Patient Safety Culture in Handling Prescriptions and Interprofessional Collaboration Practices Amongst Community Pharmacists: An Investigative Simulated Patient Study from the United Arab Emirates.

BackgroundCommunity pharmacists are in a unique position to identify drug therapy-related problems (DTRPs) in prescriptions and mitigate them by communicating with prescriber. This study assessed the ability of community pharmacists (CPs) to identify DTRPs in prescriptions, the level of interprofessional collaboration among physician and CPs in mitigating the identified DTRPs, and the existing safety culture practices among CPs.MethodsTrained simulated patients (SPs), five final-year BSc Pharm female students, visited conveniently selected community pharmacies (n = 50) in Ajman emirate of the United Arab Emirates, with dummy prescriptions containing DTRPs (total 50 prescriptions with five different types of DTRPs categorized per the Pharmaceutical Care Network Europe Version 8) and assessed the DTRP-identifying ability of the CPs. SPs also observed the steps taken by the CPs to mitigate identified DTRPs and existing (if any) collaborative practices between CPs and physician. SPs documented their observations in a checklist immediately after leaving the pharmacy premises, which served as the data source. Statistical analyses were performed with chi-square at alpha = 0.05.ResultsOf the 50 respondents, 44% (n = 22) were able to identify the DTRPs. DTRP identification by pharmacists was associated with labeling [chi-square = 7.879, p value = 0.019], reconciliation [chi-square = 10.359, p value = 0.001], counseling standard [chi-square = 19.09, p = 0.000] and physician visit suggestion [chi-square = 31.15, p = 0.000]. The labeling standards for prescriptions with DTRPs were “low” in five (50%), “average“ in three (30%) and “good” in two (20%) of the cases with wrong dose. Average counseling time of the CPs was 80.38 ± 71.61 seconds. The counseling standard had no significant association with counseling time [chi-square = 34.79, p = 0.250] and use of drug information sources [chi-square = 2.86, p = 0.243]. Average time spent in dispensing is 74.4 ± 73.05 seconds. None (n = 0) of the CPs communicated with the physician, and only five out of 50 (10%) of CPs checked any DI sources. However, in 19 (38%) cases, the CPs recommended the SPs to consult their physician prior to taking the medications.ConclusionCPs were generally able to identify DTRPs and mitigate DTRPs by recommending physician consultation. Nevertheless, there were no professional collaborations between the SPs and physicians. The dispensing and counseling standards were not appreciable.

General medical practitioners' perceived use of drug information sources with special reference to drug information centres

Abstract This study reports the current perceived use of available sources of drug information for general medical practitioners. The sample frame, of 463 GPs practising in Derbyshire, England, on January 1, 1990, provided a 74 per cent response rate to a pre-piloted questionnaire. Two printed sources, the Monthly Index of Medical Specialities and the British National Formulary (BNF) plus one personal service, fellow colleagues, were by far the most frequently used. The estimated average minimum number of days per year on which personal information services were consulted was 28.51 days compared with 63.83 days for non-personal, printed sources. Thus, if GPs are to be guided by independent sources then the BNF must be considered a prime vehicle. Drug information centres (DICs) were ranked last with 56 per cent of GPs having consulted such a source within the previous year. Use of DICs could not be predicted from GPs' characteristics or demographic data. Interviews with 101 of 106 randomly selected GPs generally confirmed questionnaire findings. However, only 41 per cent perceived themselves as users of DICs. Collation of queries from two local centres showed 25 per cent were known to be users. From interviews, discrepancies were seen to arise from accessing DICs outside the sample frame boundary plus confusion of DICs with poisons centres and hospital pharmacy departments. Thus, for use of DICs, questionnaire techniques may be unreliable unless source definition is considered. If using DICs, GPs very significantly had lower concurrent use of community pharmacists, GP colleagues and hospital consultants for general drug information

Diagnosis of the availability and use of drug information sources in drugstores and pharmacies in southern Brazil

This research has aimed to estimate the use of drug information sources by pharmacists in drugstores and pharmacies in southern Brazil. It consisted of sending a questionnaire through regular mail, contacting the pharmacist via phone and visiting the drugstores. Four hundred and eight (68.6%) of the 595 enrolled establishments answered the questionnaire. The information at pharmacies and drugstores is searched mainly to orient the patient. At drugstores the professionals have an average of 2.3 books, whereas at pharmacies they rely on 6.1. In a pharmacy, the chance to find more than five books is 27 times higher than in a drugstore. The more often available books are pharmaceutical specialties compendiums. There is access to Internet in 87.5% of pharmacies and 59% of drugstores. The National Agency of Health Surveillance webpage is the most accessed website, and the call centers of Pharmaceutical Companies are the most searched information service. Lack of time is the main alleged difficulty for searching information. The pharmacists working in the studied establishments miss appropriate drug information sources. Taking into consideration how important information is in the pharmaceutical practice, there is a need to emphasize this subject through an educative process, during undergraduate studies and continued education.

Patient Needs and Sources of Drug Information in Singapore: Is the Internet Replacing Former Sources?

Patients are exposed to a broad range of drug information (DI) sources; among them is the Internet, which has been increasingly used over the years, especially by adolescents. To identify patients' needs and common sources of DI; examine the differences in use of DI sources among groups, by age and sex; and better understand patients' use of the Internet as a source of DI. A quota sample of 201 outpatients from National University Hospital in Singapore was surveyed. A questionnaire was developed to examine patients' needs for and common sources of DI as well as their experience with and attitudes toward using the Internet as a DI source. Physicians (83.1%) and pharmacists (57.7%) were reported to be the most commonly used sources of DI regarding prescription drugs (n = 201), whereas pharmacists (40.9%) and relatives or friends (40.9%) were the most commonly reported sources for information about nonprescription drugs (n = 149). Respondents most commonly sought DI about adverse effects (72.6%), dosing (54.7%), and indications (54.2%). Among Internet users, people aged 31-50 years were more likely than those in other age groups to search the Internet for DI (p < 0.001). The Internet was considered to be a convenient source with a broad range of information; however, patients faced both the inability to find needed information and uncertainty about information reliability when searching for DI on the Internet. Pharmacists play an important role in counseling patients about prescription and nonprescription drugs. There is a need to educate patients on how to locate and evaluate DI on the Internet.

Needs and use of drug information sources in community pharmacies: a questionnaire based survey in German-speaking Switzerland.

To explore the types of drug information used by pharmacists in daily practice, their needs and wishes concerning drug information and their opinions about future changes of pharmacy practice. Postal survey to a random sample of 223 (26% out of 859) community pharmacies from the German-speaking part of Switzerland. The 48-item questionnaires were processed automatically with the Cardiff TELEform-Software. In addition, a telephone-survey to a random sample of 20 non-responders was performed in order to test for non-response bias. A total of 108 pharmacists (response rate 48%) reported that the official Swiss drug reference book is still the most popular source of drug information used to solve all kinds of drug related problems. The Internet as a source of drug information is of minor importance, even though 88% of the pharmacies have Internet access. Deficits in drug information were reported for paediatrics, phytotherapy, drugs during pregnancy/lactation and for therapy guidelines. According to 35% of the pharmacists, the importance of offering drug information to customers will increase in the future. Most of the pharmacists are not afraid that Internet pharmacies would replace them. The results show that the majority of the community pharmacists are only partially satisfied with the sources of drug information currently available. The Internet still plays a minor role for solving drug-related problems in daily practice, even though the available infrastructure makes the community pharmacies able to use the Internet more frequently. The pharmacists need more websites tailored to their needs. The pharmacists have clear visions about possible future developments. They do, however, have to adopt quickly to the changes ahead in order to remain competitive.

Sources of information on drugs: survey among veterinary practitioners in Slovenia

The article briefly describes the research published on use of information sources by physicians and veterinary practitioners. It specifies in particular studies on the use of drug information sources in human and veterinary medicine and outlines the situation of the veterinary profession in Slovenia. It presents the results of a questionnaire survey on the use of information on drugs by Slovenian veterinary practitioners, on how they use information technology and electronic information sources, while it also indicates new possibilities for veterinary medical libraries and professional veterinary organisations how to provide, by new information and communication technologies, relevant information for veterinary practitioners in the most convenient way.

The determinants of physician attitudes and subjective norms toward drug information sources: modification and test of the theory of reasoned action.

To improve upon the theory of reasoned action and apply it to pharmaceutical research, we investigated the effects of relevant appraisals attributes, and past behavior of physicians on the use of drug information sources. We also examined the moderating effects of practice characteristics. A mail questionnaire asked HMO physicians to evaluate seven common sources of drug information on general appraisals (degree of usefulness and ease of use), specific attributes (availability, quality of information on harmful effects and on drug efficacy), and past behavior when searching for information on a new, simulated H2 antagonist agent. Semantic differential scales were used to measure each appraisal, attribute and past behavior. Information was also collected on practice characteristics. Findings from 108/200 respondents indicated that appraisals and attributes were useful determinants of attitudes and subjective norms toward use. Degree of usefulness and quality of information on harmful effects were important predictors of attitudes toward use for several sources of information. Ease of use and degree of usefulness were important predictors of subjective norms toward use. In many cases, moderating effects of practice characteristics were in opposing directions. Past behavior had significant direct effects on attitudes toward the PDR. The findings suggest ways to improve the usefulness of the theory of reasoned action as a model of decision-making. We also propose practical guidelines that can be used to improve the types of drug information sources used by physicians.

The use of drug information sources by physicians: Development of a data-generating methodology

The aim of this study was: (1) to develop and evaluate a methodology to determine hospital physician's use of personal drug information sources; (2) a preliminary insight into the use personal drug information sources. Written case simulations appeared to be the most appropriate method. To construct the written case simulations a step-wise procedure was developed. In the first stage 5 internists formulated 35 complex cases from their daily practice in which they consulted drug information sources; after an evaluation by experts 20 cases were left over. Next, using a written questionnaire, these 20 cases were evaluated in a random sample of 50 internists according to criteria concerning aspects of the process, the contents and the context. Finally, we analyzed these ratings using an elimination-by-aspects decision rule, with the dominant criterion ‘need for consultation’. After this selection programme, two cases for each stage in the decision-making process of hospital physicians were selected which met the criteria. In general the colleague internist was the most frequently mentioned information source. Subspecialists and supporting specialists were considered less often and varied per stage in the decision-making process of physicians. The hospital pharmacist was hardly mentioned as a possible information source. The representatives of the pharmaceutical industry were not mentioned at all by the respondents.

A reasoned action approach to physicians' utilization of drug information sources.

The effects of attitude and subjective norm were investigated on physicians' intention to use seven drug information sources: the PDR, medical textbooks, medical journals/newsletters, pharmaceutical manufacturers' literature, pharmaceutical manufacturers' representatives, other physicians, and pharmacists. The effects of past behavior and practice characteristics were also examined. An eight-page mail questionnaire queried health maintenance organization physicians on their intention to use, attitude (emotional response) and subjective norm (colleagues' approval/disapproval) toward use of each source when searching for drug information on a fictitious, new H2 antagonist agent. Responses were received from 54% (108) of the 200 physicians surveyed. Positive attitudes toward use had the greatest influence upon intention to use each of the sources (b > or = .40) (except for pharmacists, for which subjective norm was the most important predictor (b = .31)). Past behavior directly affected intention to use the PDR (b = .27), and pharmaceutical manufacturers' literature (b = .26). The effects of attitude and/or subjective norm on intention to use non-commercial sources of drug information were moderated by the practice characteristics. These findings suggest that physicians' use of drug information sources is strongly influenced by their attitudes toward use. In addition, the importance of situational contingencies should not be overlooked when investigating the use of drug information sources.

Factors Related to Physicians' Attitudes and Beliefs toward Drug Information Sources

Through the use of a mail questionnaire, factors related to physicians' attitudes and beliefs toward seven drug information sources were investigated. A sample of 108 physicians was obtained from a large health maintenance organization (HMO). Responses were obtained to questions on past use, ease of use, emotional response to use, quality of information on harmful effects and drug efficacy, colleagues' opinions, degree of usefulness and availability, and intention to use. The results show that physicians differ on each of the above factors, depending upon the source of drug information. Future studies should include an analysis of these factors when investigating physicians' use of drug information sources.

Information Sources Utilized by Private Practice and University Physicians

A survey of 196 private practice and university-based West Virginia physicians was undertaken to ascertain their use of drug information sources. An overall response rate of 53% was obtained. Physicians had a mean of 3.3 drug information questions daily and spent approximately 16 minutes in finding the answers to these questions. The Physicians' Desk Reference was the most frequently used information source, especially by physicians in private practice settings. Pharmaceutical manufacturers' promotional literature and sales representatives were used significantly more often in private practice settings; colleagues, journals, and pharmacists were used to a similar extent at both sites. The hospital library was infrequently used by all respondents. A continued reliance on commercial information sources exists, in spite of the ever increasing availability of alternative noncommercial sources.

Physicians' and Pharmacists' Drug Information Sources in Nigeria

A random sample of physicians and pharmacists in Nigeria was surveyed by means of questionnaires. Results indicated that both groups made use of a wide range of information sources for general update but depended on a few sources for drug information per se. A Wilcoxon rank-sum test on the use of drug information sources by the two groups indicated no significant difference. A comparison of physicians in old and new schools with respect to use of drug information sources also showed no significant difference. A Kruskal- Wallis H test on regional scores of both physicians and pharmacists combined, however, had a significant effect. A case is made for the establishment of drug information centers in Nigeria.
