Localization of the inferior alveolar canal using ultralow dose CT with iterative reconstruction techniques.

Objectives:To compare subjective and objective localization of the inferior alveolar canal (IAC) on multidetector CT (MDCT) images obtained by ultralow doses in combination with the reconstruction techniques of filtered backprojection (FBP), adaptive statistical iterative reconstruction (ASIR), or model-based iterative reconstruction (MBIR) as compared to standard dose MDCT and FBP.Methods:Three cadavers were imaged with a reference standard dose MDCT examination (volume CT dose index: 29.4 mGy) reconstructed with FBP and 5 low dose protocols (LD1-5) (volumeCT dose index: 4.19, 2.64, 0.99, 0.53, 0.29 mGy) reconstructed with FBP, ASIR 50, ASIR 100, and MBIR. Linear measurements from the crest of the ridge to the roof of the IAC were recorded. The results from the test protocols were compared with those from the reference using Bland–Altman plots.Results:Only three test protocols allowed the identification of the position of the IAC on all the sample sites: LD1/FBP and LD1/ASIR 100 and LD2/FBP. All three protocols allowed identification of the IAC with comparable results to the reference dose protocol; the 95% confidence interval limits for the measurement differences were ± 0.41 mm, but the differences were not statistically significant. The calculated effective dose for the LD2 protocol, for a scan length of 5 cm, was 27.7 µSv.Conclusions:Using FBP, comparable IAC measurements were achieved with 91% reduction in dose compared with a standard exposure protocol. The use of ASIR and MBIR did not improve identification of the IAC in MDCT low dose images.

Adaptive statistical iterative reconstruction use for radiation dose reduction in pediatric lower-extremity CT: impact on diagnostic image quality.

For the past several years, increased levels of imaging radiation and cumulative radiation to children has been a significant concern. Although several measures have been taken to reduce radiation dose during computed tomography (CT) scan, the newer dose reduction software adaptive statistical iterative reconstruction (ASIR) has been an effective technique in reducing radiation dose. To our knowledge, no studies are published that assess the effect of ASIR on extremity CT scans in children. To compare radiation dose, image noise, and subjective image quality in pediatric lower extremity CT scans acquired with and without ASIR. The study group consisted of 53 patients imaged on a CT scanner equipped with ASIR software. The control group consisted of 37 patients whose CT images were acquired without ASIR. Image noise, Computed Tomography Dose Index (CTDI) and dose length product (DLP) were measured. Two pediatric radiologists rated the studies in subjective categories: image sharpness, noise, diagnostic acceptability, and artifacts. The CTDI (p value = 0.0184) and DLP (p value <0.0002) were significantly decreased with the use of ASIR compared with non-ASIR studies. However, the subjective ratings for sharpness (p < 0.0001) and diagnostic acceptability of the ASIR images (p < 0.0128) were decreased compared with standard, non-ASIR CT studies. Adaptive statistical iterative reconstruction reduces radiation dose for lower extremity CTs in children, but at the expense of diagnostic imaging quality. Further studies are warranted to determine the specific utility of ASIR for pediatric musculoskeletal CT imaging.

Dose reduction in paediatric cranial CT via iterative reconstruction: a clinical study in 78 patients

To assess how adaptive statistical iterative reconstruction (ASIR) contributes to dose reduction and affects image quality of non-contrast cranial computed tomography (cCT) in children. Non-contrast cranial CT acquired in 78 paediatric patients (age 0-12 years) were evaluated. The images were acquired and processed using four different protocols: Group A (control): 120kV, filtered back projection (FBP), n=18; Group B: 100kV, FBP, n=22; Group C: 100kV, scan and reconstruction performed with 20% ASIR, n=20; Group D1: 100kV, scan and reconstruction performed with 30% ASIR, n=18; Group D2: raw data from Group D1 reconstructed using a blending of 40% ASIR and 60% FBP, n=18. The effective dose was calculated and the image quality was assessed quantitatively and qualitatively. Compared to Group A, Groups C and D1/D2 showed a significant reduction of the dose-length product (DLP) by 34.4% and 64.4%, respectively. All experimental groups also showed significantly reduced qualitative levels of noise, contrast, and overall diagnosability. Diagnosis-related confidence grading showed Group C to be adequate for everyday clinical practice. Quantitative measures of Groups B and C were comparable to Group A with only few parameters compromised. Quantitative scores in Groups D1 and D2 were mainly lower compared to Group A, with Group D2 performing better than Group D1. Group D2 was considered adequate for follow-up imaging of severe acute events such as bleeding or hydrocephalus. The use of ASIR combined with low tube voltage may reduce radiation significantly while maintaining adequate image quality in non-contrast paediatric cCT.

Radiation dose reduction in paediatric coronary computed tomography: assessment of effective dose and image quality.

To assess the impact of different protocols on radiation dose and image quality for paediatric coronary computed tomography (cCT). From January-2012 to June-2014, 140 children who underwent cCT on a 64-slice scanner were included. Two consecutive changes in imaging protocols were performed: 1) the use of adaptive statistical iterative reconstruction (ASIR); 2) the optimization of acquisition parameters. Effective dose (ED) was calculated by conversion of the dose-length product. Image quality was assessed as excellent, good or with significant artefacts. Patients were divided in three age groups: 0-4, 5-7 and 8-18years. The use of ASIR combined to the adjustment of scan settings allowed a reduction in the median ED of 58%, 82% and 85% in 0-4, 5-7 and 8-18years group, respectively (7.3 ± 1.4 vs 3.1 ± 0.7mSv, 5.5 ± 1.6 vs 1 ± 1.9mSv and 5.3 ± 5.0 vs 0.8 ± 2.0mSv, all p < 0,05). Prospective protocol was used in 51% of children. The reduction in radiation dose was not associated with reduction in diagnostic image quality as assessed by the frequency of coronary segments with excellent or good image quality (88%). cCT can be obtained at very low radiation doses in children using ASIR, and prospective acquisition with optimized imaging parameters. • Using ASIR allows 25 % to 41 % reduction in the ED. • Prospective protocol is used up to 51 % of children after premedication. • Low dose is possible using ASIR and optimized prospective paediatric cCT.

Comparison of adaptive statistical iterative and filtered back projection reconstruction techniques in quantifying coronary calcium

BackgroundAdaptive statistical iterative reconstruction (ASIR) has been used to reduce radiation dose in cardiac computed tomography. However, change of image parameters by ASIR as compared to filtered back projection (FBP) may influence quantification of coronary calcium. ObjectiveTo investigate the influence of ASIR on calcium quantification in comparison to FBP. MethodsIn 352 patients, CT images were reconstructed using FBP alone, FBP combined with ASIR 30%, 50%, 70%, and ASIR 100% based on the same raw data. Image noise, plaque density, Agatston scores and calcium volumes were compared among the techniques. ResultsImage noise, Agatston score, and calcium volume decreased significantly with ASIR compared to FBP (each P < 0.001). Use of ASIR reduced Agatston score by 10.5% to 31.0%. In calcified plaques both of patients and a phantom, ASIR decreased maximum CT values and calcified plaque size. ConclusionIn comparison to FBP, adaptive statistical iterative reconstruction (ASIR) may significantly decrease Agatston scores and calcium volumes.

Qualitative and quantitative assessment of smoking-related lung disease: effect of iterative reconstruction on low-dose computed tomographic examinations.

The purpose of this research is to examine the role that differing levels of adaptive statistical iterative reconstruction (ASIR) have on the qualitative and quantitative assessment of smoking-related lung disease. Institutional board review approval was obtained. A total of 52 patients undergoing clinically indicated low-dose computed tomographic (CT) examinations of the chest (100 kVp, 65 mAs, mean radiation dose 1.0±0.12 mSv), with reconstruction of data with different levels of blended ASIR (0%, 40%, and 100%), were consented. Qualitative assessment of CT data sets was performed by 2 trained thoracic radiologists blinded to clinical history, spirometry, and quantitative data for the presence of emphysema (%/lung zone) and the degree of respiratory bronchiolitis. Quantitative analysis was performed (Apollo Image analysis, VIDA Diagnostics) to assess emphysema and airway measures of chronic obstructive pulmonary disease. The application of ASIR results in alterations in both qualitative and quantitative assessment of smoking-related lung disease. As levels of ASIR increased, both readers scored more respiratory bronchiolitis (P<0.05). At increased levels of ASIR (ie, 100% vs. 0%), the amount of emphysema measured (% below -950 HU) decreased, the number of airways measured diminished, and the airway thickness (Pi10mm) increased (P<0.001). The use of ASIR alters both the qualitative and quantitative assessment of smoking-related lung disease. Although a powerful tool to allow dose reduction, caution must be exercised when iterative reconstruction techniques are utilized when evaluating CT examinations for findings of chronic obstructive pulmonary disease.

Adaptive statistical iterative reconstruction: reducing dose while preserving image quality in the pediatric head CT examination

Over the last decade there has been escalating concern regarding the increasing radiation exposure stemming from CT exams, particularly in children. Adaptive statistical iterative reconstruction (ASIR) is a relatively new and promising tool to reduce radiation dose while preserving image quality. While encouraging results have been found in adult head and chest and body imaging, validation of this technique in pediatric population is limited. The objective of our study was to retrospectively compare the image quality and radiation dose of pediatric head CT examinations obtained with ASIR compared to pediatric head CT examinations without ASIR in a large patient population. Retrospective analysis was performed on 82 pediatric head CT examinations. This group included 33 pediatric head CT examinations obtained with ASIR and 49 pediatric head CT examinations without ASIR. Computed tomography dose index (CTDIvol) was recorded on all examinations. Quantitative analysis consisted of standardized measurement of attenuation and the standard deviation at the bilateral centrum semiovale and cerebellar white matter to evaluate objective noise. Qualitative analysis consisted of independent assessment by two radiologists in a blinded manner of gray-white differentiation, sharpness and overall diagnostic quality. The average CTDIvol value of the ASIR group was 21.8 mGy (SD = 4.0) while the average CTDIvol for the non-ASIR group was 29.7 mGy (SD = 13.8), reflecting a statistically significant reduction in CTDIvol in the ASIR group (P < 0.01). There were statistically significant reductions in CTDI for the 3- to 12-year-old ASIR group as compared to the 3- to 12-year-old non-ASIR group (21.5 mGy vs. 30.0 mGy; P = 0.004) as well as statistically significant reductions in CTDI for the >12-year-old ASIR group as compared to the >12-year-old non-ASIR group (29.7 mGy vs. 49.9 mGy; P = 0.0002). Quantitative analysis revealed no significant difference in the homogeneity of variance in the ASIR group compared to the non-ASIR group. Radiologist assessment of gray-white differentiation, sharpness and overall diagnostic quality in ASIR examinations was not substantially different compared to non-ASIR examinations. The use of ASIR in pediatric head CT examinations allows for a 28% CTDIvol reduction in the 3- to 12-year-old age group and a 48% reduction in the >12-year-old age group without substantially affecting image quality.

Adaptive statistical iterative reconstruction and bismuth shielding for evaluation of dose reduction to the eye and image quality during head CT

There is a concern regarding the adverse effects of increasing radiation doses due to repeated computed tomography (CT) scans, especially in radiosensitive organs and portions thereof, such as the lenses of the eyes. Bismuth shielding with an adaptive statistical iterative reconstruction (ASIR) algorithm was recently introduced in our clinic as a method to reduce the absorbed radiation dose. This technique was applied to the lens of the eye during CT scans. The purpose of this study was to evaluate the reduction in the absorbed radiation dose and to determine the noise level when using bismuth shielding and the ASIR algorithm with the GE DC 750 HD 64-channel CT scanner for CT of the head of a humanoid phantom. With the use of bismuth shielding, the noise level was higher in the beam-hardening artifact areas than in the revealed artifact areas. However, with the use of ASIR, the noise level was lower than that with the use of bismuth alone; it was also lower in the artifact areas. The reduction in the radiation dose with the use of bismuth was greatest at the surface of the phantom to a limited depth. In conclusion, it is possible to reduce the radiation level and slightly decrease the bismuth-induced noise level by using a combination of ASIR as an algorithm process and bismuth as an in-plane hardware-type shielding method.

Adaptive statistical iterative reconstruction reduces patient radiation dose in neuroradiology CT studies

Adaptive statistical iterative reconstruction (ASIR) can decrease image noise, thereby generating CT images of comparable diagnostic quality with less radiation. The purpose of this study is to quantify the effect of systematic use of ASIR versus filtered back projection (FBP) for neuroradiology CT protocols on patients' radiation dose and image quality. We evaluated the effect of ASIR on six types of neuroradiologic CT studies: adult and pediatric unenhanced head CT, adult cervical spine CT, adult cervical and intracranial CT angiography, adult soft tissue neck CT with contrast, and adult lumbar spine CT. For each type of CT study, two groups of 100 consecutive studies were retrospectively reviewed: 100 studies performed with FBP and 100 studies performed with ASIR/FBP blending factor of 40 %/60 % with appropriate noise indices. The weighted volume CT dose index (CTDIvol), dose-length product (DLP) and noise were recorded. Each study was also reviewed for image quality by two reviewers. Continuous and categorical variables were compared by t test and free permutation test, respectively. For adult unenhanced brain CT, CT cervical myelography, cervical and intracranial CT angiography and lumbar spine CT both CTDIvol and DLP were lowered by up to 10.9 % (p < 0.001), 17.9 % (p = 0.005), 20.9 % (p < 0.001), and 21.7 % (p = 0.001), respectively, by using ASIR compared with FBP alone. Image quality and noise were similar for both FBP and ASIR. We recommend routine use of iterative reconstruction for neuroradiology CT examinations because this approach affords a significant dose reduction while preserving image quality.

Model-based Iterative Reconstruction for Multi–Detector Row CT Assessment of the Adamkiewicz Artery

Purpose To determine if model-based iterative reconstruction (MBIR) can improve visualization of the Adamkiewicz artery on multi–detector row computed tomographic (CT) images compared with adaptive statistical iterative reconstruction (ASIR) and filtered back projection (FBP). Materials and Methods This retrospective study was approved by the institutional review board, and written informed consent for the CT examination was obtained. Thirty-three patients underwent contrast material–enhanced 64-section multi–detector row CT for assessment of aortic aneurysm or dissection. Helical data were reconstructed by using FBP, ASIR, and MBIR. The signal-to-noise ratio of the aorta and contrast-to-noise ratio of the anterior spinal artery relative to the spinal cord were measured on multiplanar reformatted images. Visualization of the Adamkiewicz artery and its continuity with the intercostal or lumbar artery were evaluated by using a four-point scale. All image analyses were performed by two blinded, independent observers. The one-way analysis of variance and the Wilcoxon signed-rank test were used for statistical analysis. Results MBIR showed significantly better signal-to-noise and contrast-to-noise ratios than did ASIR and FBP (P < .05 for all comparisons) with good interobserver agreement (intraclass correlation coefficient of 0.93 for signal-to-noise ratio and 0.75 for contrast-to-noise ratio). The visualization score of the Adamkiewicz artery was also significantly better when MBIR was used (3.4 ± 0.8 and 3.6 ± 0.7 for observers A and B, respectively) than when ASIR (2.7 ± 1.1 and 3.0 ± 1.0, respectively) or FBP (2.5 ± 1.2 and 3.1 ± 0.9, respectively) was used. Conclusion Use of the MBIR algorithm led to improved multi–detector row CT visualization of the Adamkiewicz artery when compared with the use of ASIR and FBP. © RSNA, 2013

Reducing abdominal CT radiation dose with the adaptive statistical iterative reconstruction technique in children: a feasibility study

The use of the adaptive statistical iterative reconstruction (ASIR) algorithm has been shown to reduce radiation doses in adults undergoing abdominal CT studies while preserving image quality. To our knowledge, no studies have been done to validate the use of ASIR in children. To retrospectively evaluate differences in radiation dose and image quality in pediatric CT abdominal studies utilizing 40% ASIR compared with filtered-back projection (FBP). Eleven patients (mean age 8.5 years, range 2-17 years) had separate 40% ASIR and FBP enhanced abdominal CT studies on different days between July 2009 and October 2010. The ASIR studies utilized a 38% mA reduction in addition to our pediatric protocol mAs. Study volume CT dose indexes (CTDI(vol)) and dose-length products (DLP) were recorded. A consistent representative image was obtained from each study. The images were independently evaluated by two radiologists in a blinded manner for diagnostic utility, image sharpness and image noise. The average CTDI(vol) and DLP for the 40% ASIR studies were 4.25 mGy and 185.04 mGy-cm, compared with 6.75 mGy and 275.79 mGy-cm for the FBP studies, representing 37% and 33% reductions in both, respectively. The radiologists' assessments of subjective image quality did not demonstrate any significant differences between the ASIR and FBP images. In our experience, the use of 40% ASIR with a 38% decrease in mA lowers the radiation dose for children undergoing enhanced abdominal examinations by an average of 33%, while maintaining diagnostically acceptable images.

Radiation Dose Reduction With Chest Computed Tomography Using Adaptive Statistical Iterative Reconstruction Technique

To assess radiation dose reduction and image quality for weight-based chest computed tomographic (CT) examination results reconstructed using adaptive statistical iterative reconstruction (ASIR) technique. With local ethical committee approval, weight-adjusted chest CT examinations were performed using ASIR in 98 patients and filtered backprojection (FBP) in 54 weight-matched patients on a 64-slice multidetector CT. Patients were categorized into 3 groups: 60 kg or less (n = 32), 61 to 90 kg (n = 77), and 91 kg or more (n = 43) for weight-based adjustment of noise indices for automatic exposure control (Auto mA; GE Healthcare, Waukesha, Wis). Remaining scan parameters were held constant at 0.984:1 pitch, 120 kilovolts (peak), 40-mm table feed per rotation, and 2.5-mm section thickness. Patients' weight, scanning parameters, and CT dose index volume were recorded. Effective doses (EDs) were estimated. Image noise was measured in the descending thoracic aorta at the level of the carina. Data were analyzed using analysis of variance. Compared with FBP, ASIR was associated with an overall mean (SD) decrease of 27.6% in ED (ASIR, 8.8 [2.3] mSv; FBP, 12.2 [2.1] mSv; P < 0.0001). With the use of ASIR, the ED values were 6.5 (1.8) mSv (28.8% decrease), 7.3 (1.6) mSv (27.3% decrease), and 12.8 (2.3) mSv (26.8% decrease) for the weight groups of 60 kg or less, 61 to 90 kg, and 91 kg or more, respectively, compared with 9.2 (2.3) mSv, 10.0 (2.0) mSv, and 17.4 (2.1) mSv with FBP (P < 0.0001). Despite dose reduction, there was less noise with ASIR (12.6 [2.9] mSv) than with FBP (16.6 [6.2] mSv; P < 0.0001). Adaptive statistical iterative reconstruction helps reduce chest CT radiation dose and improve image quality compared with the conventionally used FBP image reconstruction.
