Employing Dissonance-Based Interventions to Promote Health Equity Utilizing a Community-Based Participatory Research Approach and Social Network Analysis

The purpose of this paper is to examine and advocate for the consideration of relevant approaches that can be utilized to increase the effectiveness of cognitive dissonance-based interventions (DBIs) designed to promote health equity. Although DBIs informed by different paradigms have been reported to be effective in creating behavior change, particularly among at-risk populations, their long-term impacts on behavior change have apparently been difficult to sustain. We argue that a community-based participatory research (CBPR) approach could considerably improve the effectiveness and long-term impacts of DBIs by harnessing community strengths, increasing stakeholder participation, and facilitating collaborations and partnerships in the planning, implementation, and evaluation of such interventions. Then, we argue that the benefits of employing a CBPR approach in DBIs can be further enhanced when combined with an approach that intentionally utilizes Social Network Analysis (SNA). SNA applies powerful techniques to recognize the type of connections that hold a specific network together and identify that network’s key and influential stakeholders. We conclude by providing recommendations for the use of CBPR and SNA in DBIs and demonstrating the benefits of our recommendations, especially in the context of promoting health equity.

'I want to be screened just like the pirates!': The Power of Community-Based Participatory Research (CBPR) Theatre to Aid Research Participation


 
 
 Research participation is an important component of advancing whole health and eliminating health disparities, especially in communities facing environmental justice (EJ) issues. Though federally mandated, recruitment of racial and ethnic minorities can be a daunting task and recruitment of children an even greater challenge. A range of typical recruitment strategies (printed materials, word of mouth, broadcast etc.) for those of ethnically and culturally diverse backgrounds have exhibited only limited to modest success, depending on the community being served and the type of engagement. To date, there has been only limited assessment of the use of community-based participatory research (CBPR) theatre as a culturally relevant recruitment strategy.
 The ENRRICH Railyard Study used CBPR theatre to engage an underserved EJ community, and to assess the health impact of residential proximity to a major freight railyard. The railyard community is comprised of primarily low-income Hispanic families. To promote participation, a CBPR theatre play – a partnership between a community- based organisation (CBO) and a university institution – was produced, from design to production, at two elementary schools. Following the play, parental consents and surveys were sent home. The response was immediate and one of the largest to date – 74 percent of children participated in the study.
 
 
 
 The CBPR theatre, university and CBO partnership provided an opportunity to engage under-served minority children and their parents, thus encouraging participation and facilitating educationon respiratory health and the environment. This article includes experiences and lessons learned from using CBPR theatre to encourage study participation, critical for promoting sustainable change in an EJ community.
 
 
 
 
 

Inequities in Medically Assisted Reproduction: a Scoping Review.

Infertility has a high prevalence in the USA and health inequities play a large role in access to medically assisted reproduction (MAR). The aim of this study was to identify gaps in research pertaining to inequities in MAR and propose suggestions for future research directions. Searches were performed using MEDLINE and Ovid Embase. Articles that reported on MAR inequities, published between 2016 and 2021 in the USA, and written in English were included. The inequities investigated were adapted from the NIH-designated health disparities populations. Each article's inequity findings were extracted and reported, along with frequencies of inequities. Our sample included 66 studies. The majority of the studies investigated MAR outcomes by race/ethnicity and found that historically marginalized populations had poorer outcomes. LGBTQ + populations were less likely to use MAR or seek infertility care. Most studies found positive correlations with MAR use with income and education. The least commonly studied inequities in our sample were sex and/or gender and rural/under-resourced populations; findings showed that men and people from rural/under-resourced populations were less likely to access MAR. Studies that examined occupational status had varying findings. We suggest that future research be targeted toward: (1) standardizing and diversifying race/ethnicity reporting regarding MAR, (2) the use of community-based participatory research to increase data for LGBTQ + patients, and (3) increasing access to infertility care for men.

A Scoping Review of Theoretical Lenses and Methodological Approaches in Indigenous Women's Health and Well-Being Research in North America over the Past Two Decades.

Theoretical approaches influence research design, engagement, and outcomes. The relevance of critical theoretical and methodological approaches to Indigenous women's health and well-being research has increased in the last decade. It is difficult to assess the ways in which theoretical lenses can effectively interrupt and challenge systemic erasure, ongoing harms, and deficit-based (ill-health-centered) approaches to Indigenous women's health and well-being, a fact that is not broadly acknowledged. We conducted a scoping review to (a) map the type and frequency of critical theoretical lenses used by researchers focused on Indigenous women's health and well-being in North America over the past two decades and (b) identify which topics tend to use which theoretical lens. We have conducted a scoping review to examine peer-reviewed articles from eight electronic databases. In the articles selected over 2000-2021, we found an increase in the use of community-based participatory research, decolonial lenses, and feminist lenses. Over the last decade, there has been a decrease in quantitative social science approaches. While a range of critical theoretical and methodological approaches are increasingly being applied, the use of cultural resurgence and Indigenous feminism in health research is not widespread.

Examining the influence of group diversity on the functioning of community-based participatory research partnerships: A mixed methods study.

Public health has endorsed the use of community-based participatory research (CBPR) to address health inequities involving diverse and marginalized communities. However, few studies have examined how group diversity among members of CBPR partnerships influenced how well the partnerships achieve their goals of addressing health inequities through equitable collaboration. We conducted secondary, convergent, mixed methods analysis to (1) evaluate the association between group diversity and participatory decision-making within CBPR partnerships, and (2) identify the perceived characteristics, benefits, and challenges of group diversity within CBPR partnerships. Using data from a cross-site study of federally funded CBPR partnerships, we analyzed and integrated data from surveys of 163 partnerships (n = 448 partners) and seven in-depth case study interviews (n = 55 partners). Quantitatively, none of the measured characteristics of group diversity was associated with participatory decision-making within the partnerships. Qualitatively, we found that partnerships mainly benefited from membership differences in functional characteristics (e.g., skillset) but faced challenges from membership differences in sociocultural characteristics (e.g., genderand race). The integrated findings suggest the need to further understand how emergent group characteristics and how practices that engage in group diversity contribute to collective functioning of the partnerships. Attention to this area can help promote health equity achievements of CBPR partnerships.

Creating academic-community partnerships to jointly enhance advocacy and research on violence and disability: Two case examples.

This article describes the use of community-based participatory research (CBPR) to foster bidirectional and equitable academic-community partnerships in two studies related to interpersonal violence and disability. We analyzed our methods and experiences in conducting these studies to focus on the ways in which CBPR methodology was used to jointly promote and enhance research and advocacy surrounding violence and disability in the research processes themselves and the resulting assessment and intervention products. Our use of CBPR methodology allowed us to identify and address critical issues related to violence in the disability community, such as disability-related forms and experiences of violence, concerns and barriers linked to mandated reporting laws, and inaccessible measures and interventions, and to address them in research products. Additionally, our bidirectional academic-community partnerships led us to address overall accessibility of the research process itself as a means by which to amplify advocate voices in science. Full, meaningful, and equitable participation of people with disabilities at every stage of the research process allows for the creation of partnerships that jointly advance research and advocacy around violence and disability. Community-based participatory research with Indigenous youth: a critical review of literature

Community-based participatory research has gained traction as an innovative approach to research with underserved communities. Community-based participatory research with Indigenous youth, however, is less common. The purpose of this critical review is to explore the conduct of community-based participatory research with Indigenous youth over the past 20 years. Eighteen studies met inclusion criteria; they were evaluated for their respective use of theory, methodology, and findings. One study was quantitative, 16 were qualitative, and one employed mixed methods. Implications for future research, outreach and practice, and policy are discussed. Overall, youth highlighted connection to culture as a major strength and identified a need for programming centered in culture and community. The use of community-based participatory research represents a valuable opportunity to address unique challenges that Indigenous youth face in responsive, creative, and sustainable ways.

Using Collaborative Partnerships to Engage Firefighters in Rural Communities.

The purpose of this study was to utilize community-based participatory research (CBPR) methods to identify behaviors that may increase the exposure rates of firefighters to carcinogens and other occupational hazards. Key informant interviews and focus groups were conducted as part of a larger study that included exposure assessments at fire stations, in fire engines, and in personal vehicles. A purposive sample of five fire chiefs and leaders of the firefighter association was chosen, and these individuals were selected to participate in interviews. Unstructured interviews explored issues related to firefighter exposures and occupational hazards. Three focus groups were conducted over a three-month period. Following the focus groups, survey questions emphasizing the following three aspects of firefighting were developed: exposure to airborne smoke contaminants during fire suppression, accumulated exposure on turnout gear after fire suppression, and exposure to airborne contaminants at the fire departments. The use of community-based participatory research can be very beneficial, especially when conducting research with a group that may be hard to reach or have misgivings regarding researchers. By utilizing this approach in the current study, researchers were able to partner with a community that may be hard to reach and gain buy-in from community leadership.

Reproductive Justice, Public Black Feminism in Practice: A Reflection on Community-Based Participatory Research in Cincinnati

Research on reproductive justice has mainly, but not exclusively, appeared in academic literature in the context of grassroots social justice movements and as a theoretical framework for understanding the limitations of “reproductive choice” in the absence of social justice. But how can scholars design research to explore and understand reproductive (in)justice in the real lives of women of color? How can research partnerships between university scholars and community stewards be formed and sustained? What tensions and challenges are inherent in these efforts? And how can we find more equitable ways of sharing research findings and creating change with and not on behalf of our community? This paper reflects on the use of Community-Based Participatory Research (CBPR) in a reproductive justice research project focused on Black women residing in Cincinnati.

Use of Community-Based Participatory Research in Mental Health Studies With Older Adults: A Systematic Review

Community-based participatory research (CBPR), a bottom-up approach that community stakeholders and academics are involved equitably, is an effective approach for enhancing relevance and value in public health research and has gained popularity in recent decades. However, little is known about how CBPR can be used in mental health studies with older adults. This systematic review examined the current state of knowledge about how CBPR approach has been adopted in mental health research among older adults in different societies. According to the PRISMA guidelines, we searched five major databases and screened the literature using these criteria: 1) journal articles reporting use of CBPR in mental health research among older adults, 2) articles published in English language, 3) studies conducted in any settings with any mental health research. Initial search found 3,227 articles and preliminary screening identified 23 eligible articles. We found that around 90% of studies were conducted in the West. Most studies adopted CBPR to develop community-based mental health interventions or to revise current interventions or models while addressing the cultural needs of their studied population. Few studies adopted CBPR to evaluate existing mental health workshops or programmes. The extent of involvement of older adults in the CBPR approach varied across studies, from questionnaire design to programme evaluation. Our review uncovered ways of CBPR implementation across different societies and elements of successful implementation in CBPR practices in mental health research among older adults.

Using a Community-Based Participatory Research Model to Encourage Parental Involvement in Their Children’s Schools

Abstract Parental engagement with their children’s education has been shown to have positive effects for children’s academic outcomes; thus, learning ways to increase parental engagement can be beneficial for students. Because of the importance of understanding schools in the context of the community and the essential role that community can play in supporting schools, community-based participatory research (CBPR) may be a particularly effective approach to data collection because it engages community partners as well as parents and school personnel, and it gains information that leads to meaningful interventions. This study explored the use of CBPR within an economically and ethnically diverse school community with the intent of developing strategies to foster increased parental participation in their children’s education. Social workers and social work students facilitated focus groups with parents and community stakeholders to explore ways to increase parental engagement in their children’s school and to identify barriers to engagement. The article identifies next steps based on the recommendations of participants and describes the outcomes of preliminary implementation of these steps.

Practicing Hope: Enhancing Empowerment in Primary Health Care through Community-based Participatory Research.

The World Health Organization's (WHO's) Declaration of Alma Ata in 1978 made Primary Health Care (PHC) the official health policy of all WHO member countries, stressing the importance of multisectoral collaboration and community empowerment as critical for delivering quality primary healthcare and public health services to achieve social justice and health equity. Over forty years later, a divide remains between seeing individual patients in the traditional biomedical model and addressing population-level social determinants of health. One promising approach for the intentional and active integration of multi-sectoral partnering practices and community empowerment into Primary Health Care is the use of community-based participatory research (CBPR). The power of CBPR lies in its systematic approach to facilitating equitable collaboration of partners based on community priorities and strengths and is increasingly recognized for improving health equity outcomes. This paper highlights the use of CBPR as a promising practice for healthcare organizations to bridge the gap between the traditional individual patient focus and the comprehensive primary healthcare approach from WHO. We use a narrative case study from A Ministry of Sharing (AMOS) Health and Hope, a PHC organization in Nicaragua, to illustrate the use of the CBPR model as an implementation framework that facilitated the transformation of structures, policies, and practices as AMOS created multi-sector partnerships and embraced community empowerment as part of its strategic and comprehensive approach to health equity.

Environmental Engineering for the 21st Century: Increasing Diversity and Community Participation to Achieve Environmental and Social Justice.

Communities of color are disproportionately burdened by environmental pollution and by obstacles to influence policies that impact environmental health. Black, Hispanic, and Native American students and faculty are also largely underrepresented in environmental engineering programs in the United States. Nearly 80 participants of a workshop at the 2019 Association of Environmental Engineering and Science Professors (AEESP) Research and Education Conference developed recommendations for reversing these trends. Workshop participants identified factors for success in academia, which included adopting a broader definition for the impact of research and teaching. Participants also supported the use of community-based participatory research and classroom action research methods in engineering programs for recruiting, retaining, and supporting the transition of underrepresented students into professional and academic careers. However, institutions must also evolve to recognize the academic value of community-based work to enable faculty, especially underrepresented minority faculty, who use it effectively, to succeed in tenure promotions. Workshop discussions elucidated potential causal relationships between factors that influence the co-creation of research related to academic skills, community skills, mutual trust, and shared knowledge. Based on the discussions from this workshop, we propose a pathway for increasing diversity and community participation in the environmental engineering discipline by exposing students to community-based participatory methods, establishing action research groups for faculty, broadening the definition of research impact to improve tenure promotion experiences for minority faculty, and using a mixed methods approach to evaluate its impact.

Research, Writing, Editorial Reviewing, and Publishing as Social Justice Issues: The Effects of Stigma

People with disabilities face problems, not only because of their specific impairments but because of social stigma. This article focuses on stigma as it relates to a broad range of physical, social, and behavioral characteristics, and on the effect of stigma on academic research. Effects examined here include decisions regarding the ways in which research questions are defined, what research is funded and how it is funded, the ways in which research is conducted (e.g., the use of community-based participatory research versus traditional approaches), the way in which publication decisions are made, and the way in which research is received by professional colleagues and the public.

Using developmental evaluation and community-based participatory research to develop a model of supportive housing

Community-based participatory research (CBPR) and developmental evaluation (DE) have emerged over recent decades as separate approaches for addressing complex social issues. Current literature offers little with respect to the use of CBPR and DE in combination, although the two approaches are complementary. Through the current paper, we outline how CBPR and DE were used to develop a model of supportive housing for teen families. More specifically, we describe the structures and processes that contributed to this development, including (1) our partnership approach, (2) pooled resources, (3) regular opportunities for collaboration and reflection, (4) integration of multiple data sources, (5) ongoing feedback and knowledge dissemination, and (6) adjustments to program practices. We end by providing insights into the lessons that we learned through this project. Through this paper, we describe how researchers and community partners can collaboratively use CBPR and DE to develop a program model in complex community settings. Insights are offered that will be important for researchers, evaluators, and practitioners seeking to develop programming in response to complex community issues.

Voices of Community Partners: Perspectives Gained from Conversations of Community-Based Participatory Research Experiences.

Community-based participatory research (CBPR) has been documented as an effective approach to research with underserved communities, particularly with racial and ethnic minority groups. However, much of the literature promoting the use of CBPR with underserved communities is written from the perspective of the researchers and not from the perspective of the community partner. The purpose of this article is to capture lessons learned from the community partners’ insight gained through their experiences with CBPR. A multi-investigator consensus method was used to qualitatively code the transcripts of a CBPR story-telling video series. Seven major themes were identified: (1) expectations for engaging in research, (2) cultural humility, (3) respecting the partnership, (4) open communication, (5) genuine commitment, (6) valuing strengths and recognizing capacities, and (7) collaborating to yield meaningful results. The themes drawn from the community partner’s voice align with the tenets of CBPR advanced in the academic literature. More opportunities to include the community voice when promoting CBPR should be undertaken to help introduce the concepts to potential community partners who may be research cautious.

An Approach to Engaging Schools In Oral Health Initiatives: The Howard Meharry Adolescent Caries Study (HMACS).

Use of community-based participatory research (CBPR) principles can help identify strategies for development and implementation of studies that can address oral health disparities disfavoring African American youth. This paper summarizes approaches of the Howard Meharry Adolescent Caries Study (HMACS) to provide sustained oral health services beyond the life of a research study.

A Participatory Group Process within a Health Collaborative to Collect and Disseminate Needs Assessment Data.

Inclusion of community-based participatory research (CBPR) principles within an epidemiological study design is anticipated to promote the effective integration of knowledge derived from research data collection into the strategic planning and collaborative effort of a group focused on improving community-based health outcomes. This article describes how CBPR principles were used throughout survey design, data collection, analyses and results dissemination. A health needs assessment survey was developed as a partnership between an academic research group from the Virginia Commonwealth University (VCU), a neighborhood-level health and wellness collaborative, and community residents. Survey development, pilot testing, data collection, results dissemination, and action among the health collaborative members were carried out using CBPR principles. Feedback from participants at each stage was collected to improve the overall process of data use within the collaborative. Data from 1,064 adult participants living in the East End of Richmond, Virginia, were collected. The use of CBPR and epidemiological approaches was successful in promoting effective collaborative efforts, as indicated by 1) sustained organization-level partner participation in the development of survey items and donation of participant incentives; 2) positive feedback from resident- and organization-level participants in a preliminary data dissemination event; 3) strong resident participation at community-wide dissemination events; 4) increases in survey-related blog traffic occurring in conjunction with community-wide dissemination events; and 5) the use of process protocols and results within similar collaboratives across the city. Use of CBPR principles with epidemiological methods is a powerful tool for facilitating effective community-level strategic planning within health collaboratives.

Peace of mind: A community-industry-academic partnership to adapt dementia technology for Anishinaabe communities on Manitoulin Island.

IntroductionAging Technologies for Indigenous Communities in Ontario (ATICON) explores the technology needs of Anishinaabe older adults in the Manitoulin region of Northern Ontario. Our program of research addresses inequitable access to supportive technologies that may allow Indigenous older adults to successfully age in place.MethodsUsing Indigenous research methodologies (IRM) and community-based participatory research (CBPR) we explored the acceptability of CareBand - a wearable location and activity monitoring device for people living with dementia using a LoRaWAN, a low-power wide-area network technology. We conducted key informant consultations and focus groups with Anishinaabe Elders, formal and informal caregivers, and health care providers (n = 29) in four geographically distinct regions.ResultsOverall, participants agreed that CareBand would improve caregivers’ peace of mind. Our results suggest refinement of the technology is necessary to address the challenges of the rural geography and winter weather; to reconsider aesthetics; address privacy and access; and to consider the unique characteristics of Anishinaabe culture and reserve life.ConclusionAll three partners in this research, including the Indigenous communities, industry partner, and academic researchers, benefited from the use of CBPR and IRM. As CareBand is further developed, community input will be crucial for shaping a useful and valued device.

PLACEKEEPING AND SENSE OF PLACE: USING PHOTOVOICE TO HIGHLIGHT NEIGHBORHOOD DISPLACEMENT

There has been growing interest in the use of community-based participatory research (CBPR) in gerontology. Photovoice, one of several qualitative methods utilized in CBPR, pairs participants with photography to identify and represent issues of importance. This paper explores photovoice as a tool for meaning making and preserving a ‘sense of place’ in a gentrifying context in New York City. Older residents describe pending neighborhood displacement due to gentrification using photographs. Using these themes and a range of visual media, older adults mobilize preservation and resistance efforts to gentrification. The paper concludes with implications and directions for future research.
