Objective: To assess caregiver burden and related factor to constipation among caregivers of stroke patients in Department of Neurology and Neuro Intensive Care, Viet Duc University Hospital. Methodology: This is a cross-sectional study performed in 90 caregivers of stroke patients. Zarit Burden Interview was used to assess caregiver burden Results: The mean score of caregiver burden was 20.83 ± 13.02; and majority of caregivers had mild to moderate burden (47.8%), there were 6.7% of caregiver had moderate to severe burden (p < 0.05). Perceived self-health, the number of days for caregiving, and functional dependence of stroke patients had statistically significant associations with caregiver burden, in which the number of days for caregiving, and functional dependence of stroke patients statistically significant predicted caregiver burden (p < 0.05). Conclusion: Caregiver burden of caregivers of stroke patients in our study were moderate. Early interventions that focus on risk factors, support caregivers in caring for patients, and increase the patient's functional independence will help reduce caregiver burden.

Clinical characteristics and early results of the modified abramson technique to treat pectus carinatum at Viet Đuc Hospital from 2020-2023

Pectus carinatum is the second most common congenital anterior chest wall deformity. This study describe the clinical, paraclinical characteristics and evaluate the early results of the modified Abramson technique to treat pectus carinatum at Viet Duc Hospital. This is a retrospective descriptive study, in all cases diagnosed with pectus carinatum and underwent surgery according to the modified Abramson technique at Viet Duc Hospital from 1/2020 to 12/2023. In 134 patients, 131 were male, the mean age 13.6 ± 1.98 years. All patients presented with cosmetic complaints, and 85.2% patients have BMI <18.5. 94/134 cases (70.2%) had symmetrical deformities, 37/134 cases (27.6%) had asymmetrical deformities, and 3 cases (2.2%) had mixed pectus deformities. The average Haller index is 2.1 ± 0.3. All patients were operated using a bar, fixed at both ends with steel thread. Operative time was 33.6 ± 7.76 minutes, and hospital stay was 5.3 ± 0.76 days (4–7 days). There was no significant early postoperative complications. Conclusion: clinical examination and computed tomography play an important role in diagnosing pectus carinatum. The modified Abramson technique is a minimally invasive surgical method for pectus carinatum patients with short operative time, hospital stay, and the first step gives good treatment results. Efficacy of lengthening right renal veins using accompanying gonadal veins in living donor kidney transplantation.

Short harvested right renal veins (RV) are quite common in living donor kidney transplantation (KT). This technical difficulty might interfere implanting and increase warm ischemic time. Several techniques to overcome this problem have been applied, including iliac vein transposition, inverted transplant, synthetic graft, saphenous vein… Application of accompanying gonadal vein (GV), which is easily approachable and less time-consuming, has been recently published. This study aims to evaluate its effectiveness and safety. Retrospective study on KT using the gonadal vein to lengthen the short right renal vein at Viet Duc University Hospital from April 2019 to April 2022. The following data were gathered: baseline characteristics, vascular imaging in CT scan/after nephrectomy and after reconstruction (mm), reconstruction and surgical time, hospitalization days. The outcomes were determined by kidney function after transplantation (plasma creatinine, creatinine clearance) and related complications. Twenty-five cases with procured right kidney with short RV from the living donor which were reconstructed and lengthened by the accompanying GV were collected. The additional length of RV was 15.9 ± 2.4mm. Average cold ischemic time, venoplasty time, warm ischemic time were 60.4 ± 8.2, 21.2 ± 5.3, and 38.1 ± 5.6min, respectively. The average hospital stay was 15.3 ± 3.2 days. Average follow-up time was 31 ± 5.2 months, creatinine clearance was around 60ml/min after 1 year, no vascular or urologic complications was observed. Accompanying GV from a living donor to lengthen short right RV in KT is a feasible, safe, and effective technique.

Outcomes of middle meningeal artery embolization for treating chronic subdural hematoma

The study evaluates chronic subdural hematoma (cSDH) middle meningeal artery (MMA) embolization efficacy and safety. A prospective interventional study was conducted in Viet Duc Hospital from November 2021 to April 2024. All consecutive cSDH MMA embolization patients were included. Clinical and imaging data were collected before and one month after treatment. The study included 31 42-cSDH patients. Of these, 25.8% had hematoma evacuation, 83.9% were treated with surgery and embolization, and 16.1% with embolization alone. 92.9% of procedures used polyvinyl alcohol particles. The success rate was 92.9% and complications 7.1%. Asymptomatic external carotid artery vasospasm and MMA rupture occurred. Functional improvement occurred in 91.7% of patients one month after treatment. Significantly lower mean modified Rankin Scale (mRS) score (0.2±0.7 vs. 1.7±0.9; P=0.000) and higher proportion of patients with ≤2 mRS score (95.8% vs. 74.1%) were observed after treatment. Hematoma thickness decreased significantly (P=0.00) from 21.5±7.9 mm to 8.3±4.1 mm. The midline shift decreased significantly from 7.4±5.0 mm to 0.7±1.2 mm (P=0.00). Just one patient (4.2%) needed surgery. MMA embolization alone or with surgery appears to treat cSDH safely and effectively.

Survival outcomes of billroth-I reconstruction following distal gastrectomy for gastric cancer patients in Vietnam: A 5-year follow-up

This study aims to describe the long-term survivorship of Billroth I (or Pean's) reconstruction technique in patients with distal gastric cancer. A single-arm longitudinal observational study was conducted at Viet Duc University Hospital, Hanoi, Vietnam, from 2018 to 2021. A total of 103 patients who were diagnosed with primary carcinoma in the lower third of the stomach and underwent lower pole gastrectomy with Péan anastomosis were recruited. The Kaplan–Meier analysis was used to estimate the survival outcome. A multivariate Cox proportional hazard regression model was used to measure the factors associated with survivorship. Results showed that 8.7% died during the study period. The Restricted Mean Survival Time (RMST) was 61.9 (95%CI=59.4-64.4) months. RMST was significantly different across the stages of cancer, T and N. Specifically, patients with advanced stage (III) had a substantially lower RMST (49.2 months) compared to those with early-stage (I&II). Patients with T4 or N3 also had the lowest RMST compared to those with other characteristics (p<0.05). Female patients had a lower risk of dying than their male counterparts (HR=0.092, 95%CI=0.013-0.661). Patients with a higher level of Node (N) had a significantly higher risk of dying than those with a lower level (HR=5.103; 95%CI=1.716-15.178). Having comorbidities was associated with a lower risk of dying (HR=0.066; 95%CI=0.008-0.531) than not having comorbidity. To conclude, the findings indicate favourable long-term results for patients with distal gastric cancer undergoing the Billroth I (or Pean's) reconstruction technique. The practice above may be the established protocol for managing these particular patients.

ABSTRACTS Bone cysts of the talar body are a benign, rare condition. Remove bone cysts, autologous bone grafting is a treatment method many authors recognize. Approaching the calcaneal bone cysts of the talar body encounters difficulties due to the narrow surgical field, poor blood supply, and the entire surface of the talar body being covered with cartilage. Endoscopic talar joint surgery to remove cysts bone, autologous bone grafting in the treatment of calcaneal bone cysts of the talar body is a progressive step, with many advantages, overcoming the mentioned difficulties, albeit being a challenging technique. We report a case of bone cysts of the talar body treated with endoscopic ankle joint surgery to remove cysts bone and autologous bone grafting at Viet Duc University Hospital, with good postoperative results, and no recurrence after 8 months of follow-up.

MGMT promoter methylation in prognosis of patients with newly diagnosed IDH1 wildtype Glioblastoma

The objective of this study was to analyse the prognosis values of O6-methylguanine-DNA methyltransferase (MGMT) promoter methylation on the overall survival in patients with newly diagnosed isocitrate dehydrogenase 1 (IDH1) wild-type glioblastoma multiforme (GBM). A retrospective observational study was conducted on 108 IDH1 wild-type glioblastoma patients who underwent tumour resection at neurosurgical centres, including Hanoi Medical University Hospital and Viet Duc University Hospital, from October 2017 to December 2021. The MGMT status was assessed using methylation-specific Polymerase Chain Reaction (MSP). Multivariable Cox proportional hazards models and Kaplan-Meier survival analysis were performed. Of 108 patients, 79.6% had methylated MGMT. The median overall survival of patients with IDH1 wild-type GBM was 8.47 months (95%Confidence Interval (CI) =6.93-11.07). Patients with Methylated MGMT had a lower mortality risk (Hazard Ratio (HR) =0.63, 95%CI=0.41-0.98) than those without Methylated MGMT. Patients aged <55 HR=0.55, CI=0.35-0.85)>=55 years old. Having tumours on both sides of the hemisphere was associated with a higher risk of mortality (HR=3.18, 95%CI=1.68-6.00) compared to patients with tumours on the right side of the hemisphere. To conclude, our data underline the impact of the MGMT promoter methylation status in the treatment response of IDH1-wildtype GBM. In addition, age, tumour size and hemisphere were also significant prognostic factors in the overall survival of patients.

The situation of pre-hospital emergency care through traumatic patients have been admitted to emergency department of Viet Duc University Hospital in 2023

Introductions: Pre-hospital Emergency Care (PEC), especially for severe trauma is playing an important role. The study was conducted to provide an overview of the situation of PEC in Vietnam through traumatic patients at Viet Duc University Hospital. Materials and methods: A cross-sectional study enrolled all traumatic patients admitted to the Emergency Department of Hospital and had complete medical records, during the period from July to September, 2023, regardless of gender, age. Data were collected and processed using SPSS 20.0 software. Results: Characteristics of target population: 343 cases of traumatic patients were enrolled, male was majority accounted for 71.4%; The age group from 15 to 59 years old accounted for highest proportion (63%), ≥ 60 years old (32.6%). The sites of accident were mostly occurred on road (64.3%), at home (23.9%) and working place (10.2%). The brain injury accounted for the highest proportion (55.4%), multiple injuries (54.8%), extremities injury (38.5%), maxillofacial injury (25.1%) and spine injury (21.0%). Situation of PEC: First aid was mostly provided by first witness/accompanying person (64.5%), by community first aid provider (24.1%), only 11.4% provided by professional health worker. 8.7% patients were transported by taxi/car, 16.7% by unsafe ambulance, lacking official regulated medicine or equipment, or the health personnel did not know about the patient’s medical history. Some first aid were not provided on scene including: airway management 22.2%, wound care and dressing 33.0%, collar to cervical spine 14.5%, extremities splint 34.9%. Additionally, many extremities splint and wound care, 24.5% and 37.5% respectively, were done improperly. Conclusions and recommendations: The study showed some challenges such as improper first aid provision or unsafe transportation, which potentially cause unwanted risk for the patients. The author highly recommend the improvement of PEC according to the new law on this issue.

Results of using thoraflextm hybrid prothesis for the treatment of complex aortic disease at Viet Duc University Hospital

Background: The aim of this study was to introduce and evaluate the early results after the frozen elephant trunk (FET) procedure with ThoraflexTM Hybrid prothesis for the treatment of complex aortic disease at Viet Duc University Hospital. Methods: This is a retrospective descriptive study of all acceptable patients after the frozen elephant trunk (FET) procedure with ThoraflexTM Hybrid prothesis at Viet Duc University Hospital from January 2021 to May 2023. Results: There were 21 patients with a male rate of 85.7% and an average age 58.2±10 years old (range 40–73). History of patients with hypertension: 19 (90.5%); post-dissection aortic aneurysm: 2 (9.5%) patients. By cause, the number of patients with thoracic aortic aneurysm was 03 (14.3%), acute aortic dissection type A was 16 (76.1%), and aortic dissection type B was 02 (9.5%). In which there were 16 cases of emergency surgery (76.1%). Replacement of the aortic arch was performed under moderate hypothermic (280 °C on average) circulatory arrest and bilateral selective antegrade cerebral perfusion with permanent monitoring of cerebral tissue oxygenation by the MASIMO machine. Average cardio-pulmonary bypass time, aortic cross clamp time, distal hypothermic circulatory arrest time, and operating time were 137,5±22,9 minutes; 86,3±17,6 minutes; 25,2±8,9 minutes; and 7,7±10,3 hours. The average ventilation time, length of intensive care unit stay, and hospital stay were 7,2±9,7 days, 18,8±20,7 days, and 19,9 ± 6,5 days. There was one patient with post-operative cerebrospinal fluid drainage. No patient had bleeding complications, recurrent nerve palsy, reoperation, or in-hospital mortality. Conclusion: The initial results after the frozen elephant trunk procedure with ThoraflexTM Hybrid prothesis for the treatment of complex aortic disease at Viet Duc University Hospital were safe, good, and feasible.

The impact of video calls on the anxiety level among patients’ caregivers at Viet Duc Hospital during covid-19 pandemics

Objectives: To assess the impact of video call on the anxiety level of caregivers at the intensive care unit 2 at Viet Duc Hospital during COVID-19 pandemics. Research participants and methods: Intervention study comparing 2 groups on 62 caregivers of patients who were treated at Intensive Care Unit 2 of Viet Duc Hospital from January 2022 to November 2022 based on Zung Self-Rating Anxiety Scale. Results: Assessing the level of anxiety in the video call group: Severe anxiety was 71%, no pathological anxiety and very severe anxiety was 0%. Among the research participants in the video call group, the proportion of people who felt panicked for no reasons accounted for 71%, and 61.3% felt more impatient and anxious than usual, those felt limbs swaying, trembling, weak and easily tired, breathing in and out easily were 3.2%, 3.1%, 3.2% respectively. Anxiety level in video calls at mild to moderate anxiety accounted for the highest rate of 93.5%; The rate of very severe anxiety is 0%. Conclusion: Video calls for caregivers significantly reduced anxiety proportion compared to non-video calls.

Outcomes of liver transplantation for hepatocellular carcinoma: Experiences from a Vietnamese center.

Liver transplantation (LT) provides a favorable outcome for patients with hepatocellular carcinoma (HCC) and was launched in Vietnam in 2004. In this study, we evaluated the short-term and long-term outcomes of LT and its risk factors. This retrospective study analyzed HCC patients who underwent LT at Viet Duc University hospital, Vietnam, from 01/2012-03/2022. The following data were gathered: demographics, virus infection, tumor characteristics, alpha-fetoprotein (AFP) level, Child-Pugh and MELD scores, selection criteria, type of LT, complications, 30-day mortality, and disease-free and overall survival (DFS and OS). Fifty four patients were included, the mean age was 55.39 ± 8.46 years. Nearly 90% had hepatitis B virus-related HCC. The median (interquartile range) AFP level was 16.2 (88.7) ng/mL. The average MELD score was 10.57 ± 5.95; the rate of Child-Pugh A and B were 70.4% and 18.5%, respectively. Nearly 40% of the patients were within Milan criteria, brain-dead donor was 83.3%. Hepatic and portal vein thrombosis occurred in 0% and 1.9%, respectively; hepatic artery thrombosis 1.9%, biliary leakage 5.6%, and postoperative hemorrhage 3.7%. Ninety-day mortality was 5.6%. Five-year DFS and OS were 79.3% and 81.4%, respectively. MELD score and Child-Pugh score were predictive factors for DFS and OS (p < 0.05). In multivariate analysis, Child-Pugh score was the only significant factor (p < 0.05). In Vietnam, LT is an effective therapy for HCC with an acceptable complication rate, mortality rate, and good survival outcomes, and should be further encouraged.

Predicting Anxiety and Depression Among Erectile Dysfunction Patients: A Cross-Sectional Study.

In Vietnam, erectile dysfunction (ED) is prevalent and recognized to be associated with mental disorders; however, societal taboos impede a comprehensive understanding of this connection. Our study aims to investigate the factors related to higher levels of anxiety and/or depression (HAD) in individuals with ED. Between November 2022 and March 2023, a face-to-face survey was conducted at the Center for Andrology of Viet Duc University Hospital, involving 390 patients diagnosed with ED. The survey included 51 questions covering general patient information, the International Index of Erectile Function-15 (IIEF-15), the Patient Health Questionnaire-9 (PHQ-9), and the Generalized Anxiety Disorder-7 (GAD-7). The stepwise Akaike Information Criterion (AIC) method was used to identify factors associated with HAD. The study revealed an average age of 37.63 (11.84) years among participants, with a HAD prevalence of 17.69%. Several factors were associated with a higher likelihood of belonging to the HAD group in ED patients. These factors included non-office workers (OR: 1.11; 95% CI: [1.01, 1.21], p = .025), medium and high levels of work-related stress (OR: 1.23; [1.06, 1.44], p = .008; OR: 1.22; [1.04, 1.45], p = .018), multiple shameful experiences related to ED (OR: 1.16; [1.08, 1.25], p < .001), moderate and severe ED (OR: 1.17; [1.03, 1.32], p = .013; OR: 1.31; [1.14, 1.51], p < .001), and dissatisfaction with intercourse skills (OR: 1.09; [1.01, 1.17], p = .028). Our findings suggest a 16% higher likelihood of HAD status in individuals with multiple shameful experiences related to ED, while moderate and severe ED are associated with respective increases of 17% and 31% in the likelihood. These findings emphasize the importance of considering mental health in the care of individuals with ED.

Maternal and Perinatal Factors Associated With Childhood Brain Tumors: A Case-Control Study in Vietnam.

Brain cancer is the leading cause of cancer-related deaths in children and the majority of childhood brain tumors are diagnosed without determination of their underlying etiology. Little is known about risk factors for childhood brain tumors in Vietnam. The objective of this case-control study was to identify maternal and perinatal factors associated with brain tumors occurring in young Vietnamese children and adolescents. We conducted a hospital-based case-control study at Viet Duc University Hospital in Hanoi, Vietnam. Cases consisted of children with brain tumors aged 0-14years old admitted to the hospital from January 2020 to July 2022 while the controls were age and sex-matched hospitalized children diagnosed with head trauma. Perinatal characteristics were abstracted from hospital medical records and maternal medical, behavioral, and sociodemographic factors were collected through in-person interviews. Conditional logistic regression models were used to examine maternal and perinatal factors associated with childhood brain tumors. The study sample included 220 children (110 cases and 110 controls) whose average age was 8.9 years and 41.8% were girls. Children born to mothers aged greater than 30 years at the time of the child's birth had a higher risk of childhood brain tumors compared to those born to mothers aged from 18 to 30 years old (OR = 2.55; 95% CI: 1.13-5.75). Additionally low maternal body mass index prior to the current pregnancy of <18.5kg/m2 significantly increased the odds of having a child with a brain tumor in relation to normal maternal body mass index from 18.5-22.9kg/m2 (OR = 3.19; 95% CI: 1.36 - 7.50). Advanced maternal age and being markedly underweight were associated with an increased odds of having a child with a brain tumor. A population-based study with larger sample size is needed to confirm and extend the present findings.

LONG TERM OUTCOME OF LAPAROSCOPIC RADICAL NEPHRECTOMY FOR PATHOLOGICAL T1, T2 RENAL CELL CANCER AT VIET DUC HOSPITAL

Background: To evaluation of the long term outcome of laparoscopic radical nephrectomy for pathological T1, T2 renal cell cancer at Viet Duc Hospital. Methods: We retrospectively identified 73 patients who underwent RN between 2015 and 2022. We divided patients into three groups based on tumour size: &lt;4 cm, 4– 7 cm, &gt;7 cm. Clinical and paraclinical characteristics include age, gender, back pain, tumor size, tumor location, disease stage on computed tomography scans, histopathological results, postoperative outcomes and complications, postoperative survival time, and accumulated survival time. Results: the average age was 53.52 ± 12.17 years. CT scans classified renal cell carcinoma stages preoperatively according to IUAC 2009: 53.4% were stage T1a, 42.4% were stage T1b, 2.8% were stage T2a, and 1.4% were stage T2b, 2 patients experienced infection at the trocar site used for kidney extraction, 4 patients developed postoperative fever, and 1 patient had postoperative bleeding requiring a blood transfusion. One patient underwent emergency open surgery immediately after discharge due to bleeding through the drainage. The postoperative complication rate according to the Clavien-Dindo classification was 11% (8 patients). The results show that 63 patients are still alive, 5 patients have passed away due to the disease, and 5 patients have died from other causes such as traffic accidents, cardiovascular diseases, and cerebral vascular accidents. The average follow-up time is 46.7±26.5 months. The cumulative survival rates at 3, 4, 5, 6, and 7 years after radical nephrectomy for the treatment of renal cell carcinoma were 100%, 97.6%, 94.8%, 90.9%, and 83.3%, respectively. The average additional survival time after surgery was 84.11 ± 1.80 months. Conclusions: Laparoscopic radical nephrectomy for early-stage renal cell carcinoma (T1 and T2) demonstrates high surgical safety and absolute efficacy in oncology.
