Methodology for assessing the accessibility of buildings and structures to low-mobility groups of the population

The article has developed and presented a universal systematization of regulatory requirements regulating the accessibility of urban infrastructure facilities to low-mobility groups of the population. A methodology for assessing the accessibility of buildings and structures to low-mobility categories of citizens has been developed. Based on the monitoring data of public buildings and social facilities in the city of Orel and the Oryol region, a numerical implementation of the proposed accessibility assessment methodology was performed using the example of structural and functional zones of one of the health facilities – the Maloarkhangelsk clinic. The proposed systematization of regulatory requirements and methodology will allow to identify the most problematic places in ensuring the accessibility of urban infrastructure, to quantify the criteria for the adaptation of urban facilities for people with disabilities. The assessment results can serve as tools for increasing the effectiveness of strategic decisions made to improve the standard of living of the population in an urbanized area as a whole.

Analysis of access to amenities in peri-urban areas in West Africa: A GIS-based approach in Abomey-Calavi, Republic of Benin

Urban facilities and services are essential to human life. Access to them varies according to the geographical location of the population, whether urban, peri-urban or rural, and according to the modes of transport available. In view of the rapid development of peri-urban areas in developing countries, questions are being asked about the ability of the inhabitants of these areas to access these facilities and services. This study examines the ability of the inhabitants of Hêvié, Ouèdo and Togba, three peri-urban districts of Abomey-Calavi in the Republic of Benin, to access commercial, educational, school and health facilities. To this end, we have adopted a GIS-based methodology. It is a combination of isochronal method and accessibility utility measurement. The isochrones were produced according to the main modes of travel recorded on the study area and over a time t ≤ 20 min divided into intervals of 05 min. Analysis of the data enabled us to understand that the main modes of travel adopted by residents are walking, motorcycle and car. Access to educational and health facilities is conditioned by the mode of travel used. Access to commercial and entertainment facilities in t ≤ 20 min is not correlated with the modes of transport used.

Gender differences in the effects of urban environment on nighttime exercise behaviours: a qualitative study

ObjectiveWith the acceleration of urbanization, nighttime exercise behaviours have rapidly emerged. Existing research indicates a correlation between urban environments and physical activity; however, studies focusing specifically on nighttime are still insufficient, particularly regarding gender differences. This study aims to identify the key factors in urban environments that influence residents’ nighttime exercise behaviours and to explore the gender differences within these influences.MethodsPurposeful sampling was employed to conduct semi-structured interviews with 30 residents who regularly engage in nighttime exercise. All transcribed interviews were analyzed using Colaizzi’s phenomenological data analysis method.ResultsThe findings revealed that physical environment and environmental perception are the two primary factors influencing nighttime exercise behaviour. These factors are further divided into 10 specific sub-themes: lighting, green spaces, site facilities and layout, traffic coherence, entertainment facilities, smart sports equipment, sense of safety, convenience, pleasure, and sense of belonging.ConclusionFemales demonstrate a heightened sensitivity to the perception of the physical environment, placing greater emphasis on the feelings and experiences it provides. Males, on the other hand, focus more on the direct impact of the physical environment, such as its specific effects on exercise performance. Future urban planning and public policy should give greater consideration to gender differences in the use of urban exercise facilities, ensuring that nighttime exercise environments meet the needs of residents of different genders. This approach will contribute to enhancing overall community vitality and improving residents’ health.

Effectiveness of autism training program introduction at primary health care (PHC) level

Background: Autism spectrum disorder (ASD) has become a public health issue in many countries especially in developing countries. Family members who have a toddler with ASD face significant physical, emotional, and financial challenges. Early detection is crucial to achieve improvement in children with ASD. Objectives: The aim of the current research is to study the implementation of a training program for Autism in 4 health care facilities and to evaluate its initial outcomes. Methodology: An interventional study conducted among 63 health care workers dealing with children 18-30 months in 4 health care facilities (2 urban health care facilities and 2 rural health care facilities) in Giza governorate. A tailored autism health education message as well as training on using the Arabic validated version of the Modified Checklist for Autism in Toddlers (M-CHAT) were provided to the HCWs. Results: 58.7% of participating primary health care workers (HCWs) had a satisfactory level of awareness about childhood autism prior to the intervention, and 71.4% had a satisfactory level of understanding following it. When comparing the study subjects' pre-intervention knowledge scores to their post-intervention knowledge scores, there was a statistically significant improvement in their knowledge about autism (p < 0.001). Conclusion: The study's findings validate the value of specialized training programs in raising healthcare workers' awareness of childhood autism. Recommendations: Incorporation of autism spectrum disorder (ASD) within the educational curriculum of medical and nursing schools and ongoing education and training in ASD for health care workers.

Availability and readiness of public health facilities to provide differentiated service delivery models for HIV treatment in Zambia: implications for better treatment outcomes

BackgroundThere is persistent pressure on countries with a high burden of HIV infection to reach desired targets for HIV treatment outcomes. This has led to moving from the “one-size-fits-all” model to differentiated service delivery (DSD) models, which are meant to be more patient-centered and efficient but without compromising on the quality of patient care. However, for DSD models to be efficient, facilities should have indicators of HIV services available and ready to provide the DSD models. We aimed to assess the availability of HIV service indicators and the readiness of facilities to provide DSD models for HIV treatment in selected public health facilities in Zambia.MethodsWe conducted a nationwide cross-sectional survey among public health facilities in Zambia that provide antiretroviral therapy (ART) services. We used an interviewer-administered questionnaire based on a World Health Organization (WHO) Service Availability Readiness Assessment (SARA) tool to assess the availability of HIV service indicators and the readiness of facilities to implement DSD models for HIV treatment. Availability and readiness were considered latent constructs, and therefore, we used structural equation modeling (SEM) to determine the correlations between them and their respective indicators.ResultsOf 60 public health ART facilities, the overall availability of HIV service indicators was 80.0% (48/60), and readiness to provide the DSD models was 81.7% (48/60). However, only 48 and 39% of the facilities had all indicators of availability and readiness, respectively. Retention in care for HIV multidisciplinary teams was more likely to occur in urban areas than in rural areas. SEM showed that the standardized estimate between availability and readiness was significantly and positively correlated (r = 0.73, p < 0.0001). In addition, both availability and readiness were significantly and positively correlated with most of their respective indicators.ConclusionAlthough most facilities had available HIV service indicators and were ready to provide DSD models, most facilities did not have all indicators of availability and readiness. In addition, there were differences between rural and urban facilities in some indicators. There is a need for persistent and heightened efforts meant to implement DSD in HIV treatment, especially in rural areas to accelerate reaching the desired HIV treatment outcomes.

Acceptability of single-dose HPV vaccination schedule among health-care professionals in Kenya: a mixed-methods study.

The World Health Organization recommends a single-dose human papillomavirus (HPV) vaccination schedule for girls and boys to accelerate progress toward cervical cancer elimination. We applied the Theoretical Framework of Acceptability (TFA) within the context of HPV vaccination to assess the acceptability of a single-dose schedule among health-care professionals in Kenya. A REDCap survey was developed using relevant Theoretical Framework of Acceptability domains and validated with health-care professionals. Descriptive analyses and multivariate Poisson regression were conducted to assess factors associated with increased acceptability. Free-text responses were analyzed using a rapid qualitative approach, and findings were presented using a joint display. Among 385 responses, 74.2% of health-care professionals were female and 48.6% were nurses. On average, respondents had been in their position for 60 months, and one-third (33.2%) were based at level-4 facilities. The majority (75.84%) thought that giving a single-dose of the HPV vaccine to adolescent girls and young women was either acceptable or very acceptable. Qualitative findings highlighted that lack of information was the underlying reason for health-care professionals who were resistant, and most clinicians thought that a singled-dose schedule was less burdensome to clinicians and patients. Hospital directors had a non-statistically significantly lower acceptability likelihood than nurses (incident rate ratio = 0.93, 95% confidence interval = 0.45 to 1.71) and health-care professionals at urban facilities had a non-statistically significantly lower acceptability likelihood than clinicians in rural facilities (incident rate ratio = 0.97, 95% confidence interval = 0.83 to 1.13). Although not statistically significant, predictors of increased acceptability provide information to tailor strategies to increase HPV vaccination coverage and accelerate progress toward cervical cancer elimination.

Environmental and economic assessment of urban wastewater reclamation from ultrafiltration membrane-based tertiary treatment: Effect of seasonal dynamic

This study aimed to assess the environmental and economic performance of an ultrafiltration (UF) tertiary treatment of effluent from an urban wastewater treatment facility. Data from a UF demonstration plant composed of commercially available equipment, including industrial hollow-fiber membranes was used to project a full-scale facility. The results from the demonstration plant recommended different ranges of transmembrane fluxes and sparging air demands under summer and winter conditions to prevent excessive fouling. The energy balance of the full-scale facility would be 0.308 ± 0.112 kWh·m−3 in summer and 0.140 ± 0.040 kWh·m−3 in winter, with blowers' being the main consumers (86–93 %). CAPEX accounted for €0.030 ± 0.002·m−3 in summer and €0.027 ± 0.002·m−3 in winter and membrane acquisition represented 66–69 % of the investment cost. Energy expenditure was the major OPEX cost (66–79 %), with a total operating cost of €0.077 ± 0.023·m−3 and €0.042 ± 0.008·m−3 in summer and winter, respectively. The final average value obtained for the TAC was €0.107 m−3 in summer and €0.068 m−3 in winter. The environmental assessment confirmed optimizing energy consumption and membrane requirements as the main factors influencing environmental sustainability. Specifically, summer and winter emissions of 0.079–0.175 and 0.043–0.079 kgCO2eq·m−3 (Global warming potential); 8.1 · 10−4–1.7 · 10−3 and 4.8 · 10−3–8.1 · 10−3 m3·m−3 (water consumption); 0.019–0.041 and 0.010–0.019 kg oileq·m−3 (fossil fuel scarcity); and 1.4 · 10−4–2.9 · 10−4 and 7.7 · 10−4–1.4 · 10−4 kg Cueq·m−3 (mineral resource scarcity) were calculated, respectively. The obtained permeate quality complied with the most stringent Spanish and EU regulations.

Impact of urban facilities spatial inequality on sustainable travel mode.

With the implementation of sustainable development objectives in developing countries, urban planning, land use regulation, and urban mobility policies are expected to help reduce inequalities in access to urban facilities. Urban transport policies are also expected to encourage travel by non-motorised modes and public transport. These are considered to be the sustainable modes of urban transport. In this paper, we investigate how inequality of urban facilities impacts trips made by sustainable modes in the city of Santiago de Chile. We use a Poisson regression model and its geographical extension, the geographically weighted Poisson regression model (GWPR). The results suggest that the inequality of urban facilities impacts trips made by sustainable modes. The variables with the highest relevance are the spatial distribution of mixed land use, the spatial distribution of urban services related to transport infrastructure, primary and secondary education, as well as the spatial distribution of demographic variables related to people's life cycle.

Development of an Index System for Public Toilets Based on HRWS-PS Theory and Improvement Strategies

As an integral part of the urban public health infrastructure, public toilets play a significant role in shaping a city’s image. With the advancement of the economy, people have increasingly higher expectations for public toilets. While existing standards primarily focus on physical indicators, there is still room to incorporate measures that prioritize humanistic care. Therefore, based on the Human Rights to Water and Sanitation in Public Spaces (HRWS-PS) standards, this study develops a comprehensive index system for public toilets with a specific emphasis on humanistic care, utilizing expert questionnaires and employing methods such as the analytic hierarchy process (AHP) and entropy weight, and highlights safety and quality as paramount concerns for users, followed by usability and accessibility as secondary needs. Acceptability, which encompasses additional amenities, depends on meeting these secondary needs through solid basic design and construction practices. Affordability, related to consumer costs, is comparatively less demanding. By applying this framework, an analysis of the current state of upgrading urban public toilets in Shijiazhuang was conducted, leading to recommendations for improvements across four key areas: environmental sanitation, architectural design, infrastructure development, and urban planning. The final results provide practical insights for evaluating and suggesting enhancements to ongoing toilet revolution initiatives and urban basic service facilities while also informing urban renewal efforts and designs.

Forced displacement and the healthcare workforce crisis: Venezuelan healthcare workers in Peru

Abstract Background The migration of healthcare workforces tends to be economically based benefiting high income countries, while draining lower-income countries of workers and skills. However, national instability or civil conflict may also have the effect of forcing out health workers. Few articles focus on the experiences of these types of migrants. This study aims to close a gap in the migration debate by exploring the capacity of integrating displaced healthcare workers in middle-to-low resourced health systems, using Peru as a case study. Methods The study draws on a policy analysis, statistical data and expert information from Peru. Results Peru has become the second largest Latin American destination for Venezuelan forced displaced migrants, a number of which are healthcare workers. While the exact numbers of these workers is unknown, it is estimated that 4,000 and 3,000 physicians and about 2,500 nurses and health technicians from Venezuela reside in Peru. These workers find entry into the heath system difficult due to bureaucratic and costly registration and qualification validation procedures. However, during Covid-19 the conditions were relaxed, and a large number of these workers entered the healthcare workforce. They were primarily physicians and worked in urban medical facilities, though there was some distribution across the country’s departments. This avenue to the healthcare workforce allowed the mobilisation of dormant health skills and assisted to lift density numbers. Conclusions Peru’s experiences bring into focus the potential benefits of mobilising new resources through integrating displaced healthcare worker migrants. It is too early to assess the sustainability of these policies on the country’s healthcare workforce and its Universal Health Coverage goals, yet there seem to be some positive experiences. Assessing the Prospects and Risks of Delivering Sustainable Urban Development Through 3D Concrete Printing Implementation

The article presents the results of a comprehensive study of the use of 3D Concrete printing (3DCP) technology to create urban infrastructure facilities according to sustainable development principles. The work includes a study of scientific articles on the subject area under consideration, a survey of additive construction market participants, as well as an analysis and generalization of promising areas for technology development and methods for improving the quality of objects erected using 3DCP. As part of the conducted literature review, publications included in the Scopus database for the period 2015–2024 were selected for analysis using the keywords ‘Sustainable development + 3DCP’ and ‘Sustainable construction + 3DCP’. The following conclusions were made: (i) the most popular publications are review articles about the development of materials and technologies for 3DCP and (ii) the most sought-after are the studies in the field of partial application of 3DCP technology, existing equipment and materials for 3DCP, and assessment of the effectiveness and cost-effectiveness of 3DCP use. For this purpose, a questionnaire was developed consisting of three blocks: equipment and technologies; structures and materials for 3DCP; the ecology and economics of 3DCP applicability. As a result, four main risks have been identified, which represent promising areas for 3DCP development.

Monitoring patients on ART within the CCMDD programme and those attending an urban healthcare facility in KwaZulu-Natal.

South Africa has high number of patients on antiretroviral treatment, necessitating innovative approaches to decongest healthcare facilities. The Central Chronic Medicines Dispensing and Distribution (CCMDD) programme is a national initiative that identifies stable chronic patients for collection at pick-up points away from the health facility. This study aimed to compare patient satisfaction and virological suppression among those who collected medication through the CCMDD programme and routine care. This descriptive retrospective analytical study was conducted at a community health centre in Pietermaritzburg from 01 January 2018 to 31 December 2018 and included a questionnaire and access to their medical records on the national medicines database. The 117 patients in the routine care and CCMDD programme groups were assessed at baseline and evaluated at 6 months and 12 months, which were the time points for viral load (VL) testing. Of the 234 participants, 34 out of 117 (31.6%) remained in routine care at the 6-month review, and all but 7 patients had transferred to the CCMDD after 12 months. At the end of the study, 7 patients had VLs above 50 copies/mL and continued in routine care, while 97% (n= 27/234) remained virologically suppressed. None of the CCMDD programme patients moved out of the programme. Satisfaction with the CCMDD programme is indicated by the patients' continued VL suppression, highlighting its potential to decongest healthcare facilities and reduce the strain associated with medication collection.Contribution:The findings in this study validate patients being registered onto the CCMDD programme.

Assessing distributional justice around Cultural Ecosystem Services (CES) provided by urban green areas: The case of Bologna

Cultural Ecosystem Services (CES) and related flows of benefits – heavily influencing societal and individual health and wellbeing - are usually ascribed non-consumptive values. However, despite the increasing recognition of their contribution to citizens’ quality of life, the intangible nature of CES makes it difficult to quantify them and hard to be integrated in decision making and planning processes. Nevertheless, the assessment of the societal relevance of CES would largely help to improve people wellbeing and quality of life. CES depend not only on the characteristics and features of urban green areas, but also on preferences and needs of the users that interact with them and that contribute to co-produce ES related benefits and values. These diverse needs, coupled with the uneven distribution of urban green area and diverse ways of managing them in the city, could affect the way ES are produced and further exacerbate existing inequalities and disparities. This contribution investigates the case study of the city of Bologna, introducing a spatial approach based on a new and pluralistic notion of urban green area that consider sport facilities and Green Stewards activities, to assess the related CES co-production paths and distributional dimension of justice in the city. Results show a good accessibility to urban green area throughout the city of Bologna (around .70 %). Nevertheless, CES co-production paths are limited in those areas of the city with higher population vulnerability index, due to lower urban green area facilities and green stewards activities, thus highlighting areas with higher distributional and procedural injustice path.

An Approach for Measuring Spatial Accessibility to Services/Facilities by Urban Community Aged Population

Abstract. Population aging is a global trend and a world-wide challenge to many governments. The high-quality life of the elderly cannot be separated from the support of basic services and facilities. Therefore, it is necessary to conduct analysis and research on the accessibility of urban community services and facilities. It is worth noting that the research in this article is based on master thesis (Bai, 2013). Drawing on the data, methods, and conclusions from the master's thesis, in this paper, an approach is designed for measuring spatial accessibility to services/facilities by a spatially dispersed ageing population in urban areas. The local government area (LGA) of Monash in the Melbourne metropolitan area has been selected the case study area. This study combines GIS-based spatial analytical procedures and two-step floating catchment area (2SFCA) method, in conjunction with mesh block level 2011 population data, service/facility data and transportation network data, to measure and map spatial variations in potential accessibility to services and facilities deemed essential to the aged population. Service facilities considered in the study include bus stops, train stations, bank facilities, shopping centres, post offices, churches, parks, public libraries, community centres, pharmacies, GP clinics and hospitals in Monash LGA. The research results can be used to optimize the spatial planning and layout of public facilities for ageing population.

Can Transport Infrastructure Improve Enterprise Energy Efficiency? Evidence From China

As an advanced social development capital, transportation plays a vital role in promoting regional development. Concurrently, the business activities of enterprises intimately relate to these transportation services. Utilizing road construction to improve the energy efficiency of enterprises, therefore, becomes a critical way to promote the transformation of the economy from high-speed development to high-quality development. Existing studies primarily focus on the impact of transport infrastructure on productivity, while few papers focus on the impact mechanism of transport infrastructure on energy efficiency. To delve deeper into the internal relationship between transportation and regional economic development, this paper explores the effect of transport infrastructure on energy utilization efficiency at the enterprise level. Based on the environmental pollution data of Chinese industrial enterprises from 2001 to 2011, this paper analyzes the impact of transport infrastructure on enterprises’ energy efficiency and verifies the impact mechanism. The results show that the construction and improvement of urban road facilities can significantly improve firm energy utilization efficiency. Compared to low-energy consumption sectors, the impact of transportation infrastructure on the energy efficiency of high-energy consumption industry enterprises is notably more pronounced. A closer examination of the impact mechanism revealed that both industrial agglomeration and the cost effect are pivotal mechanisms through which transportation infrastructure affects enterprise energy efficiency. Refined Identification of Urban Functional Zones Integrating Multisource Data Features: A Case Study of Lanzhou, China

Identifying urban functional zones is one of the important foundational activities for urban renewal and the development of high-quality urban areas. Efficient and accurate identification methods for urban functional zones are significant for smart city planning and industrial layout optimization. However, existing studies have not adequately considered the impact of the interactions between human activities and geographical space provision on the delineation of urban functional zones. Therefore, from the perspective of integrating the spatiotemporal characteristics of human activities with the distribution of urban functional facilities, by incorporating mobile signaling, POI (point of interest), and building outline data, we propose a multifactorial weighted kernel density model that integrates ‘human activity–land feature area–public awareness’ to delineate urban functional zones quantitatively. The results show that the urban functional zones in the central city area of Lanzhou are primarily characterized by dominant single functional zones nested within mixed functional zones, forming a spatial pattern of ‘single–mixed’ synergistic development. Mixed function zones are widely distributed in the center of Lanzhou City. However, the area accounted for a relatively small proportion, the overall degree of functional mixing is not high, and the inter-district differences are obvious. The confusion matrix showed 85% accuracy and a Kappa coefficient of 0.83.

No Report, No Densification? A Spatiotemporal Analysis of Urban Densification and Reporting Practices in World Heritage Properties

As urbanization accelerates, World Heritage properties, critical conservation areas, face a growing threat of urban densification, jeopardizing their Outstanding Universal Value (OUV). States Parties, the countries that have ratified the World Heritage Convention, are responsible for submitting periodic reports on the state-of-conservation of their World Heritage properties. These reports should explicitly address any instances of urban densification that may be occurring. But do they? This research investigates the relationship between urban densification and reporting practices in World Heritage properties over time and space. Through a spatiotemporal analysis, by analyzing changes in the built-up area within the core zones of cultural World Heritage properties from 1985 to 2015. We found that urban development, including housing, infrastructure, and tourism facilities, has significantly impacted World Heritage properties and an increase in built-up area can be observed especially in properties not reporting on urban threats.

Barriers and facilitators to primary care for people living with HIV and diabetes in Harare.

People living with human immunodeficiency virus (HIV) and comorbid diabetes mellitus (DM) face significant challenges owing to the complex interplay between these chronic conditions and the need for comprehensive and integrated care. Service availability and readiness for primary care are essential for the health of individuals and populations. This study aimed to explore barriers and facilitators to the provision of care to the patients with HIV and T2DM comorbidity. The study was conducted at Primary health centres in Harare, Zimbabwe. A mixed-methods design was applied. An audit of primary care facilities identified that there was adequate infrastructure and equipment for HIV and T2DM diagnosis and treatment. However, there are gaps in the availability of essential medicines and supplies, such as test strips for blood glucose monitoring. The assessment also showed that the centres had a chronic shortage of healthcare providers, including doctors, nurses and counsellors, and there was a need for additional training and support for healthcare providers in the management of HIV and T2DM. The study concludes that the delivery of health services to patients with HIV and T2DM at primary care centres in Harare, Zimbabwe, faces significant challenges. Suggestions included improved resource allocation and multisectoral collaboration to improve the delivery of healthcare services.Contribution:The research contributes insight into disparities between urban and rural primary care facilities in providing services, emphasizing the need for targeted interventions to bridge gaps and enhance care quality.
