ObjectiveTo assess changes in epidemiology of undocumented HIV status and HIV rapid testing (RT) among Illinois parturients after the implementation of a statewide quality initiative.Study DesignThe Perinatal Rapid Testing Initiative in Illinois implemented HIV RT in every Illinois L&D unit in July 2004. Data submitted from birthing hospitals from January 2005 to March 2010 were used to calculate annual frequencies of HIV status documentation, RT utilization, maternal/baby pairs discharged with unknown HIV status, and HIV positive women identified using RT. The seroprevalence of HIV was compared between populations with and without documented status at presentation.Results862,250 women delivered over the observation period. There was a significant decrease in women presenting with undocumented HIV status, increase in RT utilization among undocumented women, and decrease in maternal/baby pairs with undocumented status at time of discharge (all p<0.001). Of all women known to be HIV positive at discharge, 9.8% were diagnosed using RT. Undocumented women had a higher HIV seroprevalence than documented women (0.09% vs 0.15%; p<0.001).ConclusionsTableHIV status and utilization of HIV rapid testing in the perinatal setting200520062007200820092010†January-March 2010 onlyTotal deliveries141,779175,230175,160171,473160,05038,558Undocumented HIV status at presentation16,575 (11.7%)12,903 (7.4%)10,679 (6.1%)9,503 (5.5%)8,557 (5.3%)1,899 (4.9%)RT utilization in undocumented women11,867 (71.6%)11,164 (86.5%)10,626 (99.5%)9,478 (99.7%)8,532 (99.7%)1,898 (99.9%)Unknown HIV status at discharge3,886 (2.74%)1,731 (0.99%)108 (0.06%)26 (0.02%)19 (0.01%)5 (0.01%)† January-March 2010 only Open table in a new tab ObjectiveTo assess changes in epidemiology of undocumented HIV status and HIV rapid testing (RT) among Illinois parturients after the implementation of a statewide quality initiative.
