Analysis of 55 cases of adenomatoid odontogenic tumor in an Indian population and review of literature.

Background This study reviews the demographic, clinical and radiographic features of adenomatoid odontogenic tumor(AOT) diagnosed in an Indian population over 50 years and also evaluate and compare follicular AOT(F-AOT) and extra-follicular AOT(EF-AOT).Material and Methods 55 diagnosed cases of AOT from 1971-2020 were studied retrospectively. The data regarding the age, sex, location, variant of AOT, duration, clinical features, radiographic appearance, treatment and recurrence were collected and analysed.Results Of the 722 odontogenic tumors diagnosed, 7.6% were AOTs with higher prevalence of extra-follicular (67.3%) than follicular (32.7%) variant. All the tumors were intraosseous with a marked predilection for maxilla over mandible, ratio 2:1. The patients mean age was 19.8 years with slightly higher female predilection (male:female ratio - 1:1.5). The anterior region (76.4%) was more frequently affected and entire quadrant was involved in 21.8% cases. Clinically, asymptomatic, slow-growing swelling was seen in 81.8% cases with duration of 15 days to 10 years. Radiographically, AOT appeared as well-corticated radiolucent lesion. Canine was the most commonly impacted tooth. Recurrence was seen in 3 cases.Conclusions Interestingly, in this series extra-follicular was twice more common than follicular AOT. Few cases involved the entire quadrant or crossed the midline of either jaws. An aggressive presentation of adenomatoid odontogenic tumor associated with calcifying epithelial odontogenic tumor: A hybrid variant

The adenomatoid odontogenic tumor (AOT) was formerly considered to be a variant of the ameloblastoma and was designated as adenoameloblastoma. It represents 3-7% of all odontogenic tumor and over 750 cases have been reported in the literature. Though this lesion shows pathognomic histological features, few overlapping patterns of the odontogenic tumors may be seen. AOT may infrequently show additional areas resembling to calcifying epithelial odontogenic tumor. Long-term follow-up research is need of the hour for such hybrid histological combination effect on the locally aggressive nature of the lesion. We present a case of a follicular variant of AOT in a 22 years male presenting with swelling on anterior maxilla associated with impacted canine.

An unusual presentation of extra-follicular variant of adenomatoid odontogenic tumor: A rare case report

Adenomatoid odontogenic tumor (AOT) is a benign, relatively rare and distinct odontogenic tumor that is exclusively derived from odontogenic epithelium. Although being odontogenic in origin, the presence of the so‑called “duct‑like structures” is a unique microscopic feature that gives the lesion a glandular, i.e. adenomatoid appearance. AOT affects young individuals mainly in the second decade with a female predominance, occurring most commonly in the anterior maxilla and is usually associated with an impacted canine. Here, we are presenting a rare case report of an AOT in the mandible in relation to 43, 44, and 45 in 30‑year‑old male patient. The tumor was a well‑circumscribed intraosseous lesion without an embedded tooth. Microscopic examination showed the characteristic histological features of AOT along with calcified epithelial odontogenic tumor‑like areas.

Adenomatoid odontogenic tumor associated with odontoma: a case report and critical review of the literature

We describe a case of adenomatoid odontogenic tumor (AOT) associated with odontoma occurring in the posterior mandible of a 32-year-old man. Although calcifications are commonly found in the AOT, the presence of rudimentary dental structures is a very rare phenomenon. Cases with similar aspects have been described as ameloblastic dentinoma, ameloblastic odontoma, adenoameloblastic odontoma and AOT associated with odontoma. After a careful analysis of the literature we describe the clinical aspects of this tumor. Further case reports and surveys of odontogenic tumors are necessary to define whether AOT associated with odontoma is a variant of AOT or a distinct clinicopathologic condition.

Adenomatoid odontogenic tumors: an analysis of 67 cases in a Thai population

The aim of this study was to determine the biologic profile of the adenomatoid odontogenic tumor (AOT) in a Thai population. Sixty-seven cases of AOT registered from January 1974 to May 2006 were studied retrospectively. Age, sex, variants of AOT, site and extent of tumors, associated impacted teeth, initial clinical diagnoses, clinical presentations, and duration of symptoms were analyzed. All AOT cases were intraosseously located, of which 55.2% were follicular and 44.8% extrafollicular. The 2 variants together were found more frequently in the maxilla than in the mandible, with a ratio of 2:1. Females were affected more often than males, with a ratio of 1.8:1. The peak incidence (56.7%) was found in the second decade of life. The majority of AOT lesions (68.7%) was found in the anterior jaws. Adenomatoid odontogenic tumors were observed more often on the left side of jaws (50.7% vs. 38.8%). Canines were the most common teeth associated with follicular AOT (67.5%), and the maxillary canines alone accounted for 51.3% of all cases. The majority of our patients presented with a painless swelling and with duration of symptoms of 6 months and longer. The distribution and characteristics of AOT in a Thai population are similar to the findings found in other populations. Interestingly, in this series we observed a case involving an impacted deciduous canine and a case associated with an impacted maxillary third molar.

Adenomatoid odontogenic tumor originated in the periodontal ligament

Adenomatoid odontogenic tumor (AOT) is a benign (hamartomatous), non-invasive lesion with slow but progressive growth. There are three variants of AOT: follicular, extrafollicular, and peripheral. This report illustrates an unusual case of AOT, i.e., arising within the periodontal ligament. A boy was brought in by his parents for evaluation of a gingival swelling. Periapical radiograph revealed thickening of the periodontal ligament with foci of calcification. Clinical diagnosis was peripheral ossifying fibroma. Biopsy was performed and microscopic examination revealed the presence of an AOT. The patient has been followed-up for eleven months without recurrence.
