Management Challenges And Outcome Of Juvenile Onset Respiratory Papillomatosis In A Tertiary Hospital In North Central Nigeria: A Preliminary Study

Background/Aim: Juvenile Onset Recurrent Respiratory Papillomatosis (JoRRP) is a benign tumour attributed to human papillomavirus infection with a high propensity to recur despite appropriate treatment. It is relatively rare but the commonest benign tumour of the larynx in children. This study is aimed at evaluating its management challenges.
 Materials and methods: This was a retrospective review of patients with JoRRP at Ear, Nose and Throat (ENT) Department, University of Ilorin Teaching Hospital over a 7-year period. The patients' biodata, mode of presentations, surgical care, histopathological findings and management outcome were reviewed.
 Results: A total number of ten patients were diagnosed with JoRRP during the study period. Male to female ratio was 1:1. All the children were delivered per vaginal and sixty percent were first children of their mothers. Hoarseness, breathy voice and worsening difficulty in breathing were the presentation in 7 (70%) of cases, while 3 (30%) presented with only hoarseness and breathy voice. They all presented first to General Practitioners and Paediatricians before ENT Specialist assessment and six (60%) patients presented to ENT department 6 months or more after the onset of symptoms. They all had direct laryngoscopy and excisional biopsy of laryngeal papillomatous lesions. Thirty percent had tracheostomy done. There is a statistical significant relationship between tracheostomy rate and recurrence rate of JoRRP.
 Conclusion: JoRRP is a rare but potentially life-threatening benign tumor of the respiratory tract. Most of our patients presented late and were misdiagnosed at presentation. The key to good management outcome remains early diagnosis, adequate management and regular follow-up.

Abstract 1013: Strengths, weaknesses, opportunities, and threats of conducting clinical trials in Nigeria: The IRONMAN study

Abstract Background: The need to increase understanding of prostate cancer (CaP) is ever increasing. Unfortunately, there is limited understanding of how CaP affects men of African ancestry (MAA) despite significant disparities in CaP prevention, diagnosis, and treatment due to genetic and sociobehavioral factors. Conducting CaP clinical trials (CTs) in Africa presents multiple opportunities to improve upon collective knowledge of CaP in MAA experiences, expand patient access to CTs, and provide opportunities for African scientists and urologists to contribute to research on the global stage. The International Registry of Men with Advanced Prostate Cancer (IRONMAN) seeks to reduce CaP disparities by sponsoring sites throughout Africa. Four Prostate Cancer Transatlantic Consortium (CaPTC) institutions in Nigeria currently participate in IRONMAN: University of Ilorin Teaching Hospital, Federal Medical Center, University of Maiduguri, and Lagos State University Teaching Hospital. Because CTs of this scale are infrequently conducted in Africa, there are special considerations when conducting CTs in low-middle-income countries (LMICs) like Nigeria. Methods: Hourlong interviews dedicated to site staff were conducted via Zoom in Spring 2023 (four total). Interviews utilized a Strengths, Weaknesses, Opportunities, and Threats (SWOT) analysis approach. Each interview was recorded, transcribed, and validated by study staff for accuracy. Themes from each were inductively coded and then collated across all four sites. Results: The urologists, oncologists, phlebotomists, pathologists, nurses, research coordinators that make up the study teams identified the holistics of conducting the IRONMAN study at their sites. Strengths included a large patient population to recruit from and appropriate resources to conduct the study (ex., lab space, clinical knowledge, staffing levels). Weaknesses included the social determinants of health that negatively impact patient participation, limited biorepository space to store samples, and the need for patient-physician trust. The greatest opportunity presented was Nigerian institutions joining more CTs in the future, particularly multi-site global trials. Described threats included staff turnover, national stability, and increased economic disparities. Discussion: While increasing the overall number of CTs in Africa is a noble cause, sponsors and institutions should note the unique circumstances in implementing CTs in Africa when designing CTs and recruiting sites. Listening to the experiences of study teams is pivotal in ensuring CTs are Africa-minded - they will be complimentary to established research infrastructure, appropriately funded, and culturally responsive all in the name of equitable research. By doing so, ethical compliance and quality data collection will ensue and reduce disparities at the micro, mezzo, and macro levels. Causation, Types and Outcome of Road Traffic-related Ocular Injury in University of Llorin, Kwara, Nigeria

Introduction: Despite transportation liberating man and making him mobile, his increasing reliance on vehicular movement has conferred great facilities on him and his activities. The most profound culprit of all modes of transport is the road, of which traffic accidents are the most disturbing consequence of its use. Road traffic-related ocular injuries often have severe consequences and form a significant part of ophthalmic emergencies. These injuries may be missed and are therefore often underreported. Aim: To determine the causation, types, and outcomes of road traffic-related ocular injuries at the University of Ilorin Teaching Hospital, Kwara, Nigeria. Materials and Methods: This was a cross-sectional study conducted on 78 eyes of consecutive patients with road trafficrelated ocular injuries who presented at the Accident and Emergency unit of the University of Ilorin Teaching Hospital, Kwara, Algeria, between September 2012 and August 2013. The materials included the HOTV chart for vision assessment, which is a standardised, easy-to-use assessment method for both literate and illiterate individuals, a pen torch, direct ophthalmoscope, binocular indirect ophthalmoscope, slit lamp biomicroscope, Perkins tonometer, 2% fluorescein strip, 0.5% GuttTetracaine, and a handheld portable slit lamp ophthalmoscope. Patients’ demographics, duration of ocular injury before presentation, Visual Acuity (VA) at presentation, type of vehicle, place of injury, classification of ocular injury, treatment, and outcome on follow-up were recorded. Results: There were 78 eyes (72 patients) with road trafficrelated ocular injuries. The age range of the subjects was 3-55 years with a mean of 31.10 years. Forty-four (61.1%) patients were young men between 20-39 years. Fifty-one (70.8%) patients were males, and 21 (29.2%) were females, with a maleto-female ratio of 2.4:1. Most of the victims were traders (22, 30.6%), and fifty-two eyes (72.2%) presented within 12 hours of ocular injury. Motorcycles were associated with ocular injury in 37 (51.4%) patients. The most common injuries were of closed globe type (contusion, lamellar laceration, superficial foreign body) in 70 patients (89.7%). Conclusion: Road traffic-related injuries have increased over the past eight years in Nigeria. Males are more commonly injured in Road Traffic Injury (RTI), and motorcycles were the most common cause of injury to road users. Three out of 36 people injured in a vehicle at the time of injury used seat belts, while none of the riders and passengers on motorcycles wore helmets. The importance of prevention, the use of safety devices, and the avoidance of risk behaviors on the road need to be emphasised.

Assessment of Medication Prescribing Indicators Using the World Health Organization Standard Indices in a Tertiary Hospital in Ilorin, Nigeria

Background: Drug use is the most common option in treating disease conditions in clinical practice. Irrational prescription and use of medicine have become a major problem in Africa. The patterns of medication prescription in this environment have been under-studied. Evaluating these patterns is essential to provide information that may guide actions towards improving the prescription standards and mitigate the problems associated with irrational prescription and use of medicine. The study aimed to assess medication prescribing patterns by all cadres of doctors in the General Outpatient Department of the University of Ilorin Teaching Hospital (UITH) using the World Health Organization (WHO) prescribing indicators. Methods: It was a six-month retrospective study of medication prescription by all cadres of the doctors attending to outpatients in the General Outpatient Department of UITH between 1st July, 2022 – 31st December, 2022. A validated data capture form was used for the study following the WHO prescribing indicator guidelines. The results were displayed using the arithmetic means, frequencies, and percentages. Results were interpreted by the recommended ideal values by the WHO. Results: 1,650 prescriptions were reviewed, 914 were included in the study. This study found that all the analyzed prescription indices fell short of the WHO-recommended threshold. An average of 2.25 medications were prescribed per patient. The percentages of encounters with injections, antibiotics, and other medications by generic name were 87.2%, 66.9%, and 73.2%, respectively. 53.8% of the medications prescribed came from the essential medicine list. Conclusion: All medication prescribing indicators assessed were found to be below the ideal value recommended by WHO. Therefore, we advocated for the training of prescribers and the formulation of policies that will promote the prescription of medications in line with WHO guidelines.

Mindfulness Skills Intervention for Alleviating Stress Manifestations Among Nursing Students in Ilorin Metropolis, Nigeria

The literature has indicated that nursing students are among the healthcare students that experience higher level of stress; which makes it imperative to provide them with relevant skills they can employ to overcome stressful life conditions. This study thus adopted mindfulness skills intervention to alleviate stress manifestations among nursing students in Ilorin metropolis, Kwara State, Nigeria. The objectives of the study were to examine the effect of mindfulness skills training on physical, behavioural and psychological manifestations of stress among the participants. Using a quasi-experimental study of the pre-test, post-test design, three null hypotheses guided the conduct of this study. The population of the study consisted of 234 University of Ilorin Teaching Hospital nursing school students. A purposive sampling technique was used to select 15 participants in the treatment group. An adapted “Stress Questionnaire” (with a Reliability Index of .88) from Sinha et al. (2016) was used for data collection. The data collected were analysed using percentage and paired t-test was used to test the null hypotheses formulated at .05 level of significance. The findings revealed that mindfulness skills intervention reduced the physical (t = 19.17, p < .05); behavioural (t = 11.25, p < .05); and psychological (t = 11.51, p < .05) manifestations of stress among the study participants. Based on these findings, it was concluded that mindfulness skills intervention is not only applicable in clinical setting but can also be applied in an educational environment; hence, a comprehensive knowledge of the process and steps involved in these training becomes imperative for school counsellors to avail themselves with for imparting positive change in clients who are going through stressful situations. Therefore, it was recommended, among others, that professional counsellors should equip nursing students with such skills in order to help them overcome the physical, behavioural and psychological manifestations of stress.

Twenty-Four-Hour Intraocular Pressure Pattern In Glaucoma Suspects in an African Population.

Twenty-four-hour intraocular pressure measurement (Phasing) among glaucoma suspects showed higher intraocular pressure (IOP), greater fluctuation, and more varying peak times occurring mostly outside clinic hours, between 12 midnight and 8 am in 68.9% of them. To characterise 24-hour variation in IOP, in glaucoma suspects and normal individuals in an African population. A cross-sectional analytical study was carried out between March and October 2019 at the University of Ilorin Teaching Hospital. A total of 204 study participants, comprising 102 primary open angle glaucoma (POAG) suspects and 102 age and sex-matched controls with healthy eyes were recruited. Participants had a comprehensive ophthalmic examination. Measurements of IOP were taken every 2 hours during a 24-hour period using an iCare tonometer. The mean age was 43.64±14.02 SD and 43.02±13.55 SD ( P =1.000) for POAG suspects and controls, respectively. The mean peak IOP was significantly higher in POAG suspects (21.87± 4.65mmHg) compared with controls (17.17± 2.32mmHg) ( P <0.001). Whereas, the mean trough IOP reading was not significantly different between the 2 groups. In both groups of participants, the peak and trough IOP in the majority were recorded outside of clinic hours. The majority (68.9%) of POAG suspects had a peak IOP between midnight and 8 am, whereas the control group peaked mostly at 4 pm (16.7%) and 4 am (41.7%). Twenty-four-hour IOP variation was 6mmHg or less in 73.6% of control eyes, while in the POAG suspects, 83.3% of eyes had variation greater than 6mmHg. Compared with age and sex-matched healthy controls, glaucoma suspects of African descent showed significantly higher mean IOP and IOP variation, with most peak IOP detected outside of clinic hours.

Impact of sleep quality on glycaemic control in type 2 diabetes: a cross-sectional study

Background: Inadequate sleep is associated with type 2 diabetes (T2DM) and has a negative impact on glycaemic management. The purpose of this research was to examine the relationship between sleep quality and glucose control in individuals with T2DM.&#x0D; Methods: A cross-sectional study was conducted at the Family Medicine Clinic of University of Ilorin Teaching Hospital (UITH), a tertiary hospital in North-Central Nigeria. Three hundred and seventy-eight participants took part in the research (103 men and 275 women). Questionnaires were used to compile the data, and the Pittsburgh Sleep Quality Index was included. Diabetic indicators were gathered using a combination of a thorough medical history assessment and a battery of diagnostic testing. Statistical analysis was conducted using the SPSS software version 20.0.&#x0D; Results: The median age of the respondents was 61.01 (S.D. ± 9.8) years. Seventy-four percent (74%) of people reported having poor sleep quality, 45.5% had poorly controlled diabetes, and 29.9% were obese. Inadequate sleep or poor sleep quality was shown to be statistically linked with poor glycaemic management (p= 0.0001). There were significant associations between glycaemic management and subjective sleep quality (OR 1.495, 95% C.I 1.039-2.152), sleep disturbances (OR 0.279, 95% C.I 0.122-0.636), and daytime dysfunction (OR 3.571, 95% C.I 2.253-5.662).&#x0D; Conclusion: This research shows that poor glycaemic management is linked to poor sleep quality in people with T2DM.

Prevalence and predictors of resistant hypertension among out-patients in Ilorin, Nigeria

Introduction: Systemic hypertension (SH) contributes the highest number of deaths from cardiovascular diseases worldwide. Patients with resistant hypertension (RH) are more prone to hypertension-mediated organ damage. RH has not been well-studied in Africa, despite the fact that the prevalence of SH is highest in Africa. The aim of the study was to establish the prevalence and predictors of RH among out-patients managed in the cardiology unit of the University of Ilorin Teaching Hospital, Ilorin, Nigeria.&#x0D; Method: A cross-sectional study of 201 patients selected via systematic random sampling between April and September 2019.&#x0D; Results: Mean age of the participants was 59.6 (SD 13.8) years, females 58.7%, 32.3% were non-obese, 17 (8.5%) consumed alcohol and three (1.5%) smoked tobacco. 30 participants (14.9%) had co-morbid diabetes mellitus. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were significantly higher among patients with RH 152.5 (SD 18) mmHg vs 131.9 (SD 18.4) mmHg (p&lt;0.001) and 89.43 (SD 13.8) mmHg vs 79.46 (SD 10.5) mmHg (p=0.008). Eighteen patients (8.96%, 95% CI: 5.5-14%) had RH. The predictors of RH were obesity (OR= 3.754; p=0.009), SBP at patients’ first clinic visit, (OR=1.029, p=0.032), DBP at patients’ first clinic visit, (OR=1.048, p=0.014), and serum phosphorus, (OR=1.047, p=0.047).&#x0D; Conclusion: The prevalence of RH among our patients is low and is similar to that in studies with similar blood pressure cut-off values and case definition.

Knowledge and Perception Towards the Predisposing Factors to Eclampsia among Pregnant Women Attending the Antenatal Clinic at the University of Ilorin Teaching Hospital, Ilorin Kwara State

Introduction: Eclampsia, one of the leading causes of maternal and perinatal morbidity and mortality globally, is one of the most dreaded causes of unfavourable pregnancy outcomes. Pre-eclampsia and eclampsia are thought to be the cause of nearly 63,000 maternal fatalities each year globally. Objective: This study aimed to assess the level of knowledge and perception towards predisposing factors to eclampsia among pregnant women. Materials and methods: This research assessed the level of awareness and perception of eclampsia risk factors among pregnant patients at the University of Ilorin Teaching Hospital and identified factors affecting those perceptions. 110 expectant women were chosen randomly from the research population for this descriptive cross-sectional survey, and the research participants responded to a self-structured questionnaire after informed consent was sought. Results: Data collected were analysed using descriptive and inferential statistics with the level of significance set at 0.05. The majority of the respondents (41.8%) were 21-30 years old, and the majority (88.2%) were married. The study revealed that the majority of pregnant had good knowledge (89.1%) about predisposing factors to eclampsia, but their perception was poor. The findings showed no significant relationship between knowledge and perception towards the predisposing factors towards eclampsia among pregnant women with a p-value (0.319) &gt; 0.05. Conclusion: Although pregnant women attending the antenatal clinic at the University of Ilorin Teaching Hospital have good knowledge towards predisposing factors to eclampsia, their perception is poor.

Predictors of burnout among resident doctors in a Nigerian teaching hospital.

Burnout is a psychological syndrome resulting from exposure to chronic work-related stress. There are, however, a few works of literature on burnout among trainee doctors in Nigeria. To determine the prevalence of burnout and its predictors among resident doctors across 16 medical specialties and/or subspecialties. The University of Ilorin Teaching Hospital (UITH), Ilorin, Nigeria. A cross-sectional study was conducted among 176 resident doctors between October 2020 and January 2021. The survey included the Proforma and Maslach Burnout Inventory-Human Services Survey for Medical Personnel (MBI-HSS MP). The mean age of participants was 35.10 (SD 4.07) years. The prevalence of burnout was 21.6% for high emotional exhaustion (EE), 13.6% for high depersonalization (DP), and 30.7% for low personal accomplishment (PA). Being a younger resident doctor aged 31-35 (OR = 3.715, 95% CI [1.270 - 10.871]) was the only significant predictor for the EE. Predictors of DP included the age group 31-35 years (OR = 7.143, 95% CI [2.297 - 22.216]), duty hours >50 hours per week (OR = 2.984, 95% CI [1.203 - 7.401]), and presence of work-related stress (OR = 3.701, 95% CI [1.315 - 10.421]). A good relationship with colleagues negatively predicted low PA (OR = 0.221, 95% CI [0.086 - 0.572]). High levels of burnout are prevalent among resident doctors, comparable to international studies. Therefore, the government and other relevant stakeholders must drive legislation and formulate policies toward addressing the work-related factors associated with burnout in the Nigerian healthcare industry. This study highlighted the determinants of burnout among Nigerian resident doctors, which necessitates targeted interventions to address them.

Provider-identified barriers to performance at seven Nigerian accident & emergency units: A cross-sectional study.

Nigeria hosts much of Africa's morbidity and mortality from emergency medical conditions. We surveyed providers at seven Nigerian Accident & Emergency (A&E) units about (i) their unit's ability to manage six major types of emergency medical condition (sentinel conditions) and (ii) barriers to performing key functions (signal functions) to manage sentinel conditions. Here, we present our analysis of provider-reported barriers to signal function performance. 503 Health Providers at 7 A&E units, across 7 states, were surveyed using a modified African Federation of Emergency Medicine (AFEM) Emergency Care Assessment Tool (ECAT). Providers indicating suboptimal performance ascribed this performance to any of eight multiple-choice barriers [infrastructural issues, absent and broken equipment, inadequate training, inadequate personnel, requirement of out-of-pocket payment, non-indication of that signal function for the sentinel condition, and hospital-specific policies barring signal function performance] or an open-ended "other" response. The average number of endorsements for each barrier was obtained for each sentinel condition. Differences in barrier endorsement were compared across site, barrier type and sentinel condition using a three-way ANOVA test. Open-ended responses were evaluated using inductive thematic analysis. Sentinel conditions were Shock, Respiratory Failure, Altered Mental Status, Pain, Trauma, and Maternal & Child Health. Study sites were the University of Calabar Teaching Hospital, the Lagos University Teaching Hospital, the Federal Medical Center, Katsina, the National Hospital Abuja, the Federal Teaching Hospital Gombe, the University of Ilorin Teaching Hospital (Kwara), and the Federal Medical Center Owerri (Imo). Barrier distribution varied widely by study site. Just three study sites shared any one barrier to signal function performance as their most common. The two barriers most commonly endorsed were (i) non-indication of, and (ii) insufficient infrastructure to perform signal functions. A three-way ANOVA test found significant differences in barrier endorsement by barrier type, study site and sentinel condition (p<0.05). Thematic analysis of open-ended responses highlighted (i) considerations disfavoring signal function performance and (ii) lack of experience with signal functions as barriers to signal function performance. Interrater reliability, calculated using Fleiss' Kappa, was found to be 0.5 across 11 initial codes and 0.51 for our two final themes. Provider perspective varied with regards to barriers to care. Despite these differences, the trends seen for infrastructure reflect the importance of sustained investment in Nigerian health infrastructure. The high level of endorsement seen for the non-indication barrier may signal need for better ECAT adaptation for local practice & education, and for improved Nigerian emergency medical education and training. A low endorsement was seen for patient-facing costs, despite the high burden of Nigerian private expenditure on healthcare, indicating limited representation of patient-facing barriers. Analysis of open-ended responses was limited by the brevity and ambiguity of these responses on the ECAT. Further investigation is needed towards better representation of patient-facing barriers and qualitative approaches to evaluating Nigerian emergency care provision.

Abdominal Trauma Imaging in a Nigerian Tertiary Hospital-Our Experience with 87 Adult Patients.

Abdominal trauma is a major cause of morbidity and mortality in low- and medium-income countries (LMICs). Abdominal trauma imaging is important in determining the location and severity of organ injury, the need for surgery, and the identification of complications. The choice of imaging in abdominal trauma in LMICs is influenced by peculiar problems, which include the availability of imaging modality, expertise, and cost. There are few reports on options of trauma imaging in LMICs, and this study aimed to identify and characterise the type of imaging done for patients presenting with abdominal trauma at the University of Ilorin Teaching Hospital. This was a retrospective observational study of patients with abdominal trauma who presented at the University of Ilorin Teaching Hospital from 2013 to 2019. Records were identified, and data were extracted and analysed. A total of 87 patients were included in the study. There were 73 males and 14 females. The abdominal ultrasound was the commonest modality done in 36 (41%) patients, whereas abdominal computed tomography was done in five (6%) patients. Eleven patients (13%) had no imaging done, and 10 of these patients proceeded to having surgery. In patients with intraoperative finding of perforated viscus, the sensitivity of radiography was 85% and specificity was 100%, whereas that of the ultrasound was 86.7% and 50% for sensitivity and specificity, respectively. The ultrasound scan was the commonest imaging done for patients who presented with features of haemorrhage (P = 0.04), odds ratio (OR) = 1.29 (95% confidence interval [CI] = 1.08-1.6), and patients with severe injury, P = 0.03, OR = 2.07 (95% CI = 1.06-4.06). Gender (P = 0.64), shock at presentation (P = 0.19), and mechanism of injury (P = 0.11) did not influence the choice of imaging. Abdominal trauma imaging in this setting was majorly via ultrasound and abdominal radiographs. An Exploratory Analysis of Human Immunodeficiency Virus (HIV) Prevalence in Nigeria

This paper involved conducting a statistical analysis on the number of individuals living with HIV/AIDS. The University of Ilorin Teaching Hospital (UITH) was chosen as the case study for the years 2014 to 2019, with factors such as year, sex, and age group taken into account. During this six-year period, a total of 2604 cases were recorded at UITH. Among both sexes, females had the highest number of people living with HIV/AIDS. Additionally, the age group of 31-45 had the highest number of individuals affected by the disease. In terms of the specific year, 2016 had the highest number of people living with the disease, totalling 460 cases. A chi-square test of independence was conducted to examine the relationship between the factors, using a significance level of 0.05. The results indicated that all the considered factors were not independent of each other, meaning they were related.

Pattern of Presentation and Outcome of Adult Patients with Abdominal Trauma - A 7-Year Retrospective Study in a Nigerian Tertiary Hospital.

Abdominal trauma is a major cause of morbidity and mortality in low- and middle-income countries. There is a paucity of trauma data in this region and this study aimed to show the pattern of presentation and outcome of patients with abdominal trauma at a North-Central Nigerian Teaching Hospital. This was a retrospective, observational study of patients with abdominal trauma who presented at the University of Ilorin Teaching Hospital from January 2013 to December 2019. Patients with clinical and/or radiological evidence of abdominal trauma were identified, and data extracted and analyzed. A total of 87 patients were included in the study. There were 73 males and 14 females (5.2:1) with a mean age of 34.2 years. Blunt abdominal injury occurred in 53 (61%) patients with 10 patients (11%) having concomitant extra-abdominal injuries. A total of 105 abdominal organ injuries occurred in 87 patients with the small bowel being the most frequently injured organ in penetrating trauma, while in blunt abdominal injury, the spleen was most commonly injured. A total of 70 patients (80.5%) had emergency abdominal surgery with a morbidity rate of 38.6% and negative laparotomy rate of 2.9%. There were 15 deaths in the period accounting for 17% of patients with sepsis as the most common cause of death (66%). Shock at presentation, late presentation >12 h, need for perioperative intensive care unit admission, and repeat surgery were associated with a higher risk of mortality (P < 0.05). Abdominal trauma in this setting is associated with a significant amount of morbidity and mortality. Typical patients present late and with poor physiologic parameters often resulting in an undesirable outcome. There should be steps targeted at preventive policies focused on reducing the incidence of road traffic crashes, terrorism, and violent crimes as well as improving health care infrastructure to cater to this specific group of patients.
