Heart Rate Variability-Guided Training for Improving Mortality Predictors in Patients with Coronary Artery Disease.

The objective of this research was to investigate whether heart rate variability (HRV)-guided training improves mortality predictors to a greater extent than predefined training in coronary artery disease patients. Twenty-one patients were randomly allocated to the HRV-guided training group (HRV-G) or the predefined training group (PRED-G). They measured their HRV at home daily and trained three times a week for six weeks. Resting heart rate, isolated vagal-related HRV indices (i.e., RMSSD, HF, and SD1), weekly averaged RMSSD, heart rate recovery, and maximum oxygen uptake were assessed before and after the training period. There was a statistically significant difference (p = 0.034) in the change in weekly averaged RMSSD in favor of the HRV-G, while no differences were found in the remaining analyzed variables (p > 0.050). Regardless of the training prescription method, exercise training decreased resting heart rate (p = 0.001; −4.10 [95% CI = −6.37–−1.82] beats per minute (bpm)), and increased heart rate recovery at 2 min (p = 0.010; 4.33 [95% CI = 1.15–7.52] bpm) and maximum oxygen uptake (p < 0.001; 3.04 [95% CI = 1.70–4.37] mL·kg−1·min−1). Heart Rate Variability-Guided Training for Enhancing Cardiac-Vagal Modulation, Aerobic Fitness, and Endurance Performance: A Methodological Systematic Review with Meta-Analysis.

Purpose: This systematic review with meta-analysis was conducted to establish whether heart rate variability (HRV)-guided training enhances cardiac-vagal modulation, aerobic fitness, or endurance performance to a greater extent than predefined training while accounting for methodological factors. Methods: We searched Web of Science Core Collection, Pubmed, and Embase databases up to October 2020. A random-effects model of standardized mean difference (SMD) was estimated for each outcome measure. Chi-square and the I2 index were used to evaluate the degree of homogeneity. Results: Accounting for methodological factors, HRV-guided training was superior for enhancing vagal-related HRV indices (SMD+ = 0.50 (95% confidence interval (CI) = 0.09, 0.91)), but not resting HR (SMD+ = 0.04 (95% CI = −0.34, 0.43)). Consistently small but non-significant (p > 0.05) SMDs in favor of HRV-guided training were observed for enhancing maximal aerobic capacity (SMD+ = 0.20 (95% CI = −0.07, 0.47)), aerobic capacity at second ventilatory threshold (SMD+ = 0.26 (95% CI = −0.05, 0.57)), and endurance performance (SMD+ = 0.20 (95% CI = −0.09, 0.48)), versus predefined training. No heterogeneity was found for any of the analyzed aerobic fitness and endurance performance outcomes. Conclusion: Best methodological practices pertaining to HRV index selection, recording position, and approaches for establishing baseline reference values and daily changes (i.e., fixed or rolling HRV averages) require further study. HRV-guided training may be more effective than predefined training for maintaining and improving vagal-mediated HRV, with less likelihood of negative responses. However, if HRV-guided training is superior to predefined training for producing group-level improvements in fitness and performance, current data suggest it is only by a small margin.

Involvement of Resting Heart Rate Variability in Chronotropic Responses During Exercise

PURPOSE:Although resting heart rate variability (HRV) and chronotropic responses (CR) are common prognostic factor for cardiovascular disease, it is still unknown whether relationships between two variables are affected by exercise. The current study was aimed to identify correlations between resting cardiac autonomic regulation and graded exercise test-derived CR.METHODS:We recruited apparently healthy men college students aged between 20 and 29 years (n=46). They were assigned to Lowand High groups which have been determined for medians involved in resting heart rate (HR) or HRV indices such as rMSSD, HF, and LF/HF ratio, respectively. CR and post-exercise heart rate recovery (HRR) were calculated from HR fluctuations during and after symptom-limited graded exercise test using the Bruce protocol.RESULTS: In HRR, there were no significant differences between all Low and High groups separated by resting levels of HR or HRV indices. However, all Low groups assigned according to resting cardiac vagal-related HRV indices such as rMSSD and HF showed significantly lower CR levels than those of High groups, respectively. Moreover, CRs derived from stage 1-3 of Bruce protocol were positively correlated resting rMSSD and HF levels, respectively.CONCLUSIONS:The findings of the current study suggest that resting levels of cardiac vagal-related HRV indices may have an impact on CR derived from a graded exercise test, as well as there is a significant association between resting cardiac vagal activity and HR response during incremental exercise.

Head Exposure to Cold during Whole-Body Cryostimulation: Influence on Thermal Response and Autonomic Modulation.

Recent research on whole-body cryotherapy has hypothesized a major responsibility of head cooling in the physiological changes classically reported after a cryostimulation session. The aim of this experiment was to verify this hypothesis by studying the influence of exposing the head to cold during whole-body cryostimulation sessions, on the thermal response and the autonomic nervous system (ANS). Over five consecutive days, two groups of 10 participants performed one whole-body cryostimulation session daily, in one of two different systems; one exposing the whole-body to cold (whole-body cryostimulation, WBC), and the other exposing the whole-body except the head (partial-body cryostimulation, PBC).10 participants constituted a control group (CON) not receiving any cryostimulation. In order to isolate the head-cooling effect on recorded variables, it was ensured that the WBC and PBC systems induced the same decrease in skin temperature for all body regions (mean decrease over the 5 exposures: -8.6°C±1.3°C and -8.3±0.7°C for WBC and PBC, respectively), which persisted up to 20-min after the sessions (P20). The WBC sessions caused an almost certain decrease in tympanic temperature from Pre to P20 (-0.28 ±0.11°C), while it only decreased at P20 (-0.14±0.05°C) after PBC sessions. Heart rate almost certainly decreased after PBC (-8.6%) and WBC (-12.3%) sessions. Resting vagal-related heart rate variability indices (the root-mean square difference of successive normal R-R intervals, RMSSD, and high frequency band, HF) were very likely to almost certainly increased after PBC (RMSSD:+49.1%, HF: +123.3%) and WBC (RMSSD: +38.8%, HF:+70.3%). Plasma norepinephrine concentration was likely increased in similar proportions after PBC and WBC, but only after the first session. Both cryostimulation techniques stimulated the ANS with a predominance of parasympathetic tone activation from the first to the fifth session and in slightly greater proportion with WBC than PBC. The main result of this study indicates that the head exposure to cold during whole-body cryostimulation may not be the main factor responsible for the effects of cryostimulation on the ANS.

Tapering for Marathon and Cardiac Autonomic Function

The purpose of this study was to investigate changes in post-exercise heart rate recovery (HRR) and heart rate variability (HRV) during an overload-tapering paradigm in marathon runners and examine their relationship with running performance. 9 male runners followed a training program composed of 3 weeks of overload followed by 3 weeks of tapering (-33 ± 7%). Before and after overload and during tapering they performed an exhaustive running test (T(lim)). At the end of this test, HRR variables (e.g. HRR during the first 60 s; HRR(60 s)) and vagal-related HRV indices (e.g. RMSSD(5-10 min)) were examined. T(lim) did not change during the overload training phase (603 ± 105 vs. 614 ± 132 s; P = 0.992), but increased (727 ± 185 s; P = 0.035) during the second week of tapering. Compared with overload, RMSSD(5-10 min) (7.6 ± 3.3 vs. 8.6 ± 2.9 ms; P = 0.045) was reduced after the 2(nd) week of tapering. During tapering, the improvements in T(lim) were negatively correlated with the change in HRR(60 s) (r = -0.84; P = 0.005) but not RMSSD(5-10 min) (r = -0.21; P = 0.59). A slower HRR during marathon tapering may be indicative of improved performance. In contrast, the monitoring of changes in HRV as measured in the present study (i.e. after exercise on a single day), may have little or no additive value.

Parasympathetic Activity and Blood Catecholamine Responses Following a Single Partial-Body Cryostimulation and a Whole-Body Cryostimulation

The aim of this study was to compare the effects of a single whole-body cryostimulation (WBC) and a partial-body cryostimulation (PBC) (i.e., not exposing the head to cold) on indices of parasympathetic activity and blood catecholamines. Two groups of 15 participants were assigned either to a 3-min WBC or PBC session, while 10 participants constituted a control group (CON) not receiving any cryostimulation. Changes in thermal, physiological and subjective variables were recorded before and during the 20-min after each cryostimulation. According to a qualitative statistical analysis, an almost certain decrease in skin temperature was reported for all body regions immediately after the WBC (mean decrease±90% CL, -13.7±0.7°C) and PBC (-8.3±0.3°C), which persisted up to 20-min after the session. The tympanic temperature almost certainly decreased only after the WBC session (-0.32±0.04°C). Systolic and diastolic blood pressures were very likely increased after the WBC session, whereas these changes were trivial in the other groups. In addition, heart rate almost certainly decreased after PBC (-10.9%) and WBC (-15.2%) sessions, in a likely greater proportion for WBC compared to PBC. Resting vagal-related heart rate variability indices (the root-mean square difference of successive normal R-R intervals, RMSSD, and high frequency band, HF) were very likely increased after PBC (RMSSD: +54.4%, HF: +138%) and WBC (RMSSD: +85.2%, HF: +632%) sessions without any marked difference between groups. Plasma norepinephrine concentrations were likely to very likely increased after PBC (+57.4%) and WBC (+76.2%), respectively. Finally, cold and comfort sensations were almost certainly altered after WBC and PBC, sensation of discomfort being likely more pronounced after WBC than PBC. Both acute cryostimulation techniques effectively stimulated the autonomic nervous system (ANS), with a predominance of parasympathetic tone activation. The results of this study also suggest that a whole-body cold exposure induced a larger stimulation of the ANS compared to partial-body cold exposure.

Heart rate modulations in overtraining syndrome

Every sports training program includes a component of repetitive overloading, but with an inadequate recovery time, such overloading may produce undesired effects, such as chronic fatigue and a lack of performance improvement. The state of underperformance known as the overtraining syndrome (OTS) is characterized by a number of physiological and psychological symptoms of maladaptation. It may take weeks, months or years to restore proper sports form after the development of OTS. Although overtraining has been studied for decades, the mechanism of overtraining and the tools for the early detection of overtraining are still not defined. In addition to other physiological, biochemical, immunological, psychological and performance markers, heart rate (HR) and its modulations are intensively investigated as a practical and reliable sign of overtraining. In general, resting bradycardia, a decrease in HR during submaximal exercise, an increase in speed of heart rate recovery and increased vagal-related heart rate variability indices are all well accepted markers of improved aerobic fitness. In contrast, changes of these HR measures in the opposite direction are commonly interpreted as indicators of detraining, chronic fatigue, non-functional overreaching or overtraining. However, based on the limited and diverse literature available, these parameters may be used for monitoring training status, optimising training programs and following the accumulation of fatigue, but their role in overtraining detection and assessment has yet to be elucidated. There is a need for well-controlled prospective studies where a longitudinal follow-up of athletes is performed. Because it is unethical to excessively train an athlete to produce OTS, an animal experimental model of OTS may advance our understanding of the 'unexplained underperformance syndrome.'.

Effect of recovery mode on postexercise vagal reactivation in elite synchronized swimmers

This study investigated the effect of whole-body cryostimulation (WBC), contrast-water therapy (CWT), active recovery (ACT), and passive condition (PAS) protocols on the parasympathetic reactivation and metabolic parameters of recovery in elite synchronized swimmers who performed 2 simulated competition ballets (B1 and B2) separated by 70 min. After determining maximal oxygen consumption (V̇O(2max400)) and blood lactate concentrations ([La(-)](b400)) during a 400-m swim trial, 11 swimmers performed 1 protocol per week in randomized order. Heart rate variability (HRV) was measured at rest (PreB1), 5 min after B1 (PostB1), before B2 (PreB2), and 5 min after B2 (PostB2). V̇O(2peak) was measured at PostB1 and PostB2, and [La(-)](b) was measured at PostB1, PreB2, and PostB2. PostB1 V̇O(2peak) and V̇O(2max400) were similar, but PostB1 [La(-)](b) was higher than [La(-)](b400) (p = 0.004). Each ballet caused significant decreases in HRV indices. At PreB2, all HRV indices had returned to PreB1 levels in the CWT, PAS, and ACT protocols, whereas the WBC protocol yielded a 2- to 4-fold increase in vagal-related HRV indices, compared with PreB1. WBC and ACT both increased [La(-)](b) recovery, compared with PAS (p = 0.06 and p = 0.04, respectively), and yielded an increased V̇O(2peak) from B1 to B2; however, it decreased after PAS (+5.4%, +3.4%, and -3.6%; p < 0.01). This study describes the physiological response to repeated maximal work bouts that are highly specific to elite synchronized swimming. In the context of short-term recovery, WBC yields a strong parasympathetic reactivation, and shows similar effectiveness to ACT on the metabolic parameters of recovery and subsequent exercise capacity.

Effect of Acute Hypoxia on Post-Exercise Parasympathetic Reactivation in Healthy Men

In this study we assessed the effect of acute hypoxia on post-exercise parasympathetic reactivation inferred from heart rate (HR) recovery (HRR) and HR variability (HRV) indices. Ten healthy males participated in this study. Following 10 min of seated rest, participants performed 5 min of submaximal running at the speed associated with the first ventilatory threshold (Sub) followed by a 20-s all-out supramaximal sprint (Supra). Both Sub and Supra runs were immediately followed by 15 min of seated passive recovery. The resting and exercise sequence were performed in both normoxia (N) and normobaric hypoxia (H; FiO2 = 15.4%). HRR indices (e.g., heart beats recovered in the first minute after exercise cessation, HRR60s) and vagal-related HRV indices [i.e., natural logarithm of the square root of the mean of the sum of the squared differences between adjacent normal R–R intervals (Ln rMSSD)] were calculated for both conditions. Difference in the changes between N and H for all HR-derived indices were also calculated for both Sub and Supra. HRR60s was greater in N compared with H following Sub only (60 ± 14 vs. 52 ± 19 beats min−1, P = 0.016). Ln rMSSD was greater in N compared with H (post Sub: 3.60 ± 0.45 vs. 3.28 ± 0.44 ms in N and H, respectively, and post Supra: 2.66 ± 0.54 vs. 2.65 ± 0.63 ms, main condition effect P = 0.02). When comparing the difference in the changes, hypoxia decreased HRR60s (−14.3% ± 17.2 vs. 5.2% ± 19.3; following Sub and Supra, respectively; P = 0.03) and Ln rMSSD (−8.6% ± 7.0 vs. 2.0% ± 13.3, following Sub and Supra, respectively; P = 0.08, Cohen’s effect size = 0.62) more following Sub than Supra. While hypoxia may delay parasympathetic reactivation following submaximal exercise, its effect is not apparent following supramaximal exercise. This may suggest that the effect of blood O2 partial pressure on parasympathetic reactivation is limited under heightened sympathetic activation.

Changes in Cardiac Tone Regulation with Fatigue after Supra-Maximal Running Exercise

To investigate the effects of fatigue and metabolite accumulation on the postexercicse parasympathetic reactivation, 11 long-sprint runners performed on an outdoor track an exhaustive 400 m long sprint event and a 300 m with the same 400 m pacing strategy. Time constant of heart rate recovery (HRRτ), time (RMSSD), and frequency (HF, and LF) varying vagal-related heart rate variability indexes were assessed during the 7 min period immediately following exercise. Biochemical parameters (blood lactate, pH, PO2, PCO2, SaO2, and HCO3 −) were measured at 1, 4 and 7 min after exercise. Time to perform 300 m was not significantly different between both running trials. HHRτ measured after the 400 m running exercise was longer compared to 300 m running bouts (183.7 ± 11.6 versus 132.1 ± 9.8 s, P < 0.01). Absolute power density in the LF and HF bands was also lower after 400 m compared to the 300 m trial (P < 0.05). No correlation was found between biochemical and cardiac recovery responses except for the PO2 values which were significantly correlated with HF levels measured 4 min after both bouts. Thus, it appears that fatigue rather than metabolic stresses occurring during a supramaximal exercise could explain the delayed postexercise parasympathetic reactivation in longer sprint runs.

Effect of cold or thermoneutral water immersion on post-exercise heart rate recovery and heart rate variability indices

This study aimed to investigate the effect of cold and thermoneutral water immersion on post-exercise parasympathetic reactivation, inferred from heart rate (HR) recovery (HRR) and HR variability (HRV) indices. Twelve men performed, on three separate occasions, an intermittent exercise bout (all-out 30-s Wingate test, 5 min seated recovery, followed by 5 min of submaximal running exercise), randomly followed by 5 min of passive (seated) recovery under either cold (CWI), thermoneutral water immersion (TWI) or control (CON) conditions. HRR indices (e.g., heart beats recovered in the first minute after exercise cessation, HRR 60 s) and vagal-related HRV indices (i.e., natural logarithm of the square root of the mean of the sum of the squares of differences between adjacent normal R–R intervals (Ln rMSSD)) were calculated for the three recovery conditions. HRR 60 s was faster in water immersion compared with CON conditions [30 ± 9 beats min − 1 for CON vs. 43 ± 10 beats min − 1 for TWI ( P = 0.003) and 40 ± 13 beats min − 1 for CWI ( P = 0.017)], while no difference was found between CWI and TWI ( P = 0.763). Ln rMSSD was higher in CWI (2.32 ± 0.67 ms) compared with CON (1.98 ± 0.74 ms, P = 0.05) and TWI (2.01 ± 0.61 ms, P = 0.08; aES = 1.07) conditions, with no difference between CON and TWI ( P = 0.964). Water immersion is a simple and efficient means of immediately triggering post-exercise parasympathetic activity, with colder immersion temperatures likely to be more effective at increasing parasympathetic activity.

Nocturnal Heart Rate Variability Following Supramaximal Intermittent Exercise

To assess the effect of supramaximal intermittent exercise on long-term cardiac autonomic activity, inferred from heart rate variability (HRV). Eleven healthy males performed a series of two consecutive intermittent 15-s runs at 95% VIFT (i.e., speed reached at the end of the 30-15 Intermittent Fitness Test) interspersed with 15 s of active recovery at 45% VIFT until exhaustion. Beat-to-beat intervals were recorded during two consecutive nights (habituation night and 1st night) before, 10 min before and immediately after exercise, as well as 12 h (2nd night) and 36 h (3rd night) after supramaximal intermittent exercise. The HRV indices were calculated from the last 5 min of resting and recovery periods, and the first 10 min of the first estimated slow wave sleep period. Immediate post-supramaximal exercise vagal-related HRV indices were significantly lower than immediate pre-supramaximal exercise values (P < .001). Most vagal-related indices were lower during the 2nd night compared with the 1st night (eg, mean RR intervals, P = .03). Compared with the 2nd night, vagal-related HRV indices were significantly higher during the 3rd night. Variables were not different between the 1st and 3rd nights; however, we noted a tendency (adjusted effect size, aES) for an increased normalized high-frequency component (P = .06 and aES = 0.70) and a tendency toward a decreased low-frequency component (P = .06 and aES = 0.74). Results confirm the strong influence of exercise intensity on short- and long-term postexercise heart rate variability recovery and might help explain the high efficiency of supramaximal training for enhancing indices of cardiorespiratory fitness.

Influence of cold water face immersion on post-exercise parasympathetic reactivation

The aim of the present study was to investigate the effect of cold water face immersion on post-exercise parasympathetic reactivation, inferred from heart rate (HR) recovery (HRR) and HR variability (HRV) indices. Thirteen men performed, on two different occasions, an intermittent exercise (i.e., an all-out 30-s Wingate test followed by a 5-min run at 45% of the speed reached at the end of the 30-15 Intermittent Fitness test, interspersed with 5 min of seated recovery), randomly followed by 5 min of passive (seated) recovery with either cold water face immersion (CWFI) or control (CON). HR was recorded beat-to-beat and vagal-related HRV indices (i.e., natural logarithm of the high-frequency band, LnHF, and natural logarithm of the square root of the mean sum of squared differences between adjacent normal R-R intervals, Ln rMSSD) and HRR (e.g., heart beats recovered in the first minute after exercise cessation) were calculated for both recovery conditions. Parasympathetic reactivation was faster for the CWFI condition, as indicated by higher LnHF (P = 0.004), Ln rMSSD (P = 0.026) and HRR (P = 0.002) values for the CWFI compared with the CON condition. Cold water face immersion appears to be a simple and efficient means of immediately accelerating post-exercise parasympathetic reactivation.

Effect of cold water immersion on postexercise parasympathetic reactivation

The aim of the present study was to assess the effect of cold water immersion (CWI) on postexercise parasympathetic reactivation. Ten male cyclists (age, 29 +/- 6 yr) performed two repeated supramaximal cycling exercises (SE(1) and SE(2)) interspersed with a 20-min passive recovery period, during which they were randomly assigned to either 5 min of CWI in 14 degrees C or a control (N) condition where they sat in an environmental chamber (35.0 +/- 0.3 degrees C and 40.0 +/- 3.0% relative humidity). Rectal temperature (T(re)) and beat-to-beat heart rate (HR) were recorded continuously. The time constant of HR recovery (HRRtau) and a time (30-s) varying vagal-related HR variability (HRV) index (rMSSD(30s)) were assessed during the 6-min period immediately following exercise. Resting vagal-related HRV indexes were calculated during 3-min periods 2 min before and 3 min after SE(1) and SE(2). Results showed no effect of CWI on T(re) (P = 0.29), SE performance (P = 0.76), and HRRtau (P = 0.61). In contrast, all vagal-related HRV indexes were decreased after SE(1) (P < 0.001) and tended to decrease even further after SE(2) under N condition but not with CWI. When compared with the N condition, CWI increased HRV indexes before (P < 0.05) and rMSSD(30s) after (P < 0.05) SE(2). Our study shows that CWI can significantly restore the impaired vagal-related HRV indexes observed after supramaximal exercise. CWI may serve as a simple and effective means to accelerate parasympathetic reactivation during the immediate period following supramaximal exercise.

Exercise-induced plasma volume expansion and post-exercise parasympathetic reactivation

The purpose of this study was to investigate the effect of exercise-induced plasma volume expansion on post-exercise parasympathetic reactivation. Before (D(0)) and 2 days after (D(+2)) a supramaximal exercise session, 11 men (21.4 +/- 2.6 years and BMI = 23.0 +/- 1.4) performed 6-min of submaximal running where heart rate (HR) recovery (HRR) and HR variability (HRV) indices were calculated during the first 10 min of recovery. Relative plasma volume changes (PV) were calculated using changes in hematocrit and hemoglobin measured over consecutive mornings from D(0) to D(+2). Parasympathetic reactivation was evaluated through HRR and vagal-related indexes calculated during a stationary period of recovery. Compared with D(0), PV (+4.8%, P < 0.01) and all vagal-related HRV indices were significantly higher at D(+2) (all P < 0.05). HRR was not different between trials. Changes in HRV indices, but not HRR, were related to PV (all P < 0.01). HRR and HRV indices characterize distinct independent aspects of cardiac parasympathetic function, with HRV indices being more sensitive to changes in plasma volume than HRR.

Cardiac parasympathetic regulation: respective associations with cardiorespiratory fitness and training load

The objective of this study was to establish the separate associations between parasympathetic modulations of the heart [evaluated through heart rate (HR) variability (HRV) indexes and postexercise HR recovery (HRR) indexes] with cardiorespiratory fitness and training load. We have measured cardiorespiratory fitness through peak oxygen consumption (Vo2 max) and estimated weekly training load with the Baecke sport score in 55 middle-aged individuals (30.8 +/- 1.8 yr, body mass index 24.5 +/- 0.4 kg/m2). HRV indexes were analyzed at rest under controlled breathing, and HRR was estimated from HR curve fitting after maximal exercise or from measurements of the number of beats recovered at 60 s after exercise. Multiple linear regressions were used to investigate the separate relationships between vagal-related HRV indexes and Vo2 max and Baecke scores. On the basis of their Vo2 max and Baecke scores, subjects were classified as fit or unfit and as low trained (LT) or moderately trained (MT), which yielded four groups: UnfitLT, UnfitMT, FitLT, and FitMT. Vagal-related HRV indexes were positively correlated with Vo2 max (P < 0.05) but not with Baecke scores. In contrast, HRR indexes were related to Baecke scores (P < 0.05) but not with Vo2 max. FitLT and FitMT had significantly higher (P < 0.05) normalized vagal-related HRV indexes than UnfitLT and UnfitMT, but HRR did not change. Moderate training was associated with significantly lower HRR indexes both in UnfitMT and FitMT compared with UnfitLT and FitLT, but there was no difference in vagal-related HRV indexes. These results indicate that vagal-related HRV indexes are related more to cardiorespiratory fitness, whereas HRR appears to be better associated with training load.

Relationship Between Very High Physical Activity Energy Expenditure, Heart Rate Variability and Self-Estimate of Health Status in Middle-Aged Individuals

In middle-aged persons, moderate physical activity energy expenditure (PAEE) has been shown to be associated with increased vagal-related heart rate variability (HRV) indexes and better health status. The purpose of this study was to determine whether a very high PAEE has greater effect on vagal-related HRV indexes and self-estimates of well-being in middle-aged subjects having distinct long-term physical activity profiles. Forty-four subjects were divided into three groups on the basis of the sport score of the Modified Baecke Questionnaire for Older Adults: sedentary (SED, n = 15), moderately-trained (MT, n = 16), and highly-trained subjects (HT, n = 13). PAEE was estimated by triaxial accelerometry during one week. Time and frequency domain HRV indexes were determined during quiet periods in the morning on 5-min R-R interval segments under controlled breathing. Quality of life was evaluated using the SF-36 health survey questionnaire. PAEE was significantly different for each group (374.5 +/- 13.8, 616.8 +/- 22.4, and 1086.6 +/- 43.2 kcal . day (-1) for SED, MT, and HT, respectively, p < 0.001). MT presented significantly higher vagal-related HRV indexes than SED and HT (p < 0.05). None of the HRV indexes was significantly greater in HT than in SED. MT and HT had similar health status scores, which were significantly higher than for SED. These results indicate that in middle-aged subjects, habitual moderate PAEE is associated with greater vagal tone and self-estimates of well-being compared to low PAEE. In contrast, very high PAEE is associated with similar vagal-related indexes as low PAEE, despite better overall health status.

Effects of increased training load on vagal-related indexes of heart rate variability: a novel sleep approach.

There is little doubt that moderate training improves cardiac vagal activity and thus has a cardioprotective effect against lethal arrhythmias. Our purpose was to learn whether a higher training load would further increase this beneficial effect. Cardiac autonomic control was inferred from heart rate variability (HRV) and analyzed in three groups of young subjects (24.5 +/- 3.0 yr) with different training states in a period free of stressful stimuli or overload. HRV was analyzed in 5-min segments during slow-wave sleep (SWS, a parasympathetic state that offers high electrocardiographic stationarity) and compared with data collected during quiet waking periods in the morning. Sleep parameters, fatigue, and stress levels checked by questionnaire were identical for all three groups with no signs of overtraining in the highly trained (HT) participants. During SWS, a significant (P <0.05) increase in absolute and normalized vagal-related HRV indexes was observed in moderately trained (MT) individuals compared with sedentary (Sed) subjects; this increase did not persist in HT athletes. During waking periods, most of the absolute HRV indexes indistinctly increased in MT individuals compared with controls (P < 0.05) but did not increase in HT athletes. Normalized spectral HRV indexes did not change significantly among the three groups. Heart rate was similar for MT and Sed subjects but was significantly (P <0.05) lower in HT athletes under both recording conditions. These results indicate that SWS discriminates the state of sympathovagal balance better than waking periods. A moderate training load is sufficient to increase vagal-related HRV indexes. However, in HT individuals, despite lower heart rate, vagal-related HRV indexes return to Sed values even in the absence of competition, fatigue, or overload.
