Abstract Objective To assess health services utilization by older adults in urban and rural areas of Brazil. Method A cross-sectional study was conducted analyzing data from the 2019 National Health Survey on older adults (≥60 years) selected from households based on 22,728 interviews (3,300 in rural and 19,426 in urban areas). For rural and urban areas, the prevalence of Family Health Strategy enrolment, time since last medical and dental visit, service use in past 2 weeks, and last blood pressure and blood glucose measurements were estimated. Also, the factors associated with medical and dental health services utilization in the past 12 months were explored. Results Self-rated health of “Very good” or “Good” was greater in urban areas (47.32%), as was the proportion of older adults reporting a medical or dental visit within the last 12 months (90.54%). Rates of blood pressure (81.30%) and glucose (45.83%) monitoring were lower in rural areas. Older individuals that had low education, resided in rural areas, and the North region, had a lower likelihood of using health services Conclusion The older population living in rural areas had poorer health status compared with the urban population.

Are dental visiting patterns and oral pain associated with dental disease among Norwegian adults? A cross-sectional study based on the Tromsø study.

The present study aims to describe the dental visiting patterns in a Norwegian adult population and their associations with sociodemographic and oral health variables, including oral pain. We further explore if the utilization of dental health services and oral pain predicts caries and periodontitis, the most common oral diseases. We use data from the seventh wave of the Tromsø study performed in 2015-2016. In this cross-sectional survey, all residents 40 years or older in Tromsø municipality in Norway were invited, of whom 21,083 (65%) participated. All participants answered questionnaires assessing sociodemographic characteristics, use of health services, and self-reported health measures, including pain. Almost 4000 participants underwent a dental examination with registration of caries and periodontitis. Associations of dental visiting patterns and utilization of dental services the past 12 months with sociodemographic-, self-reported-, and clinical oral health measures were analyzed by cross-tabulation and Pearson's χ2 tests, as well as with logistic regression analyses with caries and periodontitis as outcomes. A regular, annual dental visiting pattern was the most common, but among respondents with severe dental anxiety and poor dental health, visiting for acute problems only or never (symptomatic visiting) was the most common. Intervals of more than 24 months between visits and a symptomatic visiting pattern were associated with caries, whereas shorter than 12-month intervals and a symptomatic visiting pattern were associated with periodontitis. Many characteristics were shared among respondents with the lowest and the highest utilization of dental services, including oral pain, a difficult financial situation and poorer self-reported and clinical dental health. Regular dental visits at 12-24 month intervals were associated with beneficial oral health parameters, compared with more frequent, rarer, and symptomatic dental visiting patterns. Oral pain was an unreliable predictor of caries and periodontitis.

Andersen health care utilization model: A survey on factors affecting the utilization of dental health services among school children.

Children's quality of life, academic performance, and future achievement can all be negatively affected by poor dental health. The present study aimed to assess the need for dental health services and the factors influencing their utilization using the Andersen health care utilization model among school children. The current cross-sectional study was conducted among schoolchildren aged 13 to 15 in Bangalore, India (n = 1100). A questionnaire was developed using the concepts of the Andersen healthcare usage model. The parents of the children filled out the questionnaire. The factors were investigated using bivariate analysis and multivariate logistic regression analysis. About 78.1% of the children did not utilize dental health services. Regarding the reasons for not visiting a dentist, 65.8% said they did not have a dental problem, and 22.2% said they could not afford it. Bivariate analysis showed that age, gender, education level, occupation of the family's head of household, monthly family income, socioeconomic status, perceived oral health problems, accessibility of dental health facilities, and parental attitudes toward their children's oral health were significantly associated with using dental health services (p<0.05). Multiple regression analysis showed dental health service utilization was directly related to age (OR = 2.206), education, family size (OR = 1.33), and brushing frequency twice a day (OR = 1.575) with no significant relationship between distance to reach the dental facility, the number of dental visits, and socioeconomic status. Dental health service utilization was low in the past year. The age, number of family members, parent's education level, travel time to the dental facility, the child's oral health behaviors, and positive parental attitude all play a role in a children's utilization of dental health service.

Level of Understanding and Compliance Regarding the Rights and Obligations of BPJS Participants to Utilize Dental Health Services in the First Health Facilities

The problem of oral health in the last five years increased by five years, by 25,9% to 57.6% of the total population of Indonesia. BPJS is a government health service facility that provides various dental health services. Community participation in the Health BPJS is critical to guarantee the right to obtain a healthy life. This study aims to determine the relationship between understanding and compliance with the obligations and rights of BPJS Health participants to the use of dental health services. The method in this study was observational with a cross-sectional design. The research sample was 117 people. Data were analyzed using a statistical application program consisting of a single univariate analysis, presented in the form of a frequency distribution and then described as well as a bivariate analysis to see the relationship of understanding and compliance regarding the obligations and rights of BPJS Health participants to the utilization of dental health services using the statistical test Spearman Rank. The results showed a significant relationship between the level of understanding and compliance regarding the obligations and rights of BPJS health participants, with p-value = 0,000 for the aspect of the level of understanding and p-value = 0.017 for the aspect of the level of compliance. It is recommended that the community further improve the level of understanding and compliance related to the obligations and rights of the health BPJS participants so that they can efficiently utilize the available health services.

The impact of oral health literacy on dental anxiety and utilization of oral health services among dental patients: a cross sectional study

BackgroundLow oral health literacy levels and deficient oral health knowledge jeopardize the communication between dentists and patients in different communities. This study aimed to examine the impact and association of oral health literacy with patients’ levels of dental anxiety and their utilization of dental health services.MethodsThis cross-sectional study was conducted at the Misr International University (MIU) dental clinics. The study utilized a structured, interview led questionnaire that was administered by second year dental students, over the period of two successive academic years 2018–2019 and 2019–2020. A total of 440 student interviewed a convenience sample of 440 dental patients: including 269 females (61.1%) and 171 males (38.9%). The questionnaire consisted of four sections; a demographic section, a modified Arabic Rapid Estimate of Adult Literacy (ARELAD-30) Tool that measures the ability of the participants to read 30 commonly used dental terms. This questionnaire was modified by the authors to measure the participants’ knowledge by asking them to choose the most accurate meaning for each word based on their previous knowledge. Scoring was dependent on the participant’s immediate correct pronunciation, as well as comprehension of each word. The Arabic Modified Dental Anxiety Scale (AMDAS) was used to measure the level of dental anxiety, and the dental health service utilization was measured using the Utilization of oral health services questionnaire.ResultsOne quarter (24.1%) of the participants read the 30 items of the A-REALD correctly. The average percentage of correct responses to the meaning of the dental terms was 71.2%. There was no statistically significant association between A-REALD and knowledge scores (Spearman’s Correlation coefficient ρ = -0.008, p-value = 0.872). There was a statistically significant inverse correlation between age and MDAS (Correlation coefficient ρ = -0.146, p-value = 0.002). A-REALD scores were inversely correlated with time since last visit (Regression coefficient = -0.027, p-value = 0.036, with 95% CI: -0.052 – -0.002).ConclusionWithin the limitations of this study, it can be concluded that oral health literacy is significantly associated to dental health services utilization, while, dental anxiety is related to other variables, such as age and gender.

RELATIONSHIP BETWEEN RISK FACTORS AND PERIODONTAL DISEASE AMONG PATIENTS IN COMMUNITY HEALTH CENTERS

Background: Periodontal disease is an infection in the oral cavity that is often found in the community and is considered as the number two disease in the world after dental caries. The higher the age, the higher the index of periodontal disease and if no prevention is carried out it can affect a person's quality of life. This study aims to determine the relationship between risk factors and the occurrence of periodontal disease. Method: This type of research is observational with a cross-sectional design. The sample in this study was taken by proportional random sampling and it was obtained as many as 250 patients aged 15-55 years. The risk factors studied were: OHI-S index, instantaneous blood sugar, knowledge of periodontal disease, dental and oral health maintenance behavior, food selection behavior, smoking habit, and utilization of health services. Measurement of periodontal disease based on the state of gingival infection and the data were analyzed using the chi-square test. Result: The results of the chi-square test obtained the OHI-S index, knowledge of periodontal disease, dental and oral health maintenance behavior, food selection behavior, personal blood sugar, and utilization of dental health services (p&lt;0.05). The relationship between smoking habits and the occurrence of periodontal disease was p&gt;0.05 Conclusion: the risk factors: OHI-S, knowledge of periodontal disease, dental and oral health maintenance behavior, food selection behavior, instantaneous blood sugar levels, and utilization of dental and oral health services are significantly associated with the occurrence of periodontal disease.

Recency of immigration and utilization of dental care services in Canada

To investigate the association between recency of immigration to Canada and the utilization of dental health services. The cross-sectional study sample (n=2137) was drawn from the 2015-2016 Canadian Community Health Survey (CCHS). It consisted of Canadian residents aged 12years and older who resided in the two provinces and one territory who opted into the optional dental module and gave valid responses to the questions 'How often do you usually see a dental professional, such as a dentist, a dental hygienist or a denturologist?' and 'Length of time since immigration to Canada?' for the outcome and independent variable, respectively. Multinomial logistic regression was used to analyse the data, and all statistics were weighted using sampling weights provided by Statistics Canada. The adjusted odds ratios were lower for recent immigrants than for established immigrants and for visits more than once per year (OR=0.35; 95% CI 0.14, 0.92), about once per year (OR=0.34; 95% CI 0.13, 0.90) and for less than once per year (OR=0.22; 95% CI 0.07, 0.64) than for those who never visited a dental professional. Recent immigrants, males, individuals aged 70years or more and those with a low household income were less likely to visit a dental professional than established immigrants, females, younger age groups or those with higher incomes. Better policies are needed to address the dental health concerns of recent immigrants who may suffer from poorer dental health, to ensure that they receive the care they require.

Concerns with oral health care services for adults with cognitive and intellectual disabilities.

It is of interest to document data on oral health care services for adults with cognitive and intellectual disabilities. Hence, a study protocol was registered at the International Prospective Register of Systematic Reviews (PROSPERO) with registration number: CRD42020150759. We used PubMed, Science Direct, LILACS and SCIELO to collect data from known literature using keywords containing MESH (Medical Subject Headings) terms. The risk of bias rating for the collected data was calculated using the Newcastle-Ottawa assessment Scale. The AHRQ (Agency for Healthcare, Research and Quality) was used for classifying the level of evidence in the collected data. Analysis of available data shows that there is a lack of dentists with adequate skills to treat people with disabilities resulting in high cost for dental treatment. Thus, we conclude that inconvenient location of dental clinic, lack of dentists willing to treat people with disabilities and attitude of dental staff towards people with learning disabilities were considered as barriers and challenges faced for dental health service utilization in this context.

The Relationship Between Knowledge and Assessment of Pregnant Women with the Utilization of Dental Health Services

Improving the optimal health status requires commitment and continuous effort so that a sub-standard development goals (SDGS) program is formed, one of which aims to tackle health problems in pregnant women. Dental and oral disease during pregnancy is a public health problem. The incidence of dental and oral diseases during this period is due to a lack of knowledge about oral health maintenance. The low use of service facilities by the community is related to treatment seeking behavior and ill-health concepts from the community. The purpose of the study was to analyze the relationship of knowledge and assessment of pregnant women with the use of dental health services at the Dental Clinic at the Health Centers in Kupang City. The design of this research was cross-sectional. The samples were 250 respondents, selected using proportional random sampling technique. Data were collected using a questionnaire as an instrument, then analyzed using ordinal regression test. Factors associated with the utilization of dental health services by the pediatric population: an integrative review.

This integrative literature review aimed to analyze studies about factors associated with the utilization of dental health services by the pediatric population between zero and 15 years old, published between 2006 and 2016 and available in Portuguese, English or Spanish. A survey of articles in the Lilacs and Medline databases was carried out, using the search strategy: ("dental care/utilization" OR "dental health services/utilization") AND ("child" OR "child, preschool") AND NOT adult. To analyze the methodological quality, the adapted Critical Appraisal Skill Programme (CASP) and the Agency for Healthcare and Research and Quality (AHRQ) were used. The following predictors of use of dental health services stood out: factors associated with children or adolescents (age, frequency of tooth brushing, chronic conditions), caregivers (schooling, perception of child's dental health, perceived oral health needs), dentists (availability at night and on the weekends) and follow up of oral health by the family health team. These are inherent factors for the planning of oral health policies or programs for the pediatric population. However, these factors vary according to the context, and therefore, a contextual analysis should be conducted.

Factors influencing Patients' Utilization of Dental Health Services in Jazan, Kingdom of Saudi Arabia.

IntroductionOne way of prevention and early detection of oral diseases is by utilizing the dental health care services on a regular basis. The current study aims to know the factors that play a role in influencing the dental service utilization in Jazan, Kingdom of Saudi Arabia.Materials and methodsA cross-sectional survey using a self-administered questionnaire was designed and implemented. Study subjects comprised of patients visiting the dental clinics at Jazan University and the primary dental centers of five different suburbs in Jazan region of Kingdom of Saudi Arabia. Items in the questionnaire were grouped into three sections; “demographic details,” “self-reported dental visits,” and “potential factors” contributing to dental visits. Chi-square p-value of 0.05 or less was considered as significant and logistic regression with 95% confidence interval (CI) was performed to get more precise results.ResultsThe sample size was 395 (N) of which 44.8% were males and 53.4% were females. Less than half (45.8%) of the studied sample reported that their last visit to a dentist was within a span of one year and 33% of them think that a dentist should only be visited if they experience pain. Patients following instructions given by a dentist were 7 times [odds ratio (OR) = 0.13; CI = 0.04, 0.40] less likely to miss their regular dental appointments. Following this, patients receiving knowledge on their dental problems were seen to be twice (OR = 0.50; CI = 0.25, 0.98) less likely to be irregular with their dental visits. Finally, the patients who are better educated and literate were also 2 times (OR = 2.21; CI = 1.14, 4.28) more likely to be regular with their dental appointments in comparison with the patients who completed just their primary level education.ConclusionFindings of this study will facilitate future oral health prevention programs to be more focused, thereby reducing the gap between high and low educated sectors of the population residing in Jazan.How to cite this article: Quadri FA, Jafari FAM, Albeshri ATS, Zailai AM. Prevalence of dental caries and associated factors among 12\xa0years old students in Eritrea

BackgroundDental caries is one of the most prevalent diseases of childhood in developing countries. However, there is a paucity of epidemiological data on the prevalence and associated factors of dental caries in Eritrea. The objective of this study was to assess the prevalence and associated factors of dental caries among 12 years old school children in Eritrea.MethodsA school based cross sectional study was conducted among 225 twelve years old students in two selected schools. One school from randomly selected urban and rural subzones of the country were selected. WHO adopted questionnaire and a standard checklist were used to collect relevant data. To assess dental caries, two examiners were calibrated by a certified dentist and inter observer agreement was calculated using the Cohen’s Kappa statistic (0.82). All data analysis was done using SPSS version 20.ResultsThe prevalence of dental caries was 78%, without significant difference between males (78%) and females (79%).The mean DMFT value was 2.50 (±2.21). The decayed component contributed 98.3% of the score as it had 2.44 (±1.2) share to the mean DMFT value. The first molar was the most affected tooth with a DMFT value of 1.55 (±1.36). The mean significant caries index score (SiC) was 4.97 (±1.9) which is higher than the upper limit of SiC value of 3 set by the WHO as a global average. More than half of the respondents had never visited a dentist and out of the students who had utilized a dental health facility, 82% of visits were due to dental pain while visits for regular checkups were cited by only 6.6% of the respondents.ConclusionDental caries was found to be a common public health problem among 12 years old Eritrean students. The prevalence of dental caries, mean DMFT and SiC scores were higher than the average score of other developing countries. Gaps in dental health service utilization, dental health practices and suboptimal water fluoride levels contribute to poor dental health among school children in Eritrea.

"Does this Look Infected to You?" Social Network Predictors of Dental Help-Seeking Among Mexican Immigrants.

Compared to U.S. born Latinos, Mexican immigrants (MAs) have diminished health care access and face substantial barriers to accessing needed dental health services. However, little research has examined how MAs social networks shape their use of dental health services. Using data from 332 Mexican immigrants to the Midwest, this research examines the significance of individual and egocentric network characteristics on two measures of dental health service utilization. Findings reveal that network size, network dental service utilization, and the frequency with which MAs discuss acute problems with network ties, positively correspond to use of oral health services. Conversely, embeddedness in networks where ties hassle egos about dental issues and have low levels of dental health knowledge correspond to lower odds of using these services. This research is among the first to use ego network data and methods to examine the ways network characteristics shape oral health behaviors among this underserved population.

Kesehatan Gigi Mulut dan Pemanfaatan Pelayanan Kesehatan Gigi Mulut pada Ibu Hamil (Studi Pendahuluan di Wilayah Puskesmas Serpong, Tangerang Selatan)

Kehamilan memiliki efek penting pada kesehatan mulut terkait perubahan hormonal, pola makan dan perilaku. Wanita hamil menjadi sangat rentan terhadap penyakit gingiva dan periodontal. Kondisi kesehatan gigi dan mulut ibu hamil yang buruk dapat memberikan dampak seperti kelahiran prematur, dan bayi dengan berat badan lahir rendah (BBLR). Tujuan penelitian ini untuk memperoleh gambaran kesehatan gigi mulut ibu hamil, pengetahuan kesehatan gigi mulut dan pemanfaatan pelayanan kesehatan gigi mulut pada ibu hamil. Penelitian ini bersifat deskriptif dengan pendekatan cross sectional. Wawancara terstruktur melibatkan 34 ibu hamil di wilayah kerja Puskesmas Serpong. Pertanyaan meliputi karakteristik responden, tindakan pemeliharaan kesehatan gigi, keluhan atau masalah terkait kesehatan gigi, pengetahuan kesehatan gigi mulut dan kunjungan ke dokter gigi pada ibu hamil. Data deskriptif di Analisis secara Bivariat menggunakan Uji ChiSquare untuk melihat hubungan sosiodemografi, masalah kesehatan gigi dan pengetahuan kesehatan gigi terhadap kunjungan perawatan kesehatan gigi. Hasil penelitian menunjukkan bahwa lebih dari setengah jumlah responden ibu hamil (52,94%) merasa keadaan kesehatan gigi mulut mereka baik dan 61,8% tidak memiliki masalah kesehatan gigi mulut. Sejumlah 55,9% responden mengaku belum pernah mendapatkan informasi mengenai pentingnya kesehatan gigi mulut selama kehamilan. Hanya sekitar 35,5% responden yang ke dokter gigi kurang dari 1 tahun yang lalu. Terdapat hubungan signifikan antara masalah Kesehatan Gigi Mulut terhadap pengetahuan kesehatan gigi mulut ibu hamil dan kunjungan perawatan kesehatan gigi. Kesimpulan penelitian adalah adanya keluhan dan masalah kesehatan gigi mulut pada ibu hamil berpengaruh pada kunjungan perawatan kesehatan gigi mulut saat kehamilan dan terkait pengetahuan yang dimiliki mengenai kesehatan gigi mulut saat kehamilan.Oral Health and Dental Visit of Pregnant Women (Pilot Study in Serpong, South Tangerang). Pregnancy has a significant effect on oral health-related hormonal changes, diet and behavior. Pregnant women become very susceptible to gingival and periodontal disease. Oral health condition of pregnant women may adversely bring impacts such as preterm low birth weight (PLBW). The aim of this study is to gain an overview of oral health status, knowledge and use of dental care service in pregnant women. This was a descriptive study with cross sectional approach. The structured interviews involved 34 pregnant women in Puskesmas Serpong. The questions included the characteristics of the respondents, oral health care practice, problem related to oral health during pregnancy, oral health knowledge and dental health service utilization in pregnant women. Chi-squares test were conducted to examine bivariable relationships between sociodemographic, oral health status and knowledge to dental health services utilization. The results show that more than half of pregnant woman respondents (52.94%) descibed their oral health as good and 61.8% did not have any dental health problems. As many as 55.9% of the respondents had knowledge about the importance of oral dental health during pregnancy. Only about 35.5% of the respondents visited dentists in the last one year. There is a significant correlation between dental health problem and dental health knowledge of pregnant women to the use of dental health services. The conclusion from this study is that dental health problem in pregnant women affects the utilization of dental care during pregnancy and is associated with the knowledge about dental health during pregnancy.

The purpose of this study was to investigate the factors related to high school students' fear of dental treatment and distrust for dentists, analyze the relation between these factors and dental health services utilization, and then apply basis data for providing individualized dental health service. The period of the data collection was from November 1st to November 22th 2013, and the subjects were 300 of the first-year high school students in Daugu, and the final analysis data were total 280. using frequency, T-test, correlation and multiple regression analysis, the results of this study were as follows. The higher the distrust for dentist(<TEX>${\beta}=0.322$</TEX>) and the burden of dental visit(<TEX>${\beta}=0.161$</TEX>), fear of Dental treatment was increased. The larger the fear of Dental treatment and Past experienced pain level, distrust for dentist was increased. therefore, In order to decrease patients' distrust for dentists, good communication between patients and doctors, and efficient treat plans were needed. As a result, patients should have positive attitude on treat. Through early detection and early treatment of oral disease, effective dental health care services with decreasing the fear of dental care system should be provided.

Socioeconomic and psychosocial predictors of dental healthcare use among Brazilian preschool children

BackgroundDisparities in utilization of oral healthcare services have been attributed to socioeconomic and individual behavioral factors. Parents’ socioeconomic status, demographics, schooling, and perceptions of oral health may influence their children’s use of dental services. This cross-sectional study assessed the relationships between socioeconomic and psychosocial factors and the utilization of dental health services by children aged 1–5 years.MethodsData were collected through clinical exams and a structured questionnaire administered during the National Day of Children’s Vaccination. A Poisson regression model was used to estimate prevalence ratios and 95% confidence intervals.ResultsData were collected from a total of 478 children. Only 112 (23.68%) were found to have visited a dentist; 67.77% of those had seen the dentist for preventive care. Most (63.11%) used public rather than private services. The use of dental services varied according to parental socioeconomic status; children from low socioeconomic backgrounds and those whose parents rated their oral health as “poor” used dental services less frequently. The reason for visiting the dentist also varied with socioeconomic status, in that children of parents with poor socioeconomic status and who reported their child’s oral health as “fair/poor” were less likely to have visited the dentist for preventive care.ConclusionThis study demonstrated that psychosocial and socioeconomic factors are important predictors of the utilization of dental care services.
