Multidetector Row Computed Tomography Evaluation in Pediatric Heart Diseases-Additional Information to Echocardiography and Conventional Cardiac Catherterization

Multidetector-row computed tomography (MDCT) scanners are a widely available, accurate, and noninvasive technique for the diagnosis of pediatric cardiovascular disorders. A lots of articles published regarding the usefulness of MDCT mostly describe that it can be an alternative to the invasive catheterization and angiography. The unique diagnostic features of this imaging modality have been largely ignored or disregarded. We presented the pathological conditions that cannot be diagnosed by conventional angiography with cardiac catheterization but can be accurately diagnosed by MDCT. We focused on the diagnostic advantages of MDCT, and indicated four pathological conditions. (1) When Blalock-Taussig shunt total occlusion prevents catheter insertion into the artificial vessel and angiography is ruled out, the peripheral pulmonary artery during the peripheral pulmonary artery can be imaged and diagnosed using MDCT based on blood flow supplied from many small collateral vessels originating from the aorta. (2) The location and protrusion of the device in the vessel after coil embolization to treat patent ductus arteriosus can be accurately visualized by virtual endoscopy using MDCT. (3) Calcification of patches, synthetic blood vessels, and other prostheses that is indistinct on conventional angiograms is clear on MDCT. (4) Simultaneous MDCT observations of the anatomical relationships between arterial and venous systems on the same image can clarify the detail diagnosis for surgical treatment. MDCT is useful not only as a non-invasive alternative to conventional angiography, but also as a tool for specific morphological diagnoses. This article focuses on the review of unique advantages of MDCT in comparison to other imaging modalities with citing our previous articles.

Usefulness of multi-detector row computed tomography for management of duodenal varices by emergency balloon-occluded retrograde transvenous obliteration.

Ectopic varices outside the gastroesophageal region are less common in patients with portal hypertension and liver cirrhosis. Among ectopic varices, bleeding from duodenal varices is often massive and fatal. Several reports have recently described various treatment options for duodenal varices including interventional radiology. It is necessary to thoroughly investigate variceal hemodynamics before conducting interventional radiology. We report a case of bleeding duodenal varices treated successfully with balloon-occluded retrograde transvenous obliteration (B-RTO) after investigating the hemodynamics of the duodenal varices with multi-detector row computed tomography (MDCT). MDCT easily identified the duodenal varices together with the afferent and efferent vessels. Our case suggests the usefulness of MDCT before B-RTO for investigating the hemodynamics and vascular anatomy of the duodenal varices and selection of the most appropriate therapy.

Assessment of Coronary Stenosis by a 16-Slice MDCT Scanner in Asymptomatic Diabetic Patients Starting Dialysis Therapy

Background: The accuracy for the detection of coronary stenosis by multidetector row computed tomography (MDCT) has been getting more recognition. However, the usefulness of MDCT in patients with chronic kidney disease (CKD) has not been confirmed. Methods: Weanalyzed 19 consecutive patients with asymptomatic diabetic CKD who underwent both MDCT and coronary angiography (CAG) at the initiation of dialysis. The definition of stenosis in this study was lesions with ≧50% stenosis by CAG. Results: CAG revealed stenosis in 35 of 76 branches in 19 patients. Vessel diameter could not be evaluated by MDCT in 11 (14%) major vessels because of motion artifacts, pericardial effusion, pleural effusion, and severe calcification. Almost all of such lesions were located in the right coronary (4/11; 36%) or left circumflex (5/11; 45%) artery. The sensitivity, specificity, positive and negative predictive values of MDCT for a diagnosis of stenosis in the 65 evaluable major vessels were 86, 81, 78, and 88%, respectively. The severity of vessel calcification was increased in a stepwise manner with increments in the proportion of major vessels with ≧50% stenosis (p = 0.004 for trend). Conclusion: MDCT seemed to be an effective non-invasive method of screening patients with diabetic CKD for CAD.

Usefulness of multi-detector row computed tomography for the management of percutaneous transluminal septal myocardial ablation in patient with hypertrophic obstructive cardiomyopathy

Percutaneous transluminal septal myocardial ablation (PTSMA) has been a useful therapeutic option for medically refractory patients with hypertrophic obstructive cardiomyopathy (HOCM). Multi-detector row computed tomography (MDCT) has enabled the non-invasive detection of coronary arteries. Therefore, we describe here a patient with HOCM who was treated by PTSMA, and the usefulness of MDCT for its greater certainty and safety for the management of PTSMA.

Noninvasive Detection of Takotsubo Cardiomyopathy Using Multi-detector Row Computed Tomography

We present a 60-year-old woman with chest pain preceded by emotional stress. Trans-thoracic echocardiography demonstrated localized left ventricular hypokinesis around the apical area. Multi-detector row computed tomography (MDCT) revealed no significant stenosis in the coronary arteries, which comfirmed Takotsubo-like cardiomyopathy. We show here the usefulness of MDCT for the noninvasive differentiation of Takotsubo cardiomyopathy from acute coronary syndrome.

Differentiation of gastric ulcers with MDCT

It is important to differentiate malignant from benign gastric ulcers (GUs) because the early detection of malignancy offers the best prognosis and is essential for planning optimal therapy. However, the differential diagnosis between a malignant and benign gastric ulcer is sometimes difficult, and remains a great challenge. Recent advances in multidetector row-computed tomography (MDCT) with three-dimensional imaging software and multiplanar reformatted (MPR) images provide a potentially powerful tool for noninvasive gastric evaluation. Virtual gastroscopy (VG) is helpful in the detection and evaluation of GU in the same way as gastroscopy. In comparison with gastroscopy, VG images can depict abnormal endoluminal lesions with a wider field of view and they have no blind point because retrospective reconstruction is available. MPR images allow the radiologist to choose the optimal imaging plane to accurately evaluate the change of the gastric wall around the gastric ulcer avoiding partial volume averaging effects. This report describes the clinical usefulness of MDCT in differentiating malignant from benign GUs by using VG and MPR images.

Uses and Limitations of 40 Slice Multi-Detector Row Spiral Computed Tomography for Diagnosing Coronary Lesions in Unselected Patients Referred for Routine Invasive Coronary Angiography

Background and Aims: The value of multi-detector row computed tomography (MDCT) in routine cardiology practice is uncertain. We examined the applicability of MDCT imaging for the diagnosis of obstructive coronary artery disease in a routine clinical setting. Methods: MDCT scanning (40 slice) was performed in 111 unselected patients referred for invasive coronary angiography (ICA) and findings were compared to an independent quantitative assessment of the ICA on a segmental, vessel and patient basis. Results: Sensitivity and positive predictive value for segmental disease (72.2 and 70.9% respectively, overall) were higher in patients aged ≧60 years and history of disease ≧1 year, whereas specificity and negative predictive value were high in all groups. In the patient-based analysis, sensitivity and positive predictive value (84.7 and 87.8%, respectively) were higher, the latter in keeping with the high-patient prevalence of disease, but specificity and negative predictive value (61.5 and 55.2%) were low. Conclusions: Usefulness of MDCT was significantly influenced by age, duration of coronary artery disease and female gender, and on a patient-based analysis its diagnostic accuracy was not sufficient to replace ICA in a routine clinical setting.

Usefulness of multidetector-row CT in the evaluation of reperfused myocardial infarction in a rabbit model.

ObjectiveTo evaluate the usefulness of multidetector-row computed tomography (CT) in the evaluation of reperfused myocardial infarction.Materials and MethodsEleven rabbits were subjected to 90-min occlusion of the left anterior descending coronary artery followed by reperfusion. Multidetector-row CT was performed 31 hours ± 21 after the procedure and pre- and post-contrast multiphase helical CT images were obtained up to 10 min after contrast injection. The animals were sacrificed after 30 days and histochemical staining of the resected specimens was perfomed with 2'3'5-triphenyl tetrazolium chloride (TTC).ResultsIn all 11 cases, the areas of myocardial infarction demonstrated with TTC-staining were identified on the CT images and the lesions showed hypoenhancement on the early phases up to 62 sec and hyperenhancement on the delayed phases of 5 min and 10 min compared with normal myocardial enhancement. The percentage area of the lesion with respect to the left ventricle wall on CT was significantly correlated with that of the TTC-staining results (p < 0.001 for both early and delayed phase CT) according to the generalized linear model analysis. The areas showing hypoenhancement on early CT were significantly smaller than those with hyperenhancement on delayed CT (p < 0.0001).ConclusionMultidetector-row CT may be useful in the detection and sizing of reperfused myocardial infarction.

Dose evaluation and effective dose estimation from multi detector CT.

Computed tomography (CT) has evolved remarkably through device improvement and advancement of peripherals, including computers. In 1999, multi detector-row CT (MDCT) appeared and rapid high-speed scanning became possible. However, usefulness of MDCT in actual clinical application cannot be assessed until the exposure doses are assessed appropriately. Since CT examinations need a comparatively high dose, it is necessary to evaluate patient exposure for introduction of MDCT. Patient doses by three types of MDCTs were evaluated for cases of scanning of the chest and abdomen-pelvis. The examination conditions were the same as those in actual clinical examinations. The obtained effective doses were 9.4-28 mSv for the chest examination and 13-28 mSv for the abdomen-pelvis. The average surface doses varied between 16-43 mGy for the chest examination and 20-37 mGy for the abdomen-pelvis. The highest surface dose was 57 mGy for the abdomen-pelvis examination. The exposed doses differed according to scanning method and imaging conditions such as tube current, slice thickness and so on. It seemed that there is room for dose reduction by proper adjustment of scan conditions in MDCT examinations.
