husna consisted of Musyahadah-witnessing, tassawur-imagination, tafakkur-contemplation, tadabbur-reflection, and muhasabah-self-introspection was given to each participant over five days. A modified-Global Assessment Functioning (m-GAF) scale used to measure participants' general adaptive functional responses before and after intervention including follow-up at the first and second month after the intervention. Results. There was a significant increase of the m-GAF score (p < 0.001) and a chi-square value of 177.2 after the implementation of mindfulness-based Asmaul Husna intervention. The highest mean score difference was observed at the first and second follow-ups, conducted one and two months after the interventions. The effect size calculated using Kendall’s Wa indicates a significant effect (0.821). Conclusion. The study suggests there is a positive effect of the mindfulness-based Asmaul Husna intervention on adaptive functioning of people with schizophrenia.

The risk of violent behavior is the result of anger or fear which is a response to feelings of threat, which will show several signs and symptoms such as sharp gaze, stiff body posture, red face and harsh words, so intervention is given using the media using snakes and ladders to improve the signs and symptoms. violence. Aiming to improve the signs and symptoms of violent behavior through modality therapy using the snakes and ladders game, the method used is a case study to explore nursing care to improve the signs and symptoms by observing and pre-test and post-test checklists on clients with violent behavior. The results of the implementation that had been carried out were a decrease in 14 signs and symptoms of violent behavior, in patient I there was a decrease in 4 out of 6 signs and symptoms, in patient II there was a decrease in 7 out of 10 signs and symptoms, in patient III there was a decrease in 3 out of 5 signs and symptoms. , patient IV experienced a decrease in 6 of 9 signs and symptoms, patient V experienced a decrease in 4 of 7 signs and symptoms and patient VI experienced a decrease in 4 of 9 signs and symptoms of violent behavior. So there is an influence on the application of the snakes and ladders game media to improve signs and symptoms in patients with violent behavior.

Dual harm: Violent behaviour to others and self-harm behaviour in adults compulsorily admitted to a Dutch psychiatric hospital

BackgroundVerbal and physical violence in psychiatric hospitals can have harmful consequences for staff members, such as physical injury, traumatisation, and sick leave, and they often accompany involuntary admission. Harm to others may co-occur with self-harm, i.e., dual harm. However, little is known about the association between dual-harm and violent behaviour towards staff members and its clinical outcomes, such as seclusion and rapid tranquilisation after involuntary admission to a psychiatric inpatient unit. MethodA convenience sample of patients admitted involuntarily (N = 384; mean age = 48.03, SD = 19.92) between January 2016 and December 2019 in Western Brabant, the Netherlands, was used to design a retrospective file audit. Distinct harm groups, marked by the presence/absence of self- and/or other-harm, were investigated using multivariate linear regression modelling on the seriousness of violent acts and the total length of admission. Logistic regression analyses were used to study the association between harm groups and the administration of rapid tranquilisation, seclusion, and extended involuntary admissions. ResultsSeveral harm groups were identified, including self-harm only, other-harm only, and dual-harm groups. Psychiatric patients admitted to the hospital because of (the risk of) violence towards others had a higher risk of violent incidents during admission and some restrictive measures. In a subgroup of patients with psychotic disorders, patients with dual harm committed the most serious violent incidents compared to those in the other harm groups. ConclusionDistinct harm groups were identified in a sample of involuntarily admitted patients. In a general adult psychiatric setting, patients at risk for violent behaviour, especially dual-harm patients, should be identified and monitored as part of the risk assessment. Future research is needed to explore more clinical correlates in the proposed distinction between harmful groups and to assess long-term prognosis.

HUBUNGAN TINGKAT PATERNAL POSTPARTUM DEPRESSION (PPD) TERHADAP RESIKO PERILAKU KEKERASAN PADA ANAK DI WILAYAH KERJA PUSKESMAS PULO LOR KABUPATEN JOMBANG

People often think that only mothers can experience postpartum depression. However, in fact, many fathers experience postpartum depression and it is not handled well because fathers are often silent and prioritize the welfare of the family. This results in suppressed emotions and the risk of violent behavior in children. This study aims to analyze the relationship between the level of paternal postpartum depression and the risk of violent behavior in children in the Pulo Lor Public Health Center working area, Jombang Regency. This research is a correlational analytical research using the cross sectional method. The total sampling technique was used in this research using 150 respondents with the characteristics of a father aged 15 - 30 years who had just had their first child or second child within a year of the couple's postpartum period. Data analysis in this study used the Spearman's rho statistical test, resulting in r = 0.010. The results of this study show that there is a relationship between the level of paternal postpartum depression and the risk of violent behavior in children in the work area of the Jombang district health center.

 The aim of this research is to analyze and design nursing care for two patients, who are at risk of violent behavior. Observations, interviews, and medical record documentation studies are data collection methods. Based on the research results, it is known that both patients have a high risk of violent behavior, especially related to a history of previous violence and underlying mental disorders. Initial assessment includes identification of potential triggers for violence, evaluation of level of coping, and assessment of social support. Risk of violent behavior is the patient's second nursing diagnosis. Treatment plans are implemented with a focus on violence prevention, involving collaboration with a multidisciplinary team, including medical personnel, and psychiatrists. Nursing interventions involve therapeutic approaches to improve coping, interpersonal skills training, and close monitoring for signs of violence. Periodic evaluations are conducted to assess the effectiveness of the treatment plan and identify changes in patient behavior. Discharge planning involves coordination with family and mental health services in the community to ensure continuity of care after discharge. The conclusion of this research is that nursing care for patients at risk of violent behavior requires a careful, proactive and collaborative approach. Implementing integrated violence prevention strategies and close monitoring can help reduce the risk of violent incidents.
 
 
 

A randomized, prospective, active-controlled study comparing intramuscular long-acting paliperidone palmitate versus oral antipsychotics in patients with schizophrenia at risk of violent behavior

ObjectivePaliperidone palmitate (PP) is an effective long-acting antipsychotic injection, but its impact on the behavior of schizophrenia patients with dangerous tendencies requires further investigation. This study aims to explore the effects of long-term antipsychotic treatment on this population in the community. MethodsThis 49-week, randomized controlled trial was conducted across 21 communities in Wuhan and enrolled 134 schizophrenia patients at risk for violent behavior. With a fixed block size of 10, participants were randomly assigned to receive either intramuscular PP 1-month formulation (PP1M) or oral antipsychotic medication (OAP) at a 1:1 ratio. Changes in patients' risk for violent/aggressive behavior, family burden, social, and cognitive functioning were measured using VRAPP, MOAS, PANSS, FBS, PSP, and RBANS scales from baseline to endpoint. Longitudinal data from multiple repeated measures were analyzed using linear mixed-effects models. ResultsThe study protocol was completed by 77.6% of the patients overall. Significant improvements were observed in the risk assessment scores, MOAS total score, PANSS total score, PSP total score, and FBS total score of patients in the PP1M group from baseline to the end of treatment (all P < 0.05). Importantly, compared to patients in the OAP group, the improvements in these measures were also significantly greater in the PP1M group. Commonly observed AEs, such as hyperprolactinemia (70.3% vs. 62.65%) and muscle tension (45.3% vs. 57.8%), were considered to be the PP-related AEs. Nonetheless, the differences between the two groups did not reach statistical significance, and no new safety concerns emerged. ConclusionsOur study suggests that PP long-acting injection (LAI) is a safe and effective treatment option for community-dwelling schizophrenia patients with impulsive violence and risky behaviors.

Improving Emotional Control And Activities of Daily Living (ADL) of Patients with Schizophrenia Using Spiritual Mindfulness Through SI-DEPAPU Application

Background: Schizophrenia is a major mental health problem in the world that requires more attention. Patients with schizophrenia are at higher risk of poor psychological well-being and violent behaviors as expression of anger and delusion. Mindfulness alone is considered less efficient so it needs to be developed with technology, as such further technological development regarding its application is needed.Objective: This study aimed to determine the effects of spiritual mindfulness intervention through the SI-DEPAPU application on the ability to control anger and perform activities of daily living (ADL) among schizophrenic patients.Methods: This study used a pretest-posttest quasi-experimental design with a control group. The samples were 60 schizophrenic patients randomly selected and divided into two groups. The intervention group was given four sessions of mindfulness in two weeks through the SI-DEPAPU application. This study used the anger management ability questionnaire developed by Sadipun et al. (2018) to measure the patient's ability to control anger and the North American Nursing Diagnosis Association (NANDA) questionnaire by Wilkinson (2016) to measure ADL. The instrument was tested for its validity among 20 schizophrenic patients with a risk of violent behaviors, and the results were valid and reliable in which the r-count r-table (0.444), the control group was given standard hospital care.Results: The results showed that the patient’s ability to control anger and perform ADL increased with p=0.000 and p=0.005, (p = 0,000 0,05) respectively. The intervention group had a higher mean score than the control group.Conclusion: Mindfulness spiritual interventions through the SI-DEPAPU application could help schizophrenic patients control anger and increase their ADL.

The Influence of De-Escalation Techniques on Reducing Anger Responses in Drug Clients

Drug abuse is a concern because of the various impacts, one of the negative effects psychologically including tension/anxiety, upset and depressed, loss of self-control and unstable emotional state. Risk Violent behavior is a form of angry response that is expressed by making threats, injuring other people and or destroying the environment. Indonesia's national data for 2020, the risk of violent behavior is around 0.8% or 10,000 people. One of the treatments for the risk of violent behavior can be done with verbal de-escalation techniques. Methods: The design used in this study was pre-experimental with a one group pretest posttest design approach. Total population and sample of 27 respondents (1 treatment group) using total sampling technique. In this design, the treatment group was given verbal de-escalation techniques. The treatment group was given an examination before (pre-test) and after (post-test) verbal de-escalation therapy using the PANSS-EC Rating Scale to assess the patient's angry response at the Mitra Mulia Foundation, Banyuasin Regency. Results: Implementation of verbal de-escalation techniques has an effect on reducing angry responses in clients at risk of violent behavior. Conclusion: Actions that can be taken on clients at risk of violent behavior can be in the form of verbal de-escalation techniques.

GAMBARAN MEKANISME KOPING DAN BURNOUT PADA ORANG TUA SISWA SEKOLAH DASAR DI MASA PANDEMI COVID-19

The change situation during the Covid-19 pandemic is a challenge for parents to raise their children. The role of parents during a pandemic can lead to burnout which causes emotional problems so there is a risk of violent behavior in children. Coping mechanisms are needed by parents to adapt difficult situations during a pressing pandemic. This study aimed to describe the coping mechanisms and burnout among parents of elementary school students during the Covid-19 pandemic. The design of this research was descriptive quantitative with a cross sectional approach. The sample size used were 261 parents of students from SDK ST. Maria Immaculata Tabanan and SD Negeri 1 Sembung Gede were selected used the proportional stratified random sampling. The results showed that the parents of elementary school students were in the late adult age category with a median value of 39 years old. Most of the parents in this study were female, college educated, married, had two child and were still working. The results of statistical tests showed that as many as 52,9% of parents used adaptive coping mechanisms and 47,1% of parents used maladaptive coping mechanisms. The results of the analysis of the burnout rate of parents are in the low category as much as 25,7%, the moderate burnout category as much as 49,4% and the high burnout category as much as 24,9%. It was concluded that most of the parents of elementary school students in this study used adaptive coping mechanisms and had moderate burnout levels during the Covid-19 pandemic.

Implementation of a Violence Risk Assessment and Interventions Aimed at the Prevention of Patient-Perpetrated Violent Events Across Care Settings.

Healthcare worker violence at the hands of patients is the leading cause of workplace injury with reports demonstrating yearly increases. Literature suggests organizational strategies to reduce workplace injury are necessary to create safe environments for patients to receive care and for healthcare workers to practice, but tactics lack a care continuum focus and largely focus on deescalation and management of violent events. One healthcare system identified that the use of an assessment widely used in Canada, identifying patients at risk for violent behaviors, would supplement already existing efforts at protecting patients. The implementation of this tool across care settings is presented in this article.

Management of Nursing Care for Patients at Risk of Violent Behavior with Implementation Strategies to Control Anger in the Mental Hospital

The risk of violent behavior is a situation where a person commits an action that can harm himself or herself or others or the environment, whether physically, emotionally, sexually or verbally, accompanied by uncontrolled rage and restless noise. The purpose of this paper is to describe the management of nursing care for patients at risk of violent behavior in mental hospitals. The method that will be used is the descriptive method by implementing nursing for 6 days in the form of nursing care for clients to meet the need for preventing the risk of violent behavior by providing the application of controlling emotions with SP 1 to 5. The data collection technique is carried out using a nursing methodology approach which consists of build relationships of mutual trust and carry out assessments which include physical examination, observation, supporting examinations, enforcement of nursing diagnoses, nursing interventions, nursing implementation and nursing evaluation. The results of nursing management showed that the problem of risk of violent behavior was resolved because the client was able to control anger by applying SP 1 to SP 5, namely by deep breathing relaxation, by hitting a pillow or mattress, verbally, by praying and by obediently taking medication. Try not to cause other problems due to the risk of violent behavior. Suggestions for nurses in hospitals to continue to teach and provide education on how to control anger to clients who are at riskof violent behaviour independently.

Risk of violent behaviour in young people at clinical high risk for psychosis from the North American Prodrome Longitudinal Studies consortium.

Although violent behaviour has been studied in schizophrenia, violence risk has received little attention in individuals at clinical high risk for psychosis (CHR). This manuscript aims to report and discuss the overall results of the Structured Assessment for Violence Risk in Youth (SAVRY) from the NAPLS-3 project to explore the risk of violence in CHR youth and to determine the relationship between SAVRY violence risk scores, psychosis risk symptoms, and global functioning. We hypothesized that CHR young people are at higher risk of violence as compared to healthy comparison participants due to a similarity between risk factors for psychosis and risk factors for violence, and that this risk is associated with greater severity of symptoms, poor functioning, and risk for conversion to psychosis. Participants from the North American Prodrome Longitudinal Study consortium phase 3 (NAPLS-3) included 684 CHR and 96 HC. Assessments included the Structural Assessment of Violence Risk in Youth (SAVRY), clinical and functional measures. The majority of participants across groups were deemed to be at low risk for violence. There were significantly more CHR participants (29.8%) who had moderate or high scores on the SAVRY Summary Risk Rating compared to HC participants (3.1%). Low versus moderate-high SAVRY scores were associated with better social (p < .005) and role (p < .002) functioning and fewer positive (p < .002), negative (p < .002), disorganized (p < .01) and general symptoms (p < .002). CHR participants with higher SAVRY scores were more likely to be diagnosed with borderline personality disorder, ADHD and substance misuse. Among CHR, overall violence risk was not associated with conversion to psychosis. However, those who converted to psychosis scored lower on the protective factors index, primarily driven by less prosocial involvement and fewer resilient personality traits. This is the first study to assess violence risk in CHR adolescents. Violence risk factors overlap with risk factors for psychosis in general, perhaps accounting for the association. These findings have implications for intervention efforts to reduce violence risk and bolster resiliency in CHR youth.

Background: Low self-esteem is a negative feeling experienced by an individual towards himself. The emergence of low self-esteem is the result of biological, psychological and social factors which are characterized by a lowered body posture, a feeling of not having positive strengths or abilities, no eye contact, speaking slowly. The impact of low self-esteem is that individuals will tend to be alone, avoid crowds, feel anxious, and feel anxious. Based on data from the yearbook report of dr. H. Marzoeki Mahdi in 2021, the number of patients with paranoid schizophrenia both outpatient and inpatient care is 1144 people with 815 men and 329 women. Purpose: The purpose of this writing is to gain real experience in caring for patients with low self-esteem. Method: This study uses a case study approach through the management of nursing care in patients with impaired self-concept: low self-esteem. Result: this research was conducted in the Bratasena room RSJ dr. H. Marzoeki Mahdi Bogor in January 2022 for three days. Data collection was carried out through observation and interview techniques. Based on the study, several nursing problems were found, namely low self-esteem, social isolation, risk of violent behavior. Conclusion: The expected evaluation from this case study is that patients can have a positive self-concept, an increase in the ability to increase self-esteem followed by a decrease in signs and symptoms.

Psychoeducation in Family with Clients Recovering from the Risk of Violent Behavior

Recovery is a change in which a person improves his health and well-being, lives in the direction of life he chooses,and strives to achieve life goals according to all his abilities. Recovery conditions are very necessary for everyclient with mental disorders (ODGJ), where strengthening hope, especially from within ODGJ itself, activates themto be active independently with family, friends, and the community in their environment. ODGJ with ViolentBehavior has anger that is expressed excessively and is not controlled verbally to the point of injuring others andor not harming the environment. Through this research with a pre-test-posttest control group design. The resultsfound in this study, it was found that the average recovery rate of ODGJ in the intervention group was 116.17 andthe average recovery rate of ODGJ in the control group was 94.03 so it can be concluded that the average recoveryrate of the intervention group was 22.14 greater than the control group with a significance of P-Value &lt; 0.05. Theresults showed that there was a significant difference in the recovery rate of ODGJ in the intervention group andthe control group. There was also an increase in the recovery rate in the control group, this was possible becauseof other interventions to increase the recovery of ODGJ with the risk of violent behavior either in the form ofindividual, family, or community interventions. Thus, to take sides with all elements in the ODGJ environment,encourage a sustainable family psychoeducation program policy, and be accompanied by monitoring of familycapabilities, especially effective communication within the family in the form of regular family visits by utilizing atiered health facility system.

Changes in cognitive, physiological, affective, to behavioral and social functions to cause the risk of violent behavior. This KIAN aims to provide an overview of Mental Nursing Care for Schizophrenic Patients at Risk of Violent Behavior. Violent behavior is a response to stressors faced by a person, this response can cause harm to oneself, others, and the environment. The function of the strategy for implementing violent behavior is to control the emotions of the hallucinations experienced by the client.

Penurunan Gejala Risiko Perilaku Kekerasan Melalui Acceptance and Commitment Therapy (ACT) di RSUD Tombulilato

Background: Violent behavior is one of the symptoms that becomes a reason for families to treat clients in mental hospitals because of the risk of endangering themselves and others. Acceptance Commitment Therapy (ACT) is a specialist nursing action given to clients with violent behavior.Purpose: The purpose of this study was to determine the effect of Acceptance and Commitment Therapy (ACT) on symptom reduction with the risk of violent behavior at Tombulilato Hospital.Method: The research design used a quasi-experimental design with one group pre and posttest. The number of respondents in this study, namely patients with violent behavior amounted to 18 respondents at Tombulilato Hospital.Result: From the results of the study, it was found that there was an effect of Acceptance and Commitment Therapy (ACT) on the reduction of symptoms with the risk of violent behavior at the Tombulilato Hospital. The conclusion in this study was that the average behavior of respondents before ACT was carried out at Tombulilato Hospital was 1.611 with a standard deviation of 0.50 while the average behavior of respondents after ACT was carried out at Tombulilato Hospital was 1.1667 with a standard deviation of 0.38. Statistical results obtained p value = 0.007 (p &lt; 0.05),Conclusion: it can be concluded that there is an effect of Acceptance and Commitment Therapy (ACT) on symptom reduction with the risk of violent behavior in RSUD Tombulilato

Predisposing and Precipitating Factors of Schizophrenic Clients with the Risk of Violent Behavior and Hallucination

The management of schizophrenia clients especially those who are at risk of violent behavior and hallucinations clients, its takes a long time. The management can be successful if it focuses on redisposing and precipitation factors. This study aimed to determine the predisposing and precipitation clients of risk factors for violent behavior and hallucinations. This research was descriptive analytic study. The sample was obtained with a total sampling of 48 respondents who met the inclusion criteria. Data were collected using a checklist of predisposing and precipitation factors and analyzed by frequency distribution. The predisposing factors found that caused schizophrenic clients to experience the risk of violent behavior and hallucinations were history of previous mental disorders (72.9%), unpleasant experiences and negative self-concept (100%) and the condition of clients who did not work (79.2%). Most of the precipitation factors found were drug withdrawal (50%), problem solving was 41.7%, and not working which reached 66.7%. Clients with schizophrenia who are at risk of violent behavior and hallucinations can be managed properly by considering the predisposition and precipitation that triggers the client to experience schizophrenia and relapse.
