Neurofilament Light Chain as a Biomarker of Global Cognition in Individuals With Possible Vascular Mild Cognitive Impairment.

Neurofilament Light Chain (NfL) is a biomarker of axonal injury elevated in mild cognitive impairment (MCI) and Alzheimer's disease dementia. Blood NfL also inversely correlates with cognitive performance in those conditions. However, few studies have assessed NfL as a biomarker of global cognition in individuals demonstrating mild cognitive deficits who are at risk for vascular-related cognitive decline. To assess the relationship between blood NfL and global cognition in individuals with possible vascular MCI (vMCI) throughout cardiac rehabilitation (CR). Additionally, NfL levels were compared to age/sex-matched cognitively unimpaired (CU) controls. Participants with coronary artery disease (vMCI or CU) were recruited at entry to a 24-week CR program. Global cognition was measured using the Montreal Cognitive Assessment (MoCA) and plasma NfL level (pg/ml) was quantified using a highly sensitive enzyme-linked immunosorbent assay. Higher plasma NfL was correlated with worse MoCA scores at baseline (β = -.352, P = .029) in 43 individuals with vMCI after adjusting for age, sex, and education. An increase in NfL was associated with worse global cognition (b[SE] = -4.81[2.06], P = .023) over time, however baseline NfL did not predict a decline in global cognition. NfL levels did not differ between the vMCI (n = 39) and CU (n = 39) groups (F(1, 76) = 1.37, P = .245). Plasma NfL correlates with global cognition at baseline in individuals with vMCI, and is associated with decline in global cognition during CR. Our findings increase understanding of NfL and neurobiological mechanisms associated with cognitive decline in vMCI.

Diabetes and vascular mild cognitive impairment among Chinese ≥50 years: A cross-sectional study with 2020 participants.

With the decline of cognitive function in vascular cognitive impairment, the burden on the family and society will increase. Therefore, early identification of vascular mild cognitive impairment (VaMCI) is crucial. The focus of early identification of VaMCI is on the attention of risk factors. Therefore, this study aimed to investigate the relationship between diabetes and VaMCI among the Chinese, hoping to predict the risk of VaMCI by diabetes and to move the identification of vascular cognitive impairment forward. We collected data from seven clinical centers and nine communities in China. All participants were over 50 years of age and had cognitive complaints. We collected basic information of the participants, and cognitive function was professionally assessed by the Montreal Cognitive Assessment scale. Finally, logistic regression analysis was used to analyze the correlation between each factor and VaMCI. A total of 2020 participants were included, including 1140 participants with VaMCI and 880 participants with normal cognition. In univariate logistic regression analysis, age, heavy smoking, and diabetes had a positive correlation with VaMCI. At the same time, being married, high education, and light smoking had a negative correlation with VaMCI. After correction, only diabetes (OR=1.04, 95% CI: 1.01-1.09, p=0.05) had a positive correlation with VaMCI, and high education (OR=0.60, 95% CI:.45-.81, p=0.001) had a negative correlation with VaMCI. In our study, we found that diabetes had a positive correlation with VaMCI, and high education had a negative correlation with VaMCI. Therefore, early identification and timely intervention of diabetes may reduce the risk of VaMCI and achieve early prevention of VaMCI.

The AHA/ASA and DSM-V diagnostic criteria for vascular cognitive impairment identify cases with predominant vascular pathology.

There are major challenges in determining the etiology of vascular cognitive impairment (VCI) clinically, especially in the presence of mixed pathologies, such as vascular and amyloid. Most recently, two criteria (American Heart Association/American Stroke Association (AHA/ASA) and Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V)) have been proposed for the clinical diagnosis of VCI but have not as yet been validated using neuroimaging. This study aims to determine whether the AHA/ASA and DSM-V criteria for VCI can distinguish between cases with predominantly vascular pathology and cases with mixed pathology. A total of 186 subjects were recruited from a cross-sectional memory clinic-based study at the National University Hospital, Singapore. All subjects underwent clinical and neuropsychological assessment, magnetic resonance imaging (MRI) and carbon 11-labeled Pittsburgh Compound B ([11C] PiB) positron emission tomography (PET) scans. Diagnosis of the etiological subtypes of VCI (probable vascular mild cognitive impairment (VaMCI), possible VaMCI, non-VaMCI, probable vascular dementia (VaD), possible VaD, non-VaD) were performed following AHA/ASA and DSM-V criteria. Brain amyloid burden was determined for each subject with standardized uptake value ratio (SUVR) values ⩾1.5 classified as amyloid positive. Using κ statistics, both criteria had excellent agreement for probable VaMCI, probable VaD, and possible VaD (κ = 1.00), and good for possible VaMCI (κ = 0.71). Using the AHA/ASA criteria, the amyloid positivity of probable VaMCI (3.8%) and probable VaD (15%) was significantly lower compared to possible VaMCI (26.7%), non-VaMCI (33.3%), possible VaD (73.3%), and non-VaD (76.2%) (p < 0.001). Similarly, using the DSM-V criteria, the amyloid positivity of probable VaMCI (3.8%) and probable VaD (15%) was significantly lower compared to possible VaMCI (26.3%), non-VaMCI (32.1%), possible VaD (73.3%), and non-VaD (76.2%) (p < 0.001). In both criteria, there was good agreement in differentiating individuals with non-VaD and possible VaD, with significantly higher (p < 0.001) global [11C]-PiB SUVR, from individuals with probable VaMCI and probable VaD, who had predominant vascular pathology. The AHA/ASA and DSM-V criteria for VCI can identify VCI cases with little to no concomitant amyloid pathology, hence supporting the utility of AHA/ASA and DSM-V criteria in diagnosing patients with predominant vascular pathology. Selective vulnerability of hippocampal sub-regions in patients with subcortical vascular mild cognitive impairment.

Early diagnosis of subcortical vascular mild cognitive impairment (svMCI) is clinically essential because it is the most reversible subtype of all cognitive impairments. Since structural alterations of hippocampal sub-regions have been well studied in neurodegenerative diseases with pathophysiological cognitive impairments, we were eager to determine whether there is a selective vulnerability of hippocampal sub-fields in patients with svMCI. Our study included 34 svMCI patients and 34 normal controls (NCs), with analysis of T1 images and Montreal Cognitive Assessment (MoCA) scores. Gray matter volume (GMV) of hippocampal sub-regions was quantified and compared between the groups, adjusting for age, sex, and education. Additionally, we explored correlations between altered GMV in hippocampal sub-fields and MoCA scores in svMCI patients. Patients with svMCI exhibited selectively reduced GMV in several left hippocampal sub-regions, such as the hippocampal tail, hippocampal fissure, CA1 head, ML-HP head, CA4 head, and CA3 head, as well as decreased GMV in the right hippocampal tail. Specifically, GMV in the left CA3 head was inversely correlated with MoCA scores in svMCI patients. Our findings indicate that the atrophy pattern of patients with svMCI was predominantly located in the left hippocampal sub-regions. The left CA3 might be a crucial area underlying the distinct pathophysiological mechanisms of cognitive impairments with subcortical vascular origins.

A Nomogram Including Total Cerebral Small Vessel Disease Burden Score for Predicting Mild Vascular Cognitive Impairment in Patients with Type 2 Diabetes Mellitus.

Total cerebral small vessel disease (CSVD) burden score is an important predictor of vascular cognitive impairment (VCI). However, few predictive models of VCI in type 2 diabetes mellitus (T2DM) patients have included the total CSVD burden score, especially in the early stage of VCI. To develop and validate a nomogram that includes the total CSVD burden score to predict mild VCI in patients with T2DM. A total of 322 eligible participants with T2DM who were divided into mild and normal cognitive groups were enrolled in this retrospective study. Demographic data, laboratory data and imaging markers of CSVD were collected. The total CSVD burden score was calculated by combining the different CSVD markers. Step-backward multivariable logistic regression analysis with the Akaike information criterion was applied to select significant predictors and develop a best-fit predictive nomogram. The performance of the nomogram was assessed in terms of discriminative ability, calibrated ability, and clinical usefulness. The nomogram model consisted of five variables: age, education, hemoglobin A1c level, serum homocysteine level, and total CSVD burden score. A nomogram with these variables showed good discriminative ability (area under the receiver operating characteristic curve was 0.801 in internal verification). In addition, the Hosmer-Lemeshow test (χ2 =9.226, P=0.417) and bootstrap-corrected calibration plot indicated that the nomogram had good calibration. The Brier score of the predictive model was 0.178. Decision curve analysis demonstrated that when the threshold probability ranged between 16% and 98%, the use of the nomogram to predict mild VCI in patients with T2DM provide a greater net benefit. The nomogram, composed of age, education, stroke, HbA1c level, Hcy level, and total CSVD burden score, had good predictive accuracy and may provide clinicians with a practical tool for predicting the risk of mild VCI in T2DM patients.

Sex-specific neuropsychological correlates of apathy and depression across neurodegenerative disorders.

Apathy and depression are common neuropsychiatric symptoms across neurodegenerative disorders and are associated with impairment in several cognitive domains, yet little is known about the influence of sex on these relationships. We examined the relationship between these symptoms with neuropsychological performance across a combined cohort with mild or major neurodegenerative disorders, then evaluated the impact of sex. We conducted a cohort analysis of participants in the COMPASS-ND study with mild cognitive impairment (MCI), vascular MCI, Alzheimer's disease, mixed dementia, Parkinson's disease, frontotemporal dementia, and cognitively unimpaired (CU) controls. Participants with neurodegenerative disease and CU controls were stratified by the presence (severity ≥1 on Neuropsychiatric Inventory Questionnaire) of either depressive symptoms alone, apathy symptoms alone, both symptoms, or neither. A neuropsychological battery evaluated executive function, verbal fluency, verbal learning, working memory, and visuospatial reasoning. Analysis of covariance was used to assess group differences with age, sex, and education as covariates. Groups included depressive symptoms only (n=70), apathy symptoms only (n=52), both (n=68), or neither (n=262). The apathy and depression+apathy groups performed worse than the neither group on tests of working memory (t(312) =-2.4, p=0.02 and t(328) =-3.8, p=0.001, respectively) and visuospatial reasoning (t(301) =-2.3, p=0.02 and t(321) =-2.6, p=0.01, respectively). The depression, apathy, and depression+apathy groups demonstrated a similar degree of impairment on tests of executive function, processing speed, verbal fluency, and verbal learning when compared to participants without apathy or depression. Sex-stratified analyses revealed that compared to the male neither group, the male apathy and depression+apathy groups were impaired broadly across all cognitive domains except for working memory. Females with depression alone showed deficits on tests of executive function (t(166) =2.4, p=0.01) and verbal learning (t(167) =-4.3, p=0.001) compared to the female neither group. This study demonstrated that in neurodegenerative diseases, apathy with or without depression in males was associated with broad cognitive impairments. In females, depression was associated with deficits in executive function and verbal learning. These findings highlight the importance of effectively treating apathy and depression across the spectrum of neurodegenerative disorders with the goal of optimizing neuropsychological outcomes.

Abstract WMP32: Effect of Immersive Motor-Cognitive Training on Executive Function in Patients With Vascular Mild Cognitive Impairment: Aquasi-Experimental Study

Background: Cerebrovascular disease is a common cause of vascular cognitive impairment (VCI) and may progress to vascular dementia at a relatively fast rate. Strengthening intervention in patients with mild VCI can delay cognitive decline and prevent vascular dementia. Purpose: To investigate the effect of an immersive motor-cognitive training program on executive function in patients with mild VCI. Methods: A total of 60 patients with mild VCI were divided into two groups: the intervention group (n=30) and control group (n=30). The participants were recruited from an outpatient Neurology clinic between May 2021 and January 2022. The intervention group was received immersive motor-cognitive training for 2 months, while the control group was received traditional motor-cognitive training during the same duration. Before the intervention and at the end of the first and second months of the intervention, executive function was assessed as the primary outcome using the Trail-Making Test (TMT)-A/B. General cognitive, speech function and balance function were evaluated as secondary outcomes using the Mini-Mental State Examination (MMSE)/Montreal Cognitive Assessment (MoCA), Boston Naming Test, and Berg Balance Scale, respectively. Data analysis was performed using independent sample t-test, Chi-Square test, nonparametric test, and generalized estimation equation model. Results: Among 60 patients, 54 were completed the 2 month-follow-up. Our results showed that there is a significant time effect ( F =14.36, P =0.001) regarding the TMT-A score. In addition, significant time (F=7.28, P =0.001) and interaction effects (F=7.43, P =0.001) on the TMT-B score were noted. The MMSE score showed significant time (F=64.66, P =0.001) and interaction effects (F=12.01, P =0.002), and similar results were observed for the MoCA score with significant time (F=38.29, P=0.001) and interaction effects (F=22.89, P =0.001). The time effect of the Boston Naming Test score was also found to be statistically significant ( F =2.92, P =0.003). However, no significant differences were observed in the Berg Balance Scale score. Conclusion: Immersive motor-cognitive training may improve the executive function and general cognitive function in patients with mild VCI.

Cognitive Profiles of Mild Traumatic Brain Injury and mild Vascular Cognitive Impairment: A Comparative Study

Abstract Objective The cognitive profile of any neurological disorder is very important throughout the duration of a person's treatment. It has a role in diagnosis, prognosis, and even after remission of active symptoms. It is a common trend among clinicians to monitor and compare the cognitive profiles of different disease conditions to locate the area of maximum dysfunction, with respect to a particular diagnosis. This study correlates the cognitive profiles of mild traumatic brain injury (mTBI) and mild vascular cognitive impairment (mVCI). Methods The study population comprised 30 mTBI and 30 mVCI patients medically diagnosed by a neurologist. The patients were been selected from the neuromedical outpatient department (OPD) and neurosurgery OPD of the Sree Chitra Tirunal Institute for Medical Sciences and Technology (SCTIMST), Trivandrum. Simple random sampling had been used to select the sample. The patients diagnosed with the stated disease conditions were referred for neuropsychological assessment. Testable and cooperative patients were recruited for the study. To stabilize the data and for a transparent comparison, 30 healthy controls with no medically diagnosed illnesses were also added to the study. The results were analyzed using R. Result and Conclusion The study concluded that cognitive profiles of mTBI and mVCI patients were significantly different from the cognitive profiles of healthy controls, but there was no statistically significant difference between the cognitive profiles of mVCI and mTBI patients except in confrontation naming and recognition memory.

Associations of white matter hyperintensities with cerebrovascular architecture in Alzheimer’s disease and related dementias

AbstractBackgroundMRI‐detected white matter hyperintensities (WMHs), a marker of cerebrovascular‐pathology, are common in Alzheimer’s disease (AD) and related dementias (ADRDs). In stroke, WMH‐volume was found to correlate most with stroke risk‐factors in the anterior cerebral arterial‐territory. Since the relationship between WMHs and arterial‐territories remains unexplored in the ADRDs, we systematically investigated WMHs’ spatial distribution (1) in 8 ADRDs relative to cognitively unimpaired (CU), and (2) across arterial‐territories per ADRD.MethodsThe extensive CCNA‐COMPASS‐ND cohort (N = 927) allowed us to investigate 9 clinical categories: CU, subjective (SCI) and mild (MCI) cognitive impairment, vascular MCI (V‐MCI), AD, vascular AD (V‐AD), Lewy‐body dementia (LBD), fronto‐temporal dementia (FTD), and Parkinson’s disease (PD) (Fig.1A). FLAIR and T1w MRI‐scans were used to segment WMHs (Dadar et al.,2017). Following registration to standard space, an arterial‐territory atlas (Schirmer et al.,2019) was used to calculate WMH‐volumes in 10 regions (Fig.1B). Statistical analyses were run on whole‐brain‐WMH‐volume and regional‐WMH‐ratios (region‐size‐normalized‐WMH‐volume/whole‐brain‐WMH‐volume). Data‐analyses used a series of linear models accounting for clinical category, age, and sex – followed by pairwise group‐comparisons corrected for multiple comparisons.Results 1) Relative to CU: Whole‐brain‐WMH‐volumes were higher for V‐MCI, V‐AD, and FTD. WMH‐ratios were higher in MCA and LMCA for 2 ADRDs (SCI;V‐MCI) and lower in PCA, LPCA, and RPCA for SCI. WMH‐ratios were higher in ACA or LACA and lower in PCA, LPCA, or RPCA for 4 ADRDs (V‐MCI;V‐AD;FTD;PD) (Fig.1C). Sex‐analyses: Men’s WMH‐volumes were higher in whole‐brain (CU;V‐MCI;FTD), while their WMH‐ratios were higherin LMCA (CU), but lower in ACA (SCI;MCI), LACA (MCI), and RACA (SCI). 2) MCA contained higher WMH‐ratios than ACA and PCA in all clinical categories. Relative to PCA, ACA’s WMH‐ratio was higher in 2 ADRDs (V‐MCI;V‐AD) and lower in 4 (CU;MCI;AD;PD). Pairwise‐group comparisons between all left/right arterial‐territories demonstrated consistent results (Fig.2). Asymmetry‐analyses: The right arterial side had higher WMH‐ratios in ACA (CU;V‐MCI), MCA (CU;SCI;MCI;V‐MCI;AD;V‐AD;PD), and PCA (MCI;AD).ConclusionWe identified arterial‐territories of increased susceptibility to WMH‐formation per ADRD. The two most prevalent ADRDs (AD;V‐AD) were found to accumulate WMHs in a territory‐specific manner – higher WMH‐ratios in ACA for V‐AD and PCA for AD. Overall, the arterial‐territory‐specific WMH‐signatures identified may improve ADRD‐diagnosis accuracy.

Changes in Brain Glutathione in patients with mild vascular cognitive impairment

AbstractBackgroundOxidative stress (OS) has been implicated in age‐related neurodegeneration and may be important in prodromal states such as mild vascular cognitive impairment (mVCI). Higher peripheral OS is reported in mVCI patients; however, the role of central antioxidant defenses in mVCI is unclear. Glutathione (GSH) is a major brain antioxidant, and so brain GSH in possible mVCI vs. controls was assessed.MethodPossible mVCI patients (1 standard deviation (SD) below population norms in verbal memory, executive function (EF), processing speed, or working memory, age 55‐85, and currently enrolled in a cardiac rehabilitation program due to having 2 or more vascular risk factors or previous vascular event) and cognitively‐normal (CN) controls are recruited from a 6‐month exercise rehabilitation program. All participants received 1H magnetic resonance spectroscopy (MEscher–GArwood Point Resolved Spectroscopy) to quantify brain GSH at baseline in the anterior cingulate (AC) and occipital region (OC). Spectroscopic analysis was completed using the Gannet toolkit (vers. 3.1) in Matlab (vers. 2020b), a cut‐off of 20% was used to reject spectra of poor quality.ResultIn 30 participants (mVCI n = 16, CN n = 14), anterior cingulate (AC) GSH (I.U. ± SD) was higher in mVCI (1.86 ± 0.45) compared to CN (1.63 ± 0.50) (F(1,18.8) = 4.6, P = 0.04), this difference became trending after correcting for cerebrospinal fluid volume ((F(1,18.2) = 3.9, P = 0.06). No difference was detected for OC‐GSH before or after correcting for cerebrospinal fluid volume. Higher AC‐GSH was correlated with lower Montreal Cognitive Assessment score (rho (26) = ‐0.42, p = 0.02).ConclusionPreliminary data suggest an upregulation in anterior cingulate glutathione in mVCI, which might reflect a compensatory increase in antioxidants as a response to increase peripheral oxidative stress previously reported in these patients. Recruitment is ongoing and additional participants are needed to reinforce findings.

Investigating the effects of aerobic exercise primed anodal transcranial direct current stimulation on γ‐aminobutyric acid concentrations in mild vascular cognitive impairment

AbstractBackgroundMild vascular cognitive impairment (mVCI) is characterized by minor deficits in cognition and memory due to underlying vascular pathology. Anodal transcranial direct current stimulation (atDCS), a non‐invasive neuromodulation technique, has been demonstrated to improve cognition. The effects of atDCS have been suggested to be mediated by γ‐aminobutyric acid (GABA)–a major inhibitory neurotransmitter associated with cerebral perfusion and oxygenation, and cognition and memory. However, previous studies have reported mixed results regarding changes in GABA concentrations following atDCS. Physical activity has been reported to increase cortical GABA levels, and in particular, aerobic exercise (AE) is associated with improved cognition in older adults. Preliminary data from an ongoing trial (EXPRESS‐V) was used to evaluate whether GABA concentration in the motor cortex was altered following AE and/or atDCS and whether this was associated with changes in global cognition in mVCI.MethodIn this double‐blind, randomized clinical trial, participants received one of four interventions for two weeks: AE and atDCS, AE and sham atDCS, exercise education and atDCS, or exercise education and sham atDCS. mVCI was determined based on a modified Gorelick criteria. At baseline and endpoint, GABA concentration in the motor cortex was measured using magnetic resonance spectroscopy and quantified with the Mescher‐Garwood point resolved spectroscopy pulse sequence, and global cognition was assessed using the Montreal Cognitive Assessment. Wilcoxon signed rank test was used to determine changes in GABA concentrations (ΔGABA). Spearman correlation was used to determine the association between ΔGABA and changes in global cognition (ΔMoCA).ResultTen participants have enrolled so far, with nine participants having completed the study (22.2% female, age = 66.4(9.8), years of education = 8.8(3.4), 55.6% White, baseline MoCA score = 22.8(3.1)). To date, ΔGABA was insignificant between baseline and endpoint (Z = 23.0, p = 0.953). However, a positive trend between ΔGABA and ΔMoCA was observed (ρ = 0.650, p = 0.058).ConclusionPreliminary results from nine participants showed a positive trend between ΔGABA and ΔMoCA. Since the trial is ongoing, with the target of recruiting 80 participants by 2024, analyses on unblinded data upon study completion will determine whether AE and atDCS leads to greater ΔGABA, and whether this is accompanied by greater ΔMoCA in mVCI.

Towards inclusive trial design for vascular cognitive impairment in the diverse context of India

AbstractBackgroundThe burden of vascular contribution to cognitive impairment and dementia is substantially higher in low and middle‐income countries like India compared to the high‐income countries (HIC). Nearly 40% of 5.3 million people with dementia are estimated to be due to vascular aetiology. This high burden of vascular cognitive impairment (VCI) is due to a greater prevalence of vascular risk factors such as diabetes, hypertension and stroke as well as low awareness and a wide treatment gap. Lack of validated diagnostic instruments in various regional languages and availability brain imaging pose challenges to diagnosis of VCI. These factors pose unique challenges to reducing the burden of VCI in India. Novel clinical trials are being implemented to risk of progression of VCI, but these efforts are mainly restricted to HIC. The limited availability of evidence for effective treatment and preventive strategies in the context of diverse countries like India are major impediments to effective public health interventions to reduce burden.MethodWe present results from a large cohort of 2000 individuals from multiple centres across the country including individuals at high risk of developing VCI, patients with vascular mild cognitive impairment and vascular dementia. Languages spoken by patients included Kannada, Telugu, Hindi, Malayalam, Urdu, Bengali and English among others. A were illiterate and around 60% were bilingual.ResultsHarmonised cognitive and functional measures adapted to multiple Indian languages and educational variability were used to detect VCI in the diverse population. MRI imaging to detect vascular imaging biomarkers was performed to detect infarcts, cortical atrophy, microbleeds and white matter lesions, for accurate identification of vascular damage and neurodegeneration. Collaborations between multiple centres in a systematic manner are ongoing. Novel public health initiatives with support of policy makers are also being implemented to improve brain health by increasing awareness and reducing risk of stroke and dementia in the community.ConclusionsThese large‐scale efforts towards developing clinical‐trial readiness of VCI cohorts in the community across India will be crucial to fill research evidence gaps and implement clinical trials in India in concurrence with ongoing global efforts.

Accelerated brain ageing in participants with higher cerebrovascular lesion load across aging, cognitive impairment, and mixed presentation

AbstractBackgroundWe aimed to investigate the association between cortical brain age and white matter hyperintensities (WMH) in diverse forms of clinically‐defined vascular conditions. Our hypothesis was that a higher WMH burden would be associated with higher cortical brain age in all clinical subtypes.MethodWe used standardized MRI data obtained from participants in the COMPASS‐ND cohort of the CCNA. We used a standard linear support vector regression algorithm to estimate brain age, matching chronological age to cortical anatomical measurements obtained with the FreeSurfer toolbox on T1‐weighted MRI, sex, and intracranial volume. We calculated brain‐PAD (i.e., predicted brain age minus real age) and applied bias adjustment to remove age‐dependency in the estimates. A validated automated technique utilizing T2‐weighted and fluid attenuated inversion recovery MRIs was used to compute WMH loads.ResultParticipants included 107 CIE, 240 mild cognitive impairment (MCI), 115 vascular‐MCI (V‐MCI), 81 probable Alzheimer’s disease (AD), and 50 V‐AD. There was a significant difference in brain‐PAD [ F (4,599) = 56, P &lt; 0.001, ANCOVA test] among groups, whilst adjusting for sex and chronological age. All four categories of patients exhibited a significantly higher mean brain‐PAD than CIE (P &lt; 0.001), with the AD cohort having the highest brain‐PAD. There was a significant difference in WMH loads [ F (4,586) = 48, P &lt; 0.001, ANCOVA test] between groups, whilst adjusting for sex and age. All cohorts showed a positive correlation between brain‐PAD and WMH, indicating accelerated ageing in people with higher WMHs. We observed moderate and significant correlations between brain‐PAD and WMH loads for V‐MCI (r = 0.34, P &lt; 0.001) and V‐AD (r = 0.36, P = 0.001) but weaker, while significant correlations in CIE (r = 0.20, P = 0.035) and AD cohorts (r = 0.24, P = 0.025).ConclusionWe observed a positive and significant link between brain‐PAD and WMH in all categories. This finding indicates on the importance of treatment and prevention strategies for vascular risk factors, which might be able to slow down the progression of cerebrovascular lesions and delay the effect on cortical thickness.

T1w/T2w MRI ratio brain signature across degenerative syndromes and their cognitive impairment continuum and mixed presentations

AbstractBackgroundWe aimed to investigate the ratio’s signature in diverse clinically‐defined neurodegenerative syndromes and their cognitive impairment continuum. Our hypothesis was that higher cognitive impairment would be associated with lower cortical T1w/T2w ratio in all clinical subtypes.MethodWe used standardized MRI data obtained from participants in the COMPASS‐ND study of the Canadian Consortium on Neurodegeneration in Aging. T1w images were resampled down to T2w resolution; after co‐registration, intensities were standardized between 1‐100 and the ratio computed. Brain segmentation was then performed using FreeSurfer 6.0 on T1w images with the Desikan‐Killiany‐Tourville and default subcortical atlases. Participants included 109 cognitively intact elderly (CIE), 131 subjective cognitive decline (SCD), 252 mild cognitive impairment (MCI), 113 vascular‐MCI (V‐MCI), 55 mixed vascular‐Alzheimer’s dementia (V‐AD), 76 Alzheimer’s disease (AD), 21 Lewy‐body dementia (LBD), 33 fronto‐temporal‐dementia (FTD), 77 Parkinson’s disease (PD), 35 PD‐MCI and nine PD with dementia (PDD). Since the between‐vendor distributions differed (Fig1), Z scores were computed separately for each scanner vendor (GE, Philips, or Siemens) with CIE as a reference group. ANCOVA with age as covariate and FDR correction were conducted between each group and CIE.ResultNo significant effect of age was observed. Compared to CIE, significantly higher T1w/T2w ratios were observed in SCD for most of the brain (57/86 regions), in MCI for six frontal areas only, while decline was observed in AD throughout the brain (Fig1). V‐MCI showed lower ratios in subcortical areas only while V‐AD had pronounced lower ratios in subcortical areas as well as in 11 cortical areas. PD had marked lower ratios throughout the brain with the exception of an intact corpus callosum (Fig2). Contrariwise, FTD only displayed a lower ratio in the corpus callosum whereas PD‐MCI and LBD did not show any significant difference with CIE.ConclusionWe observed clear distinct patterns of T1w/T2w ratios for SCD, MCI, AD and PD groups. Since this ratio does not seem specific to a neural biological substrate, the meaning of these differences remains to be determined. Nevertheless, these results suggest that simple MRI measures such as the T1w/T2w ratio can highlight underlying mechanisms in various neurodegenerative processes.

Characterization of various type of cognitive impairment using multiomics – a pilot imaging study from a local institution

AbstractBackgroundIn addition to clinical diagnosis, multi‐modal imaging has been employed for early detection of various types of dementia. Image features from various types of images provided information for accurate diagnosis. This study aimed to evaluate the role of multiple imaging, including MRI T1W, T2W, resting state functional MRI (rs‐fMRI) and 18‐F Flutemetamol PET image in diagnosis of various types of cognitive impairments.Methods56 participants (16 Alzheimer’s Disease (AD), 10 AD type Mild Cognitive impairment (A‐MCI), 18 vascular type MCI (V‐MCI), 12 Vascular dementia (VD) and 25 Healthy Control (HC) were recruited in an local memory clinic. All types of images were re‐orientated, segmentation and normalized to the whole brain mask template. 45 brain regional volumes (Vol) were calculated from T1w using Freesurfer. Brain regional white matter hyperintensities (WMH) were obtained from T2w images. Interhemispheric functional connectivity (IFC) of brain regions were measured from rs‐fMRI. Standard uptake value (SUV) were measured and calculated from the PET images. Also, patient demographics including age, sex and HK‐MoCa score (Demo) were recorded. 70% of each group of participants were assigned to the training group, whereas the rest 30% were assigned to test group. The neural network toolbox of MATLAB was employed to build four characterization models: Model 1 based Vol and WMH; Model 2 based on Vol, WMH and IFC; Model 3 based on Vol, WMH, IFC, and demo; Model 4 based on Vol, WMH, IFC, demo and SUV.ResultsThe receiver operating characteristics (ROC) curves showed that the Model 1, 2 and 3 achieved satisfactory accuracy, with 79%, 68% and 70% respectively. While Model 4 achieved excellent accuracy of 100%. The sensitivity and specificity of each model for each group was over 75% to 100%. The model with more image features attained higher accuracy in dementia diagnosis.ConclusionArtificial neural network model with multiomics can accurately differentiate AD, A‐MCI, V‐MCI, VD and HC. Further study is suggested with more participants for models verification.

Language Impairment in Vascular Dementia: A Clinical Review.

Vascular cognitive impairment (VCI) encompasses a wide range of conditions, including cognitive impairment associated with stroke or vascular brain injury, mild vascular cognitive impairment, and vascular dementia (VD). Knowledge of language impairment associated with VD is far less extensive than that of Alzheimer's disease. Although not prevalent in VD, impairment in language skills has been reported. A better understanding of the neurolinguistic features associated with the different presentations of VD could facilitate medical diagnosis. In this article, we report data on language impairment in VD, with particular attention to their primary or secondary functional origin. To better appreciate this functional origin, we also outline the main characteristics of impairment in other cognitive functions. Key elements that should be considered in the speech-language assessment of individuals with possible or proven VD are also highlighted.

Changes in Plasma Amyloid-β Level and Their Relationship With White Matter Microstructure in Patients With Mild Cognitive Impairment

Objective To investigate the changes in plasma amyloid-β (Aβ) level and their relationship with white matter microstructure in the patients with amnesic mild cognitive impairment(aMCI) and vascular mild cognitive impairment (vMCI).Methods A total of 36 aMCI patients,20 vMCI patients,and 34 sex and age matched healthy controls (HC) in the outpatient and inpatient departments of the First Affiliated Hospital of Anhui Medical University were enrolled in this study.Neuropsychological scales,including the Mini-Mental State Examination,the Montreal Cognitive Assessment,and the Activity of Daily Living Scale,were employed to assess the participants.Plasma samples of all the participants were collected for the measurement of Aβ42 and Aβ40 levels.All the participants underwent magnetic resonance scanning to obtain diffusion tensor imaging (DTI) data.The DTI indexes of 48 white matter regions of each individual were measured (based on the ICBM-DTI-81 white-matter labels atlas developed by Johns Hopkins University),including fractional anisotropy (FA) and mean diffusivity (MD).The cognitive function,plasma Aβ42,Aβ40,and Aβ42/40 levels,and DTI index were compared among the three groups.The correlations between the plasma Aβ42/40 levels and DTI index of aMCI and vMCI patients were analyzed.Results The Mini-Mental State Examination and the Montreal Cognitive Assessment scores of aMCI and vMCI groups were lower than those of the HC group (all P<0.001).There was no significant difference in the Activity of Daily Living Scale score among the three groups (P=0.654).The plasma Aβ42 level showed no significant difference among the three groups (P=0.227).The plasma Aβ40 level in the vMCI group was higher than that in the HC group (P=0.014),while it showed no significant difference between aMCI and HC groups (P=1.000).The plasma Aβ42/40 levels in aMCI and vMCI groups showed no significant differences from that in the HC group (P=1.000,P=0.105),while the plasma Aβ42/40 level was lower in the vMCI group than in the aMCI group (P=0.016).The FA value of the left anterior limb of internal capsule in the vMCI group was lower than those in HC and aMCI groups (all P=0.001).The MD values of the left superior corona radiata,left external capsule,left cingulum (cingulate gyrus),and left superior fronto-occipital fasciculus in the vMCI group were higher than those in HC (P=0.024,P=0.001,P=0.003,P<0.001) and aMCI (P=0.015,P=0.004,P=0.019,P=0.001) groups,while the MD values of the right posterior limb of internal capsule (P=0.005,P=0.001) and left cingulum (hippocampus) (P=0.017,P=0.031) in the aMCI and vMCI groups were higher than those in the HC group.In the aMCI group,plasma Aβ42/40 level was positively correlated with FA of left posterior limb of internal capsule (r=0.403,P=0.015) and negatively correlated with MD of the right fonix (r=-0.395,P=0.017).In the vMCI group,plasma Aβ42/40 level was positively correlated with FA of the right superior cerebellar peduncle and the right anterior limb of internal capsule (r=0.575,P=0.008;r=0.639,P=0.002),while it was negatively correlated with MD of the right superior cerebellar peduncle and the right anterior limb of internal capsule (r=-0.558,P=0.011;r=-0.626,P=0.003).Conclusions Plasma Aβ levels vary differently in the patients with aMCI and vMCI.The white matter regions of impaired microstructural integrity differ in the patients with different dementia types in the early stage.The plasma Aβ levels in the patients with aMCI and vMCI are associated with the structural integrity of white matter,and there is regional specificity between them.

Does acupuncture have advantages in the rehabilitation of vascular mild cognitive impairment? A systematic review and meta-analysis

BackgroundVascular mild cognitive impairment (VMCI) is a common impairment caused by vascular factors. VMCI often occurs after stroke, and it is the main clinical manifestation of long-term disability. Many patients are treated with acupuncture in combination with other therapies. However, evidence regarding the effectiveness of this treatment regimen is lacking. AimsThis meta-analysis aimed to evaluate the efficacy of acupuncture therapy for treating VMCI. MethodsThis systematic review was conducted in accordance with the preferred reporting and meta-analysis guidelines. The CNKI, Wanfang, VIP, CBM, Cochrane Library, PubMed and Embase databases were searched from inception to August 20, 2022. After two researchers independently screened the literature, they extracted the data and evaluated the risk of bias in the included studies. Revman 5.3 software was used for the meta-analysis. Summary of reviewThirty-two randomized controlled trials (RCTs) were included. The overall effective rate of acupuncture for treating VMCI was 3.06, 95% CI [2.39, 3.91], (P < 0.05). Montreal Cognitive Assessment (MoCA), Mini-Mental State Examination (MMSE), Barthel Index and Activities of Daily Living (ADLs) scores significantly differed between the treatment and control groups, with weighted mean differences (WMDs) [95% CI] (P value) of 1.97 [1.44, 2.49] (P < 0.05), 2.02 [1.50, 2.54] (P < 0.05), 5.54 [3.81, 7.28] (P < 0.05), and 3.43 [2.53, 4.33] (P < 0.05), respectively. The overall effective rate of electroacupuncture (EA) for treating VMCI was better than that of the control group (RR = 2.25, 95% CI, [1.13, 4.50], P < 0.05). MoCA, MMSE, Barthel index and ADL scores differed significantly between the treatment and control groups, with WMDs [95% CI] (P value) of 1.79 [1.20, 2.38] (P < 0.05), 1.45 [0.87, 2.03] (P < 0.05), 5.78 [2.38, 9.18] (P < 0.05), and 3.15 [2.15, 4.15] (P < 0.05), respectively. Acupuncture alone and combined with drug therapy were thus superior to drug therapy alone for improving cognitive function. EA also has potential advantages. ConclusionsAcupuncture combined with another therapy is better than other therapies alone, such as simple drug therapy, for treating VMCI. However, variations in study duration (4–12 weeks) limit us from drawing any definitive conclusions about long-term effects. Therefore, more RCTs with rigorous designs and reasonable treatment and follow-up durations are needed.

Serum microRNA-501-3p is a potential diagnostic tool for detecting mild cognitive impairment: Ehime genome study.

Tight junction disruption and dysfunction are involved in the progression of blood-brain barrier (BBB) breakdown. Recent investigations have revealed BBB disruption in patients with vascular cognitive decline. Our previous studies showed that miR-501-3p negatively regulates cerebral endothelial tight junction protein-1, resulting in the disruption of the BBB, and playing an important role in the development of vascular cognitive impairment. BBB breakdown in white matter lesions is often seen in the patients with vascular mild cognitive impairment (MCI). We therefore hypothesize that most early-phase MCI patients may demonstrate elevated expression of miR-501-3p and sought to investigate whether serum exosome miR-501-3p levels could be a clinical indicator for detecting mild cognitive impairment. One hundred and seventy-eight subjects (aged 73 [68-75] years, 53% male) were recruited for this study. The Japanese version of the Montreal Cognitive Assessment (MoCA-J) was used for detecting MCI. Serum exosome miR-501-3p expression levels were measured by qPCR methods. Patients were divided into two groups depending on whether their miR-501-3p ∆Ct values were above ("High"; n = 74) or below ("Low"; n = 104) cutoff levels determined by ROC curve. MCI was detected significantly more often in the miR-501-3p-High group (vs. -Low group, 63.5% vs. 47.1%, respectively; p < 0.05). Multivariate logistic regression analysis showed a significant association between MCI status and High miR-501-3p (odds ratio 2.662; p < 0.01), improved vs. known risk factors. In non-diabetic patients, High miR-501-3p was positively associated with MCI status (odds ratio 3.633; p < 0.01) and also positively associated with MCI status in those with atherosclerosis (odds ratio 3.219; p < 0.01). The present study demonstrates that elevated expression of blood exosomal miR-501-3p can indicate the presence of MCI in human patients. Early detection of vascular injuries may allow a reduction in progressive dementia through the management of vascular risk factors.

Functional abnormalities of the cerebellum in vascular mild cognitive impairment.

The alterations in cerebellar activity that occur in vascular mild cognitive impairment remain largely unexplored. This study aimed to investigate potential associations between abnormal cerebellar functional connectivity (FC) and changes in cognitive function by examining intracerebellar and cerebellar-cerebral FC. MRI data were collected from seventy-two patients with vascular mild cognitive impairment (VMCI), comprising 38 patients with small vessel mild cognitive impairment (SVMCI) and 34 with poststroke mild cognitive impairment (PSMCI), and from 43 demographically matched healthy controls (HCs). Changes in FC between subregions within the cerebellum and from each cerebellar subregion to the selected cerebral seed points in VMCI patients were calculated, and the association of these changes with cognitive function was examined. Compared with HCs, we found that VMCI patients had 11 cerebellar subregions showing significant differences (mainly decreases) in FC with brain regions in the default-mode network (DMN), sensory-motor network (SMN), and frontoparietal network (FPN). In the intracerebellar FC analysis, 47 (8%) cerebellar connections had significant intergroup differences, mainly a reduced magnitude of FC in VMCI patients. In the correlation analysis, higher Montreal Cognitive Assessment (MoCA) scores were correlated with stronger intracerebellar FC (left crus II-right lobule VI, left crus II-right lobule VIIb) and cerebellar-cerebral FC (right lobule X-left precuneus, vermal lobule IX-right inferior parietal lobule) in both the SVMCI and PSMCI groups. These findings suggest prominent intracerebellar and cerebellar-cerebral FC abnormalities in VMCI patients, contributing evidence for a possible role of the cerebellum in cognitive processes.
