Clinical value of grayscale ultrasound combined with real-time shear wave elastography nomogram in risk prediction of thyroid cancer

ObjectivesThis study constructed a nomogram based on grayscale ultrasound features and real-time shear wave elastography (SWE) parameters to predict thyroid cancer.MethodsClinical data of 217 thyroid nodules of 201 patients who underwent grayscale ultrasound, real-time SWE, and thyroid function laboratory examination in Ma’anshan People’s Hospital from January 2019 to December 2020 were retrospectively analyzed. The subjects were divided into a benign nodule group (106 nodules) and a malignant nodule group (111 nodules). The differences in grayscale ultrasound features, quantitative parameters of real-time SWE, and laboratory results of thyroid function between benign and malignant thyroid nodules were analyzed. We used a chi-square test for categorical variables and a t-test for continuous variables. Then, the independent risk factors for thyroid cancer were analyzed using multivariate logistic regression. Based on the independent risk factors, a nomogram for predicting thyroid cancer risk was constructed using the RMS package of the R software.ResultsMultivariate logistic regression showed that the grayscale ultrasound features of thyroid nodules were the shape, margin, echogenicity, and echogenic foci of the nodules,the maximum Young’s modulus (SWE-max) of thyroid nodules, and the ratio of thyroid nodule and peripheral gland (SWE-ratio) measured by real-time SWE were independent risk factors for thyroid cancer (all p < 0.05), and the other variables had no statistical difference (p > 0.05). Based on the shape (OR = 5.160, 95% CI: 2.252–11.825), the margin (OR = 9.647, 95% CI: 2.048–45.443), the echogenicity (OR = 6.512, 95% CI: 1.729–24.524), the echogenic foci (OR = 2.049, 95% CI: 1.118–3.756), and the maximum Young’s modulus (SWE-max) (OR = 1.296, 95% CI: 1.140–1.473), the SWE-ratio (OR = 2.001, 95% CI: 1.403–2.854) of the thyroid nodule to peripheral gland was used to establish the related nomogram prediction model. The bootstrap self-sampling method was used to verify the model. The consistency index (C-index) was 0.979, ROC curve was used to analyze the nomogram scores of all patients, and the AUC of nomogram prediction of thyroid cancer was 0.976, indicating that the nomogram model had high accuracy in the risk prediction of thyroid cancer.ConclusionsThe nomogram model of grayscale ultrasound features combined with SWE parameters can accurately predict thyroid cancer.

Comparison of Diagnostic Values of ACR TI-RADS versus C-TIRADS Scoring and Classification Systems for the Elderly Thyroid Cancers.

To compare the diagnostic value of the Thyroid Imaging Reporting and Data System (TI-RADS) of the American College of Radiology (ACR) versus the Chinese Thyroid Imaging Reporting and Data System (C-TIRADS) scoring and classification system for elderly thyroid cancers. A total of 512 nodules from 465 patients aged ≥60 with surgical pathology-proven thyroid nodules were enrolled in our study. The ultrasound features of thyroid nodules were independently evaluated by the ACR TI-RADS and C-TIRADS classification systems, and the receiver operating characteristic curve (ROC) was plotted. The optimal cut-off values of the ACR TI-RADS and C-TIRADS scoring and classification systems for diagnosing elderly thyroid nodules were estimated, and the diagnostic efficacy was analyzed. The ACR TI-RADS and C-TIRADS scores and classifications of thyroid cancers were both higher than benign nodules (both P < 0.05). The area under the curve (AUC) of ACR TI-RADS and C-TIRADS scoring and classification systems were 0.861, 0.897, 0.879, and 0.900, respectively, and the AUC of the scoring system was greater than the classification system for both criteria. When the Youden index was the highest, the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the ACR TI-RADS scoring and classification systems were consistent, ie, they were 89.66%, 41.70%, 89.93%, and 59.00%, respectively; the sensitivity, specificity, PPV, and NPV of the C-TIRADS scoring and classification systems were also consistent, ie, they were 88.71%, 44.26%, 90.23%, 59.69%, respectively. The diagnostic efficacy between the two systems was not statistically significant. ACR TI-RADS and C-TIRADS systems had relatively high diagnostic efficacy for elderly thyroid cancer. The diagnostic efficiency of the scoring systems of ACR TI-RADS and C-TIRADS were higher than the classification systems. In addition, the two systems had high clinical practical values, while there is still a significant risk of missed diagnosis.

Cytological and Ultrasound Features of Thyroid Nodules Correlate With Histotypes and Variants of Thyroid Carcinoma.

Prognosis is excellent for papillary thyroid carcinoma (PTC), noninvasive follicular thyroid neoplasia with papillary-like nuclear features (NIFT-P), and follicular thyroid carcinoma (FTC) but is poor for poorly differentiated thyroid carcinoma (PDTC) and anaplastic thyroid carcinoma (ATC). Among PTCs, the prognosis is more favorable for follicular (FV-PTC) and classic (CV-PTC) than for tall cell (TCV-PTC), and solid (SV-PTC) variants. To associate histotypes and variants of thyroid carcinoma with ultrasound and cytological features. Histology of 1018 benign tumors and 514 PTC (249 CV, 167 FV, 49 TC, 34 SV, and 15 other variants), 52 NIFT-P, 50 FTC, 11 PDTC, and 3 ATC was correlated with fine-needle aspiration biopsy categories (Italian classification: TIR1, TIR2, TIR3A, TIR3B, TIR4, and TIR5) and ultrasound features at the Endocrinology Unit, University Hospital of Pisa. In total, 1117 patients with thyroid nodule(s) who underwent thyroidectomy were included. Of PTC, 36.3% had indeterminate cytology (TIR3A or TIR3B), 56.6% were suspicious for malignancy or malignant (TIR4 or TIR5); 84.0% FTC and 69.3% NIFT-P were TIR3A or TIR3B; 72.5% FV-PTC and 73.6% SV-PTC were TIR3A or TIR3B; 79.9% CV-PTC and 95.9% TCV-PTC were TIR4 or TIR5. The association of a hypoechoic pattern, irregular margins, and no microcalcifications was more frequent in TCV-PTC than in CV-PTC (P = .02, positive predictive value = 38.9%; negative predictive value = 85.5%). At cytology, most FTC, NIFT-P, FV-PTC, and SV-PTC were indeterminate, most CV-PTC and TCV-PTC were suspicious for malignancy or malignant. Ultrasound can be helpful in ruling out TCV-PTC.

Reevaluation of the Correlations between Ultrasound Features of Thyroid Nodules and Grades of Bethesda Classification

AIM: To reevaluate the correlations between ultrasound (US) features of thyroid nodules (THNs) and grades of Bethesda classification, to select correctly the patients who must undergo fine needle aspiration (FNAB). MATERIAL AND METHODS: In this study, we have included 260 cytologies of thyroid gland between the period of 2014–2018. The procedures are performed at radiology department of Hygeia Hospital. In our study are excluded the cases with a high risk of hemorrhage and the patients which did not accept the anesthetic procedure because of anxiety. The study includes only the first punctions with their respective Bethesda classification and not repeated FNAB cases. First using the z test, we compared the percentage occupied by the Bethesda categories that are indicative of surgery (BIV + BV + BVI) at US features that suspect malignancy (hypoechogenicity, microcalcifications, abnormal contours, central vascularization), with the percentage occupied by group (BIV + BV + BVI) at the US features which indicate benignity (hyperechoic, no microcalcifications, peripheral vascularization, cystic-solidocystic, spongiform, normal contours). Furthermore, We have evaluated utilizing the odds ratio if there was a correlation between TR4 and TR5 categories in ACR/TIRADS classification and the categories (BIV+BV+BVI) for any statistical significance. The significance of the dimensions of the nodule was tested as an indicator for surgical intervention. For this purpose, the percentage occupied by the nodules with a diameter larger than 1.5 cm at (BIV + BV + BVI) group was compared with the percentage occupied by nodules smaller than 1.5 cm at BIV + BV + BVI. In addition, we observed if there was a strong statistical connection between nodules larger than 1.5 cm and the Bethesda categories that suggested malignancy. There was no statistical test made for the features “taller than wide” and microcalcifications because of the small number of cases. It was also made a comparison of percentages (BIV + BV + BVI) even for three clinical features: Men versus women, solitary nodule versus multinodular goiter, left lobe versus right lobe. We compared the percentages occupied by the (BIV + BV + BVI) group of categories in patients over 45 years old with the percentages occupied by this group at patients younger than 45 years old. We also noted which of Bethesda categories is more frequent. CONCLUSIONS: The features that are more indicative for FNAB are hypoechogenicity, consistency, intranodal vascularization, and extralobar positioning. If a THN has one of the above features and has a dimension of more than 10 mm, it has an indication for FNAB. Indications for FNAB increase with the increasing of the abovementioned features of a THN. The combination of US features that suggest malignancy, TR4 and TR5, with BIII category is a strong indicator for surgical intervention. The results of this study are similar with the results of prior studies, and we could not distinguish any specific US feature that has an absolute indication for FNAB. The appropriate determination of the US features of a THN in correlation with the patient’s clinic information will determine the proper indication for a FNAB.

Thyroid Parenchyma Microcalcifications on Ultrasound for Predicting Lymph Node Metastasis in Papillary Thyroid Carcinoma: A Prospective Multicenter Study in China.

Objective: Cervical lymph node metastasis (LNM) was found to be clinically significant prognostic factors of patients with papillary thyroid carcinomas (PTC). Ultrasound (US) characteristics of thyroid nodules and thyroid parenchyma may be used to predict LNM. To investigate the value of nodular US features as well as thyroid parenchymal microcalcifications on US in predicting LNM in patients with PTC.Methods: This prospective study was approved by the Institutional Review Board. From January 2018 to June 2019, 971 consecutive patients with solitary PTC who underwent preoperative neck US evaluation were included from six hospitals in China. The US features of thyroid nodules as well as thyroid parenchyma microcalcifications were carefully evaluated based on the static images and dynamic clips. Univariate and multivariate analyses were performed to determine independent predictors of LNM.Results: Of the 971 patients, 760 were female, 211 were male. According to the pathological examination, 241(24.82%) patients were found with cervical LNM (LNM positive group), while 730 (75.18%) patients were not (LNM negative group). Multiple logistic regression analysis showed that young age (<55 years old) (OR = 1.522, P = 0.047), large size (>10 mm) (OR = 1.814, P < 0.001), intratumoral microcalcifications (OR = 1.782, P = 0.002) and thyroid parenchyma microcalcifications (OR = 1.635, P = 0.046) were independent risk factors for LNM of PTC.Conclusions: Young age, large nodule size, intratumoral microcalcifications, as well as thyroid parenchyma microcalcifications on US are independent predictors of cervical LNM for patients with PTC.

Diagnosis of thyroid cancer using a TI-RADS-based computer-aided diagnosis system: a multicenter retrospective study

ObjectiveThe purpose of this study was to use a computer-aided diagnosis (CAD) system based on the Thyroid Imaging, Reporting, and Data System (TI-RADS) to improve the diagnostic performance of thyroid cancer by analyzing clinical ultrasound imaging data. MethodsA retrospective diagnostic study of ultrasound image sets was conducted at five hospitals in China. A CAD system based on TI-RADS was applied in this study, and the diagnostic performance of CAD system was tested through multi-center data. The performance of the CAD system was compared with the consensus of three experienced radiologists. The interobserver agreement for cancer diagnosis was calculated between the CAD system and the consensus of the three experienced radiologists. ResultsThe CAD system performed well in the diagnosis of thyroid cancer, with an area under the curve (AUC) value of 0.902 (95% CI: 0.884–0.918), and obtained results similar to those of the three experienced radiologists. The CAD system performed better in the internal test set than in the external test set (AUC: 0.930 vs 0.877, respectively). The performance of the CAD system in the diagnosis of thyroid cancer for nodules of different sizes (<1 cm, 1–2 cm and ≥2 cm) was basically similar (accuracy: 84.6% vs 85% vs 84.2%). The CAD system can recognize 15 ultrasound features of thyroid nodules, most of which reached the level of 3 experienced radiologists (12/15, 85%). ConclusionThe CAD system achieved an improved AUC and similar sensitivity and specificity in the diagnosis of thyroid cancer compared with the consensus of experienced radiologists.

The Role of Ultrasound in the Diagnosis of Malignant Thyroid Nodules

Background: Thyroid nodules are very common in clinical practice. Although most of thyroid nodules are benign, it is crucial to checkout which nodules are more likely to be malignant. Ultrasound is a major diagnostic tool for screening and evaluating thyroid diseases because it is safe, non-invasive, non-radioactive and effective.Objective: The aim is to identify the role of ultrasound in assessing thyroid nodules and to review various ultrasound criteria predicting malignancy.Patients and methods: A case series study conducted during the period from January 2015 to February 2016 at the First Surgical Unit, Department of Surgery, Baghdad Teaching Hospital by a team of surgeons. One hundred eighty Patients who underwent surgical intervention for nodular thyroid disease were included in the study. The ultrasound features of these patients were analyzed with respect to the number of nodules (solitary or multiple), size, echogenicity, consistency, the presence of calcification, border definition, vascular pattern and whether there was lymphadenopathy and / or local invasion to adjacent structures. The final diagnosis was confirmed by histopathological examination after surgery. The ultrasound features of thyroid nodules were studied for their correlation with benign and malignant lesions.Results: Among 180 patients with nodular thyroid disease, thyroid malignancy was found in 22 (12.2%) patients. Ultrasound features of thyroid nodules included significant echogenicity, consistency, calcification, border definition, vascular pattern and the presence of local invasion. Ultrasound features suggesting malignant nodules are solid and hypoechoic nodules, ill-defined borders, nodules with local invasion to adjacent structures, the presence of microcalcification and nodules that show increased vascularity on doppler study.Conclusion: Ultrasound is useful in evaluating nodular thyroid disease and is valuable for identifying many malignant and potentially malignant nodules. Despite that, no single ultrasound feature is reliable in making the diagnosis of thyroid malignancy, combining ultrasound suggestive criteria could aid in predicting malignant nature of a given nodule.

Rates of lymphocytic thyroiditis and ultrasound features of citologically-interrogated thyroid nodules based on the area of residence in a Sicily province.

To verify the prevalence of autoimmune thyroiditis (AIT) and the ultrasound characteristics (composition and volume) of thyroid nodules with respect to the area of residence in the province of Messina, some areas having environmental issues. Fine-needle aspiration-interrogated nodules (n = 902) of 809 patients were evaluated upon stratification into 8 areas of residence. Overall, women were younger than men (55.3 ± 14.0 vs. 58.6 ± 12.6 years, P = 0.0083). Patients residing in three areas (one hosting two garbage dumps, one hosting a petrochemical complex and a thermoelectrical power plant, and one hosting several ceramic factories [CFA]) were younger than those residing in the city of Messina (MEA) (52.9 ± 13.4 vs. 57.7 ± 13.6 years, P < 0.0001). Also, patients residing in those three areas had a greater rate of AIT, diagnosed either ultrasonographically/serologically (22.2% of patients) or cytologically (26.3% of nodules), compared with MEA (11.7% of patients, P = 0.0007 or 20.2% of nodules, P = 0.0815). Rates of AIT ranged 12.5-28.6% in the remaining four areas. Overall, nodules in women were smaller than in men (3.6 ± 5.7 vs. 6.1 ± 9.4 ml, P = 0.0006). Compared with the other seven areas, patients living in CFA had the largest nodules (6.8 ± 6.8 ml, P = 0.0040-0.0291), with the nodule volume being inversely correlated to patient's age (r = -0.4955, P = 0.0431). Rates of AIT and associated ultrasound features of thyroid nodules vary in different areas of our province. Further studies correlating these rates and features with exposure to specific toxicants are warranted.

Evaluation of the Bethesda System and the ACR TIRADS in an Endemic Goiter Region

ABSTRACT Background Various factors can affect incidence of thyroid disorders and disease profiles may show abrupt changes in endemic goitrous areas. In this study, it was aimed to analyze the American College of Radiology (ACR) Thyroid Imaging Reporting and Data System (TI-RADS) and the Bethesda System for Reporting Thyroid Cytopathology (BSRTC) in terms of risk of malignancy and general recommendations in an endemic goiter region (EGR). Methods In this retrospective study, a total of 500 patients who had thyroidectomy following thyroid fine needle aspiration biopsy were enrolled. For the assessment of thyroid cytology, BSRTC was used and for the evaluation of ultrasound features of thyroid nodules, ACR TIRADS lexicon was adopted. For the assessment of thyroid cytology, Bethesda classification was used and for the evaluation of ultrasound features of thyroid nodules, ACR TIRADS lexicon was adopted. Results In the EGR setting, benign category of BSRTC had a cancer risk of 6.2% which was two times more than the 2017 BSRTC revision reported. Nodules 10–14.9 mm in diameter had nearly 4 times higher malignancy risk than nodules >15 mm. In this group of patients, the risk of malignancy for TIRADS level 1, 2, 3, 4 and 5 was 1.16%, 2.94%, 7%, 45.64% and 94.44%, respectively. The malignancy rates for Bethesda system category I, II, III, IV, V and VI were as follows: 14.43%, 6.2%, 19.05%, 36.73%, 75.68% and 100%. Conclusions There are slight differences between the common set of standards and this study results regarding risk of malignancy. This brings up the question whether there is need for revision for the use of categories and the appropriate management in endemic goiter regions.

OR27-1 Interobserver Agreement in the Assessement of Thyroid Nodules Ultrasound Features: A Blinded Multicenter Study

Background: Single-center trials have demonstrated an acceptable level of inter-observer agreement in the evaluation of the ultrasound (US) features of thyroid nodules, but limited data are available about the consistency in the assessment of US findings among different thyroid centers. Aim of the study: to assess the inter-observer agreement between different thyroid centers and among different specialists in the evaluation of the main US features of thyroid nodules. Materials and methods A blinded retrospective analysis of 100 electronically-recorded US images was conducted in three large-volume thyroid centers by seven qualified thyroid imaging experts, two radiologist and five endocrinologists. The following US features were evaluated: composition (solid, predominantly solid, predominantly cystic, and cystic); echogenicity (hyperechoic, isoechoic, mildly and deeply hypoechoic); margins (well-defined, ill-defined, microlobulated, and spiculated); calcifications (absent, microscopic, macroscopic, eggshell); hyperechoic foci of uncertain significance; comet-tail artifacts; vascularity (no vascular signals, perinodular and/or slight intranodular flow, and marked intranodular flow). Thyroid nodules were also classified according to four major US classification systems: AACE/ACE/AME, EU-TIRADS, ATA and ACR. The inter-observer agreement was calculated using cross-tabulation expressed in Cohen's Kappa. Kappa values were evaluated, according to Landis and Koch, as follows: 0-0.20 poor, 0.21-0.40 fair, 0.41-0.60 moderate, 0.61-0.80 substantial, and 0.81-1.0 almost perfect agreement. A sub-analysis assessed how many times and how many operators evaluated as suspicious for malignancy the US features of each nodule. Results: The inter-observer agreement resulted in a K-correlation coefficient of 34.5%, 44.0%, 42.3% and 38.8% for the ATA, AACE/ACE/AME, ACR, and EU-TIRADS classification systems, respectively. The interobserver agreement for the main thyroid nodule US findings resulted as follows: composition 53.2%; echogenicity 46.9%; margins 33.2%; comet-tail 10.6%, microcalcifications 46.8%; macrocalcifications 37.7%; eggshell calcifications 64.9%; intranodular vascularity 45.9%. Conclusion: The level of agreement among different thyroid centers in the description of suspicious US features in thyroid nodules ranged from fair to moderate, with the lowest level of consistency for the characteristics of margins and the presence of comet-tails. A similar range of variability was demonstrated also for the four US classification systems. An universally accepted lexicon of thyroid US features and a dedicated training in thyroid US findings definition are needed to improve the inter-observer agreement and the predictive value of US classification systems in real world practice.

OR27-3 A Novel Risk Stratification System for Thyroid Nodules with Indeterminate Cytology: A Pilot Cohort Study

Background Thyroid ultrasound (US) and US-guided fine needle aspiration biopsy (FNAB) have been widely used to stratify the risk of malignancy in thyroid nodules (TN). Cytology material characterized as category III and IV by the Bethesda System for Reporting Thyroid Cytopathology is associated with thyroid cancer (TC) risk ranging between 5-30%. The use of molecular markers may help defining the management strategy in these indeterminate nodules. The goal of this study was to create a risk stratification system based on US features and the results of molecular profiling of cytologically indeterminate TN and to assess its diagnostic accuracy. Methods We performed a cohort study including patients with TN who underwent thyroid US and FNAB revealing indeterminate cytology (Bethesda III, IV). Final pathology diagnosis was available for all patients. DNA and RNA were extracted from pathology material and subjected to Ion Torrent™ Oncomine™ Comprehensive Assay v3 (OCAv3) next-generation sequencing, analyzing single nucleotide variants (SNV), small insertions and deletions (INDEL), copy number variants (CNV) and gene fusions (GF) from 161 cancer driver genes. Each genetic alteration was annotated (X) based on its strength of association with TC per TCGA and COSMIC database. US features of TN were similarly categorized (Xus) by two independent clinicians, blinded to pathology report. The total risk score (TRS) was estimated based on the following formula TRS= (XSNV/INDEL)*n + XGF + XCNV + XUS , where X is the annotated score and n is the number of SNVs/INDELs. ROC curve was performed to assess the diagnostic accuracy of US alone, OCAv3 alone and TRS. Results We analyzed 84 TN in 50 patients (39 females and 11 males), age 47.2 ± 14.8 years. Thirty-four patients (68%) were diagnosed with TC: 21 papillary (PTC), 6 follicular variant of PTC (FVPTC), 1 noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP), 2 Hürthle cell (HTC), 1 poorly differentiated carcinoma (PD) and 3 medullary TC (MTC). The molecular signature revealed the BRAF V600E mutation as the most common in PTC, while KRAS and NRAS mutations were associated with FVPTC. A NIFTP showed a PAX8-PPARG fusion. HTC had pathogenic variants of SLX4, ATM and NRAS genes, PD was associated with HRAS mutation, and MTC with RET pathogenic variants. Benign lesions were characterized by either no mutations or the presence of GNAS, HRAS, NRAS and ESR1 variants. The diagnostic accuracy of US alone was 68% (sensitivity 41.2%, specificity 93.8%), while for OCAv3 alone it was 79% (sensitivity 65%, specificity 93.8%). TRS was associated with the highest diagnostic accuracy of 83.5% (sensitivity 79.4%, specificity 88%). Conclusions Combining risk stratification based on thyroid US and molecular signature improves diagnostic accuracy of TC in patients with indeterminate TN. Our pilot study requires validation in independent larger cohorts.

Correlation of high-resolution ultrasonography findings of thyroid nodules with ultrasound-guided fine-needle aspiration cytology in detecting malignant nodules: A retrospective study in Malabar region of Kerala, South India

Aims:The purpose of our study is to assess the role of high-resolution real-time gray-scale ultrasonography with ultrasound-guided fine-needle aspiration cytology (FNAC) in detecting malignant nodules in the thyroid gland.Settings and Design:It is a retrospective study of 25 patients (aged 16–63 years) who underwent high-resolution ultrasound (HRUSG) of the thyroid gland and ultrasound-guided FNAC from February 2017 till November 31, 2017.Materials and Methods:A total of 25 patients with thyroid nodules detected at ultrasound were included in this study. The characteristics of each nodule were determined. The results were then compared with FNAC (ultrasound-guided) findings.Statistical Analysis Used:Sensitivity, specificity, positive predictive value, negative predictive value and accuracy were used.Results:Of the 25 nodules examined, 5 (20%) were found to be malignant on cytology.ConclusionsGray-scale ultrasound features of thyroid nodules are useful to distinguish malignant thyroid nodules from those with benign nodules. From our study, it is obvious that the HRUSG findings of hypoechogenicity, microcalcification, and poorly defined margins have high diagnostic accuracy for identifying malignant thyroid nodules.

Evaluation of selected ultrasound features of thyroid nodules with atypia of undetermined significance/follicular lesion of undetermined significance for the Bethesda reporting system for thyroid cytology.

Background and objectiveThe risk of malignancy from “atypia of undetermined significance/follicular lesion of undetermined significance” (AUS/FLUS) is estimated to lie between 5% and 15%; however, some authors suggest that the risk of malignancy in AUS/FLUS depends upon specific clinical situations. This was a retrospective study which aimed to determine the incidence and risk of thyroid cancer (TC) based upon selected ultrasound features from patients with thyroid nodules (TN) classified as AUS/FLUS.MethodsUnivariate and multivariate logistic regression analyses were used to identify significant associations between ultrasound features and the risk of TC.ResultsOf 127 patients with TN classified as AUS/FLUS who underwent thyroidectomy, 114 (89.8%) had benign disease while 13 (10.2%) had TC. Univariate analysis identified several significant predictors for TC (all p<0.05), including microcalcifications, hypoechogenicity, the prevalence of irregular margins, a taller rather than a wide form, high vascularity, and fast tumor growth. Multivariate analysis further showed that microcalcifications (odds ratio =21.37; p=0.024) and fast growth (odds ratio =22.70; p=0.021) were significant and independent factors associated with the risk of developing TC.ConclusionMicrocalcifications and fast growth of the TN could therefore be used as predictive factors for the development of TC in patients with AUS/FLUS.

Assessment of Malignancy Risk in Thyroid Nodules Using a Practical Ultrasound Predictor Model: “Alpha Score”

Objective: The aim of this study was to develop a simple predictor model to diagnose malignancy by using ultrasound features of thyroid nodules and the association with cytopathological diagnosis obtained by fine needle aspiration. Materials and Methods: The likelihood of malignancy from ultrasound features was assessed in thyroid nodules obtained by fine-needle aspiration biopsy (FNAB) according to cytopathological findings reported using Bethesda System. A score was developed depending on the presence of each ultrasound feature evaluated. Results: 429 nodules were assessed, 103 (24%) were malignant. The following ultrasound features were associated with malignancy, according to the logistic regression analysis and were assigned a score of 0, +1, +2 depending on the presence or absence of each one: hypoechogenicity, solid appearance, irregular margins, microcalcifications, absence of a halo, diameter of ≥10 mm and intranodular vascular flow. The area under the curve of the proposed model was 0.900, demonstrating its predictive capacity. 4 risk categories were stablished based on the score obtained. Malignant nodules scored higher than the benign nodules (7.24 ± 1.87 vs. 3.74 ± 1.83). Conclusions: The proposed predictive model demonstrated to be useful and easy to apply when stratifying thyroid nodule risk of malignancy using presented US features and applying the proposed risk categories to increase the accuracy at selecting nodules that need to be studied with FNA.

Pre-operative features of non-invasive follicular thyroid neoplasms with papillary-like nuclear features: An analysis of their cytological, Gene Expression Classifier and sonographic findings.

To investigate the corresponding cytological diagnoses, Gene Expression Classifier (GEC) results and ultrasound features of thyroid nodules diagnosed as non-invasive follicular thyroid neoplasms with papillary-like nuclear features (NIFTP), as well as any coexisting pathology. We performed a retrospective review of thyroid nodules histologically diagnosed as NIFTP at our institution between 1st April 2016 and 1st April 2017. The following data points were collected: demographics, nodule size, ultrasound features, cytological diagnosis, GEC results, origin of sample (in-house vs outside hospital) and any additional pathology identified in the resection specimen. The case cohort included 87 nodules diagnosed as NIFTP (size range: 1-7cm, mean: 2.5cm) from 82 patients (age range: 22-82, mean age: 50.4, M:F-1:4.1). Corresponding FNA results were available for 72 nodules (82.8%) and were categorised as follows: benign (n=5, 6.9%), atypia of unknown significance/follicular lesion of undetermined significance (n=29, 40.3%), follicular neoplasm/suspicious for follicular neoplasm/follicular neoplasm with oncocytic features (n=27, 37.5%), suspicious for papillary thyroid carcinoma (n=6, 8.3%) and malignant (n=5, 6.9%). GEC results were available for 32 (44.4%) nodules, with the majority of cases classified as suspicious (81.3%). On ultrasound, most of the nodules were predominantly solid (81.8%), vascular (93.8%), non-calcified (86.5%), and either hypoechoic (44.9%) or isoechoic (38.8%). In addition to NIFTP and other benign findings in the background thyroid, 75 separate malignant tumours were identified in 38 (46.3%) patients, many of which were papillary thyroid microcarcinomas (86.5%) with lymph node metastases present in two cases. The majority of thyroid nodules histologically diagnosed as NIFTP have indeterminate cytology (77.8%) and are classified as suspicious (81.3%) by GEC testing. Taken together, these findings can guide clinicians toward a more conservative therapeutic approach.

Sonographic Criteria Predictive of Malignant Thyroid Nodules: Which Lesions Should be Biopsied?

The objective of this study was to evaluate the ultrasound features of thyroid nodules and their association with malignancy, focusing on establishing feature-oriented ultrasound criteria to determine proper management of a thyroid nodule. A sample of 379 thyroid nodules were biopsied (from a total of 357 patients aged 59.8 ± 14.8 years) and 300 were included in the final study (271 benign nodules and 29 malignant ones). Ultrasound features were recorded for each nodule: size, echogenicity, homogeneity, contours, shape, texture, peripheral halo, calcifications, and the presence of adenopathy. Statistical analysis of the data was performed using the Mann-Whitney U test and chi-square test. The sensitivity and the specificity of variables seen to have a statistically significant association with the malignancy of nodules were assessed and a logistic regression was performed. A taller-than-wide shape, an ill-defined contour, the presence of a halo, microcalcifications, and adenopathy were found to have a statistically significant relationship (P < 0.05) with malignancy, although with a low sensitivity and a high specificity. The presence of at least one suspicious feature yields great sensitivity (89.7%) in detecting malignant disease. The ultrasound features of thyroid nodules alone do not allow the radiologist to make a confident diagnosis regarding the malignancy of a nodule without performing a biopsy. However, a nodule showing a taller-than-wide shape, microcalcifications, a peripheral halo, an ill-defined contour, or associated adenopathy should be considered for cytology.

Association between lifestyle and anthropometric parameters and thyroid nodule features.

Thyroid nodularity has been associated with obesity, but data regarding associations of body composition parameters with specific ultrasound features of thyroid nodules are lacking. The aim of the present study was to assess associations between thyroid nodule ultrasound characteristics, lifestyle, and anthropometric parameters. This was a cross-sectional study in the general apparently healthy population of Northern Greece. Thyroid ultrasound data together with medical history, demographic, and anthropometric characteristics were individually recorded. Body composition was evaluated using bioelectrical impedance. Three hundred and six subjects [215 females (70.3%), aged 20-83 years] were included. Ultrasound revealed one or more thyroid nodules in 168 subjects (54.9%). Subjects with thyroid nodules were more frequently females (p = 0.033), older (p < 0.001) and had higher fat mass (p = 0.011), total body fat percentage (p < 0.001) and waist circumference (p = 0.045) than subjects without nodules. In logistic regression analyses, age and female gender were the only independent predictors of presence of thyroid nodules, as well as specific sonographic features. Additionally, total body fat percentage was positively correlated with nodule size (rho = 0.210, p = 0.006) and was the only independent predictor of hypoechoic thyroid nodule(s) and peripheral vascularity, while lack of exercise was predictive of internal vascularity. Body fat accumulation and lack of exercise, used as surrogate markers of sedentary lifestyle, influence thyroid nodule size and could predict some ultrasonographic characteristics, like hypoechoicity and internal vascularity. Therefore, routine thyroid examination of obese patients and promotion of active lifestyle may be warranted to prevent thyroid nodule formation and possibly progression to malignancy.

JOURNAL CLUB: Retrospective Evaluation of Ultrasound Features of Thyroid Nodules to Assess Malignancy Risk: A Step Toward TIRADS.

The aim of this retrospective study was to develop a thyroid nodule scoring system for malignancy potential to better select nodules for ultrasound (US)-guided fine needle aspiration (FNA). US-guided FNA was performed on 2375 thyroid nodules in successive patients. Cytologic or histopathologic confirmation of disease state in 2002 lesions showed that 148 were malignant. We developed an extended scoring system to provide more decision levels than standard scoring by including weak (macrocalcification, eggshell calcification, hypoechogenicity, solid consistency) as well as strong indicators of malignancy (microcalcification, hypoechogenicity, lobulated or ill-defined margins, taller-than-wide shape, suspicious lymph nodes) and by including contraindications (hyperechogenicity, comet-tail artifact, complete halo, cystic or microcystic). ROC analysis was used to compare detection accuracy and decision thresholds resulting from standard scoring using five major features with extended scoring. Although an accuracy advantage was found for the extended scoring over standard scoring, we discount this finding because our scoring involved a preliminary analysis. However, the extended scoring offers more reporting options for certainty of malignancy: standard scoring gave four potential thresholds for reporting; extended scoring gave nine. The most useful of the additional thresholds captured nearly 88% of malignancies but resulted in only 28% of patients without malignancy undergoing biopsy. Our extended Thyroid Imaging Reporting and Data System scoring provides more operating points to support treatment decisions. A simplified decision rule-biopsy every nodule with at least two weak features or one strong feature-preserves the most useful of the new decision thresholds from extended scoring.

Thyroid ultrasound features and risk of carcinoma: a systematic review and meta-analysis of observational studies.

Background: Thyroid nodules are a common finding in the general population, and their detection is increasing with the widespread use of ultrasound (US). Thyroid cancer is found in 5–15% of cases depending on sex, age, and exposure to other risk factors. Some US parameters have been associated with increased risk of malignancy. However, no characteristic seems sufficiently reliable in isolation to diagnose malignancy. The objective of this meta-analysis was to evaluate the diagnostic performance of US features for thyroid malignancy in patients with unselected thyroid nodules and nodules with indeterminate fine-needle aspiration (FNA) cytology.Methods: Electronic databases were reviewed for studies published prior to July 2012 that evaluated US features of thyroid nodules and reported postoperative histopathologic diagnosis. A manual search of references of review and key articles, and previous meta-analyses was also performed. A separate meta-analysis was performed including only nodules with indeterminate cytology. Analyzed features were solid structure, hypoechogenicity, irregular margins, absence of halo, microcalcifications, central vascularization, solitary nodule, heterogeneity, taller than wide shape, and absence of elasticity.Results: Fifty-two observational studies (12,786 nodules) were included. Nine studies included nodules with indeterminate cytology as a separate category, comprising 1851 nodules. In unselected nodules, all US features were significantly associated with malignancy with an odds ratio varying from 1.78 to 35.7, and microcalcifications, irregular margins, and a taller than wide shape had high specificities (Sp; 87.8%, 83.1%, 96.6%) and positive likelihood ratios (LHR; 3.26, 2.99, 8.07). Absence of elasticity was the single feature with the best diagnostic performance (sensitivity 87.9%, Sp 86.2%, and positive LHR 6.39). The presence of central vascularization was the most specific US feature in nodules with indeterminate cytology (Sp 96% and positive LHR 2.13).Conclusions: US features in isolation do not provide reliable information to select nodules that should have a FNA performed. A combination of US characteristics with higher likelihood ratios and consequently with higher post-test probabilities of malignancy—microcalcifications, or a taller than wide shape, or irregular margins, or absence of elasticity—will probably identify nodules with an increased risk for malignancy. Further studies are required to standardize elastography techniques and evaluate outcomes, especially in nodules with an indeterminate cytology.
