Diagnosis of cystoid macular edema by comparison of pre-operation and post-operation macular thickness calculated by optical coherence tomography (OCT) after cataract extraction surgery with and without pre-operative use of NSAIDS.

ABSTRACT… Objective: To compare mean macular thickness after cataract extraction surgery with and without pre-operative NSAIDs. Study Design: Randomized Controlled Trial. Setting: Department of Ophthalmology, Madinah Teaching Hospital, Faisalabad. Period: 21-10-2022 to 20-04-2023. Methods: Total 640 patients (320 in each group) were involved in the study. In group-A patients received one drop of NSAID three times a day for one day prior surgery. In group-B, patients not received NSAID. Results: Results showed that there was a total 395 males and 245 females. Mean age of the patients was 48.24±5.54 in group A and 47.05±5.20 in group B. Comparison of macular thickness showed that thickness was 215.69±14.20 in group A and 227.99±06.09 in group B. There was a significant difference in macular thickness of two groups (P <0.001). Conclusion: In conclusion, preoperative usage of NSAIDs revealed statistically significant (p0.001) and clinically relevant benefits in avoiding macular edoema and preserving visual acuity in patients undergoing cataract surgery as compared to the non-NSAIDs group.

Is Floppy Eyelid Syndrome predictive of Intraoperative Floppy Iris Syndrome? A prospective and translational study

Intraoperative floppy iris syndrome (IFIS) is defined as an excessive billowing of the iris during cataract surgery. Floppy eyelid syndrome is defined as an excessive hyperlaxity of the eyelids. Based in our daily experience, we suspected FES to be associated with IFIS. This association has been investigated. The goal of the study was to explore whether FES was predictive of IFIS. We conducted a prospective observational single-center study at the University Hospital of Nice. IFIS and FES were investigated in patients undergoing cataract surgery from November 2019 to May 2021. In addition, tarsus and iris samples were harvested in 2 fresh cadavers, and Verhoeff staining was used to identify elastin fibers. We included 452 eyes (n=312 patients, 50.9% male) with a mean age of 71.7years (±11.4). IFIS was diagnosed in 88 (19.5%) patients, including 20 (4.4%) grade 1, 34 (7.5%) grade 2, and 33 (7.3%) grade 3. FES was diagnosed in 35 (7.7%) patients. PEX was found in 23 (5.1%) patients. On multivariate analysis, FES (P<0.001), pseudo exfoliation (P=0.017), intracameral dilatation (P<0.004), senior surgeons (P=0.009) and α1-ARA (P<0.001) were associated with IFIS. Elastin fibers were identified in small amounts in the tarsus samples but not in the iris samples. FES is predictive of IFIS. Surgeons should be aware of this association to prevent intraoperative complications.

Visual and Surgical Outcomes of Outreach Cataract Surgeries in Ilam District of Nepal: An Observational Study

Introduction: Cataracts are the leading cause of preventable blindness worldwide. Although cataract blindness is reversible, its service coverage remains poor, particularly in rural and hilly areas of Nepal. The study aimed to evaluate visual outcomes of outreach cataract surgeries and associated intraoperative and post-operative complications. Methods: This cross-sectional study was conducted at a district of Eastern Nepal after ethical clearance was taken from the Institutional Review Committee (Reference number: 88). Total 131 subjects had cataract surgeries at the surgical camp, and their pre-operative, one-day and one-month post-operative visual acuities were compared, and intraoperative and post-operative surgical complications were recorded. Visual acuity ≥ 6/12 was considered normal. Data was collected in excel and analyzed in SPSS. Results: The mean age of participants was 71.8±9.51 years, with 70 (53.43%) male. A total of 156 eyes from 131 subjects underwent cataract surgeries and among them 25 (19.08%) cases underwent bilateral cataract surgeries. At the day one of surgery, visual acuity improved to normal in 137 (87.82%) of cases, while after one-month, visual acuity improved to normal in 150 (96.15%) of cases with best-corrected glasses. There were 4 (2.56%) intraoperative complications, which included surgery-induced subluxated bag, premature entry, iris trauma, and posterior capsule rent. Conclusions: The visual and surgical outcomes of cataract surgeries performed at an outreach surgical camp were excellent with minimal surgical complications.

AVALIAÇÃO DA QUALIDADE VISUAL EM PACIENTES SUBMETIDOS A CIRURGIA DE CATARATA COM IMPLANTE DE LENTE INTRAOCULAR VIVITY

Cataract is an eye condition characterized by the clouding of the lens, the eye's natural lens, which results in a progressive loss of vision. This disorder affects millions of people worldwide, being defined as the leading cause of preventable blindness on the planet. The only treatment is the removal of the lens with the implantation of an intraocular lens (BMC Ophthalmology, 2016). This is a descriptive, observational, qualitative, and quantitative study based on the application of a questionnaire to patients undergoing phacoemulsification at a private ophthalmology center in Aracaju, the Instituto de Olhos Cristiano Mendonça, by ophthalmology specialist Dr. C.Q.M. This questionnaire was developed as an adaptation of the Visual Function Index (VF-14) by Pereira et al. (2017). It was initially designed to assess the visual impairment caused by cataracts. The objective of this study was to evaluate personal satisfaction and verify the effectiveness of the implantation of the AcrySof IQ Vivity intraocular lens in patients who previously underwent cataract surgery, associating it with postoperative refractive outcomes. Most participants who underwent this surgery achieved excellent refractive results and visual acuity, along with a positive association with the VF-14 questionnaire.

AVALIAÇÃO ULTRASSONOGRÁFICA DO CONTEÚDO GÁSTRICO EM PACIENTES SUBMETIDOS À CIRURGIA DE CATARATA

Introduction: Cataract surgery is the most commonly performed ophthalmologic procedure worldwide, and the disease is one of the main causes of reversible blindness worldwide. Objective: To analyze the ultrasound evaluation of gastric contents in patients undergoing cataract surgery. Methodology: Series report evaluating 24 patients in the perioperative period at the bedside using ultrasound to investigate the presence of gastric content that could indicate the risk of aspiration or other anesthetic complications. Results: Thus, 16 patients (66.6%) were female, and 8 (33.3%) were male. The mean age observed was 66.4 years (42-80). The average fasting time was 13 hours and 6 minutes, with the shortest fasting time at 7 hours and the longest at 24 hours. The mean residual gastric volume was 43.7 mL, with a minimum of 0 mL and a maximum of 138.9 mL. Conclusion: The substantial variance in gastric volumes assessed in the study underscores the need for further perioperative intervention studies focused on shortened fasting times and the use of fast-absorbing carbohydrates to confirm their added benefit to safety, documented as an empty stomach through bedside ultrasound.

Patients’ perceptions on reluctance to undergo cataract surgery in Sedibeng district hospital, Gauteng province

Background: Over 50 million people are blind, and 135 million people suffer from severe visual impairments, with cataracts contributing to 51% of global blindness cases. Despite efforts to promote cataract surgery (CS), many patients remain hesitant, underscoring the importance of understanding their perspectives.Aim: This study aimed to explore and describe patients’ perceptions regarding reluctance to undergo CS to improve awareness and increase surgery uptake.Setting: The research was conducted in the Sedibeng District, Gauteng province, South Africa.Methods: A qualitative descriptive phenomenological design was employed to capture the lived experiences of patients through face-to-face, individual semi-structured interviews with 15 patients aged 38 – 72 years prior to their CS. Purposive sampling was used to select participants, and Colaizzi’s seven-step analysis was used for data analysis.Results: The findings highlight several barriers perceived by participants, including long waiting times; fear; lack of knowledge; limited financial, material and human resources; power outages; uncontrolled blood sugar levels; and coronavirus disease 2019 (COVID-19) pandemic restrictions. Nevertheless, some view it positively as a means to avert blindness and enhance vision.Conclusion: To address the challenges, the study recommends boosting the number of eye health care professionals, prioritising budget allocation, addressing electricity interruptions and conducting community outreach educational programmes to improve access to CS in the district.Contribution: The insights gained from this study can help health care providers and policymakers optimise care and support for CS patients through policy formulation and education. Addressing these knowledge gaps will likely increase surgery uptake and improve patients’ quality of life.

Clinical Evaluation of Violet Light Filtration and High-Resolution Lathing on a Diffractive Extended Depth of Focus IOL.

This study is a prospective, randomized, subject/evaluator-masked clinical trial in a single-center clinical setting. The purpose of the study is to compare the clinical performance of Tecnis Symfony Optiblue IOL (models ZXR00V and ZXW150) with violet light filter (VLF) and manufacturing improvements versus Tecnis Symfony IOL (models ZXR00 and ZXT150) with ultraviolet light filter (UVF) in patients undergoing cataract surgery. Patients with cataracts aged ≥ 22years were randomly assigned 1:1 to bilateral implantation with ZXR00V/ZXW150 (VLF group) or ZXR00/ZXT150 (UVF group). Key endpoints at 6months postoperative included patient reported nighttime dysphotopsia symptoms, 25% low contrast visual acuity with glare, and patient satisfaction. Sixty patients were implanted with ZXR00V/ZXW150 (30) or ZXR00/ZXT150 (30). At 6months, the VLF group did not show a statistically significant differences in mean monocular photopic uncorrected distance visual acuity (UCDVA), best-corrected distance visual acuity (BCDVA), uncorrected near visual acuity (UCNVA) (40cm), or distance-corrected near visual acuity (DCNVA) (40cm) compared to the UVF group. At 1month, patients in the VLF group reported significantly less difficulty due to halo (p = 0.016) and starburst (p = 0.028) symptoms. By the 6months, dysphotopsia complaints were no longer significantly different between the groups. Although the VLF group trended toward better low contrast visual acuity and patient satisfaction, statistical significance was not reached. Managing the patients' expectations is key to achieving success. At the 1-month visit the patients who reported dysphotopsia complaints in the VLF group had significantly less difficulty with starbursts and halos as compared to the UVF group. By the 6-month visit, there was no significant difference between the two groups in the difficulty with starbursts and halos. ClinicalTrials.gov identifier, NCT06567834.

Evaluation of postoperative outcomes after cataract surgery in patients with Juvenile Idiopathic Arthritis-Associated Uveitis.

To evaluate the postoperative outcomes of cataract surgery and intraocular lens (IOL) implantation on visual outcomes and complications in pediatric patients with Juvenile Idiopathic Arthritis-Associated Uveitis. This study included 25 patients (30 eyes) with uveitis and cataracts who underwent cataract surgery. The study was conducted as a retrospective study. Patients were divided into two groups based on their preoperative treatment: those who received immunomodulatory therapy (IMT) along with systemic corticosteroids and those who received IMT in combination with a biologic therapy. Best corrected visual acuity (BCVA), intraocular pressure (IOP), central macular thickness (CMT), macular volume (MV), and postoperative complications were assessed at various time points up to 12 months. All surgeries were performed by a single surgeon. Significant improvement in BCVA were observed postoperatively in both study groups, with approximately 80% of eyes showing improvement. Mean BCVA improved from 0.89 ± 0.63 to 0.37 ± 0.33 in group 1and from 0.82 ± 0.68 to 0.35 ± 0.31 in group 2 after 12 months. Postoperatively, there was a decrease in mean IOP, with no significant variance observed between the two groups. Complications were observed in 73% of cases, including posterior capsular opacification (PCO), secondary glaucoma, and macular edema (ME), but did not significantly differ between groups. Cataract surgery in children with uveitis associated with JIA presents significant challenges due to the risk of postoperative complications, even with adequate preoperative and postoperative care. The similar improvement in BCVA observed in both groups suggests that different systemic therapies were sufficient for satisfactory control of inflammation before and after surgery.

Alterations in optical coherence tomography angiography parameters after cataract surgery in patients with diabetes

This prospective study aimed to investigate microvascular changes in diabetic patients undergoing cataract surgery. Foveal avascular zone (FAZ) area, as well as vessel density (VD) in the macula and radial peripapillary capillary plexus (RPC), were compared before and after surgery. Sixty eyes (72.3%) had no diabetic retinopathy (no DR group) and 23 eyes (27.7%) had non-proliferative diabetic retinopathy (DR group). In the no DR group, the FAZ area in the superficial capillary plexus (SCP) decreased from 0.41 ± 0.13 to 0.38 ± 0.11 mm2 (P = 0.036), while no significant change was observed in the DR group (0.33 ± 0.12 to 0.30 ± 0.12 mm2, P = 0.091) at 6 months postoperatively. VD in the RPC increased from 34.4 ± 2.3% to 35.6 ± 2.3% in the no DR group (P = 0.009), but there was no significant change in the DR group (33.0 ± 3.5% vs. 34.0 ± 2.3%, P = 0.051) after 6 months. VD in the macula did not change in either group. Phacoemulsification reduced the FAZ area in the SCP and increased the VD in the RPC in diabetic patients without diabetic retinopathy at six months postoperatively.

Importance of different topographic, biometric, and retinal parameters for calculating IOL power

The objective is to evaluate the parameters significantly related to calculating the power of the implanted lens and to determine the importance of different biometric, retina, and corneal aberrations variables. A retrospective cross-sectional observational study used a database of 422 patients who underwent cataract surgery at the Oftalvist Center in Almeria between January 2021 and December 2022. A random forest based on machine learning techniques was proposed to classify the importance of preoperative variables for calculating IOL power. Correlations were explored between implanted IOL power and the most important variables in random forests. The importance of each variable was analyzed using the random forest technique, which established a ranking of feature selections based on different criteria. A positive correlation was found with the random forest variables. Selection: axial length (AL), keratometry preoperative, anterior chamber depth (ACD), measured from corneal epithelium to lens, corneal diameter, lens constant, and astigmatism aberration. The variables coma aberration (p-value = 0,12) and macular thickness (p-value = 0,10) were almost slightly significant. In cataract surgery, the implanted IOL power is mainly correlated with axial length, anterior chamber depth, corneal diameter, lens constant, and preoperative keratometry. New variables such as astigmatism and anterior coma aberration and retina variables such as the preoperative central macular thickness could be included in the new generation of biometric formulas based on artificial intelligence techniques.

Pseudo-plateau iris secondary to multiple peripheral pigment epithelial cysts: A case report

Iris epithelial cysts are rare, spontaneous, epithelium-lined spaces that arise from the iris-pigmented epithelial layer and ciliary body and can cause a narrowing of the anterior chamber angle leading to a pseudo-plateau iris configuration (PPI). We present the case of an 80-year-old female patient with a history of decreased visual acuity, diagnosed with bilateral cataracts and pseudo-plateau iris secondary to multiple peripheral pigment cysts. This report describes the detection process, management strategy and course of treatment of a patient with bilateral cataracts who underwent cataract surgery by manual incision, and clinically presented with Pseudo-plateau iris secondary to Iris epithelial cyst. It highlights the importance of early detection and correct differentiation from malignant conditions, and anterior segment pathologies to prevent associated permanent complications.

Macular changes after cataract surgery.

This article aims to highlight if the central macular retina suffers changes after cataract surgery, and to evaluate the eventually discovered changes regarding persistence. This retrospective study, which lasted over three years (2021-2023), included patients who underwent cataract surgery performed by one surgeon. Spectral-domain optical coherence tomography (SD-OCT) imaging assessed macular changes and measured the central macular thickness. A total of 240 eyes with diagnosed senile cataract were included in this study. The mean age was 66 years ± 4 years. Preoperative central foveal thickness was 210 ± 27.3 µm, the postoperative thickness on day 1 was 234.3 ± 40.2 µm, at 6 months 230.5 ± 35.2 µm, and the 1-year follow-up 229.2 ± 30.3 µm. Macular changes after cataract surgery are easily confirmed by SD-OCT. The evaluation and monitoring of macular changes can be done by using central macular thickness assessment. The study provides data from a Romanian pool of patients. The values correlated well with those from similar studies of SD-OCT examinations, but differences were still observed, as there were different devices for performing SD-OCTs.

Alpha-adrenergic antagonists and iris dynamics: Challenges and solutions in cataract surgery

BackgroundAlpha-1 adrenergic receptor antagonists (α1-ARAs) are frequently used in treatment of Hypertension and symptomatic benign prostatic hypertrophy (BPH). Numerous studies have demonstrated the association between α1-ARAs like Tamsulosin and increased surgical risks for patients undergoing cataract surgery. This study aims to identify and study the effects of α1-ARAs on iris parameters and the subsequent operative challenges encountered during cataract surgery.MethodsA cross-sectional, prospective study involving 30 patients on α1-ARAs planned for cataract surgery and equal number of age and sex matched controls were subjected to evaluation of changes on iris parameters and subsequent challenges in cataract surgery.ResultsThe study group had statistically significant lesser pupil diameter. Iris thickness at sphincter muscle region (SMR) was similar between groups (P = 0.53). Significantly lower values of iris thickness at dilator muscle region (DMR) found in treated subjects (P = < 0.001). There was statistically significant difference between DMR/SMR ratio of two groups (P < 0.001). Multiple regression analysis revealed longer duration of α1-ARAs treatment correlated with reduced DMR/SMR ratio (P = 0.001; r = 0.47).Conclusionα1-ARAs have implications for pupil size regulation and surgical procedures involving the eye. Tamsulosin is more potent than alfuzosin in inducing IFIS. Systemic α1-ARAs lower values of DMR thickness, DMR/SMR ratio and reduces pupillary diameter. Therefore, ophthalmologists, primary care physicians, urologists, and patients should be aware of the potential difficulties that these drugs pose for cataract surgery.

Effectiveness of a new commercial ocular spray containing Biosecur in reducing conjunctival microbial flora as surgical prophylaxis in patients undergoing cataract surgery

IntroductionThis study is aimed to evaluate the antiseptic activity of a new ophthalmic spray containing Biosecur citrus extract as a preoperative prophylactic treatment in patients undergoing cataract intervention.MethodsA prospective, multicenter study was conducted. Patients applied Oftasecur spray on the eye scheduled for surgery, three times daily for 3 days before surgery. Conjunctival swabs were performed before and after the treatment. Qualitative and quantitative microbiological analyses were assessed.ResultsHundred and forty-seven patients (M: F = 76:71; mean age 70.74 ± 9.41 years) were enrolled. 58.5% of patients presented detectable microbial loads in the eye to be operated, before treatment. Most predominant bacterial species were: Staphylococcus epidermidis, coagulase-negative Staphylococci and Staphylococcus aureus. Patients were divided into three subgroups, according to the microbial loads measured at baseline: lower than 10 CFU/ml (41.7%), between 10 and 100 CFU/ml (31.7%), equal or greater than 100 CFU/ml (26.6%). A twofold reduction in positive patients who received Oftasecur: 44 eyes (51.2%) resulted in negative after ocular treatment. A significant reduction in bacterial loads was observed (p < 0.001). No ocular adverse reactions were recorded.ConclusionOftasecur spray effectively reduced conjunctival bacterial burden. It may represent a proper and safe antiseptic adjuvant treatment in patients undergoing cataract surgery.

Ten-year trends of delayed sequential bilateral cataract surgery (DSBCS) in Sweden: a register-based study.

To study the trend of delayed sequential bilateral cataract surgery (DSBCS) in Sweden in the past decade. This register-based cohort study utilized data from the Swedish National Cataract Register (NCR) from 2010 through 2019. Register files from patients who underwent cataract surgery in both eyes during the study period were linked using their social security numbers. Bilateral surgeries on different days were classified as DSBCS. The study investigated the association between DSBCS within 3months and several variables with stratification and multivariate logistic regression. The following variables were used: operation year, region, private or public unit, age, sex, indication for surgery, type of intraocular lens (IOL), preoperative visual acuity, ocular comorbidity, posterior capsule rupture and perioperative difficulties. During the study period, 368,106 patients underwent DSBCS, of which 62.6% (n = 230,331) had bilateral surgery within 3months. The median time between the surgeries was 61days (interquartile range 26-161days), showing regional variations. Better visual acuity in the fellow eye, presence of ocular comorbidity, various perioperative events and complications were associated with longer time to surgery of the second eye. Conversely, cataract surgery in more recent years, private clinic, increasing age, anisometropia and multifocal IOL were associated with shorter timespan between surgeries. The majority of DSBCS were conducted within a 3-month timeframe, with the interval between surgeries decreasing throughout the study period. Several rational factors were associated with the time difference, in addition to regional variations. Many patients would probably benefit from less time between the surgeries, and we encourage a clinical practice taking the whole patient's visual function into account.

Visual, Refractive, Functional, and Patient Satisfaction Outcomes After Implantation of a New Trifocal Diffractive Intraocular Lens.

To describe the visual, refractive, functional, and patient satisfaction outcomes of the Clareon® PanOptix® trifocal intraocular lens (IOL). This was a prospective longitudinal descriptive study. Patients who underwent cataract surgery with implantation of Clareon® PanOptix® (Alcon Laboratories, Inc.) were included. Monocular refractive outcomes and visual acuity at distance, intermediate, and near were evaluated 1- and 6-months post-op. Binocular contrast sensitivity (M&S® Technologies), binocular defocus curve, and patient satisfaction with the IOL Satisfaction (IOLSAT) and Questionnaire for Visual Disturbance (QUVID) questionaries were assessed at 6-month post-op. Seventy-six Clareon® PanOptix® were implanted bilaterally in 38 patients. The mean age of the patients was 67.63±5.18 years. At 1-month post-op, the monocular Corrected Distance Visual Acuity (CDVA), CIVA and CNVA were 0.00±0.09, 0.02±0.17, and 0.12±0.12 LogMAR, respectively, and CDVA and CNVA were stable at 6-month post-op (p>0.05). No statistical differences were found in post-op spherical equivalent at 1 and 6 months (-0.08±0.27 D and -0.05±0.24 D; p=0.351). A 100% of eyes were within ±0.5 D at 1 month and 6-month post-op. Binocular defocus curve shows three peaks of maximum visual acuity (VA) at 0D (-0.04±0.08 LogMAR), at -1.50D, and -2.50 D (0.01±0.10 LogMAR and 0.03±0.07 LogMAR, respectively). Contrast sensitivity decreased at high spatial frequencies. In patient satisfaction, IOLSAT questionary reveals 78.94% patients "Never" or "Rarely" Needing Glasses and according QUVID questionnaire, 100% of patients report no hazy vision. The PanOptix® IOL platform with the new material Clareon® provides good visual outcomes for distance, intermediate, and near vision, with adequate contrast sensitivity and low visual disturbances.

Clinical Performance of an Extended Range of Vision Intraocular Lens After LASIK.

To evaluate the clinical and patient-reported outcomes (PROMs) of cataract surgery with implantation of a wavefront-shaping extended depth of focus (EDOF) intraocular lens (IOL) in eyes with previous laser in situ keratomileusis (LASIK) for myopia correction. This prospective observational study enrolled 50 eyes with previous LASIK from 25 patients (age: 46 to 70 years) who underwent cataract surgery with implantation of the AcrySof IQ Vivity IOL (Alcon Laboratories, Inc). Visual and refractive outcomes were evaluated during a 3-month follow-up. PROMs were evaluated with the Intraocular Lens Satisfaction (IOLSAT; Alcon Vision, LLC) (general satisfaction), Catquest-9SF (difficulty in doing vision-related activities), and Questionnaire for Visual Disturbances (QUVID; Alcon Vision, LLC) (photic phenomena) questionnaires. Mean 3-month postoperative binocular uncorrected distance, intermediate, and near visual acuities were 0.02 ± 0.11, -0.02 ± 0.09, and 0.14 ± 0.13 logarithm of the minimum angle of resolution (logMAR), respectively. Mean binocular distance-corrected intermediate and near visual acuities were 0.05 ± 0.11 and 0.32 ± 0.14 logMAR, respectively. All eyes had a postoperative spherical equivalent within ±0.75 diopters (D). The mean defocus curve showed visual acuity values better than 0.20 logMAR for defocus levels from +0.50 to -1.50 D. Most patients (92.0%) confirmed that they had no vision-related difficulties limiting their normal daily life. Perception of starbursts, halos, glare, hazy vision, blurry vision, and double vision was referred by 24%, 24%, 28%, 28%, 8%, and 4% of patients, respectively. Most of symptoms were described as mild and not or a little bothersome. The wavefront-shaping EDOF IOL evaluated provides efficacious distance, intermediate, and near visual restoration in eyes with previous LASIK, with minimal photic phenomena associated. Visual Outcome and Patient Satisfaction with Implantation of Trifocal Intraocular Lens After Radial Keratotomy.

To investigate the visual outcomes and patient satisfaction following trifocal intraocular lens (IOL) implantation after radial keratotomy (RK). This was a retrospective chart review wherein we studied 14 eyes from 7 patients who had undergone cataract surgery and had trifocal intraocular lens (IOL) implanted in the Eastern Province of Saudi Arabia's King Fahd University Hospital and Kahhal private centre. Data such as demographic characteristics, the time between RK and cataract removal, intraoperative and postoperative complications, preoperative and postoperative uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), and refraction were evaluated. In addition, visual complaints and satisfaction were assessed through a questionnaire. All eyes included had 8 RK sections. The mean spherical equivalent was +1.81 ± 4.27 D preoperatively and was -0.41 ± 1.2 D at the last follow-up. The mean uncorrected distance visual acuity (UDVA) increased from 0.32 ± 0.2 prior to surgery to 0.64 ± 0.2 following surgery. The mean efficacy index was 1.01 and a safety index was 1.26. In the last follow-up, 29% reported having 0.5 spherical equivalents, while 86% were within 1D. Improvement in cylindrical correction was noticed in the last follow-up with 36% not more than 0.5 D and 43% within 1D. In total, 80% of patients had a distance-corrected near visual acuity (DCNVA) of better than 20/40. The survey response rate was 100%, and two patients reported a lower degree of satisfaction. Glare and difficulty seeing at night were the most often reported complaints, with a mean of 2.86 ± 1.95 and 22 ± 1.9, respectively. Our results suggest that implantation of a trifocal IOL is safe and effective in patients with previous RK. However, several factors, including the patient's preoperative expectations, glare testing, refractive error, number of cuts, optical zone size, and visual phenomena related to RK, should be considered before implantation.

The Clareon Vs AcrySof PanOptix Trifocal IOL: A Comparative Study of Patient Satisfaction and Visual Performance.

To evaluate patient-reported outcomes of cataract surgery using the Clareon Panoptix and Panoptix Toric trifocal lenses and to compare these to the data collected previously for the AcrySof Panoptix and Panoptix Toric. Prospective, open-label, multicenter analysis of satisfaction, spectacle independence, presence of unwanted side effects, and best-corrected visual acuity among patients undergoing cataract surgery who had been implanted at least 1 month previously with the Clareon PanOptix or PanOptix Toric trifocal IOL bilaterally. Results were compared to outcomes measured two years ago from a similar study with the AcrySof version of the same lens. No significant differences in patient satisfaction rates were reported between the two cohorts. Spectacle independence was similar with 88% of Clareon Panoptix and 83% of AcrySof Panoptix patients having no need for any corrective lenses. Quality of vision was not statistically different with 7% of Clareon vs 15% of AcrySof patients reporting "very much" or more of glare/halo severity. Significantly more AcrySof (66%) than Clareon (42%) patients achieved a spherical equivalent outcome within 0.25 D of target. Best corrected distance visual acuity differences were not statistically significant, and no safety concerns were reported. The Clareon PanOptix trifocal lens provides similar satisfaction and spectacle independence and has a similar side effect profile and BCVA outcome to the identical lens made of the predecessor AcrySof lens.

Distribution of Ocular Biometry and Intraocular Lens Power in Patients Undergoing Cataract Surgery in an Eye Camp in Northern Pakistan

Purpose: To report ocular biometric data of patients undergoing cataract surgery in an eye camp in a city of Northern area of Pakistan. Design: Cross-sectional descriptive study. Place and Duration of Study: A surgical eye camp was set up in the month of September 2022 for 6 days in a private hospital in Skardu. Methods: A total of 119 patients were selected with a non-probability consecutive sampling technique. Ocular biometry of permanent local residents with significant cataractswas included. Incomplete biometry data and coexisting diseases like corneal opacity, pterygium, and severe dry eye causing errors in biometry were excluded. Keratometry was done with an automated refractor-keratometer. Axial length was calculated with an A-scan (ultrasound contact method) and intraocular lens (IOL) power was calculated. Results: A total of 119 patients were selected for biometry among 3908 individuals visiting an eye camp. The mean axial length was 22.13 ± 1.25 SD mm. The mean keratometry readings (K1) was 43.28 ± 1.83 Diopter and (K2) 44.26 ± 1.67 D. The mean intraocular lens (IOL) power was 24.78 ± 3.43 D. The mean K readings were more in males as compared to females (p-value &lt;0.005). There was no significant difference in axial length and IOL power among genders. Conclusion: This study gives normative data on ocular biometric parameters of patients undergoing cataract surgery at high altitudes in Skardu, Pakistan. These biometric values can be used as reference values in the local population
