Inequalities in orthodontic outcomes in England: treatment utilisation, subjective and normative need.

To investigate inequalities in three aspects of access to orthodontic care: uptake of orthodontic treatment, normative need and subjective need in England. We used data from two surveys in England: 12 and 15-year-olds from the 2013 Child Dental Health Survey (CDHS 2013) and 12-year-olds from the 2008/2009 NHS Dental Epidemiology Programme for England (NDEP 2008/2009). Summary variables representing orthodontic status were calculated. Two regression-based summary measures of inequalities were used to investigate the relationship between deprivation level and orthodontic outcomes: Slope and Relative indices of Inequality. There were significant absolute and relative inequalities in uptake of orthodontic treatment. The least deprived were 1.9 times more likely to have received orthodontic treatment compared to the most deprived in both surveys. Normative need was not associated with deprivation in either the analyses of CDHS 2013 (SII= 0.03, 95% CI: -0.04, 0.1; RII=1.06, 95% CI: 0.91, 1.24) or the NDEP 2007/2008 (SII= 0.03, 95% CI: -0.02, 0.07; RII=1.06, 95% CI: 0.96, 1.18). There was greater willingness to have teeth straightened in more deprived children from CDHS 2013 (SII=-0.09, 95% CI: -0.16, -0.03; RII=0.85, 95% CI: 0.75, 0.96) but not in NDEP 2007/2008 (SII=0.03, 95% CI: 0, 0.06; RII=1.07, 95% CI: 0.99, 1.15). Being deprived was associated with lower uptake of orthodontic treatment. Normative need was not related to deprivation. The association between deprivation and subjective need was only partly established, with poorer children showing a greater desire to have their teeth straightened in one survey.

Uptake of orthodontic treatment by children and adolescents in Germany. Results of the cross-sectional KiGGS Wave 2 study and trends.

For patients with considerable malfunctions, or where these could potentially develop, statutory health insurers completely cover the cost of correcting malpositionings of the teeth and jaws through orthodontic treatment in Germany. Based on the self-reported information from the second wave of the German Health Interview and Examination Survey for Children and Adolescents (KiGGS Wave 2, 2014-2017), the uptake of orthodontic treatment was analysed. A total of 25.8% of the girls and 21.1% of the boys aged 3 to 17 years are receiving regular orthodontic treatment. Uptake of treatment is very much dependent on age. The highest rates are found among 13-year-old girls (55.0%) and 14-year-old boys (50.8%). Compared to the data from previous KiGGS waves, the trend over approximately ten years has seen a significant increase of orthodontic treatment across all age groups. More health services research and a broader discourse on current treatment practices and their benefits are desirable.

The child perception questionnaire is valid for malocclusions in the United Kingdom

The purpose of this study was to validate the child perception questionnaire (CPQ(11-14)) with a sample of schoolchildren in Greater Manchester, United Kingdom. We made a longitudinal survey of children from 1999 to 2002, using the index of orthodontic treatment need (IOTN) at baseline when the children were 11 to 12 years old, the CPQ(11-14), and their uptake of orthodontic treatment 3 years later. CPQ(11-14) scores corresponded to differences in IOTN scores. These were related to the child's emotional and social well-being. Regression analysis showed that CPQ(11-14) scores were higher for girls, for higher grades of the dental health component of the IOTN, and for children who thought that their teeth needed straightening. CPQ(11-14) has acceptable reliability and validity, and is likely to be a useful measure for orthodontic trials. The impact of malocclusion on a child's quality of life might be substantial.

The influence of social class, gender, and peers on the uptake of orthodontic treatment.

The accurate prediction of the future demand for orthodontic treatment requires an understanding of the factors which influence the uptake of orthodontic treatment. This paper examines the influence of social class, gender, and peer group on the uptake of orthodontic treatment. A representative random sample comprising five-hundred-and-forty 15- and 16-year-old adolescents were interviewed in school. The results indicate that familiarity with orthodontic appliances among a subject's peer group has a greater influence on the uptake of orthodontic treatment than the subject's social class or gender.

The effects of social class on the uptake of orthodontic treatment.

The relationship between social class and uptake of orthodontic treatment was investigated in a longitudinal cohort study of 1018 children living in South Glamorgan, Wales. Previous studies have shown that working class people make less use of dental services and receive inferior dental care than middle class people. The present investigation examined the role of one factor which appears likely to contribute to this effect: namely, the uptake of orthodontic treatment by families from different social classes. If a significant association were shown then findings relating to the effectiveness of orthodontic treatment might be confounded by this social class factor.
