Endless loop tachycardia among patients with devices having advanced preventive algorithms: A case series and brief review.

Endless loop tachycardia (ELT) is the commonest pacemaker mediated tachycardia (PMT) encountered among patients with cardiac implantable electronic devices (CIEDs). Despite being enabled with various preventive algorithms, we encountered several cases having recurrent, long, and symptomatic ELT. We retrospectively analyzed consecutive device checkups at device clinic at a single center of eastern India between January 2019 to March 2023. There were 20 cases of confirmed and sustained ELT among 4520 device checks. Although mostly benign, in two cases ELT led to clinical worsening in patients having left ventricular (LV) systolic dysfunction. Even with good ventricular function, ELT resulted in improper atrioventricular (AV) synchrony leading to disabling symptom in one case. The differentiation of ELT from sinus tachycardia and atrial tachycardia (AT) was not always easy. Magnet application is certainly useful to differentiate. The situations that provoked ELT in this study were-long AV delays, VIP (ventricular intrinsic preferences)/MVP (managed ventricular pacing), atrial non-capture, atrial under/over sensing, premature ventricular contractions (PVCs)/couplets, premature atrial contractions (PAC) and slower ventriculo-atrial (VA) conduction. Rate responsive shortening of post-ventricular atrial refractory period (PVARP) also promoted its occurrence and hindered troubleshooting. When ELT occurred despite post-PVC extension of PVARP, lowering the atrial sensitivity, switching to bipolar sensing and manual setting of longer PVARP after measuring VA conduction time were useful. "Rate responsive PVARP" had to be turned off in a few cases to prevent ELT. On the contrary, an over aggressive prolongation of PVARP led to repetitive non-reentrant ventriculo-atrial synchrony (RNRVAS) in two cases. Checking VA conduction during implantation and noninvasive program stimulation (NIPS) during follow up were useful to check the tendency for ELT. Clinically significant ELT is rare but not uncommon among devices having in-built preventive algorithms. Manual adjustments are often useful to troubleshoot the same.

Orthodromic Reciprocating Tachycardia Relying on Aberrant Conduction: The Need for a Larger Circuit.

Aberrant conduction during orthodromic reciprocating tachycardia (ORT) prolongs the ventriculoatrial conduction time, which can be essential for the maintenance of tachycardia in specific cases. We searched for ORT relying on aberrancy among 220 cases in our center. Three patients showed the phenomenon of aberrancy-dependent ORT. All accessory pathways were located at the lateral regions of the atrioventricular annulus. None of them had a baseline bundle branch block (BBB). Creating a functional BBB was necessary to induce the tachycardias. In two cases, termination of tachycardias was directly associated with resolution of the aberration. In the other case, re-entry required both BBB and slow pathway conduction. We conclude that extra transseptal time caused by aberrancy can be an integral part of the ORT circuit, which explains the infrequent and unsustainable episodes of ORT in certain patients and is useful in understanding the circuit and localizing the pathway.

PO-05-226 ORTHODROMIC RECIPROCATING TACHYCARDIA RELYING ON ABERRANT CONDUCTION: NEED FOR A LAGER CIRCUIT

Aberrant conduction during orthodromic reciprocating tachycardia (ORT) can prolong the ventriculo-atrial conduction time, which is found essential for the reentry in certain situations. To investigate the prevalence and different mechanism of aberrancy-dependent ORT. ORT cases relying on aberrancy from 220 cases in our center were searched using the following criteria: (1) Multiple episodes of ORT with aberrant conduction in ECG ; (2) No baseline BBB during sinus rhythm; (3) Absence of narrow QRS form during tachycardia unless isoproterenol used; (4) Restoration of bundle branch conduction giving rise to tachycardia termination or circuit alteration. ECGs and intracardiac tracings during study were carefully examined. 3 patients from 220 patients (1.4%) with ORT showed the phenomenon of dependence on aberrancy. Clinical tachycardia episodes of the patients were short in time. All accessory pathways (APs) were located at anterolateral regions of atrioventricular annulus. None of them had baseline bundle branch block. Creating functional bundle branch block was necessary to induce the tachycardias. In the first patient with ORT and left bundle branch block (LBBB), a late-delivered premature ventricular contraction (PVC) was able to advance the next A and reset the tachycardia, which was diagnostic for ORT with aberrancy. Meanwhile, left bundle branch conduction was restored which narrowed the next QRS complex. Tachycardia was then terminated by the advanced wavefront reaching the ventricular insertion site of AP which was refractory. In the second case, termination of the tachycardia was always preceded by a spontaneous LBBB restoration, with the V and subsequent A at MA advanced, suggesting anterograde block in the AV node. In the other case was, induction required both right BBB and slow pathway conduction. When isoproterenol was given, fast pathway recovered followed by BBB restoration, which made anterograde limb again switched to slow pathway. All pathways were successfully ablated during the procedure. All patients have been free of arrhythmias for at least 6 months. Negative SA-VA difference during entrainment of a supraventricular tachycardia: a new criterion for the diagnosis of focal atrial tachycardia

Introduction Ventricular overdrive pacing during tachycardia is one of the most common and useful maneuvers to assess de differential diagnosis of supraventricular tachycardia (SVT). Focal atrial tachycardia (FAT) usually presents long ventriculo-atrial (VA) interval, which is a passive interval since FAT is not an atrio-ventricular reentry. Therefore, during ventricular entrainment of a FAT, the stimulus-atrial (SA) interval will depend solely on the retrograde VA conduction time, if retrograde conduction exists. This retrograde conduction time is usually shorter than the VA Interval during tachycardia, thus being able to cause a negative SA-VA difference. This does not occur in other types of SVT, where the SA-VA difference helps to differential diagnosis, but positive values have been described. Purpose Our aim was to analyze whether the SA-VA value discriminates FAT from the other types of SVT, in patients with SVT and VA conduction when stimulating the ventricle during tachycardia. Methods Multicenter data of SVT entrainment were collected retrospectively, including FAT, atrioventricular nodal reentry tachycardia (AVNRT), and atrioventricular reciprocating tachycardia (AVRT). The numeric value of SA-VA difference was calculated for each case. The best SA-VA cut-off point for the diagnosis of TAF was determined, evaluating the sensitivity and specificity of this value for the diagnosis of TAF with respect to the gold standard (classical criteria). Results Continuous ventricular pacing during tachycardia succeeded in accelerating the atrium to the pacing rate in 80 cases out of the total studied cases: 32 (40%) AVNRT, 28 (35%) AVRT and 20 (25%) FAT. All FAT cases had a negative SA-VA value and all AVNRT and AVRT cases obtained positive SA-VA (Figure 1). SA-VA value lower than 0 ms was the best cut-off point (using ROC curve, Figure 2) with an area under the curve of 1. SA-VA difference lower than 0 ms (or negative SA-VA difference) showed 100% sensitivity and specificity for the diagnosis of FAT. Conclusions Negative SA-VA difference is a new and accurate criterion for the diagnosis of focal atrial tachycardia.

Miniseries 2-Septal and paraseptal accessory pathways-Part III: Mid-paraseptal accessory pathways-revisiting bypass tracts crossing the pyramidal space.

The mid-paraseptal region corresponds to the portion of the pyramidal space whose right atrial aspect is known as the triangle of Koch. The superior area of this mid-paraseptal region is also para-Hisian, and is close to the compact atrioventricular node and the His bundle. The inferior sector of the mid-paraseptal area is unrelated to the normal atrioventricular conduction pathways. It is, therefore, a safe zone in which, if necessary, to perform catheter ablation. The middle part of the mid-paraseptal zone may, however, in some patients, house components of the compact atrioventricular node. This suggests the need for adopting a prudent attitude when considering catheter ablation in this area. The inferior extensions of the atrioventricular node, which may represent the substrate for the slow atrioventricular nodal pathway, take their course through the middle, and even the inferior, sectors of the mid-paraseptal region. In this review, we contend that the middle and inferior areas of the mid-paraseptal region correspond to what, in the past, was labelled by most groups as the 'midseptal' zone. We describe the electrocardiographic patterns observed during pre-excitation and orthodromic reciprocating tachycardia in patients with pathways ablated in the middle or inferior sectors of the region. We discuss the modification of the ventriculo-atrial conduction times during tachycardia after the development of bundle branch block aberrancy. We conclude that the so-called 'intermediate septal' pathways, as described in the era of surgical ablation, were insufficiently characterized. They should not be considered the surrogate of the 'midseptal' pathways defined using endocardial catheter electrode mapping.

Ventriculoatrial conduction and related pacemaker-mediated arrhythmias in patients implanted for atrioventricular block: An old problem revisited

Ventriculoatrial (VA) conduction and related pacemaker-mediated tachyarrhythmias (PMT) have not been systematically investigated in a large cohort of patients implanted for symptomatic atrioventricular (AV) block. Two hundred fifty consecutive patients (71±14 years, 63% male) implanted for symptomatic second- or third-degree AV block were screened for retrograde VA conduction and related PMTs including endless loop tachycardia (ELT) and repetitive nonreentrant VA synchrony (RNRVAS). After a mean post-implantation period of 38±12 months, AV block was persistent in 137 (55%) and variable in 113 (45%) patients. Retrograde 1:1 conduction was present in 76 patients (30%) with a mean VA conduction time of 258±65 ms. The incidence of VA conduction varied considerably according to the presence and type of anterograde conduction block. Retrograde conduction was present in 24 of 137 patients (18%) with persistent AV block. Fifteen of the 76 patients (20%) with VA conduction had ≥1 documented PMT. The recorded arrhythmias were ELT in 11 and RNRVAS in 4 patients. VA conduction time was significantly longer in patients with than without PMT (297±64 vs. 249±62 ms, resp., P<0.01). None of the patients without VA conduction had documented PMTs. Retrograde VA conduction and related PMTs are not uncommon in patients implanted for symptomatic AV block. Testing for retrograde conduction should therefore be performed in all patients with AV block in order to optimize device programming and prevent PMTs.

Catheter Ablation of an Unusual Decremental Accessory Pathway in the Left Coronary Cusp of the Aortic Valve Mimicking Outflow Tract Ventricular Tachycardia

The anatomic location of atrioventricular (AV) bypass tracts (or accessory pathways) is variable, but these generally traverse the tricuspid or mitral annulus and insert into the atrium and ventricle near the AV ring. There is only 1 case report of an accessory pathway ablated in the left coronary cusp (LCC),1 and this pathway had bidirectional conduction, was not decremental, and was associated with an orthodromic tachycardia. We report here the first case of an antegrade slowly conducting, decremental, accessory pathway that was also successfully ablated in the aortic LCC. This pathway generated antidromic tachycardia having a QRS morphology mimicking those seen in outflow tract ventricular tachycardia. A 42-year-old woman was evaluated for palpitations and presyncope for 2 years. These were commonly associated with shortness of breath and chest heaviness that would resolve spontaneously. Her baseline ECG was normal without ventricular preexcitation. Echocardiogram, coronary angiogram, and thyroid function were also unremarkable. On her third presentation to a hospital, a broad complex tachycardia was captured (representative 12-lead ECG shown in Figure 1). The cycle length was 480 ms, the QRS duration was 150 ms, and a left bundle branch morphology with a right inferior axis was noted. She was referred for ablation of suspected ventricular outflow tract tachycardia. Consent was obtained for mapping and ablation of this tachycardia. During placement of catheters, a spontaneous initiation of tachycardia occurred, which was noted to have 1:1 AV association. A quadripolar catheter was placed at the right ventricular apex and His bundle, with a decapolar catheter in the coronary sinus. During ventricular pacing, retrograde conduction occurred with a long ventriculoatrial interval. Tachycardia was readily initiated with ventricular premature beats showing retrograde decremental conduction (Figure 2). Endless loop tachycardia below the upper tracking rate of a pacemaker: A case report

An 82-year-old female with a history of hypertrophic cardiomyopathy (HCM), sick sinus syndrome (SSS), and an implanted DDD pacemaker was admitted to our hospital for congestive heart failure caused by rapid atrial fibrillation. After administration of amiodarone, atrial fibrillation (AF) became atrial flutter (AFL). Electrophysiological investigation revealed counterclockwise AFL. Catheter ablation of the cavotricuspid isthmus was performed. Burst pacing from the coronary sinus ostium to confirm the block line of the isthmus induced rapid, regular, ventricular pacing at a rate of 110bpm. The differential diagnosis of this tachycardia included ectopic atrial tachycardia and pacemaker-mediated, endless loop tachycardia (ELT). We diagnosed this arrhythmia as ELT, because temporary reprogramming of the pacemaker mode from DDD to VVI terminated the tachycardia. In this patient, pacing parameters favored ELT (long atrioventricular delay [AVD] and short postventricular atrial refractory period [PVARP]), and atrioventricular and ventriculoatrial conduction time was prolonged as a result of amiodarone administration.The ELT continued because the rate was lower than the programmed upper tracking rate. Reprogramming the parameter (decreasing AVD and increasing PVARP) resulted in termination of ELT.

Pacemaker repetitive nonreentrant ventriculoatrial synchrony. Why did automatic mode switching occur?

Repetitive nonreentrant ventriculoatrial synchrony during dual-chamber pacing is characterized by long intervals alternating with short intervals. This arrangement activated automatic mode switching in a St Jude dual-chamber pacemaker in which the algorithm requires an atrial sensed event for automatic mode switching initiation. Automatic mode switching activation by an atrial sensed event (retrograde P wave) was puzzling because the programmed postventricular atrial period was longer than the retrograde ventriculoatrial conduction time. The explanation is presented in the form of questions and answers to facilitate the understanding of pacemaker function and complex timing cycles.

Atrial lead malfunction presenting as new onset pacemaker-mediated tachycardia

This report describes the de novo occurrence of pacemaker-mediated tachycardia (PMT) in a patient with a dual-chamber implantable cardioverter-defibrillator and stable retrograde ventriculoatrial conduction time. The same rate-adaptive post-ventricular atrial refractory period (PVARP) duration had previously prevented PMT. Oversensing of atrial false signals from a defective lead shortened the PVARP with consequent sensing of retrograde conduction.

Differentiation of narrow complex tachycardia

Many different criteria and pacing maneuvers exist to determine the etiology of a narrow QRS complex tachycardia (NCT). This report describes a case of NCT wherein the initial conflicting findings at the time of the EP study were then clarified by the development of left bundle branch block and prolongation of ventriculoatrial conduction time confirming the diagnosis of a left sided accessory pathway. The accessory pathway was then successfully ablated.

Successful Radiofrequency Catheter Ablation of Left Anterolateral Accessory Pathway Associated with a Coronary Sinus Diverticulum Using an Irrigated-Tip Catheter

Background: Left posterior-posteroseptal accessory pathways associated with the coronary sinus diverticulum have been reported. We report a rare case of a manifest left anterolateral accessory pathway associated with a coronary sinus diverticulum.Case: Twelve years-old boy, his 12-leads ECG revealed sinus rhythm with positive delta waves in leads II, III, and aVf, was suffering from recurrent supraventricular tachycardia. At the first admission, he had undergone the catheter ablation of left anterolateral region of the mitral annulus via the femoral artery. The Delta waves disappeared but tachycardia was easily induced. During tachycardia, the ventriculoatrial conduction time was not so short around there. Coronary sinus venography revealed the divericulum near its ostium. An additional ablation was performed near the neck of the diverticulum, revealing the shortest ventriculoatrial conduction time, via the femoral artery. However, the tachycardia re-occurred one month later. At the second admission, ventriculoatrial conduction during ventricular pacing was mapped meticulously using Ensite NavX system. The pathway where the shortest ventriculoatrial conduction time and accessory pathway potential were recorded at the neck of coronary sinus diverticulum was successfully ablated using an irrigated-tip catheter via the femoral vein.

Successful Ablation of Left-Sided Accessory Pathways in a Patient with Coronary Sinus Atresia Using 3-Dimensional Mapping System

A 62-year-old man with recurrent paroxysmal supraventricular tachycardia was referred to our hospital for ablation. A 12-lead electrocardiogram revealed sinus rhythm with no delta waves. A 12-lead electrocardiogram during the palpitations recorded a regular, narrow QRS tachycardia at the rate of 186/min. A coronary angiography was performed after the coronary sinus could not be cannulated. On levophase coronary angiography, there was dual venous drainage into the right and left atria and no coronary sinus ostium. A regular, narrow QRS tachycardia with a cycle length of 300 ms was induced by atrial extrastimulus pacing. A transseptal catheterization was performed using the standard Brockenbrough technique. Mapping of the mitral annulus was performed during ventricular pacing, using a 7-French deflectable ablation catheter and the 3-dimensional electroanatomical mapping system. The earliest retrograde atrial activation was recorded in the posterolateral region of the mitral annulus. Though the ventriculoatrial conduction time was prolonged after radiofrequency energy was delivered to this site, the ventriculoatrial conduction after transvenous adenosine triphosphate administration was maintained. After re-mapped, the earliest retrograde atrial activation site was shifted to the left anterior region radiofrequency energy eliminated this accessory pathway. Thirty minutes after effective radiofrequency discharge, there was persistent ventriculoatrial dissociation. Six months after the procedure, the patient has been free from any symptoms.

The atrioventricular interval during pre-excited tachycardia: A simple way to distinguish between decrementally or rapidly conducting accessory pathways

Recognition of the presence and role of decremental fibers during wide QRS tachycardia requires carefully executed intracardiac studies. This study sought to determine the value of the atrioventricular (AV) conduction time during pre-excited tachycardia to differentiate a fast from a decrementally conducting accessory pathway (AP). Fifty-one patients with 56 pre-excited tachycardias were included in the study: Group I: 27 patients with 31 antidromic tachycardia (ADT) using an atriofascicular pathway, Group II: 2 patients with pre-excited tachycardia due to bystander AV conduction, Group III: 3 patients with ADT and a short AV Mahaim fiber, and Group IV: 19 patients with 21 ADT using a fast conducting right-sided AP. The AV interval was measured in the His bundle electrogram and related to the tachycardia cycle length (TCL) by making an AV/TCL index. An AV interval > or = 150 ms during pre-excited tachycardia yielded a 91% sensitivity, 90% specificity, positive predictive value of 94%, and negative predictive value of 83% for AV conduction over a decrementally conducting pathway, whereas a > or =0.55 AV/TCL index yielded a sensitivity of 89%, specificity of 100%, positive predictive value of 100%, and negative predictive value of 84%. In 3 of 4 patients with Mahaim fibers and a <0.55 AV/TCL index, a prolonged ventriculoatrial (VA) conduction time was found. An AV interval > or =150 ms during pre-excited tachycardia is a fast and reliable method for detecting a decrementally conducting AP. Correcting the AV interval by the tachycardia cycle length improved specificity and positive predictive accuracy.

Electrophysiological Characteristics and Radiofrequency Ablation of Accessory Pathways With Slow Conductive Properties

Atrioventricular accessory pathways (AP) with unusually long ventriculo-atrial (VA) conduction times are present in a significant subset of patients with the Wolff-Parkinson-White (WPW) syndrome, not including patients with the permanent form of atrioventricular junctional reciprocating tachycardia. We compared the electrophysiological characteristics and outcomes after radiofrequency (RF) ablation in 34 patients with the WPW syndrome, a VA interval >80 ms, and paroxysmal tachycardia with an RP/PR ratio <1 (the slow group), vs 80 patients with WPW syndrome and a VA interval <80 ms (the fast group). AP were found in the posteroseptal region significantly more often in the slow than in the fast group. In addition, the decremental conductive properties of the AP were more common in the slow than in the fast group. Catheter ablation of AP was highly successful in both groups, although ablation required a greater number of RF applications and longer procedure times in the slow group, especially for AP with decremental conductive properties. A posteroseptal AP location was more common in AP associated with long conduction times than in AP with typical conductive properties. Both types of AP were successfully ablated, although the slow group required longer procedures and more RF energy deliveries.

Conduction time oscillations precede the spontaneous termination of human atrioventricular reciprocating tachycardia.

Prior clinical research indicates that conduction slowing is the primary mechanism leading to the spontaneous termination of reentrant tachycardia in humans. Yet, some experimental models indicate that cycle length oscillations and enhanced conduction are important prerequisites. The role of oscillations in conduction times and enhanced conduction in the spontaneous termination of human reentrant tachycardia has not been adequately investigated. The electrophysiologic features preceding the spontaneous termination of orthodromic atrioventricular (AV) reciprocating tachycardia (RT) were evaluated in 21 patients, each of whom had a sustained (>60 seconds) and a spontaneously terminating (>/=10 beats and </=60 seconds) episode of AVRT during the same electrophysiologic study. Atrio-His, His-ventricular, interventricular, ventriculoatrial and atrial conduction times were measured for each beat of spontaneously terminating AVRT and for paired beats of sustained AVRT. Beats of spontaneously terminating and sustained tachycardia were pooled and Hadi multivariate outlier analysis was used to identify whether significant beat-to-beat alterations in conduction times preceded the spontaneous termination of reentry. Cycle lengths of sustained (348+/-62 msec) and spontaneously terminating AVRT (351+/-70 msec) were similar. Significant beat-to-beat oscillations in conduction times preceded the spontaneous termination of AVRT in 10 of the 21 (48%) patients. An apparent enhancement in atrio-His or ventriculoatrial conduction times immediately preceded the spontaneous termination of AVRT in 11 patients (52%), while an apparent conduction delay occurred in only 2 patients (10%). Moreover, significant oscillations in conduction times were present in 9 of the 11 patients (82%) with enhanced conduction, but only in 1 of the 10 (10%) remaining patients (p=0.002. Conduction time oscillations, which are related to apparent enhancement in atrio-His or ventriculoatrial conduction, frequently precede the spontaneous termination of reentry in humans.

Electrogram patterns associated with successful radiofrequency ablation of accessory pathways in children.

Electrograms observed prior to successful and unsuccessful ablation trials in 33 patients (362 attempts) with manifest pathways and 18 patients (194 attempts) with concealed pathways were compared to identify the electrogram patterns that are associated with successful radiofrequency ablation of accessory atrioventricular connections in young patients (mean age 12.7 years; range 4-22 years). Success was defined as permanent or transient interruption of conduction in the accessory connection. Predictors of success in patients with manifest pathways were local ventricular preexcitation (p &equals 0.0001), left-sidedness (43 or 174) of the accessory connection compared (p &equals 0.04) to right-sidedness (27 of 172), a probable Kent bundle potential (29 of 84 versus 39 of 256; p &equals 0.0001), and short antegrade atrioventricular conduction intervals (53.1 +/- 31.9 ms versus 64.6 +/- 32.0 ms; p &equals 0.02). Predictors of success in patients with concealed pathways were short ventriculoatrial conduction times (103.3 +/- 35.8 ms versus 117.9 +/- 34.8 ms; p &equals 0.01), and left-sided (42 of 125) pathways (p &equals 0.03; versus right-sided, 11 of 60). The presence of a Kent bundle potential was not significant. We conclude that specific electrogram patterns can predict successful ablation of either manifest or concealed accessory pathways. Use of these criteria may reduce the delivery of unnecessary energy to young myocardium.

Studies in rat isolated hearts suggest that adenosine is more potent for slowing antegrade atrioventricular (AV) than retrograde ventriculo-atrial (VA) conduction. The potency (EC 50 ) and maximal effect (E max ) of adenosine to slow AV and VA conductions were determined in 16 patients (mean age 48 years). These patients were selected from a group that had a common form of AV nodal reentrant tachycardia who had prior to the study been subjected to selective radiofrequency ablation of the slow pathway conduction, but with intact antegrade and retrograde fast pathway conduction. During high right atrial and ventricular pacing at comparable cycle lengths (400–500 msec). adenosine was injected as an intravenous bolus at an initial dose of 0.5 mg followed by stepwise increase of 1 mg given every 5 minutes until AV or VA block occurred. The dose-response curves for adenosine-induced prolongation of AV and VA conduction time were determined. The 1:1 AV and VA conduction cycle lengths before injection of adenosine were 340 ± 50 and 326 ± 75 msec, respectively. Adenosine induced dose-dependent prolongation of AV nodal conduction time and caused typical Wenckebach AV block in all 16 patients with EC 50 and E max values of 1.4 ± 0.8 mg and 54 ± 2%, respectively. Adenosine also induced dose-dependent prolongation of VA conduction time and VA block, however, with significantly (P &lt; 0.051 higher EC 50 and shorter E max , i.e., 6.1 ± 1 mg and 20 ± 2%, respectively. The mean dose of adenosine to induce AV block is significantly lower than that to induce VA block (3.3 ± 1.6vs. 7.7 ± 3.3 mg, P &lt; 005). Notably, adenosine induced Wenckebach VA block in 10 of 16 patients and Mobitz type II block in 6 of 16 patients. In conclusion, adenosine is more potent in slowing AV than VA conduction, suggesting that the electrical wavefront entering the N zone of AV node is less affected by adenosine during VA than during AV conduction. In addition, retrograde fast pathway may at least in part involve conduction through the N zone of AV node.

Radiofrequency Catheter Ablation of an Accessory Pathway in a Young Man with Dextroversion

We report an observation of a radiofrequency catheter ablation of an accessory pathway (AP) in a patient with Wolff-Parkinson-White syndrome (WPW) and dextroversion. Atrioventricular rings were mapped by the ablation catheter to locate the shortest local atrioventricular conduction time in sinus rhythm and ventriculoatrial conduction time during orthodromic tachycardia or ventricular pacing. Successful ablation confirmed a right posteroseptal AP localization. Thus, the electrocardiographic modifications due to an AP in this location in the presence of dextroversion were defined.
