Abstract Introduction Vein of Marshall ethanol infusion (VOM-EI) creates a local ablation which includes epicardial bridges, AF triggers, vein innervation and facilitates mitral isthmus (MI) ablation. Recent trials demonstrated that in persistent AF patients the addition of VOM-EI to RF catheter ablation (CA) increased success rate vs CA alone. Moreover, a novel CA strategy (VOM-EI, PVI, and specific linear lesions set) is associated with a high rate of freedom from AF. Nevertheless, in these studies VOM-EI and RFCA were performed as different step of the same procedure. We hypothesize that performing electro-anatomical mapping (EAM) right after the VOM-EI would lead to an overestimation of ethanol-induced scar because of a certain amount of reversible oedema. This could affect the durability of the lesion set and possibly the success rate. Aim We postulated that VOM-EI performed at least 1 month earlier than RFCA would show a significantly smaller EAM low voltage area as compared to the standard strategy. Methods Patients with recurrent persistent AF with no history of previous ablation were enrolled in this randomized, dual centre, pilot study. All included patients were randomized 1:1 to group 1 (experimental): VOM-EI and after at least one month later high-definition (HD)-EAM and RFCA (PVI, MI, LA dome, cavo-tricuspid isthmus). group 2 (control): to a standard procedure (VOM-EI then 5 minute later HD-EAM and RFCA). Primary endpoint was ethanol induced HD-EAM scar area. According to the endpoint patients with unsuccessful VOM-EI were excluded. Moreover patiets with or no branches morphology, VOM dissection, and mechanical leakage were also excluded because these are demonstrated to be related to a smaller VOM-EI related lesion. Clinical and procedure related data were also collected. Results Forty consecutive patients met the inclusion criteria. The two groups did not differ for baseline characteristics (see table 1). Mean ethanol-induced HD-EAM scar area was significantly smaller in 20 patients where VOM-EI was performed at least 1 month earlier as compared to the standard procedure group (2,5±2,4 vs 11,2±9,5 cm2, p=0,018). The set of linear lesions was successful achieved in all patients. Notably the two group did not differ for any procedural technical aspect of mitral isthmus block (table 1). Neither VOM-EI nor AF ablation procedure related complications were recorded. Conclusion This is the fist randomized pilot study to demonstrate that a consistent amount of ethanol-induced related lesion could be reversible at least in its endocardial aspect of the left atrium. This would possibly affect the long-term stability of the lesion set. Notably, we still had a high rate of block at first mitral line attempt which could possibly be related to a persistent effect of VEOM-EI on the epicardial aspect of the left atrium. Larger population, randomized trial is needed to confirm this preliminary result and if it would affect the procedure success.Table 1Picture 1

Acute impact of vein of marshall ethanol infusion combined with anatomical ablation validated by pacing in redo ablation of persistent atrial fibrillation

Abstract Background Catheter ablation with radiofrequency in persistent atrial fibrillation (PeAF) has limited success and a significant proportion of patients requires redo ablation. Several trials showed that addition of vein of Marshall ethanol infusion (Vom-EI) to catheter ablation, compared with catheter ablation alone, increases the possibilities of remaining free of AF. No data are available about the effect on VoM-EI in Pe-AF patients that undergo a redo ablation. Purpose We evaluated acute impact on lesion formation post-VoM-EI and the mitral line block validation after a methodical approach including VoM-EI and check of pulmonary vein isolation (PVI), roof-line, mitral line (ML) and cavo-tricuspid isthmus line in a population of PeAF patients undergone redo ablation. We aimed also the results of a short follow-up. Methods Consecutive patients undergoing redo ablation for PeAF were enrolled. All patients underwent check of PVI, left atrium (LA) roofline and cavotricuspid isthmus line and, if necessary, ablation was completed. In all patients, after a detailed electroanatomical map of the LA (filter at 0.05-0.5 mV if the patient was in sinus rhythm or 0.05-0.3 mV in the case of AF), we proceeded with the VoM-ETHO. LA map was thus repeated to assess the extension of the newly-formed low voltage area (LVA). According to the newly-formed LVAs, the validation of mitral line was obtained by the evidence of bidirectional block. In presence of conduction through the mitral line also after endocardial revision, we proceeded to mapping and ablating in the CS epicardial gaps in the "anchored wall" or in the "free-wall"of the great cardiac vein (GCV). Results Twenty consecutive patients (64±8 years and 65% male) undergone redo ablation for AF with VoM-EI were included in this study. All patients underwent PVI in the previous procedure but only in 11 roof line has been performed. In twenty patients (52%) reconnection of PV was observed (12/20 in right superior pulmonary vein). In 4/11 roof line was not complete. The medium value of basal LA-LVAs was 3.38土5.27 cmq and the newly-formed LVAs after the VoM-EI procedure was 9.21土5.63 cmq. All patients achieved bidirectional block validated across ML: in 8/20 after epicardial gaps ablations into the anchored wall of GCV and in 6/20 after epicardial gaps ablation in the free wall of GCV. The ML procedural time was 18.82土12.79 minutes. The number of radiofrequency irrigation was 20.11土10.56. No major complications occurred. Five patients had VOM dissection without consequences. During a short follow-up period (6土5 months), only one patient had AF recurrences after the blanking period of one month. Conclusions VoM ethanol ablation added to PVI and linear lesions in the context of a methodical and anatomical approach during redo ablation of PeAF patients seems to have promising results and to be safe. Longer follow-up is needed to understand the role of this technique in redo ablation.

Low voltage area in the left atrial anterior wall predict Marshall-PLAN failure in patients with persistent atrial fibrillation

Abstract Background The most appropriate ablation strategy for persistent atrial fibrillation (PeAF) has not yet been determined. However, ethanol infusion of the vein of Marshall (VOM) combined with a linear set of ablation lesions (namely the Marshall-PLAN) has yielded encouraging results (1). Purpose Our study aimed at analyzing the relationship between low voltage areas (LVA) and arrhythmias recurrences in a PsAF population treated with the Marshall-PLAN. Methods we conducted a retrospective study on consecutive patients undergoing first catheter ablation (CA) for PsAF between 2021 and 2022 at our center. Pulmonary vein isolation (PVI), mitral isthmus ablation (VOM ethanol infusion, endocardial ablation and epicardial ablation in the great cardiac vein), left atrial (LA) roofline and cavo-tricuspid isthmus ablation were carried out in all the patients. Bidirectional block was validated for all the linear lesions as well as for PVI. Pre-ablation LA electroanatomic maps were built with CARTOv3 system and the PentaRay mapping catheter (minimum 3000 points). LVA were identified as areas >1cm2 (containing ≥3 neighboring points with ≤10mm distance) with bipolar voltage <0.5mV in SR and <0.24 mV in AF (2). All patients were followed for at least 12 months. Results Overall, 93 patients (69±7years, 30% female) with PsAF were included. Their left-ventricle ejection fraction and left atrial volume indexed for body surface area (LAVI) were 57% and 92±20 ml/m2. LA maps demonstrated at least one LVA in 70 subjects (75.3%). The mean relative surface of LA in which a LVA was evidenced was 8±14%. LVA were found within the anterior LA wall in 49 patients (53%), in the posterior wall in 33 patients (35.5%), in the inferior wall in 22 patients (24%), in the lateral wall in 14 patients (15.1%) and in the pulmonary vein region in 44 patients (47.3%). After 19±6 months of follow-up, 28 (30%) patients had AF recurrence. At Cox regression analysis (including LVA burden and LAVI dimensions), LVA localized within the anterior LA wall were predictive of arrhythmia recurrences (hazard ratio [HR] 2.0 [95% CI, 1.1–4.7], P=0.04). Kaplan-Meier estimates of survival free from recurrences at 20 months were 51% and 88% in patients with and without LVAs in the anterior LA wall (Log-rank 0.006). Conclusions The Marshall-PLAN led to a global AF-free survival of 70% after 19±6 months in our cohort of patients with PsAF. Importantly however, the data show that the subset of patients with LVA in the anterior LA wall (i.e., the LA region not targeted by the pre-specified ablation set here evaluated) have a 55%-only AF-free survival while that of patients without LVA in the anterior LA wall was of 88%. As a result, our study supports the relevance of atrial substrate assessment and the possibility of CA strategy optimization in the subset of patients with PsAF and LVA in the anterior LA wall (3).

A proposed index of myocardial staining for vein of marshall ethanol infusion

Abstract Background Mitral isthmus(MI) conduction block is a fundamental step in anatomical approach treatment for persistent atrial fibrillation(PeAF). However, MI block is hardly achievable with endocardial ablation only. Retrograde ethanol infusion(EI) into the vein of Marshall(VOM) facilitates MI block. Fluorographic myocardial staining(MS) during VOM-EI could be helpful in predicting procedural alcoholization outcome even if its role is qualitatively assessed in the routine. Purpose The aim was to quantitatively assess MS during VOM-EI and to evaluate its association with MI block achievement. Methods Consecutive patients undergoing catheter ablation for PeAF at our Institution from February 2022 to May 2023 were considered. Patients with identifiable VOM were included. A proposed index of MS(MSI) was retrospectively calculated in each included patient. Correlation of MSI with low voltage zones(LVZ) extension after VOM-EI and its association with MI block achievement were assessed. Results 42 patients out of 49 (85.8%) had an identifiable VOM. MI block was successfully achieved in 35 patients out of 42 (83.3%). MSI was significantly associated with the occurrence of MI block (OR 1.24 (1.03 – 1.48); p = 0.022). A higher MSI resulted in reduced ablation time (p = 0.014) and reduced radiofrequency applications (p = 0.002) to obtain MI block. MSI was also associated with MI block obtained by endocardial ablation only (OR 1.07 (1.02 – 1.13); p = 0.002). MSI was highly correlated with newly formed LVZ extension (r = 0.776; p = 0.001). ATRIAL VOLTAGE ANALYSIS FOLLOWING VEIN OF MARSHALL ETHANOL INFUSION PREDICTS MITRAL LINE BLOCK

Abstract Background The vein of Marshall (VoM) is a promising target for atrial fibrillation (AF) treatment, matching with the ‘Coumel triangle‘ by housing triggers, autonomic connections, and serving as a substrate for perimitral flutters. Lateral mitral line (ML) represents a fundamental part for AF treatment, but achieving bidirectional block through endocardial ablations is challenging. VoM ethanol infusion (VOM–EI) facilitates ML block, with the newly–formed bipolar lesion serving as an effectiveness index. Voltage analysis assessment after VOM–EI in predicting ML block is poorly investigated. Purpose To compare unipolar and bipolar low–voltage areas (LVAs) along VOM trajectory after VOM–EI, and their role in predicting ML block. Methods We enrolled 59 patients undergoing catheter ablation of persistent AF or ML–dependent atrial flutter. High–density voltage mapping of left atrium was followed by VOM–EI and LA remapping. The area width difference was obtained and defined as ΔLVA. Normal bipolar voltage cutoffs were 0.50 mV in sinus rhythm or 0.29 mV in AF. Unipolar cutoffs were 2.7 mV and 1.1 mV, respectively. After VOM–EI, anatomical lesions included PVI, linear lesion for dome and ML isthmus. Systematic lines block validation was performed, defining ML block after coronary sinus (CS) electrograms sequence inversion (septal–to–lateral) during left atrial appendage pacing. Radiofrequency (RF) applications into the CS–great cardiac vein targeted epicardial gaps if present. Ablation time for ML block (AblTime) was obtained. Results In our cohort, 94.5% achieved ML block. Bipolar and unipolar low voltage areas after VOM–EI were 9,9 ± 6,9 cm2 and 12,2 ± 5,9 cm2 respectively. Bipolar ΔLVAs were significantly lower than unipolar ΔLVA (8.2 ± 6.5 cm2 vs. 9.4 ± 6.0 cm2; p=0.03) (Fig 1). A strong linear correlation between AblTime and bipolar ΔLVA (R: 0.76), and a significant correlation between AblTime and unipolar ΔLVA (R: 0.6) were found (Fig 2). Patients requiring RF applications into the CS for ML block (13/59, 22%) showed lower ΔLVAs in logistic regression for both bipolar (p<0.01) and unipolar (p=0.03) analyses. Conclusions VOM–EI induces unipolar LVAs wider than bipolar along the mitral isthmus trajectory. Unipolar and bipolar voltage analysis predicts a short time ML block achievement. Moreover, wider unipolar and bipolar LVAs following VOM–EI correlates with higher likelihood to avoid the targeting of epicardial gaps via the CS musculature.

Vein of Marshall Ethanol Infusion for AF Ablation; A Review.

The outcomes of persistent atrial fibrillation (AF) ablation are modest with various adjunctive strategies beyond pulmonary vein isolation (PVI) yielding largely disappointing results in randomised controlled trials. Linear ablation is a commonly employed adjunct strategy but is limited by difficulty in achieving durable bidirectional block, particularly at the mitral isthmus. Epicardial connections play a role in AF initiation and perpetuation. The ligament of Marshall has been implicated as a source of AF triggers and is known to harbour sympathetic and parasympathetic nerve fibres that contribute to AF perpetuation. Ethanol infusion into the Vein of Marshall, a remnant of the superior vena cava and key component of the ligament of Marshall, may eliminate these AF triggers and can facilitate the ease of obtaining durable mitral isthmus block. While early trials have demonstrated the potential of Vein of Marshall 'ethanolisation' to reduce arrhythmia recurrence after persistent AF ablation, further randomised trials are needed to fully determine the potential long-term outcome benefits afforded by this technique.

Clinical effect of vein of Marshall ethanol infusion on mitral isthmus ablation.

This study aimed to investigate the effect of Marshall ethanol infusion (VOM-Et) in the vein on mitral isthmus (MI) ablation. Patients with persistent atrial fibrillation (AF) were grouped into vein of VOM-Et combined with radiofrequency (RF) ablation (VOM-Et-RF) and RF groups. The primary outcome was MI block immediate block rate after surgery. Stratified analysis was also performed for factors affecting the outcome measures. A total of 118 consecutive patients underwent AF ablation at Taizhou Hospital of Zhejiang Province from January 2018 to December 2021. Successful bidirectional perimitral block was achieved in 96% of patients in VOM-Et-RF (69 of 72) and in 76% of patients in the RF group (35 of 46) (P < 0.01). In the subgroup analysis, male sex, elder than 60 years, Left atrial diameter <55 mm, and AF duration <3 years were associated with the benefits of VOM-Et in AF Patients. The vein of Marshall ethanol infusion for catheter ablation can improve the MI block rate. Male sex, elder age, smaller Left atrial diameter and shorter AF duration may have significant benefits for VOM-Et.

Use of intracardiac echocardiography in vein of Marshall ethanol infusion for ablation of persistent atrial fibrillation

BackgroundFew methods have been reported to demonstrate real-time effects during vein of Marshall (VOM) ethanol infusion in persistent atrial fibrillation (PeAF). ObjectiveThis study was to evaluate the impact of left atrial (LA) monitoring using intracardiac echocardiography (ICE) during VOM ethanol infusion. MethodsSeventy-four consecutive patients with PeAF who underwent VOM ethanol infusion followed by radiofrequency (RF) ablation were included. Patients with findings on ICE consistent with echogenic streaming in the LA and with increased myocardial local echogenicity along the VOM area were placed into one group (group A) and those without into the other group (group B). Outcomes between the 2 groups were compared. ResultsForty-six patients (62%) were placed into group A. A new ethanol-induced low-voltage area in group A was larger than that in group B (8.5 cm2 [5.5–10.2 cm2] and 4.0 cm2 (2.4–6.3 cm2]; P < .001). The RF ablation time required to achieve MI block was reduced in group A patients (263.0 seconds [196.0–351.0 seconds] vs 417.0 seconds [315.0–709.5 seconds] in group B patients; P < .001). MI block was achieved in 46 patients (100%) via an endocardial approach in group A and 27 patients (96.4%) in group B (extra coronary sinus ablation in 4 patients). One patient developed clinically significant pericardial effusions and required pericardiocentesis in group B. ConclusionPresence of increased myocardial local echogenicity at the ridge and consistent echogenic streaming in the LA detected by ICE-based imaging during VOM ethanol infusion suggests increased ablated tissue in that region and lower RF ablation time during ablation for PeAF.

Efficacy and safety of the vein of Marshall ethanol infusion with radiofrequency catheter ablation for the treatment of persistent atrial fibrillation in elderly patients.

Increasing age is a significant risk factor for atrial fibrillation (AF) recurrence after catheter ablation (CA). We accomplished this study to evaluate the efficacy and safety of the vein of Marshall (VOM) ethanol infusion (VOM-EI) with CA in elderly patients with persistent AF (PsAF). This retrospective observational study included 360 consecutive adult patients with PsAF, of which 141 were in the Elder group (age ≥65 years) and 219 were in the Younger group (age <65 years), who underwent the VOM-EI and radiofrequency CA (RFCA) between May 2020 and April 2022. The efficacy endpoint was no recurrence of AF within one year after CA. The VOM-EI was successfully performed in 90.8% of patients from the Elder and 88.6% from the Younger group. All patients achieved PVI; 97.9% of patients from the Elder and 98.6% from the Younger group reached LA roof block, and 93.6% of patients from the Elder and 95.9% from the Younger group achieved MI block. There was no significant difference in 1-year survival without recurrence of AF between the two groups (83.0% and 84.5%, respectively). The incidence of complications within 30 days after the procedure from the two groups was low and did not differ significantly. The VOM-EI combined with RFCA proved to be an effective and safe strategy for treating PsAF in elderly and younger patients.

Acute mitral isthmus block during catheter ablation with vein of Marshall ethanol infusion: Angiographic considerations

Achieving bidirectional mitral isthmus block is still challenging. Conventional ablation methods involve radiofrequency applications on the endocardial aspect of the lateral mitral isthmus, and often epicardial applications inside the coronary sinus. To evaluate the impact of the systematic use of ethanol infusion in the vein of Marshall on the achievement of acute mitral isthmus block of additional epicardial component lesion. We evaluated patients referred to two centres for long-standing persistent atrial fibrillation ablation or recurrent peri-mitral flutter. All patients had pulmonary vein isolation and mitral isthmus line using ethanol infusion in the vein of Marshall for the first procedure and additional radiofrequency ablation lesion if necessary. For redo procedures, additional ablations (atrial lines and complex fractionated atrial electrogram ablations, if needed) were also performed. We included 149 patients, and ethanol infusion in the vein of Marshall was not performed in 27 patients (18%). Among 122 patients, 115 had long-standing persistent atrial fibrillation (94.2%) and seven had peri-mitral flutter (5.8%). The mean duration of continuous atrial fibrillation was 53 months before ablation. Acute bidirectional mitral isthmus block was obtained in 115 (94.2%) of the 122 patients who received ethanol infusion in the vein of Marshall (77% when considering the total population). The mean radiofrequency delivery time to obtain mitral isthmus block was 2.6minutes for the endocardial mitral isthmus radiofrequency ablation and 2.6minutes for the epicardial mitral isthmus radiofrequency ablation. Failure to obtain mitral isthmus block was associated with increased mitral isthmus length and left atrial dilation. No major complications related to ethanol infusion in the vein of Marshall were observed. Ethanol infusion in the vein of Marshall, when feasible (82%), was a safe approach to obtaining a high success rate (94%) of acute bidirectional endocardial and epicardial mitral isthmus block.

The vein of Marshall (VoM) is a promising therapeutic target because it contains triggers and autonomic parasympathetic and sympathetic activity implicated in arrhythmogenesis of AF. Moreover, as the VoM colocalizes with the trajectory of the "mitral isthmus", alcohol infusion into the vein facilitates bidirectional block across the line eliminating protected epicardial connections (1,2). Purpose We evaluated acute impact on lesion formation post-VOM-ETHO and the mitral line block validation after an approach including VOM-ETHO, pulmonary vein isolation (PVI), roof-line, mitral line (ML) and cavo-tricuspid isthmus line in a population of PeAF patients (Figure 1). We aimed also to report procedural outcomes after a short follow-up period. Methods After a detailed electroanatomical map of the left atrium (LA) (filter at 0.05-0.5 mV if the patient was in sinus rhythm or 0.05-0.3 mV in the case of AF), we thus proceeded with the VoM-ETHO using the technique previously described (3,4). LA map was thus repeated to assess the extension of the newly-formed low voltage area (LVA). According to the newly-formed LVAs, mitral isthmus ablation was completed endocardially. PVI, roof-line and cavo-tricuspid isthmus line were performed to complete the ablation setting. Bidirectional block across line was then validated during pacing from left atrial appendage. In case of persistence of conduction through the mitral line due to epicardial gaps, additional ablations were applied in the "anchored wall" or in the "free-wall" of the great cardiac vein (GCV). Results Twenty-one PeAF patients (67±6 years; 67% male) underwent ablation. The medium value of basal LA-LVAs was 3.1±63.8 cmq and the newly-formed LVAs after the VoM-ETHO procedure was 12.35±67.28 cmq. All patients had bidirectional block validated across the roof-line. Bidirectional block of the ML was achieved in 19/21 patients: in 12/21 patients after endocardial line only and in 9/21 after epicardial gaps ablations into the coronary sinus. The ML procedural time was 11.3±65.9 minutes. No major complications occurred. One patient had mild pericardial effusion due to VoM perforation with a spontaneous resolution. After a short follow-up period (6±63 months), no relapse of AF was observed in 19/21 (90%) patients. 12/21 patients were free from antiarrhythmic drugs (AADs). Interestingly, recurrences happened in patients in whom bidirectional block of the mitral line was not achieved. Conclusions VoM ethanol ablation added to PVI and linear lesions in the context of a methodical and anatomical approach seems to have promising results in PeAF patients. This strategy seems to be safe and reproducible. A favourable outcome depended on the interventional setting completion and the procedural end-points validation is crucial.

Efficacy and feasibility of vein of Marshall ethanol infusion during persistent atrial fibrillation ablation: A systematic review and meta-analysis.

Catheter ablation (CA) is currently used to treat persistent atrial fibrillation (PeAF). However, its effectiveness is limited. This study aimed to estimate the effectiveness of the vein of Marshall absolute ethanol injection (VOM-EI) for PeAF ablation. Adjunctive vein of Marshall ethanol injection (VOM-EI) strategies are more effective than conventional catheter ablation (CA) and have similar safety outcomes. We extensively searched the literature for studies evaluating the effectiveness and safety of VOM-EI + CA compared with CA alone. The primary endpoint was the rate of acute bidirectional block of the isthmus of the mitral annulus (MIBB). The secondary endpoints were atrial fibrillation (AF) or atrial tachycardia (AT) recurrence over 30seconds after a 3-month blanking period. Weighted pooled risk ratios (RRs) and corresponding 95% confidence intervals (CIs) were calculated using a random effects model. Based on the selection criteria, nine studies were included in this systematic review, including patients with AF (n = 2508), persistent AF (n = 1829), perimitral flutter (n = 103), and perimitral AT (n = 165). There were 1028 patients in the VOM-EI + CA group and 1605 in the CA alone group. The VOM-EI + CA group showed a lower rate of AF/AT relapse (RR = 0.70; 95% CI = 0.53-0.91; p = .008) and a higher rate of acute MIBB (RR = 1.29; 95% CI = 1.11-1.50; p = .0007) than the CA alone group. Our meta-analysis revealed that adjunctive VOM-EI strategies are more effective than conventional CA and have similar safety outcomes.

Impact of Vein of Marshall Ethanol Infusion Combined with Anatomical Ablation for the Treatment of Persistent Atrial Fibrillation: A Long-Term Follow-Up Based on Implantable Loop Recorders.

The best ablation treatment for persistent atrial fibrillation (PeAF) patients is still debated. The vein of Marshall (VOM) seems to be a promising target for ablation and could be combined with a linear set of ablation lesions. The aim of our study is to evaluate the incidence of AF recurrences in a PeAF population treated with a comprehensive ablation approach consisting of VOM ethanol infusion (EI), pulmonary vein isolation (PVI), a left atrial (LA) roofline, a mitral line (guided by the newly formed lesion after alcohol infusion into the VOM and validated by pacing), and a cavotricuspid isthmus line. Consecutive patients undergoing the first ablation procedure of catheter ablation (CA) for PeAF were enrolled. All patients underwent VOM-EI, PVI, and ablation lines along the roof of the LA, mitral, and cavotricuspid isthmus. LA voltage mapping before and after VOM-EI was also performed. An implantable loop recorder (ILR) was implanted at the end of the ablation in each patient. Thirty-one consecutive patients (66 ± 8 years and 71% male) affected by PeAF were included in this study. The VOM-EI procedural phase lasted 21.4 ± 10.1 min. PV isolation and lines were validated in all subjects. The ML block was achieved within 10.8 ± 8.7 min. At a mean follow-up of 12 ± 7 months, 27 out of 31 (87%) patients remained free from AT/AF recurrences. Among the patients with recurrences, two (50%) had incomplete ablation lesions and three (75%) had "suboptimal" VOM-EI. In 23/31 patients (74%), antiarrhythmic drugs (AADs) were discontinued after 1 month of follow-up. No significant complications were reported during the follow-up. this single-center experience demonstrates that VOM-EI systematically combined with an anatomical ablation set in patients with PeAF resulted in feasible, safe, and effective freedom from AF/AT recurrences in 87% of the population after a 1-year follow-up period according to an ILR.

Double-wire technique to facilitate vein of Marshall cannulation and ethanol infusion in atrial fibrillation: a case series

BackgroundThe vein of Marshall (VOM) ethanol infusion is increasingly performed in combination with catheter ablation in atrial fibrillation (AF). The cannulation of the VOM can sometimes be challenging. This study aimed to evaluate the double-wire technique in cases of difficult cannulation of the VOM.Case presentationPatients with AF scheduled for combined catheter ablation and VOM ethanol infusion were consecutively enrolled. The procedure was performed via the femoral vein. If the regular cannulation technique with one angioplasty wire failed or took more than 20 min, the double-wire technique using a stabilizing wire and a cannulation wire was performed. The unique technique was used mainly in two scenarios, when the Eustachian ridge was too prominent as a barrier for catheter manipulation or when the VOM ostium was close to the coronary sinus ostium. Of 162 patients scheduled for VOM ethanol infusion, the double-wire technique was applied in 6 (3.7%) patients and led to a 100% successful cannulation rate of the VOM. Of the six patients, two had a prominent Eustachian ridge, and four had a VOM ostium close to the coronary sinus ostium. The mean cannulation time was 33.3 ± 7.3 min. The ethanol infusion was successfully performed in 5 patients. One patient had a collateral circulation in the distal VOM, and ethanol infusion was not performed.ConclusionsThe double-wire technique can facilitate VOM cannulation and ethanol infusion in challenging cases.Word count: 231.

Angiographic assessment of vein of Marshall in atrial fibrillation: Implications for identification and cannulation

BackgroundThe vein of Marshall (VOM) ethanol infusion improves rhythm control in atrial fibrillation (AF). The identification and cannulation of the VOM can be technically challenging. This study aimed to assess the angiographic morphology of the VOM and investigate its value in the VOM ethanol infusion. MethodsPatients with AF (n = 162) scheduled for combined catheter ablation and VOM ethanol infusion were enrolled. The VOM morphologic features in the right anterior oblique (RAO), the left anterior oblique (LAO), and the LAO cranial views were analyzed. The impact of morphology on the identification and cannulation of the VOM was investigated. ResultsThe VOM was identified in 159 (98.1 %) and cannulated in 150 (92.6 %) patients. The VOM identification rate in the RAO and LAO/LAO cranial view was 97.3 % and 89.3 %, respectively. Of 134 patients with VOM identification in the LAO/LAO cranial view, 104 (77.6 %) had a VOM ostium clock location (VOMoClock) of ≤3 and 3–4 o'clock. The VOM cannulation success rate in the ≤3, 3–4, 4–5, and 5–6 o'clock groups was 100 %, 92.6 %, 88.5 %, and 77.8 %, respectively (p = 0.032). The median (interquartile range) cannulation time in the four groups was 10.5 (6.3), 12.0 (9.0), 13.0 (23.0), and 34.0 (30.0) minutes, respectively (p < 0.001). The diameter of the coronary sinus ostium in the RAO view and the VOMoClock were independent predictors for difficult cannulation. ConclusionsThe VOM morphologic features in different angiographic views provide valuable information which could facilitate the identification and cannulation of the VOM.

Impact of vein of Marshall ethanol infusion on left atrial function: an MRI study

Abstract Funding Acknowledgements Type of funding sources: None. Background Previous data suggested that left atrial (LA) function may be impaired by extensive atrial fibrillation (AF) ablation. Ethanol infusion of Vein of Marshall (EI-VOM) as an adjunctive therapy to pulmonary vein isolation is an emerging therapeutic strategy that has shown to improve efficacy in persistent AF ablation procedures and mitral isthmus line. The impact of EI-VOM scar formation on LA function has not been addressed. Purpose To analyse the effect of EI-VOM on LA function, evaluated by magnetic resonance imaging (MRI) strain parameters. Methods Twelve patients referred for de novo persistent AF or any repeat procedure were included. MRI was performed previous (baseline) and 3-month after AF ablation. MRI images were analyzed off-line with specialized post-processing software. LA volumes, LA strain parameters, including LA global longitudinal strain (GLS), LA reservoir (LASr), conduit (LAScd) and contractile (LASct) strain; LA ejection fraction (LAEF), LA passive emptying function (LAPEF) and LA active emptying function (LAAEF) were measured from SSFPs two-chamber cine sequences. Results Two patients were excluded due to absence of VOM and failed ethanolization. Ten patients (83%) underwent both MRI studies (64 [55-71]years; 80% men; de novo ablation of persistent AF (50%)). Six patients received additional mitral isthmus line and three posterior wall isolation. Mean baseline indexed LA volume was 62.7ml/m2 [47-73]. Baseline LA strain and EF parameters were: LA GLS 17.16±15.3, LASr 17.80±14.9%, LAScd -5.96±3.7%, LASct -10.35±9.5%; LAPEF 5.89±9.8, LAAEF 23.87±17.0 and LAEF 27.6±18.3%. At 3-months follow-up, no AF recurrences were detected. No differences were found regarding LA volume (57.6ml/m2 [42-64], p=0.671), strain parameters (LA GLS Δ0.62, p=0.903; LASr Δ0.02, p=0.991; LAScd Δ-0.84, p=0.563; LASct Δ0.54, p= 0.874) and LAEF parameters (LAPEF Δ4.82, p=0.592; LAAEF Δ3.03, p=0.903; and LAEF Δ7.0, p=0.213). Conclusions EI-VOM appears as a safe adjunctive strategy in AF ablation in terms of LA function. The present study did not show impairment in the main MRI strain parameters.
